Neurophysiology of complex spinal cord untethering.

Surgery of complex spinal dysraphisms can be challenging. A number of surgical maneuvers can place the conus and the cauda equina at risk for neurological injury during cord untethering, and the identification of functional neural structures within the lumbosacral region is often not possible solely on the basis of anatomy. Therefore, the assistance of intraoperative neurophysiological monitoring can be invaluable during these procedures. We describe the intraoperative neurophysiological monitoring strategy developed at our institution over the past 12 years when dealing with tethered cord sur

[Focus on the Brain Under Anesthesia: The Safe Brain Initiative's EEG Bootcamp].

In current anaesthetic practice, monitoring remains largely focused on cardiorespiratory parameters, while the brain, the primary target organ of anaesthesia, is insufficiently monitored. EEG-based neuromonitoring enables direct, continuous, and non-invasive evaluation of the cerebral effects of anaesthetics, yet its clinical application is often limited to numerical indices derived from proprietary algorithms. This simplification of a complex biosignal is prone to artifacts, delays, and systematic bias, particularly in elderly and frail patients. We argue that index-guided anaesthesia alone ma

Intraoperative electroencephalographic burst suppression: a narrative review and clinical framework for interpretation and management.

Burst suppression is common during general anaesthesia yet remains one of the most confusing electroencephalography (EEG) patterns for practicing anaesthetists. The literature alternately frames burst suppression as neuroprotective, harmful, or clinically neutral, leaving clinicians uncertain about how to interpret its appearance and respond in real time. This narrative review reframes burst suppression as a context-dependent biomarker of cortical energy balance rather than a uniformly protective or purely iatrogenic phenomenon. From a mechanistic perspective, physiologic burst suppression ref

Future Advances in Neurolaryngology.

Neurolaryngology as a subspecialty of laryngology has developed considerably in the last four decades with more laryngologists, neurologists, speech and swallow therapists, and neurophysiologists taking interest in the field. The North American and Japanese laryngology societies have increasingly focused on conditions which are mainly concerned with aberrations of the nervous system affecting the larynx directly or indirectly. In the last few years, societies in Europe and the Asia-Pacific have also recognized the need to collaborate both within their organizations and with other societies glo

Recommendations on the use of neuromonitoring in thyroid and parathyroid surgery.

INTRODUCTION: Thyroid and parathyroid surgery (TPTS) is associated with risk of injury to the recurrent laryngeal nerve, superior laryngeal nerve and voice changes. Intraoperative neuromonitoring (IONM), intermittent or continuous, evaluates the functional state of the laryngeal nerves and is being increasingly used. This means that points of consensus on the most controversial aspects are necessary. OBJECTIVE: To develop a support document for guidance on the use of IONM in TPTS. METHOD: Work group consensus through systematic review and the Delphi method. RESULTS: Seven sections were iden

2.5D heterogeneously integrated microsystem for high-density neural sensing applications.

Heterogeneously integrated and miniaturized neural sensing microsystems are crucial for brain function investigation. In this paper, a 2.5D heterogeneously integrated bio-sensing microsystem with μ-probes and embedded through-silicon-via (TSVs) is presented for high-density neural sensing applications. This microsystem is composed of μ-probes with embedded TSVs, 4 dies and a silicon interposer. For capturing 16-channel neural signals, a 24 × 24 μ-probe array with embedded TSVs is fabricated on a 5×5 mm(2) chip and bonded on the back side of the interposer. Thus, each channel contains 6 × 6 μ -

Central Hypoventilation Following Pontomedullary Tumor Decompression - Is Routine Brainstem Monitoring Adequate?

Neurophysiological monitoring assesses the functional integrity of the brainstem by using monitoring and mapping techniques. We report an operated case of a pontomedullary lesion in a patient who developed central hypoventilation postoperatively. The intraoperative use of neurophysiological and cardiovascular monitoring was unable to predict/prevent this hypoventilation. We describe the inherent limitations of monitoring the respiratory system, including spontaneous respiration. Moreover, we suggest the novel application of diaphragmatic motor evoked potential for real-time monitoring of respi

[Intraoperative Visual Evoked Potential Monitoring].

Visual evoked potential (VEP) is recorded from the back of the head, which is elicited by retinal stimulation transmitted through optic nerve, optic chiasm, optic tract lateral geniculate body, optic radiation and finally cortical visual area. VEP monitoring did not prevail since 1990s because marked intra-individual difference and instability of VEP recording limited the clinical usefulness under inhalation anesthetic management and techniques of VEP monitoring at the time. However, recent advances in techniques including a new light-stimulating device consisting of high-luminosity LEDs and i

Advances in Multimodality Monitoring in Traumatic Brain Injury.

