Systemic inflammatory response syndrome after percutaneous nephrolithotomy: an assessment of risk factors.

OBJECTIVES: To analyze the risk factors for systemic inflammatory response syndrome (SIRS) after percutaneous nephrolithotomy (PCNL) and to quantitatively predict the probability of SIRS after PCNL. METHODS: Medical records on 209 patients who underwent PCNL were retrospectively analyzed. The chi2 test, the t-test and a logistic regression model were used to identify key risk factors of SIRS after PCNL. A predictive equation was then formulated to assess the risk of SIRS according to the results from the logistic model. Subsequently, the accuracy of the equation by calculating sensitivity, sp

Simultaneous bilateral multipuncture tubeless percutaneous nephrolithotomy in patient with orthotopic bladder substitution.

We report the successful treatment of a patient that presented with bladder tumor, bilateral multiple renal stones, right lower ureteral stone, and bilateral hydroureteronephrosis with progressively rising serum creatinine. Initially, he was managed by drainage of the upper tract by left percutaneous nephrostomy tube until serum creatinine dropped to normal value then he was managed by radical cystectomy and orthotopic bladder substitution. Three weeks later, he was subjected to simultaneous bilateral multi-puncture tubeless percutaneous nephrolithotomy. In addition, we report on the feasibili

[Percutaneous renal access for percutaneous nephrolithotomy guided by contrast enhanced ultrasound: a single-center preliminary experience in China].

OBJECTIVE: Contrast enhanced ultrasound (CEUS) is an innovative technique that employs microbubble contrast agents to demonstrate parenchymal perfusion. Ultrasound contrast agent was reported to be directly used in human internal lumen to improve the observation capacity of ultrasound. However, CEUS has never been reported to be used in the guidance of percutaneous renal access in percutaneous nephrolithotomy (PCNL). This study aimed to assess the efficacy of CEUS-guided renal access in PCNL. METHODS: In this retrospective study, percutaneous renal access was performed under real-time monitor

Endoscopic Combined Intrarenal Surgery vs Percutaneous Nephrolithotomy: A Systematic Review and Meta-Analysis.

INTRODUCTION: Endoscopic combined intrarenal surgery (ECIRS) has emerged as a significant advancement in the management of patients with a high stone burden or large, complex stones. This technique has been reported to improve the stone-free rate (SFR) while reducing complications commonly associated with percutaneous nephrolithotomy (PCNL). However, most available evidence comes from single-center studies with relatively small cohorts. We conducted a systematic review and meta-analysis to compare perioperative outcomes between the two procedures. METHODS: A systematic search of PubMed, Embas

Laparoscopy-assisted percutaneous nephrolithotomy in a pelvic ectopic kidney: A minimally invasive approach.

Ectopic pelvic kidneys present unique anatomical challenges in the management of nephrolithiasis due to their abnormal position and proximity to surrounding structures. This case report describes a 58-year-old male patient with a 26-mm renal stone in a left pelvic ectopic kidney, successfully treated using a laparoscopy-assisted percutaneous nephrolithotomy (PCNL) approach. The procedure involved initial ureteral catheter placement, laparoscopic mobilisation of the kidney and fluoroscopic-guided percutaneous access for stone fragmentation and removal. Laparoscopic assistance improved visualisa

Safety and efficacy of Pharmacologic thromboprophylaxis following percutaneous nephrolithotomy: a retrospective cohort study.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is the standard treatment for large or complex renal stones; however, it carries risks such as bleeding and venous thromboembolism (VTE). Current guidelines do not recommend routine pharmacologic thromboprophylaxis in these patients due to concerns about bleeding and limited supporting evidence. This study evaluates the safety and efficacy of unfractionated heparin thromboprophylaxis in patients at moderate to high VTE risk undergoing PCNL. METHODS: In this retrospective cohort study, 202 patients undergoing PCNL were assigned to either heparin

Administration of Intravenous Amino Acids Attenuates Postoperative Hypothermia in Patients Undergoing Percutaneous Nephrolithotomy.

OBJECTIVE: Amino acids attenuate hypothermia during the perioperative period by increasing thermogenesis and stimulating energy consumption. Percutaneous nephrolithotomy (PCNL) induces profound hypothermia owing to the use of large volumes of irrigating fluids. The role of amino acids in this group of surgeries for hypothermia and shivering prevention has been evaluated in this study because there is no available literature of concern. METHODS: This prospective randomised controlled trial was conducted in patients undergoing PCNL. Group A received amino acids at 60 mL h-1 an hour before surge

Novel Irrigation Protocol for Renal Pelvis Sterilization During Percutaneous Nephrolithotomy: A Pilot Study.

While percutaneous nephrolithotomy (PCNL) remains the treatment modality of choice for kidney stones larger than 2 cm, infectious complications are most common, ranging from 5% to 32%. We present here a novel technique for potentially improving collecting system sterility during PCNL and initial postoperative outcomes. Retrospective chart analysis data of our irrigation protocol were collected from our first 56 patients between February and July 2019. Traditional prone PCNL was performed in a standard manner using fluoroscopic guidance. Using a ureteral catheter that was placed cystoscopically

Factors predicting postoperative febrile urinary tract infection following percutaneous nephrolithotomy in prepubertal children.

BACKGROUND: Predictive tables and scoring systems can predict stone clearance. However, there is a paucity of evidence regarding the prediction of complications during percutaneous nephrolithotomy (PCNL), particularly in children, which remains under-researched. To our knowledge, no studies have evaluated the risk factors for febrile urinary tract infection (FUTI) after pediatric PCNL. OBJECTIVES: To assess the predictive factors of FUTI in prepubertal children after PCNL and determine whether any prophylactic cephalosporins are superior for decreasing the FUTI rate. STUDY DESIGN: Data from

Percutaneous Nephrolithotomy, Ileal Conduit- Lithotripsy and Litholapaxy for a Neglected Encrusted Ureteral Stent.

Urolithiasis can result as a complication of urinary diversión, favored by urinary stasis, intestinal mucus, urinary tract bacteriuria, the metabolic derangements and the presence of foreign bodies. We present a 52- year-old male who underwent radical cystectomy with construction of a Bricker uretero-ileostomy. 5 years later he was found with a forgotten ureteral stent, a 6 cm calculi occupying the whole ileal conduit and a 13 mm calculi in the left renal pelvis. We present our experience in the successful endourological management of an encrusted neglected ureteral stent in an ileal conduit,

Endoscopic combined intrarenal surgery: From percutaneous nephrolithotomy to the future of stone management.

The surgical management of complex renal calculi has undergone a paradigm shift from traditional prone percutaneous nephrolithotomy (PCNL) to the more versatile supine approach. This evolution paved the way for endoscopic combined intrarenal surgery (ECIRS), a technique that synergizes antegrade and retrograde access into a seamless "four-hand" collaboration. This review explores the historical trajectory and biomechanical advantages of ECIRS, emphasizing its role in overcoming the limitations of PCNL and retrograde intrarenal surgery monotherapies. Central to this approach is the "intermediat
