Concentrated Exposure Therapy vs Internet-Based Cognitive Behavioral Therapy for Obsessive-Compulsive Disorder: A Randomized Clinical Trial.

IMPORTANCE: Given the high prevalence, chronicity, and associated disability of obsessive-compulsive disorder (OCD), optimizing treatment is a global priority. The Bergen 4-day treatment (B4DT), a concentrated exposure-based intervention, may reduce treatment burden and improve access to evidence-based care. OBJECTIVE: To evaluate the efficacy of B4DT compared with internet-based cognitive behavioral therapy (iCBT) in reducing OCD symptom severity. DESIGN, SETTING, AND PARTICIPANTS: Single-center randomized controlled trial (1:1 allocation). Between May 8, 2024, and August 8, 2025, adults (a

Neural correlates of response inhibition following cognitive behavioral therapy in obsessive-compulsive disorder: a randomized controlled EEG study.

BACKGROUND: Cognitive behavioral therapy (CBT) with exposure and response prevention is an established treatment for obsessive-compulsive disorder (OCD), yet the electrophysiological indices through which symptom improvement occurs remain incompletely understood. Response inhibition deficits have been implicated in OCD and may represent a key neurocognitive target of treatment. The present randomized controlled trial (Clinicaltrials.gov registiration no: NCT07000812) examined whether CBT is associated with changes in behavioral and electrophysiological indices of response inhibition. METHODS:

Exploring genetic and epigenetic markers for predicting or monitoring response to cognitive-behavioral therapy in obsessive-compulsive disorder: A systematic review.

Growing evidence has identified potential biomarkers of cognitive-behavioural therapy (CBT) efficacy in obsessive-compulsive disorder (OCD). Genetic and epigenetic mechanisms (e.g., polymorphisms, DNA methylation) contribute to OCD pathogenesis and CBT response variability, establishing them as a key research focus. To evaluate their associations with CBT outcomes in OCD, we conducted a systematic review of PubMed, Web of Science, CNKI, and Cochrane Library (from inception to January 2025), identifying eight studies that met rigorous inclusion criteria. The identified predictors included: (1)

Pavlovian fear reversal learning predicts outcome of exposure-based cognitive behavioral therapy for adult obsessive-compulsive disorder.

Extinction of conditioned fear responses is assumed to relate to beneficial effects of exposure-based treatment, but few studies investigated the role of fear conditioning for exposure techniques in clinical settings. Specifically, no study addressed the relationship between psychophysiological indicators of extinction and outcome of exposure and response prevention (ERP) in adult obsessive-compulsive disorder (OCD). This study investigated whether skin conductance responses (SCRs) to threat and safety signals in a pre-treatment Pavlovian reversal learning experiment predicted the outcome of i

Working memory load-dependent modulation of neural activity predicts response to cognitive behavioral therapy in obsessive-compulsive disorder.

Cognitive behavioral therapy (CBT) is an effective treatment for obsessive-compulsive disorder (OCD). However, CBT does not lead to a satisfying symptom reduction in a considerable number of patients with OCD. The identification of variables that predict insufficient treatment response could improve efficient treatment selection and inform the development of specific augmentative treatments. In the current study, we tested whether prediction of treatment response can be improved by including neurobiological markers during working memory (WM) performance. Forty-four patients with a primary OCD

Symptom Dimensions of Pediatric Obsessive-Compulsive Disorder: Exploratory Examination of Associations with Clinical Features and Response to Cognitive-Behavioral Therapy.

Several overarching symptom dimensions have been identified within pediatric obsessive-compulsive disorder (OCD), including doubting, obsessing, washing, ordering, neutralizing, and hoarding. These unique symptom presentations may have important clinical implications, especially in terms of potentially distinct associations with age, functional impairment, insight, family accommodation, and symptoms of anxiety and depression. Such symptom dimensions may also differ in their responsiveness to cognitive-behavioral therapy (CBT). Existing literature on the features associated with and the clinica

Comparing the 'When' and the 'Where' of Electrocortical Activity in Patients with Tourette Syndrome, Body-Focused Repetitive Behaviors, and Obsessive Compulsive Disorder.

