Examining the symptom-level specificity of negative problem orientation in a clinical sample.

Given the equivocal state of the literature as to the symptom-level specificity of the cognitive variable labeled negative problem orientation (NPO), we targeted NPO-symptom relations. A clinical sample (N = 132) of adults diagnosed with an anxiety disorder, mood disorder, or obsessive-compulsive disorder completed self-reports of NPO and symptom types (worry, depression, obsessive-compulsive, panic, and social anxiety). Symptom-level specificity was examined using a combination of zero-order and regression analyses that controlled for the other assessed symptoms. Results were that NPO shared

Daily mood patterns and bulimic behaviors in the natural environment.

OBJECTIVE: Negative affect has been purported to play an important role in the etiology and maintenance of bulimic behaviors. The objective of this study was to identify daily mood patterns in the natural environment exhibited by individuals with bulimia nervosa and to examine the relationship between these patterns and bulimic behaviors. METHOD: One hundred thirty-three women aged 18-55 meeting DSM-IV criteria for bulimia nervosa were recruited through clinical referrals and community advertisements. Ecological momentary assessment was used to collect multiple ratings of negative affect, bin

[Catatonia - About four clinical cases and literature review].

INTRODUCTION: Catatonia is a neuropsychiatric syndrome characterized by the presence of at least three out of the twelve following signs: stupor, catalepsy, waxy flexibility, mutism, negativism, posturing, mannerisms, stereotypies, agitation, grimacing, echolalia, and echopraxia. We will describe four cases of catatonia that occurred over one year in a single medicine department. CLINICAL CASES: All four patients were over 60 years old. The causes of catatonia were diverse: hemorrhagic shock occurring in the context of mixed dementia, schizoaffective disorder, a severe major depressive episod

Catatonia in Ugandan children with nodding syndrome and effects of treatment with lorazepam: a pilot study.

BACKGROUND: Nodding syndrome (NS) is a severe neuropsychiatric syndrome of an unknown etiology affecting children and adolescents mostly in Eastern Africa. Symptoms of NS and catatonia seem to overlap. We investigated the presence and types of catatonic symptoms in NS and their response to one or two doses of lorazepam, the first-line treatment for catatonia. METHODS: A cross-sectional descriptive study with systematic assessment of catatonia in 33 patients with NS using a modified version of the Bush Francis Catatonia Rating Scale. Sixteen patients met criteria for catatonia and were observe

The response of agitated behavior to pain management in persons with dementia.

OBJECTIVES: Behavioral disturbances and pain are common in nursing home (NH) patients with dementia. An association between pain and increased agitation has been suggested, and recently a significant reduction of agitation has been demonstrated by pain treatment in patients with moderate to severe dementia. We now examined which specific agitated behaviors respond to individualized pain treatment. DESIGN: Cluster randomized clinical trial. SETTING: 60 clusters (i.e., clusters defined as single independent NH units) in 18 NHs within five municipalities of Western Norway. PARTICIPANTS: 352 pa

Association between low-activity serotonin transporter genotype and heroin dependence: behavioral and personality correlates.

In previous studies, serotonin (5-HT) system disturbance was found involved in a variety of behavioral disorders, psychopathologies, and substance use disorders. A functional polymorphism in the promoter region of the human serotonin transporter gene (5-HTTLPR) was recently identified and the presence of the short (S) allele found to be associated with a lower level of expression of the gene, lower levels of 5-HT uptake, type 2 alcoholism, violence and suicidal behavior. In the present study, 101 heroin addicts (males, West European, Caucasians) and 101 healthy control subjects matched for rac

Evidence for a discrete behavioral phenotype in the oculocerebrorenal syndrome of Lowe.

The oculocerebrorenal syndrome of Lowe (OCRL) is an X-linked disorder characterized by congenital cataracts, cognitive impairment, and renal tubular dysfunction. Although there is a wide range of intellectual function in affected individuals, it is often compromised by a high prevalence of maladaptive behaviors, including tantrums, stubborness, and stereotypy. Whether these behaviors simply reflect the multiple disabilities found in some developmentally impaired individuals with or without OCRL, or a specific genetically-determined behavioral phenotype of OCRL, is unknown. Controls were matche

Lithium-Associated Hyperparathyroidism Followed by Catatonia.

OBJECTIVE: To familiarize the medical community with the less common adverse effects of lithium on parathyroid function, we present a case of lithium-associated hyperparathyroidism followed by the development of new-onset catatonia in a patient with schizoaffective disorder. METHODS: To allow for the safe resumption of lithium, the patient received laboratory screening of serum lithium, blood urea nitrogen, serum creatinine, calcium, and thyroid-stimulating hormone levels. The hypercalcemia was evaluated by measuring parathyroid hormone (PTH), ionized calcium, and 25-hydroxy vitamin D levels.

Catatonia as a putative nosological entity: A historical sketch.

