Unwitnessed out-of-hospital cardiac arrest: is resuscitation worthwhile?

STUDY OBJECTIVE: To determine the epidemiology of unwitnessed out-of-hospital cardiac arrest and the factors associated with survival after resuscitation using the Utstein style data collection. METHODS: We conducted a prospective cohort study in a 525,000-population city served by a single EMS system comprising a tiered response with physicians in the field. We studied consecutive unwitnessed out-of-hospital cardiac arrests that occurred between January 1, 1994, and December 31, 1995. We determined survival from cardiac arrest to discharge from hospital and the factors associated with surviv

Survival from profound metabolic acidosis due to hypovolaemic shock. A world record?

This case describes the unexpected survival of an adult man who presented to the emergency department with hypovolaemic shock secondary to a splenic haemorrhage. Before surgery he had a pH 6.527, base excess (BE) -34.2 mmol/l and lactate 15.6 mmol/l. He underwent a splenectomy after which his condition stabilised. He was managed in the intensive care unit postoperatively where he required organ support including renal replacement therapy but was subsequently discharged home with no neurological or renal deficit. Although there are case reports of patients surviving such profound metabolic acid

Extended therapeutic hypothermia for several days during extracorporeal membrane-oxygenation after drowning and cardiac arrest Two cases of survival with no neurological sequelae.

Drowning associated with hypothermia and cardiopulmonary resuscitation has a very poor prognosis. We report two such cases, where impossible oxygenation due to severe pulmonary oedema was treated with extracorporeal membrane-oxygenation (ECMO). Following cardiac arrest, mild therapeutic hypothermia for 24h was maintained as recommended, but subsequent rewarming precipitated additional pulmonary oedema. Little is currently known about how long to maintain therapeutic hypothermia to optimize neurological outcome and suppress reperfusion injury. In our patients, therapeutic hypothermia during ven

Pseudallescheria boydii infection of the central nervous system.

Pseudallescheria boydii is a rare cause of central nervous system infection characteristically presenting as a neutrophilic meningitis or multiple brain abscesses. Factors predisposing to central nervous system infection with this fungus include immunosuppression and near drowning. The organism is infrequently cultured from fluid obtained by lumbar puncture, delaying clinical recognition and appropriate antifungal therapy. All untreated patients with P boydii infection of the central nervous system died. We describe a patient who developed a persistent neutrophilic meningitis with focal neurol

Etiology of Sudden Cardiac Arrest in Patients with Epilepsy: Experience of Tertiary Referral Hospital in Sapporo City, Japan.

It has been reported that epilepsy patients had higher risk of sudden death than that of the general population. However, in Japan, there is very little literature on the observational research conducted on sudden fatal events in epilepsy. We performed a single-center, retrospective study on all the out-of-hospital cardiac arrest (OHCA) patients treated in our emergency department between 2007 and 2013. Among the OHCA patients, we extracted those with a history of epilepsy and then analyzed the characteristics of the fatal events and the background of epilepsy. From 1,823 OHCA patients, a tota

Molecular determination of infection source of a sporadic Legionella pneumonia case associated with a hot spring bath.

To determine the infection source of a sporadic Legionella pneumonia case associated with a hot spring bath, we used five molecular methods, including repetitive element polymerase chain reaction (rep-PCR), arbitrarily primed PCR (AP-PCR), ribotyping, restriction endonuclease analysis (REA), and macrorestriction endonuclease analysis (MREA) by pulsed-field gel electrophoresis. L. pneumophila serogroup (SG) 3 strain EY 3702, isolated from an intratracheal specimen of a 71-year-old Japanese female who developed pneumonia after nearly drowning in a hot spring spa bath, produced rep-PCR and AP-PCR

Some prehistory of New Zealand intensive care medicine.

In taking 1960 as the foundation year for the practice of intensive care medicine in New Zealand, this paper briefly looks into the previous two centuries for some interventions in life-threatening conditions. With the help of descriptions in early 19th century journals and books by perceptive observers, the author focuses on some beliefs and practices of the Maori people during pre-European and later times, as well as aspects of medical treatment in New Zealand for early settlers and their descendents. Dr Laurie Gluckman's book Tangiwai has proved a valuable resource for New Zealand's medical

Fatal drowning as a result of an airplane crash--case report.

An ULM (Ultra Light Motorized) plane Aeroprakt 22-L with two men on board dived unexpectedly into the Dworackie lake near the town of Olecko in Warmia and Mazury district, Northeastern Poland. The pilot and the passenger had multiorgan blunt injuries, including subdural and subarachnoid hemorrhage, fractured ribs, fractured thigh (in one of the deceased) and pulmonary contusions as a result of the impact with the lake. The multiorgan injuries and unconsciousness resulting from head trauma prevented the casualties from saving themselves. The underlying cause of the death was the crash into the

[Pediatric emergencies].

Management of acute emergencies in children has a number of specific aspects, with which the first person providing medical care needs to be familiar. For example, intubation requires knowledge of the particular anatomy of infants and young children. In addition, a specific and flexible approach to such matters as oxygen requirement, blood volumes, thermoregulation and the venous system is essential. Administration of adrenaline should initially be via inhalation, since this can be done rapidly and easily. Among the causes of cardiac arrest, sudden infant death predominates in babies, while in

Vomiting is not associated with poor outcomes in pediatric victims of unintentional submersions.

