Traumatic Brain Injury and Accelerated Epigenetic Aging Among Post-9/11 Veterans.

OBJECTIVE: Military service over the last several decades has been associated with an increased risk of injuries, including traumatic brain injury (TBI). Veterans with a history of TBI often experience poor health outcomes and have higher rates of premature mortality. In this study, we examined whether accelerated biological aging could help explain negative health outcomes following TBI. SETTING, PARTICIPANTS, AND DESIGN: We evaluated the association between TBI and rate of epigenetic aging (assessed using DunedinPACE) using data from post-9/11 veterans ( N = 1152) enrolled in the VA Mid-Atl

Evaluating Decision Regret in Patients Who Have Undergone Sacral Neuromodulation.

OBJECTIVE: To quantify decision regret in individuals who had undergone sacral neuromodulation (SNM). Secondary objectives evaluated for correlation of subjects' regret score with symptom relief, complications, device explant status, or military deployment status. METHODS: Each subject's decision regret was assessed via a telephone survey utilizing a validated decision regret tool, and regret scores were calculated. A score of 50 was utilized a cutoff, below which patients were deemed to have minimal regret while those scores above 50 were associated with significant decision regret. Comparat

Tobacco Research in the Military: Reflections on 20 Years of Research in the United States Air Force.

The U.S. military is one of the world's largest employers. Approximately 30% of active duty military personnel smoke cigarettes and more than 14% use smokeless tobacco. The military has historically supported tobacco use and more recently is attempting to combat its use. Through 20 years of collaborative research with the United States Air Force, we have learned that smoking bans are effective, recruits who have never previously smoked cigarettes initiate tobacco use, smokeless tobacco serves as a gateway for smoking initiation, smoking is associated with discharge, smoking adds significant pr

Vaccines for military use.

Vaccines have long been used by military forces in order to prevent communicable diseases and thereby preserve the fighting force. A tradition that began with the mass vaccination of the Continental Army against smallpox during the War of the American Revolution in the late 18th century continues today with routine and deployment-based vaccination of military forces against potential pathogens of nature and biological weapon threats. As their role has expanded in recent years to include humanitarian and peacekeeping missions, the military's use of vaccines against infectious diseases has conco

Associations of Lifetime Traumatic Brain Injury Characteristics With Prospective Suicide Attempt Among Deployed US Army Soldiers.

OBJECTIVE: To estimate associations of lifetime traumatic brain injury (TBI) characteristics with prospective suicide attempt among US Army soldiers. METHOD: The Army STARRS (Study to Assess Risk and Resilience in Servicemembers) Pre/Post Deployment Study surveyed 3 Brigade Combat Teams that were deployed to Afghanistan in 2012. Lifetime TBI and past-month postconcussive/post-TBI symptoms were evaluated at predeployment baseline. Recency and number of TBIs were quantified, and TBI severity was classified on the basis of reports of alteration/loss of consciousness and memory lapse. Suicide att

Role of modifiable organisational factors in decreasing barriers to mental healthcare: a longitudinal study of mission meaningfulness, team relatedness and leadership trust among Canadian military personnel deployed on Operation LASER.

OBJECTIVES: The literature presents complex inter-relationships among individual-factors and organisational-factors and barriers to seeking mental health support after deployment. This study aims to quantify longitudinal associations between such factors and barriers to mental health support. DESIGN: A longitudinal online survey of Canadian Armed Forces (CAF) personnel collected data at 3 months post-deployment (T1), 6 months post-deployment (T2) and 1 year post-deployment (T3). SETTING: In 2020, as part of Canada's response to the COVID-19 pandemic, 2595 CAF personnel deployed on Operation

Factors associated with antisocial behavior in combat veterans.

The objective of this study was to identify factors associated with antisocial behavior in 1,543 Marines who deployed to combat zones in support of conflicts in Iraq and Afghanistan during 2002-2007. Five factors were associated with antisocial behavior in multivariate analyses: post-traumatic stress disorder (PTSD) symptoms, deployment-related stressors, combat exposure, younger age, and being divorced. PTSD symptoms had a stronger association with antisocial behavior than any other variable. A unique and important finding of this study was the association between deployment-related stressors

Obstetric complications on deployed operations: a guide for the military surgeon.

Modern military general surgeons tend to train and then practice in 'conventional' surgical specialties in their home nation; however, the reality of deployed surgical practice, either in a combat zone or on a humanitarian mission, is that they are likely to have to manage patients with a broad range of ages, conditions and pathologies. Obstetric complications of war injury include injury to the uterus and fetus as well as the mother and both placental abruption and uterine rupture are complications that military surgeons may have little experience of recognising and managing. On humanitarian

US Navy Women's Experience of an Abnormal Cervical Cancer Screening.

INTRODUCTION: Recent policy revisions allow greater inclusion of military women in operational and/or deployable positions (ie, shipboard, overseas, and war zone duty assignments), but these positions can create unique health care challenges. Military members are often transient due to deployments and change of duty stations, impacting timely follow-up care for treatable health conditions. There has been minimal research on challenges or strategies in preventive health screening and follow-up for US military women. METHODS: The purpose of this qualitative research study was to describe US Nav

The role of perceived threat in the emergence of PTSD and depression symptoms during warzone deployment.