Kahlbaum was the first to propose catatonia as a separate disease following the example of general paresis of the insane, which served as a model for establishing a nosological entity. However, Kahlbaum was uncertain about the nosological position of catatonia and considered it a syndrome, or "a temporary stage or a part of a complex picture of various disease forms". Until recently, the issue of catatonia as a separate diagnostic category was not entertained, mainly due to a misinterpretation of Kraepelin's influential views on catatonia as a subtype of schizophrenia. Kraepelin concluded that

Symptom profile and short term outcome of catatonia: an exploratory clinical study.

BACKGROUND: Catatonia is a potentially life-threatening but treatable neuropsychiatric condition. Although considered more common in low income countries, data is particularly sparse in these settings. In this study we explore the symptomatology, treatment, and short-term outcome of catatonia in Ethiopia, a low income country. METHODS: The study was a prospective evaluation of patients admitted with a DSM-IV diagnosis of catatonia. Diagnosis of Catatonia and its severity were further assessed with the Bush-Francis Catatonia Rating Scale (BFCRS). RESULTS: Twenty participants, 5 male and 15 fe

A comparative study of "Idiopathic catatonia" with catatonia in schizophrenia.

Catatonia has been defined as a cluster of signs and occurs secondary to or as a subgroup of schizophrenia, mood disorders or organic syndrome. This study specifically examined the distinct variety of catatonia that did not meet any standard psychiatric diagnostic criteria on globally recognised psychiatric rating tools and compared the clinical features with the catatonia that occurred in association with a diagnosis of schizophrenia. The inpatients in a tertiary psychiatric ward in Ahmedabad, India, between 2002 and 2005 who presented with two or more catatonic signs present for more than 24

Influence of failed vascular bypass procedures on conversion of below-knee to above-knee amputation levels.

From 1975 to 1980, 79 major limb amputations were performed from a group of 552 lower extremity vascular reconstructions with glutaraldehyde-stabilized umbilical vein grafts. All amputations were performed in instances in which limb salvage had been the indication for the attempted bypass (n = 520)--an incidence of 15.2%. Twelve of 51 below-knee (BK) amputations were converted to above-knee (AK) levels (crude failure rate, 23.5%). The highest crude failure rate occurred after failed bypass to the popliteal artery (33.3%); the lowest occurred after failed tibial bypass (16.7%), and the peroneal

The risks for adolescents of negatively biased self-evaluations of social competence: the mediating role of social support.

This study conducted among 544 adolescents (M = 15.1 years, SD = .82) examined whether perceived social support from parents and peers mediated the relationship between biased self-evaluations of social competence and internalizing problems. The results showed negative links between bias in self-evaluation and depressive symptoms, social anxiety and social avoidance. Bias in self-evaluation of social competence was more strongly related to perceived peer support than perceived parental support. Gender differences were observed in the mediating role of social support. Among boys, parental suppo

Do Metacognitions and Intolerance of Uncertainty Predict Worry in Everyday Life? An Ecological Momentary Assessment Study.

Cognitive models of generalized anxiety disorder (GAD) suggest that excessive worry is due to positive and negative metacognitive beliefs and/or intolerance of uncertainty. Empirical support mainly derives from cross-sectional studies with limited conclusiveness, using self-report measures and thereby possibly causing recall biases. The aim of the present study therefore was to examine the power of these cognitive variables to predict levels of worry in everyday life using Ecological Momentary Assessment (EMA). Metacognitions and intolerance of uncertainty were assessed using well-established

Over and over again: rumination, reflection, and promotion goal failure and their interactive effects on depressive symptoms.

Research indicates that examining failure experiences using an immersed processing style versus a non-immersed, self-distanced open style influences cognitions about the self, motivation, and subsequent depressive symptoms. However, the effect of processing goal failure experiences using these different processing styles have not been adequately incorporated into existing self-regulation theories of depression. In a cross-sectional study, we examined the interactive effects of rumination (versus reflection) and failure to attain promotion goals on depressive symptoms. As predicted, greater lev

Continual training of attentional bias in social anxiety.

Using the dot-probe paradigm, it has been shown that high social anxiety is associated with an attentional bias toward negative information. In the present study, individuals with high social anxiety were divided into two groups randomly. One group was the attentional bias training group (Group T), and the other was the control group (Group C). For Group T, 7 days' continuous training of attentional bias was conducted using the dot-probe paradigm to make socially anxious individuals focus more on positive face pictures. The results showed that the training was effective in changing attentional

A prospective test of the hopelessness theory of depression in children.

Providing a developmental extension of the cognitive theories of depression, researchers and theorists have suggested that during early to middle childhood, attributional styles may mediate rather than moderate the association between negative life events and the development of depression. Within the context of the hopelessness theory of depression, we tested this hypothesis in a 6-month longitudinal study of 4th- and 5th-grade children. Using path analysis, we found support for the mediating role of attributional styles among both 4th and 5th graders. Supporting recent refinements in the hope
