Long-term follow-up circular myotomy for esophageal atresia.

Between January 1976 and March 1979, esophageal atresia was successfully repaired in six infants with circular myotomy (Livaditis technique) and end-to-end anastomosis. Children were 1 day-6 mo in age and weighed 2.4 to 8.0 kg at the time of repair. Four children required one and two children required two circular myotomies in the proximal esophagus to bridge gaps between esophageal ends, which varied from 1.2-3.0 cm. Follow-up has been 1-4 yr (mean-3 yr). Operative complications included: opening of the mucosa at myotomy (2) and anastomotic leak (2). Subsequently, three children developed ana

Reflux after peroral endoscopic myotomy: The dilemma and the options.

Per oral endoscopic myotomy (POEM) is rapidly emerging as the treatment of choice for achalasia cardia, but its success is marred by problematic reflux. Although symptomatic reflux rates are low and often comparable to that after laparoscopic Hellers myotomy (LHM), a high incidence of pathologic reflux has been noted after POEM. This poses a dilemma as to what is true reflux, and in determining the indications and optimal endpoints for managing post-POEM reflux. The two pertinent reasons for the difference in reflux rates between LHM and POEM are the variation in length and location of myotomy

Peroral endoscopic myotomy in children with achalasia: A review of the literature.

Achalasia is a rare esophageal motility disorder characterized by the inability of the lower esophageal sphincter (LES) to relax, accompanied by the loss of peristalsis in the esophageal body. Although more prevalent in adults, pediatric achalasia poses unique diagnostic challenges due to its atypical presentation. Peroral Endoscopic Myotomy (POEM) has emerged as a minimally invasive alternative to laparoscopic Heller myotomy for managing achalasia, particularly in children, but its efficacy and safety remain under-investigated. A comprehensive literature review was conducted to assess the rol

Efficacy and safety of peroral endoscopic myotomy for esophageal diverticula.

Background and study aims  Epiphrenic diverticula are rare and mainly occur in patients with underlying esophageal motility disorders. The current standard treatment is surgical diverticulectomy often combined by myotomy and is associated with significant adverse event (AE) rates. The aim of this study was to examine the efficacy and safety of peroral endoscopic myotomy in reducing esophageal symptoms in patients with esophageal diverticula. Patients and methods  We performed a retrospective cohort study including patients with an esophageal diverticulum who underwent POEM between October 2014

A comparative study of robotic and laparoscopic approaches to Heller myotomy.

OBJECTIVE: Minimally invasive Heller myotomy for achalasia is commonly performed laparoscopically, but recently done with robotic assistance. We compare outcomes of the 2 approaches. METHODS: From January 2010 to January 2020, 447 patients underwent Heller myotomy with anterior fundoplication (170 with robotic assistance and 277 laparoscopically). End points included short-term and longitudinal esophageal emptying according to timed barium esophagram, symptom relief according to Eckardt score, and time-related reintervention. Normal esophageal morphology, present in 328 patients, was defined

Per oral endoscopic myotomy in a pediatric patient with achalasia.

INTRODUCTION: Achalasia is the most common primary motor disorder of the esophagus. Its reported incidence is low, even more in pediatric patients. Laparoscopic Heller myotomy is the current stan dard of treatment. During the last years, per-oral endoscopic myotomy (POEM) has been positioned as a safe and effective therapeutic alternative as the Heller procedure for esophageal achalasia. Ob jective: To describe the POEM technique and report the first pediatric case in our country. CLINICAL CASE: 11-year-old patient, previously healthy, who presented with progressive dysphagia for solids and l

A novel strategy to initiate a peroral endoscopic myotomy program.

OBJECTIVES: The standard of care for achalasia remains laparoscopic Heller myotomy with partial fundoplication. Peroral endoscopic myotomy (POEM) has been introduced as an alternative, but safety and long-term comparative efficacy are not yet established. We report our experience in developing a POEM program using a novel hybrid approach. METHODS: We developed a hybrid approach to POEM with a POEM followed by laparoscopic evaluation, extension of the myotomy, if necessary, and partial fundoplication. We reviewed the results of the programme from April 2012 until May 2015. Starting in 2014, we

Comprehensive Evaluation of the Learning Curve for Peroral Endoscopic Myotomy.

BACKGROUND & AIMS: Peroral endoscopic myotomy (POEM) is a minimally invasive endoscopic surgical procedure that is effective in treatment for spastic esophageal motility disorders. However, little is known about the learning curve for endoscopists. We aimed to evaluate the effects of various factors on the POEM learning curve. METHODS: We performed a retrospective study of 1346 patients who underwent POEM for achalasia at Shanghai Zhongshan Hospital in China from August 2010 through July 2015. We used risk-adjusted cumulative sum and moving average methods to evaluate outcomes. The primary ou

The role of Heller myotomy and POEM for nonachalasia motility disorders.

