Melanocytic nevi simulant of melanoma with medicolegal relevance.

A group of melanocytic benign nevi are prone to be misdiagnosed as nodular or superficial spreading melanoma. This review illustrates the most frequent forms of these nevi in direct comparison with their malignant morphologic counterparts. The nevi are: hyper-cellular form of common nevus to be distinguished from nevoid melanoma, Spitz nevus (vs spitzoid melanoma), Reed nevus (vs melanoma with features of Reed nevus), cellular atypical blue nevus (vs melanoma on blue nevus), acral nevus (vs acral melanoma), Clark dysplastic nevus (vs superficial spreading melanoma), desmoplastic nevi (vs desmo

A Melanocytic Lesion Extending From the Right Ear to the Nasopharynx in a Pediatric Patient: A Case Report.

BACKGROUND: Blue nevus is a benign dermal melanocyte tumor that mainly arises from the skin. We report an extremely rare case of blue nevus in a pediatric patient with extensive progression from the middle ear and inner ear to the nasopharynx through the Eustachian tube. CASE REPORT: A 2-year-old girl with blue tympanum was referred to our department. Computed tomography scans and magnetic resonance imaging were performed, followed by a tissue biopsy and histopathologic evaluations. Radiologic examinations revealed that the lesion had progressed beyond the middle ear into the inner ear and th

Oral melanocytic nevi: a clinicopathologic study of 100 cases.

OBJECTIVE: The purpose of this study was to evaluate the clinical and histopathologic features of oral melanocytic nevi (OMN). STUDY DESIGN: One hundred cases of OMN were retrieved from the files of two oral pathology services, and the data were analyzed with regard to histologic type and clinical features. RESULTS: Intramucosal nevus was the most common type (61%), followed by common blue nevus (23%), compound nevus (7%), and junctional nevus (3%). Combined nevus and cellular blue nevus were rare (2% each). The hard palate was the most commonly affected site (33%), followed by the buccal mu

Malignant melanoma primary in lymph node. The case of the missing link.

When malignant melanoma occurs in a lymph node, it is presumed to be metastatic, and the primary cutaneous/mucosal process is discovered to be either active, spontaneously regressed, previously excised, or occult. Nevus cell aggregates and nodal blue nevi are well-documented morphologic curiosities occurring in lymph nodes. It has been hypothesized that these nevus cell aggregates and blue nevi could be progenitors of lymph nodal malignant melanoma in patients without an obvious extranodal site of origin. We document a prototypical case of primary malignant melanoma evolving from precursor nev

TERT Promoter Mutational Analysis as an Ancillary Diagnostic Tool for Diagnostically Challenging Melanocytic Neoplasms.

Telomerase reverse transcriptase promoter mutations (TPMs) have been shown to be common in melanoma and uncommon in benign nevi. To assess the use of TPMs as an ancillary diagnostic tool, we report the concordance of the TPM status with the final diagnosis in clinical cases with distinct differential diagnostic scenarios: dysplastic nevus versus melanoma, atypical Spitz nevus versus melanoma, atypical deep penetrating nevus (DPN) versus melanoma, and atypical blue nevus versus malignant blue nevus. In a control cohort, we found a positive TPM in 51/70 (73%) of the total melanomas with the high

Somatic mutations in GNAQ in amelanotic/hypomelanotic blue nevi.

A recent study indicates that somatic mutations in codon 209 of GNAQ, a gene encoding the signaling protein G-protein α subunit q, may be present in up to 80% of blue nevi. Given that mutations in GNAQ represent dominant dark skin (Dsk) mutations caused by increased dermal melanin, the primary aim of this study was to ascertain whether amelanotic/hypomelanotic blue nevi exhibited somatic mutations in GNAQ like their melanotic counterpart. Genomic DNA was isolated for genotyping per protocol using techniques including laser capture microdissection to isolate nevus cells from amelanotic/hypomela

Pigmented nevi of the oral mucosa: a clinicopathologic study of 36 new cases and review of 155 cases from the literature. Part I: A clinicopathologic study of 36 new cases.