Background/Objectives: Tourette Syndrome (TS), Obsessive Compulsive Disorder (OCD), and Body-Focused Repetitive Behaviors (BFRB) are three disorders that share many similarities in terms of phenomenology, neuroanatomy, and functionality. However, despite the literature pointing toward a plausible spectrum of these disorders, only a few studies have compared them. Studying the neurocognitive processes using Event-Related Potentials (ERPs) offers the advantage of assessing brain activity with excellent temporal resolution. The ERP components can then reflect specific processes known to be potent

Local structural and functional MRI markers of compulsive behaviors and obsessive-compulsive disorder diagnosis within striatum-based circuits.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a classic disorder on the compulsivity spectrum, with diverse comorbidities. In the current study, we sought to understand OCD from a dimensional perspective by identifying multimodal neuroimaging patterns correlated with multiple phenotypic characteristics within the striatum-based circuits known to be affected by OCD. METHODS: Neuroimaging measurements of local functional and structural features and clinical information were collected from 110 subjects, including 51 patients with OCD and 59 healthy control subjects. Efficacy and acceptability of a self-guided internet-delivered cognitive-behavioral educational program for obsessive-compulsive symptoms with international recruitment.

Cognitive-behavioural therapy is an effective treatment for obsessive-compulsive disorder (OCD). However, there are many barriers in accessing this treatment, with stigma being a particularly prominent barrier for many patients. Self-guided internet-delivered cognitive-behavioural therapy (ICBT), which does not require any contact with a therapist, has the potential to overcome this barrier. However, there is limited research on the efficacy of self-guided ICBT for OCD. The aim of the current study was to examine the efficacy of self-guided ICBT for OCD in a large international sample. Two hun

Benefit of cognitive behavioral therapy for post-traumatic stress disorder and obsessive-compulsive disorders in Huntington's disease: a case report.

We describe the management of post-traumatic stress disorder (PTSD) and obsessive-compulsive disorders (OCD), through Cognitive Behavioral Therapy (CBT) in a Huntington's disease (HD) patient.Even with a psychopharmacological treatment to manage PTSD and OCD, psychiatric disorders remained in the HD patient. We proposed CBT essentially based on progressive exposure. The symptomatology disappeared shortly after starting the psychotherapeutical treatment and was maintained over time. We discuss the benefits of CBT on this patient considering the therapeutical alliance and the cognitive disorders

Predictors of Suicidal Thoughts in Children with Autism Spectrum Disorder and Anxiety or Obsessive-Compulsive Disorder: The Unique Contribution of Externalizing Behaviors.

Children with autism spectrum disorder (ASD) are at elevated risk of suicidal ideation, particularly those with comorbid anxiety disorders and/or obsessive-compulsive disorder (OCD). We investigated the risk factors associated with suicidal ideation in 166 children with ASD and comorbid anxiety disorders/OCD, and the unique contribution of externalizing behaviors. Suicidal ideation was reported in the child sample by 13% of parents. Controlling for child age, sex, and IQ, perceived loneliness positively predicted the likelihood of suicidal ideation. In addition, externalizing behaviors positiv

Long-Term Stability of Benefits of Cognitive Behavioral Therapy for Obsessive Compulsive Disorder Depends on Symptom Remission During Treatment.

BACKGROUND: Cognitive behavioral therapy (CBT) is an effective treatment for obsessive-compulsive disorder (OCD) and may afford stable long-term improvements. It is not clear, however, how stability or symptom recurrence can be predicted at the time of termination of CBT. METHOD: In a 1-year follow-up intention-to-treat study with 120 OCD patients receiving individual CBT at a university outpatient unit, we investigated the predictive value of international consensus criteria for response only (Y-BOCS score reduction by at least 35%) and remission status (Y-BOCS score ≤ 12). Secondly, we appl

Guided Internet-Based Cognitive Behavioral Therapy in Japanese Patients With Obsessive-Compulsive Disorder: Protocol for a Randomized Controlled Trial.