OBJECTIVE: Numerous studies have shown that level of exposure to combat-related stressors is a robust risk factor for posttraumatic stress disorder (PTSD) and depression among military personnel deployed to a warzone. Threat perception of warzone experiences assessed retrospectively has been consistently linked to increased risk for PTSD and depression months or even years after returning from deployment. However, little is known about concurrent relations between perceived threat, deployment stress, and stress-related symptoms during deployment. Using a novel in-theater web-based assessment s

Gynecologic disorders diagnosed during deployment to Southwest/Central Asia, active component females, U.S. Armed Forces, 2008-2013.

Service women in the U.S. Armed Forces face unique challenges that may lead to or exacerbate gynecologic disorders - particularly during deployment. This report documented that approximately one in 10 military women who served in Southwest/Central Asia were diagnosed with a gynecologic disorder at least once during deployment. In addition, gynecologic disorders accounted for approximately one of every 20 medical evacuations of female service members from the war zone. A majority of clinically significant gynecologic disorder cases were attributable to irregular menstruation/bleeding or unspeci

Deployment-related respiratory health: bridging the gap between clinical and epidemiological associations and disease mechanisms: an official American Thoracic Society Workshop Report.

Land-based military deployers to Afghanistan and/or Southwest Asia (SWA) encountered exposure to high concentrations of respirable particulate matter (PM) from multiple sources, including desert dust, burn pit smoke, and military occupations. Adverse lung health effects following deployment have been noted, including upper and lower respiratory tract symptoms, asthma, and small airway and other abnormalities on lung biopsy. The American Thoracic Society (ATS) convened a workshop in 2018 to review studies assessing postdeployment respiratory health, describe emerging research, and highlight kno

The role of insect-brain-inspired technology in drone warfare.

For many years, the potential for insect-like robots to be deployed in a military context has been explored in fiction, but also supported by research funding from defence agencies around the world. Recent conflicts have seen a rapid increase in the deployment of unmanned aerial vehicles (UAVs or drones); but is this at all related to new insights in insect science? In fact, the key advances behind this expansion are largely the result of hardware improvements, communication infrastructure and mainstream engineering solutions, combining to substantially lower the cost and increase the reliabil

The Royal Army Dental Corps today.

The Royal Army Dental Corps (RADC) provides dental care to the British Army both in barracks and on deployment While a significant proportion of most RADC dental officers' careers is spent working in permanent military primary care dental clinics, it is the ability of all RADC dental officers to provide dental care when deployed that provides unique challenges only seen in the military environment. All RADC dental officers are able to deliver high quality care in temporary facilities. During a career the majority of RADC dental officers will have at least one job in a medical regiment providin

Longitudinal measures of hostility in deployed military personnel.

Increases in anger and hostility are commonly found after military deployment. However, it is unknown how anger and hostility develop over time, and which veterans are more at risk for developing these complaints. Data of 745 veterans one month before deployment to Afghanistan and one, six, twelve and 24 months after deployment were analyzed in a growth model. Growth mixture modeling revealed four classes based on their growth in hostility. Most of the participants belonged to a low-hostile group or a mild-hostile group that remained stable over time. Two smaller groups were identified that di

Incidence and Risk Factors for Tinnitus Among Military Service Members in the Millennium Cohort Study.

PURPOSE: Military personnel may be repeatedly exposed to high-noise environments that may increase tinnitus risk. Previous military research on tinnitus has often utilized small samples of personnel with specific experiences and exposures (e.g., combat deployment), with few examinations leveraging longitudinal data. The purpose of this study was to determine the incidence and associated risk factors for new-onset tinnitus in a large prospective and representative military cohort. METHOD: This study utilized data from the Millennium Cohort Study, the largest and longest running longitudinal he

Examination of the Factor Structure and Correlates of the Perceived Military Healthcare Stressors Scale.

The current study evaluated the factor structure, reliability estimates, correlates, and predictive utility of the Perceived Military Healthcare Stressor Scale (PMHSS) in a sample of active duty military medical personnel (N = 1,131) deployed to Joint Base Balad in Iraq. The sample was composed of an approximately even split of male (51.2%) and female (48.8%) participants who ranged in age from 18 to 60 years. The PMHSS is a 21-item measure that was designed to assess the impact of specific medical stressors that military healthcare providers may encounter while deployed. An exploratory factor

Assessing military veterans for posttraumatic stress disorder: a guide for primary care clinicians.

PURPOSE: To educate primary care providers on the physical effects of posttraumatic stress disorder (PTSD), explain why military veterans are at great risk, and describe how to identify PTSD in primary care clients. DATA SOURCES: Current scientific and psychiatric literature on PTSD. CONCLUSIONS: PTSD is prevalent in the military community because of the frequency and type of trauma seen in the combat zone. With the ongoing military deployments, assessment for the presence of PTSD is increasingly important for comprehensive and high-quality primary care. Clients with trauma histories, such a