The best-defined primary esophageal motor disorder is achalasia. However, symptoms such as dysphagia, regurgitation and chest pain can be caused by other esophageal motility disorders such as Diffuse Esophageal Spasm (DES), Nutcracker Esophagus (NE) and the Hypertensive Lower Esophageal Sphincter (HTN-LES). Most patients with DES and HTN-LES who complain of dysphagia improve after a myotomy. Patients with NE whose main complaint is chest pain, often do not have relief of the pain and can even develop dysphagia as a consequence of the myotomy. POEM is a relatively new procedure, and there are n

The potential and perspective of peroral endoscopic esophageal myotomy for achalasia.

The therapeutic spectrum of this disorder consists of medical therapy, endoscopic balloon dilatation, endoscopic Botox injection, open or laparoscopic cardia myotomy, and most recently transesophageal endoscopic myotomy (POEM peroral endoscopic myotomy). The most important requirement is a well-experienced team in interventional flexible endoscopy. The endoscopist as well as the assisting staff should have experience in advanced therapeutic endoscopic techniques and hemostasis to handle all necessary endoscopic instruments such as injection needles, needle knife, triangle knife, coagulation gr

Z-myotomy of the inferior oblique for small incomitant hypertropias.

BACKGROUND: Small hypertropic deviations due to inferior oblique muscle overaction may cause symptomatic diplopia and frustration because incomitant deviations render prism correction unsatisfactory. The several most common surgical options for inferior oblique weakening risk overcorrection of these smaller deviations. In this study we report the results of a "Z-myotomy" of the inferior oblique muscle to correct small incomitant hypertropias due to mildly overacting inferior oblique muscle. METHODS: The medical records of patients who underwent inferior oblique Z-myotomy at a single center fr

EndoFLIP system for the intraoperative evaluation of peroral endoscopic myotomy.

BACKGROUND: Peroral endoscopic myotomy (POEM) has been recently introduced in clinical practice for the treatment of achalasia. The endoluminal functional lumen imaging probe (EndoFLIP) system) uses impedance planimetry for the real-time measurement of the diameter of the oesophago-gastric junction. OBJECTIVE: The aim of this study is to prospectively evaluate the effect of POEM on the oesophago-gastric junction using EndoFLIP. METHODS: All the patients who underwent POEM in a single centre between April and July 2013 were enrolled in the study. EndoFLIP was used intraoperatively, immediatel

Clinical results of thoracoscopic Heller's myotomy in the treatment of achalasia.

OBJECTIVES: Ideal treatment for achalasia permanently eliminates the dysfunctional lower oesophageal sphincter, relieving dysphagia and regurgitation. The aim of this study was to review the results in a series of patients undergoing video-imaged thoracoscopic Heller's myotomy (THM). METHODS: Records of all patients undergoing THM by a single surgeon at one institution were analysed. Follow-up was conducted using a structured questionnaire together with oesophageal manometry and/or 24 h pH monitoring when clinically indicated. RESULTS: Twenty-five consecutive patients (13 males, 12 females,

[Long-term outcome of myotomy and semi-fundoplication in achalasia].

The basic principle behind the treatment of achalasia consists of alleviating swallowing disorders by reducing resistance in the lower esophageal sphincter without inducing gastroesophageal reflux. Only a few studies are available on long-term results after operative treatment. Fifty-one patients were studied with regard to long-term results after open transabdominal extramucosal myotomy of the distal esophagus along with partial anterior fundoplication (Dor procedure). Clinical data were collected by standardized interviews, and symptoms were assigned a score ranging from 0 to 3 according to

Application of the principles of myotomy and strictureplasty for treatment of esophageal strictures.

BACKGROUND/PURPOSE: Strictures of the esophagus in the pediatric population may be congenital or acquired. Regardless of the etiology, the treatment goal is to relieve the symptoms and allow patients to eat normally. The cornerstone of nonoperative management is repeated esophageal dilatations. However, when nonoperative management fails, operative intervention becomes necessary. In this report the authors present 4 cases of severe upper esophageal strictures managed by applying the principles of myotomy and strictureplasty. METHODS: Retrospective review of 4 patients undergoing esophageal st

[Treatment of achalasia of the cardia using thoracoscopic esophago-myotomy].

We performed left thoracoscopic esophagomyotomy in four patients suffering from achalasia cardiac. In one patients the esophageal mucosa was opened during the myotomy. This was sutured through thoracoscopy and the per os feeding of the patient started on the 8th postoperative day, while in the other three patients this was done on the 2nd postoperative day. The mean emission of the patients was on the 6th postoperative day. Comparison of the status before and 6 weeks following the operation was done on the basis of results of different examinations i. e. x-ray, esophago-gastroscopy, manometry,

Evaluation of the carbon dioxide laser for performance of gastrointestinal myotomies.

Using a conventional carbon dioxide laser set at 8-W continuous-wave energy, a technique was developed in the animal laboratory to incise only the muscularis of the pylorus leaving the mucosa and submucosa intact. Similar esophageal myotomies were performed with less success, due to the rich submucosal venous plexus causing obscuring hemorrhage and to the reapproximation of the longitudinal esophageal muscle over a 3-week period. The technique for esophageal myotomy requires further refinement, but pyloromyotomy appears applicable for controlled human trials.