INTRODUCTION: The outcome of submersion victims depends on submersion duration and the availability of timely and effective resuscitation. The prognostic implication of vomiting during resuscitation of submersion victims is unclear. The study sought to determine whether vomiting during resuscitation in children treated for unintentional submersion injuries adversely impacts outcome. METHODS: This was a retrospective study of unintentional submersion victims under age 18 treated at an urban tertiary-care children's hospital from 2003-2009. Submersion and victim details were obtained from hospi

McArdle's disease in childhood: report of a new case.

McArdle's disease (glycogenosis type V) is an inherited glycogen storage disease characterized clinically by myalgia, cramps and sometimes myoglobinuria, triggered by exercise. The onset of exercise intolerance is usually in late childhood or adolescence and diagnosis is exceptionally established during infancy. We report the case of a 6-year-old girl who had been complaining of aching muscles for a long time, and who presented after a near-drowning incident, with extensive muscle necrosis, probably secondary to myophosphorylase deficiency-induced cramps. These unusual manifestations led to th

Heimlich manoeuvre: adjunctive emergency procedure to relieve choking and asphyxia.

Choking on aspirated food or a foreign body (i.e. meat, mushroom, coin, chewing gum and a balloon) is a common cause of laryngeal obstruction, particularly in those persons who are intoxicated by alcohol or who have bulbar palsy (degeneration of motor neurons in the brain stem nuclei of the glossopharyngeal and vagal nerve). The rima glottis in the larynx is an important site where aspirated food or material becomes lodged, thereby causing laryngeal obstruction (choking). Because the lungs still contain air, intentional compression thrusts to the abdomen (Heimlich manoeuvre) will theoretically

Hypothermia and poikilothermia induced by a kappa-agonist opioid and a neuroleptic.

When an opioid acting selectively at the kappa opioid receptor is administered subcutaneously to rats along with a neuroleptic at an ambient temperature of 20 degrees C a marked hypothermia ensues. The combination of U-50,488H (a kappa agonist) and chlorpromazine (a neuroleptic) caused a drop in body temperature amounting to as much as 11 degrees C, with all animals recovering after 24-48 h. Naloxone partially reversed the hypothermia. Similar, but less dramatic, decreases in body temperature occurred with other neuroleptics and weaker kappa agonists. The induction of poikilothermia was indica

Radionuclide imaging in the assessment of lung injury.

Nuclear medicine imaging techniques are being more widely applied to study a variety of lung injuries. Ventilation and perfusion imaging is often superior to other roentgenographic techniques for establishing the diagnosis and demonstrating the extent of such injuries. At several large burn centers, the Xe-133 washout technique has become the cornerstone for diagnosing early inhalation injury. The overall accuracy of this procedure is 92%. Regional decreases in ventilation and perfusion can reliably localize aspirated radiolucent foreign bodies. Disproportionate perfusion changes are often the

Critical Hypophosphatemia in a Special Operations Combat Dive Candidate: A Case Report.

In contrast to shallow water (hypoxic) blackout and swimming-induced pulmonary edema (SIPE), acute electrolyte disturbance secondary to acute respiratory alkalosis is not considered a common Combat Swimmer injury but has the potential to be life-threatening. We present the case of a 28-year-old Special Operations Dive Candidate who presented to the Emergency Department after a near-drowning incident with altered mental status, generalized weakness, respiratory distress, and tetany. He was found to have severe symptomatic hypophosphatemia (1.00mg/dL) and mild hypocalcemia secondary to intention

Unusual presentation of sand aspiration in a 14-mo-old child.

Accidental sand aspiration is an unusual but potentially life-threatening condition in children. Sand aspiration often occurs in cases of near drowning and accidental burial in sand. The authors report a case of accidental sand aspiration in a 14-mo-old girl while she was playing with sand. Though child was symptomatic and chest radiograph was showing obstructive emphysema initially, there was spontaneous resolution of radiographic findings and clinical condition within 48 h. Child was treated conservatively and was discharged. Follow up examination after 2 mo revealed persistent wheeze but no

Acquired heterotopic ossification in the settings of cerebral anoxia and alternative therapy: two cases.

Acquired Heterotopic Ossification (HO) has been well described in the literature as a recognized complication following spinal cord injury, traumatic brain injury and joint arthroplasty. Commonly, large proximal limb joints are affected. The underlying mechanisms for ectopic bone formation remain poorly elucidated. Post-stroke hemiplegia as a cause of neurogenic HO is rare, and no published reports of HO occurring after anoxic brain injury in adults have been documented. This study reports two unusual cases of acquired HO: (1) Polyarticular HO involving the ankle joint in a 24-year-old Chinese

[Malignant Transformation of Cerebellopontine Angle Epidermoid Cyst:A Case Report].

Intracranial epidermoid cysts are benign cystic lesions that typically exhibit slow growth. Their malignant transformation into squamous cell carcinoma is rare. We report a 77-year-old woman who was admitted to our hospital because of a near-drowning incident due to a seizure sustained in her bathtub. Magnetic resonance imaging(MRI)revealed an extra-axial tumor occupying the right cerebellopontine angle. The lesion appeared hyperintense in diffusion-weighted images and exhibited contrast enhancement after gadolinium injection. Cerebrospinal fluid examination revealed noninfectious meningitis,
