An approach to the neonate with sudden dysrhythmia: diagnosis, mechanisms, and management.

The clinical course of a newborn can be rendered more complex by the sudden development of a disturbance in heart rhythm and conduction. The caregiver's ability to recognize a particular dysrhythmia and its most probable mechanism is crucial for optimal management. During the past 15 years, there has been significant progress in the understanding of the electrophysiologic basis of rhythm disturbances in the newborn period. This understanding has led to more precise diagnosis and effective treatment regimens for infants with dysrhythmias. Using recent case studies from one intensive care nurser

Possible mechanisms for vaginal infection with Staphylococcus aureus: inferences drawn from studies of nosocomial infection of newborn infants and surgical patients.

Previous epidemiologic studies of staphylococcal infections in newborn infants and surgical patients provide clues to the method of infection in toxic shock syndrome. In newborn infants, staphylococci were shown to be transmitted most often by the hands of nursery personnel to the umbilical stump, which afforded a warm, moist milieu for bacterial growth. Studies indicated that many surgical wound infections were due to the patients' own organisms, which were carried into the operating room on the patient's skin. It is hypothesized that in toxic shock syndrome the tampon becomes contaminated wi

Reducing Variation in the Management of Apnea of Prematurity in the Intensive Care Nursery.

BACKGROUND: Premature infants have bradycardia and/or desaturation events due to apnea of prematurity that resolve as the infants mature. Despite American Academy of Pediatrics guidelines recommending a standard "event-free" period before discharge, length of observation in our Intensive Care Nursery was variable. By June 2018, for infants born <36 weeks' gestation in the Intensive Care Nursery, we aimed to standardize time to discharge after the last documented event at 5 days, when the baseline mean was 3.6 days (range 0-6 days). METHODS: A quality-improvement team used the Model for Improv

Diarrhea associated with adherent enteropathogenic Escherichia coli in an infant and toddler center, Seattle, Washington.

During November 1983, the Seattle-King County Department of Public Health investigated an outbreak of diarrhea associated with enteropathogenic Escherichia coli, serogroup 0111:K58, in an infant and toddler day-care center. Of the 25 children in the center, ranging in age from 4 to 30 months (median age 11 months), diarrhea occurred in 14 characterized by watery, greenish stools. The median duration of diarrhea was 12 days. Two of the ill children were hospitalized because of severe dehydration. Stool cultures from the children diagnosed initially did not yield the common bacterial pathogens,

Ultraviolet light burn: a cutaneous complication of visible light phototherapy of neonatal jaundice.

Visible light phototherapy is an easily administered and effective treatment for neonatal indirect hyperbilirubinemia. Reported cutaneous side effects include transient rashes and the uncommon bronze baby syndrome. A more hazardous side effect is ultraviolet burn. Two premature infants developed phototherapy-induced erythema, one associated with a second-degree burn, after exposure to fluorescent daylight bulbs inadvertently used without Plexiglass shields, thus allowing prolonged ultraviolet A (UVA) exposure. Premature infants, especially during the first two weeks of life, may be significant

Brain disorders associated with corticotropin-releasing hormone expression in the placenta among children born before the 28th week of gestation.

AIM: To evaluate the relationship between placenta corticotropin-releasing hormone (CRH) expression and brain structure and function abnormalities in extremely preterm newborns. METHODS: In a sample of 1243 infants born before the 28th week of gestation, we evaluated the relationship between CRH expression in the placenta and the risk of brain ultrasound scan abnormalities identified while these infants were in the intensive care nursery, low scores on the Bayley Scales of Infant Development-II of 900 of these children at age two years and head circumference measurements then more than one an

Obstetric and perinatal events: the accuracy of maternal report.

Physicians commonly rely on maternal recall of events during pregnancy, delivery, and the perinatal period. Investigators who are doing research in the reproductive and perinatal areas also rely on maternal recall. However, there is little information regarding its accuracy. This study evaluates the accuracy of maternal knowledge of events during pregnancy, delivery, and the perinatal period by comparing maternal report with information recorded on the medical records of the mother-infant pair. Results are presented as sensitivity, specificity, and chance-corrected agreement. Mothers demonstra

Hypertension in infancy.

Hypertension in infants is rare and requires a thorough evaluation. The incidence of hypertension in infancy has risen in recent years, reflecting both better monitoring methods and increasingly successful salvage of smaller and smaller newborns. Overall mortality and morbidity rates for uncontrolled hypertension in infants are unknown. With appropriate treatment, the prognosis for resolution of hypertension is good. In most cases, hypertension is short-lived and blood pressures return to normal even when medication is discontinued. Recent experience with improved antihypertensive agents in in

Outbreak of echovirus 11 infection in hospitalized neonates.

Between July 18 and August 5, 1986, a cluster of echovirus 11 infections occurred in hospitalized neonates. Ten infants were affected and one died. All cases occurring after the index case were infants who were in the nursery for at least 1 day when the index patient was also present. Risk factors for secondary infection included low birth weight or gestational age and receipt of antibiotics, red blood cell transfusions, nasogastric intubation or gavage feedings. Because viral infection had not been suspected in the index patient, isolation measures were not instituted until after onset of sec

Acquisition of nonmaternal Enterobacteriaceae by infants delivered in hospitals.