Pigmented nevi are uncommon oral lesions. This study adds data on 36 new cases. The most common type of nevus was the intramucosal type (20 cases), followed by the common blue nevus (11 cases). Only three cases were of the compound type and only two were of the junctional type. The hard palate was the most frequent location for the blue nevus, whereas the buccal mucosa was the most frequent site for the intramucosal nevus. One nevus (intramucosal type) was located on the tongue, and to our knowledge, this is the first reported example of pigmented nevus at this site. The individual data on the

Combined nevus of the tarsal conjunctival.

We present an unusual case of combined nevus of the tarsal conjunctiva and eyelid margin in a pregnant woman who noticed morphological changes of the lesion during pregnancy. The patient underwent a wide full-thickness eyelid excisional biopsy including tarsal conjunctiva. Histopathologically, a subepithelial combined nevus (nevocellular and blue nevus) was observed. Conjunctival nevi are benign lesions with wide variation in clinical and histopathological features. Tarsal conjunctival involvement, as in the present case, is extremely rare (less than 1% of conjunctival nevi), especially the bl

Generalized congenital epithelioid blue nevi (pigmented epithelioid melanocytomas) in an infant: Report of case and review of the literature.

The epithelioid blue nevus (EBN) is a variant of the blue nevus characterized by heavily pigmented epithelioid melanocytes and lightly or nonpigmented spindle cells. It may be associated with Carney complex, a multiple neoplasia syndrome. Congenital cases of EBN not associated with Carney complex are rarely reported. We herein describe an infant who presented with multiple blue-gray nodules and papules involving the head, trunk, and extremities at birth, the corresponding histopathologic findings, and genetic testing results.

[Treatment of nevus of Ota and Ito and epidermal nevus syndrome].

Nevus of Ota, nevus of Ito and nevus of Hori are special melanocytic nevi that have a slate-brown or blue/grey coloring. They are pigmented disorders characterized by its heterotopic melanocytic dermal location and by blue/brown unilateral and sometimes bilateral facial patch in case of nevus of Ota, and in the supraclavicular, scapular, and deltoid region in case of nevus of Ito. It is more common in patients with Asian and dark-skinned ethnic backgrounds. Histologically, elongated, dendritic melanocytes are seen scattered mainly throughout the upper third of reticular dermis. An acquired var

Blue nevi of the palpebral conjunctiva: report of 2 cases and review of literature.

PURPOSE: To describe two patients with blue nevi of the palpebral conjunctiva and to review the existing literature on common and cellular blue nevi of the conjunctiva. METHODS: Report of two cases and literature review. RESULTS: We present two cases of blue nevi in the palpebral conjunctiva: an atypical cellular blue nevus of the left upper eyelid and a common blue nevus around the lacrimal punctum of the left lower eyelid. Both patients underwent full thickness eyelid excision with wide margins. There was no tumor recurrence at 11 and 4 months postoperatively. CONCLUSIONS: Blue nevi are a

Blue nevi of the Müllerian tract: case series and review of the literature.

BACKGROUND: Blue nevi are rare in the cervix and vagina. Melanocytes are not normally found in these sites and have been hypothesized to arise either from the Schwann cells of stromal nerves or from melanocytic precursors which have aberrantly migrated from the neural crest to rest in the Müllerian stroma. Because of their rarity (3 previous cases in the literature), vaginal blue nevi have not previously been studied with immunohistochemical and ultrastructural analysis. DESIGN: We describe 3 cases of blue nevus occurring in the Müllerian tract, 1 in the vagina and 2 in the endocervix. RESUL

Hyalinizing Spitz nevus: spindle and epithelioid cell nevus with paucicellular collagenous stroma.

Hyalinizing spindle and epithelioid cell nevus (Spitz nevus) is an uncommon melanocytic lesion. The histologic features of this benign tumor can mimic those of certain benign (dermatofibroma and desmoplastic cellular blue nevus) and malignant (metastatic carcinoma and malignant melanoma) neoplasms. We report a woman with a hyalinizing Spitz nevus and review the clinical characteristics, histologic features, and differential diagnosis of this lesion. The lesion presented as an asymptomatic red firm nodule on her abdomen. Microscopic examination showed a symmetric lesion in the dermis. The neopl

Dysplastic changes in different types of melanocytic nevi. A unifying concept.