BACKGROUND: Cognitive behavioral therapy for obsessive-compulsive disorder has been established, but access to this therapy in Japan is limited. Internet-based cognitive behavioral therapy may improve treatment accessibility and sufficiently improve obsessive-compulsive symptoms. There are few randomized controlled trials examining the effectiveness of internet-based cognitive behavioral therapy in patients with obsessive-compulsive disorder. We designed a randomized controlled trial protocol to assess the effectiveness of guided internet-based cognitive behavioral therapy in Japanese patients

Parent-led Behavioral Intervention for a Treatment-refusing Adult With Obsessive-Compulsive Disorder With Poor Insight and Extreme Family Accommodation.

Obsessive-compulsive disorder (OCD) with poor insight can contribute to the refusal to receive treatment, leaving the burden of care on parents or other caregivers. The case presented here involved a young adult with OCD with delusional levels of insight paired with extreme family-accommodating behaviors that had resulted in impaired quality of life and functioning for the young adult and his parents. A parent-led behavioral intervention [The Supportive Parenting for Anxious Childhood Emotions (SPACE)] that was developed for children with anxiety was adapted and utilized for a young adult with

Cognitive-behavioral therapy vs risperidone for augmenting serotonin reuptake inhibitors in obsessive-compulsive disorder: a randomized clinical trial.

IMPORTANCE: Obsessive-compulsive disorder (OCD) is one of the world's most disabling illnesses according to the World Health Organization. Serotonin reuptake inhibitors (SRIs) are the only medications approved by the Food and Drug Administration to treat OCD, but few patients achieve minimal symptoms from an SRI alone. In such cases, practice guidelines recommend adding antipsychotics or cognitive-behavioral therapy consisting of exposure and ritual prevention (EX/RP). OBJECTIVE: To compare the effects of these 2 SRI augmentation strategies vs pill placebo for the first time, to our knowledge

Modified Cognitive Behavior Therapy for Severe, Treatment Resistant Obsessive-Compulsive Disorder in an Adolescent With Autism Spectrum Disorder: The Importance of Parental Involvement.

The prevalence of obsessive-compulsive disorder (OCD) is higher among young people with autism spectrum disorders (ASD). Case studies and randomized controlled trials show that modified cognitive behavioral therapy (CBT) can be effective for young people with OCD and co-morbid ASD. This case study describes modified CBT for an adolescent with severe, treatment-resistant OCD and co-morbid ASD, and highlights the importance of family accommodation and parental mental health in pediatric OCD. Modifications to the standard evidence-based CBT for OCD protocol included extended psychoeducation, visu

Efficacy of distress tolerance techniques of dialectical behavior therapy in persons with obsessive-compulsive disorder: a study protocol for a randomized controlled trial.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a chronic and debilitating mental health condition that significantly impairs the quality of life. Although cognitive-behavioral therapy (CBT), particularly exposure and response prevention (ERP), is considered the gold standard, many patients remain symptomatic or find these approaches challenging due to emotional and cognitive barriers. Distress tolerance techniques (DTT), a component of dialectical behavior therapy (DBT), have demonstrated efficacy in various conditions, including substance dependence, binge eating, and depression, by manag

Identity-Related Stress and Obsessive-Compulsive Disorder in Sexual and Gender Minority Individuals: A Test of the Cognitive Behavioral-Minority Stress Model.

Pinciotti and Feinstein's (2025) cognitive behavioral-minority stress model of obsessive-compulsive disorder (OCD) proposes to explain the development, presentation, and maintenance of OCD among sexual and gender minority (SGM) individuals. Extrapolating from extant theory and research about the development and maintenance of OCD and, separately, the influence of identity-based stressors on psychopathology among SGM individuals, this model proposes that a combination of early life experiences and identity-based stressors lead to a general predisposition of inflated responsibility to prevent ha
