Observer variation in assessing neurophysical signs among patients with head injuries.

As an early phase in the development of a valid and reliable measure of the neurophysical sequelae after head injury, we carried out assessments of interrater agreement on representative items. The purpose of this study was to determine the degree of agreement among expert raters who independently measured neurophysical signs on patients undergoing physical rehabilitation after brain injury. Agreement was described using the index of crude agreement, expected agreement and Kappa. Therapists showed a high degree of agreement on those items forming part of a routine neurological assessment: preh

The Efficacy of the Manual Circumlaryngeal Therapy for Muscle Tension Dysphonia: A Systematic Review and Meta-analysis.

OBJECTIVE: Muscle tension dysphonia (MTD) is the most common functional voice disorder. Behavioral voice therapy is the front-line treatment for MTD, and laryngeal manual therapy may be a part of this treatment. The objective of this study was to investigate the effect of manual circumlaryngeal therapy (MCT) on acoustic markers of voice quality (jitter, shimmer, and harmonics-to-noise ratio) and vocal function (fundamental frequency) through a systematic review with meta-analysis. DATA SOURCES: Four databases were searched from inception to December 2022, and a manual search was performed. R

The effect of breathing exercises combined with manual therapy on muscle tension dysphonia in traditional singers: a blinded randomized controlled trial.

BACKGROUND: Muscle tension dysphonia (MTD) is common in traditional singers and can lead to serious complications, but there is inconclusive evidence about how MTD should be treated in singers as professional voice users. This study was conducted to investigate the effects of breathing exercises combined with manual therapy versus breathing exercises and manual therapy on voice quality in traditional singers suffering from MTD. METHODS: In this blinded randomized clinical trial, 60 patients with MTD were randomly allocated to four groups: (1) breathing exercises, (2) manual therapy, (3) combi

Effects and Differences of Voice Therapy on Spasmodic Dysphonia and Muscle tension dysphonia: A Retrospective Pilot Study.

OBJECTIVES: Laryngeal and voice findings of adductor spasmodic dysphonia (AdSD) and muscle tension dysphonia (MTD) are similar, and it is difficult to distinguish between both disorders. The purpose of this study is to ascertain the effect of voice therapy on MTD and AdSD and clarify their difference. METHODS: A total of 49 patients, including 22 patients with MTD (MTD group) and 27 patients with AdSD (SD group), were included in the study. The MTD scores were evaluated, and aerodynamic analysis (maximum phonation time [MPT], mean airflow rate [MFR], highest pitch, lowest pitch, and pitch ran

[Neurological examination of the elderly. Principle modifications and most-frequent abnormalities].

This article will review age-related changes in the neurological examination. The involutional process is a diffuse one, affecting all levels of the neuraxis simultaneously. Most of the numerous little manifestations distinguishing them so sharply from the youthful are really features of the neurology of the elderly. Some signs are characteristic: attitude of general flexion, general poverty of movement, gait, minor extrapyramidal signs, primitive reflexes. However, these features may also be caused by neurological diseases common in the elderly, such as dementia or cervical myelopathy, which

The effects of paraoesophageal structures and vagotomy on the canine lower oesophageal sphincter function.

The effects of paraoesophageal structures and vagotomy on lower oesophageal sphincter (LOS) function were studied in the dog using modern oesophageal manometric technique. The isolation of the gastro-oesophageal junction from surrounding structures by a stiff-walled Silastic-tube led to a slight decrease in LOS pressure, but the difference was not statistically significant. LOS response to abdominal compression also decreased slightly, but the difference was without statistical significance. Bilateral transabdominal, transthoracic and cervical vagotomy did not produce any marked changes in res

[Myoelectrical changes of activity of the Vater's ampulla after various forms of vagotomy in the dog].

The effects of varying the extent of vagotomy on the myoelectrical activity of the sphincter of Oddi have been successfully studied in 12 dogs undergoing either truncal, selective, or highly selective vagotomy. Both the amplitude and the shape of the single action potentials as well as the interval change in the electrical activity in the sphincter of Oddi in relation to the extent of the vagal denervation. No significant change of frequency and amplitude of the myoelectrical activity occurred after highly selective vagotomy which proves to be the best method.

[Surgical management of the adult spastic hand].

The adult spastic hand, of varying causes, but dominated by vascular hemiplegia and brain damage, associates motor disorders and problems of tonus. The variety of forms of brain damage explains the wealth and diversity of the symptoms. These symptoms, often the most serious along with cognitive disorders, justify the expression "central neurological hand". Each case is an individual one. The effect on the hands may be unilateral or bilateral with spasticity involving the fingers/thumb/wrist. The clinical evaluation leading to a decision tree must take into account spasticity, retraction and pa

Contractile performance of papillary muscles of renovascular hypertensive and isoproterenol-pretreated rats.