We observed histopathologic changes previously described in dysplastic melanocytic nevi in association with a dermal component characteristic of other types of melanocytic nevi or overlapping with features of other varieties of nevi. In order to determine the frequency of these changes, we studied 2,164 cases of compound melanocytic nevi that fulfilled the histopathologic criteria for the diagnosis of compound dysplastic nevus, including architectural pattern, cytologic features, and mesenchymal changes. Of the 2,164 compound dysplastic melanocytic nevi, 1,895 (87.6%) had the histopathologic c

Concurrent Occurrence of Trichilemmal Cyst and Nevus of Ota: Case Series.

Nevus of Ota or congenital oculodermal melanosis (ODM) is characterized by brown or blue/gray asymptomatic brown or blue/gray flat lesions of the skin, mucosae, episcleral/sclera, and uvea, which are located near the trigeminal nerve's ophthalmic and mandibular branches. The main ophthalmic complications are glaucoma and predisposition to uveal melanoma. "trichilemmal cyst" is also known as "wen" "pilar cyst" or " isthmus catagen cyst". It occurs in the scalp and mimics sebaceous cysts clinically. The swelling appears smooth in outline and is filled with cytokeratin. An unusual case of a 32-ye

Cellular blue nevi with myxoid change--diagnostic difficulties and the review of the literature.

We report three cases of a rare morphologic variant of the cellular blue nevus, which usually occurs in young patients on the buttocks and lumbar area. This variant is characterized by the myxoid degeneration of the stroma, which separates alveolar nests of nevus cells. The uncommon occurrence and atypical pathologic features may bring about diagnostic pitfalls and unnecessary excessive treatment modalities, therefore, we tried to re-evaluate the criteria of diagnosis of cellular and malignant blue nevi.

Pigmentary disorders in oriental skin.

Brown hyperpigmented disorders may be melanotic in which there is a normal number of epidermal melanocytes but melanin pigment is increased in the epidermis (eg, melasma), melanocytotic, in which melanocytes are increased (eg, café-au-lait macules), and nonmelanotic hyperpigmentation (eg, minocycline pigmentation). Blue hyperpigmented disorders may also be melanotic in which there is a normal number of epidermal melanocytes, but melanin pigment is present in the upper dermis (eg, gray/slate pigmentation in Riehl's melanosis), melanocytotic in which melanocytes are present in both the epidermis

Electron microscopy. Ultrastructural observations of the extracellular sheath of dermal melanocytes in the nevus of Ota.

Under the electron microscope, we studied dermal melanocytes in lesions of six cases of nevus of Ota and also those in common blue nevus and in nevus caeruleus tardus associated with progressive systemic sclerosis. An extracellular sheath surrounded most of the dermal melanocytes in nevus of Ota in four patients ages 21, 22, 25, and 30, but not those in two younger patients ages 9 months and 10 years. Some dermal melanocytes possessed extracellular sheath as well as basal laminae. Some possessed only an extracellular sheath. The thickness of the extracellular sheath varied among dermal melanoc

Multiple halo neurofibromas.

The second case of multiple halo neurofibromatosis is reported. Halo neurofibromas are rare benign tumors that appear on the neck and trunk of adults. The clinical and histopathological differential diagnosis of the other neuroectodermally derived halo lesions, such as neural nevus, nevocellular nevus, spindle and epitheloid cell nevus, blue nevus, and malignant melanoma, also is considered. An autoimmune hypothesis is proposed to explain the depigmentation in the halo neurofibroma. The fact that the patient has café au lait spots that contain giant pigment granules probably indicates that he

Linear arrangement of multiple deep penetrating nevi: report of first case and review of literature.

BACKGROUND: Deep penetrating nevus is a recently described variant of melanocytic nevi with clinical and histopathological features that may be confused with malignant melanoma, blue nevus, pigmented Spitz nevus, or congenital melanocytic nevus. We report a case with linear arrangement of multiple deep penetrating nevi. To our knowledge, such presentation has never been reported in the literature. OBSERVATIONS: We describe a patient with multiple darkly pigmented lesions in the right periauricular area, above and behind the ear. The histopathological features of these lesions were consistent