PURPOSE OF REVIEW: Multimodal monitoring (MMM) has the potential to transform the management of secondary brain injury following traumatic brain injury (TBI) to enable more personalized care. Despite its promise, the optimal combination of monitoring modalities and the interpretation of complex data streams remain insufficiently mature for MMM to be considered standard of care. This review evaluates the current state of MMM technologies, their clinical applications, and their emerging role in improving TBI management and guiding the path toward broader clinical adoption. This review focuses on

Intraoperative electroencephalogram-derived measures and their association with postoperative delirium: a systematic review and meta-analysis.

BACKGROUND: Postoperative delirium (POD) is a common and serious complication in older surgical populations. Intraoperative EEG may help identify those at risk. Proprietary processed EEG (pEEG) devices were not developed to identify POD risk and studies utilising such devices may confound attempts to identify non-proprietary EEG-derived measures associated with POD. We performed a systematic review of non-proprietary intraoperative EEG-derived measures and their association with POD including a secondary analysis excluding studies that utilised pEEG-guided anaesthesia. METHODS: This review fo

Fast or Slow? A Comparison Between Two Transcranial Electrical Stimulation Techniques for Eliciting Motor-Evoked Potentials During Supratentorial Surgery.

PURPOSE: During intraoperative neurophysiological monitoring of motor pathways, two types of transcranial electrical stimulation are available, i.e., constant-current and constant-voltage stimulation. Few previous studies, performed only during spinal surgery, analyzed and compared them during intraoperative neurophysiological monitoring. The aim of our study was to compare these two stimulation techniques for eliciting motor-evoked potentials during intraoperative neurophysiological monitoring in a group of patients affected by supratentorial lesions. METHODS: Supratentorial lesions from 16

Advanced perioperative assessment of neurological function in acute Stanford A aortic dissection.

OBJECTIVE: Acute Stanford Type A aortic dissection (AAAD) is a critical condition in vascular surgery, and total aortic arch replacement surgery is the preferred method to save patients' lives. In recent years, as clinical research has advanced, there has been a growing realization of the close association between poor postoperative outcomes in patients and neurological functional deficits. Neurological function monitoring is a medical technique used to evaluate and monitor the functional status of the nervous system. METHODS: This monitoring involves the assessment of various aspects of the

A Bibliometric Analysis of Intraoperative Neuromonitoring in Spine Surgery.

Intraoperative neuromonitoring (IONM) techniques are usually implemented during spine surgery to avoid nefarious abuse of the nervous system, which can cause postoperative problems. A lack of bibliometric analysis on the topic of IONM in spine surgery has been identified. Therefore, the aims of this study are to provide information about the main contributors to this field and their publication dynamics, as well as conceptual and cooperative networks. Results have shown that a steady publication increase has been occurring since 1991, with high levels of citations in the first decade, but irre

Is There Any Benefit of Neuromonitoring During Descending and Thoracoabdominal Aortic Aneurysm Repair?

OBJECTIVE: Paraplegia remains the most feared and a devastating complication after descending and thoracoabdominal aneurysm operative repair (DTA and TAAAR). Neuromonitoring, particularly use of motor-evoked potentials (MEPs), for this surgery has gained popularity. However, ambiguity remains regarding its use and benefit. We systematically reviewed the literature to assess the benefit and applicability of neuromonitoring in DTA and TAAAR. METHODS: Electronic searches were performed on 4 major databases from inception until February 2014 to identify relevant studies. Eligibility decisions, me

Clinical applications of cerebral function monitoring in neonates.

The cerebral function monitor is a device for trend monitoring of changes in the amplitude of the electroencephalogram, typically recorded from one or two pairs of electrodes. Initially developed and introduced to monitor cerebral activity in encephalopathic adult patients or during anaesthesia, it is now most widely used in newborns to assess the severity of encephalopathy and for determining prognosis. The duration and severity of abnormalities of the amplitude-integrated electroencephalogram tracing is highly predictive of subsequent neurologic outcome following neonatal hypoxic-ischemic en

Clinical neurophysiology of cranial nerve disorders.

Electrophysiologic techniques are available to measure many of the cranial nerves. The procedures can be done using equipment available in standard clinical neurophysiology laboratories. These studies can aid in localization of cranial nerve lesions as well help identify the underlying pathology and possibly aid in prognosis. The trigeminal pathways can be measured using the blink and masseter responses. The facial nerve is measured by the blink response and by direct facial stimulation; techniques such as lateral spread can identify specific abnormalities. The spinal accessory nerve is measur