To determine whether Escherichia coli strains that colonize the intestinal tract of newborn infants in hospitals are of maternal origin or come from the environment, plasmid profiles of E. coli strains isolated from the stools of infants were compared with those from the stools of their mothers. Twenty-nine mother-infant pairs were studied in three different hospitals. In only 4 of 29 pairs, plasmid profiles of E. coli or other Enterobacteriaceae were shared by infant and mother; vertical transmission seemed to be uncommon, unlike findings in previous reports. In one hospital, 8 of 10 infant f

[Supporting families in the management of gastroesophageal reflux disease in children].

Gastroesophageal reflux disease (GERD) in children is a frequent reason for medical consultation. It is defined as the involuntary passage of gastric contents into the esophagus, with or without regurgitation and vomiting. It can become pathological if it leads to embarrassing symptoms and complications. Often confronted with this pathology, nursery nurses sometimes find themselves at a loss when it comes to treating the symptoms of pathological GERD in a toddler, as well as in supporting the parents. In order to give them some ideas, a review of the literature on the benefits of non-medicinal

Management and Early Outcomes of Neonates Born to Women with SARS-CoV-2 in 16 U.S. Hospitals.

OBJECTIVE: There is a paucity of evidence to guide the clinical care of late preterm and term neonates born to women with perinatal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. The objective of this case series is to describe early neonatal outcomes and inpatient management in U.S. hospitals. STUDY DESIGN: We solicited cases of mother-infant dyads affected by novel coronavirus disease 2019 (COVID-19) from the Better Outcomes through Research for Newborns (BORN) Network members. Using a structured case template, participating sites contributed deidentified, retrospec

Evaluation of a Simple Weight-Based Screening Tool for Asymptomatic Neonatal Hypoglycemia.

The objective of this study was to determine if the use of weight-based criteria to screen for asymptomatic hypoglycemia is consistent with using percentile curves to determine small and large for gestational age status. A retrospective chart review of 892 term infants in the normal nursery was performed. The degree of agreement in screening recommendations between weight-based criteria and the Lubchenco, Oken, and Olsen percentile curves was determined using Cohen's κ and McNemar's test. Agreement was lower between Lubchenco and Olsen or Oken (0.470, 0.509, respectively) than with weight-base

Cystic periventricular leukomalacia and type of cerebral palsy in preterm infants.

A case series design was used to identify cases of cystic periventricular leukomalacia (N = 31) identified by neurosonography at one regional tertiary intensive care nursery. Patients were preterm infants born at < or = 32 weeks of gestation who had cysts involving predominantly the middle-posterior or posterior periventricular regions. Neurodevelopmental evaluations were made for 26 (96%) of 27 survivors. All infants assessed had cerebral palsy (i.e., 54% quadriplegia, 42% diplegia, and 4% hemiplegia). Most cognitive delays and all sensory impairments occurred in children with quadriplegia. P

The etiology of neurosensory hearing defects in preterm infants.

We retrospectively investigated the influence of gestational age, perinatal risk, and the duration of incubator care periods in 193 surviving preterm infants with a gestational age between 28 and 36 weeks raised in our intensive care nursery incubators from 1965--1967. 24 (12.4%) of the children showed definite neurosensory hearing defects, which were particularly encountered in the high frequencies. No correlation could be substantiated between hearing difficulties and hyperbilirubinemia, streptomycin application and gestational age. This study does not support the assumption that the duratio

Early presentation of Hirschsprung's disease with initial meconium passage: A diagnostic challenge.

Hirschsprung's disease is a congenital condition characterized by an absence of enteric ganglionic cells. In this paper, we report a case of an 8-day-old male infant presenting with a history of constipation and progressive abdominal distension. Meconium had been passed in the newborn nursery. No stools were passed after 4 days of age, and the last one had required rectal stimulation. We emphasize the diagnostic challenges associated with Hirschsprung's disease in neonates who initially pass meconium. Timely recognition and surgical intervention achieved a favorable outcome.

Jaundice in the healthy newborn infant: a new approach to an old problem.

We measured the serum bilirubin concentrations in 2,416 consecutive infants admitted to our well baby nursery. The maximal serum bilirubin concentration exceeded 12.9 mg/dL (221 mumol/L) in 147 infants (6.1%), and these infants were compared with 147 randomly selected control infants with maximal serum bilirubin levels less than or equal to 12.9 mg/dL. A serum bilirubin concentration greater than 12.9 mg/dL was associated strongly with breast-feeding (P = .0000) and percentage of weight loss after birth (P = .0001), as well as with maternal diabetes, oriental race, decreased gestational age, m

Copious amount of bloody stool in a newborn shortly after birth.

Melena neonatorum is not uncommon; it presents as bloody stool in newborns and is caused by ingestion of maternal blood. This case presents an infant who had multiple episodes of copious amount of blood in his stool starting at 10 min of life, which raised multiple concerns requiring further investigation. The alkali denaturation test confirmed that the blood was maternal; however, due to the amount of bloody stool and the time required for the alkali denaturation test to result, further workups such as abdominal radiograph and hemoglobin/hematocrit were completed to ensure nothing was overloo