Previous studies suggested that left ventricle papillary muscles from neurogenic hypertensive rats with 15 days of sino-aortic denervation (SAD) develop intense depression of their inotropic state. Considering that these animals have an increased cardiac sympathetic tonus that could produce the observed depression of the contractile response we did the present study simulating a cardiac sympathetic hyperactivity pretreating rats, during 15 days, with isoproterenol (IPA, 0.1 mg/kg, i.p., divided in 3 daily doses). Because SAD rats also develop hypertension, the effect of blood pressure overload

Clinical Evaluation of Pelvic Floor Muscles Versus Instrumental Assessment with Myotonometry: A Diagnostic Validity Study.

Pelvic floor evaluation is essential for assessing its mechanical and elastic properties to guide treatment decisions. The lack of objective instrumental tools creates challenges in both clinical practice and research. This study aimed to evaluate the diagnostic validity of subjective palpation for classifying pelvic floor muscle tone in comparison to biomechanical parameters obtained with myotonometry in healthy young women. A secondary aim was to explore the consistency of measurements between a novice and an experienced examiner. An observational study was conducted in the laboratories of t

Does More Compression Mean More Pressure? A New Classification for Muscle Tension Dysphonia.

Objective The aims of this study were to test the hypothesis that greater supraglottic compression (anteroposterior or lateral) correlates with higher subglottic pressure (SGP) and to develop a classification of muscle tension dysphonia (MTD), based on the degree of supraglottic compression during speech. Method A prospective, cross-sectional study was conducted in a series of 37 consecutive patients diagnosed with MTD with an altered aerodynamic profile characterized by high SGP (more than 90 mmH2O). Supraglottic anteroposterior and lateral compression were categorized in three grades and ass

A Study of Vocal Facilitating Techniques Compared to Manual Circumlaryngeal Therapy in Teachers With Muscle Tension Dysphonia.

OBJECTIVE: There are several voice therapy techniques been suggested in patients with muscle tension dysphonia (MTD), however a few studies are compared between them. The purpose of present study was to compare the effect of vocal facilitating techniques (VFTs) and manual circumlaryngeal therapy (MCT) in teachers with MTD. STUDY DESIGN: This is a randomized clinical trial study. METHODS: Sixteen teachers with MTD (female; mean age: 38.6 ± 4.6 years) participated in this study. The participants allocated to two treatment groups: the first group received VFTs (n: 8) and MCT was presented to th

Self-Assessment of Vocal Fatigue in Muscle Tension Dysphonia and Vocal Nodules: A Preliminary Analysis of the Discriminatory Potential of the Croatian Adaptation of the Vocal Fatigue Index (VFI-C).

In this paper the authors hypothesize that the intensity of subjective experience of vocal fatigue (VF) potentially reflects primary (hyper) functional vocal disorders (primary muscle tension dysphonia - MTD) differently than structural vocal disorders of functional etiology (MTD type 2a). Given the reported benefit of self-evaluation in the quantification of VF, the goal of this study was to test the potential of the Croatian adaptation of the Vocal Fatigue Index questionnaire (VFI-C) in order to show different VF intensities for the two above-mentioned diagnoses of vocal disorders. In total,

Gastric electrical stimulation reduces gastric tone energy dependently.

OBJECTIVE: Impaired gastric accommodation is one of the major pathophysiological factors in functional dyspepsia (FD). Gastrointestinal electric stimulation (GES) as a potential therapy for treating patients with motility disorders has been applied in animal models and clinical practice. However, the response of proximal gastric tone to different GES has not been fully understood. Therefore, the aims of this study were to investigate the effects of GES with different parameters on gastric tone and to estimate the relationship between gastric volume, stimulation energy, and symptoms. Coronary vasomotor tone: a heterogeneous entity.

Changes in coronary vasomotor tone are undoubtedly involved in the pathogenesis of many ischaemic coronary syndromes. The term coronary artery 'spasm' should be limited to the situations characterized by focal coronary constriction sufficient to cause transient total or sub-total coronary occlusion. Mild coronary artery constriction is also observed in many other situations, without even causing coronary artery occlusion. We present suggestive evidence that the mechanisms underlying coronary artery 'spasm' and 'physiological' coronary artery constriction are different.

Neurophysiology of the cerebellum and clinical correlations: a review.

The cerebellum is a complex structure tightly connected to the cerebral cortex, brainstem, and spinal cord. It plays an important role in movement coordination and motor planning. Lately, it has been also recognized as a key component in cognitive circuits. The specific motor functions of the cerebellum include the control of the initiation, execution, and velocity of movements, as well as the maintenance of balance, motor coordination, and muscle tonus. Cerebellar lesions typically result in ataxia but can also lead to other manifestations, such as abnormal eye movements, severe vertigo, or i
