Traumatic posterior atlantoaxial dislocation combined with Jefferson fracture and odontoid process fracture with vertebral artery injury is rare. The management of such injury raises controversial issues and is still open to debate. A 74-year-old Chinese male presented with sustained neck pain and stiffness after falling from height. The patient was neurologically intact. Preoperative radiographs demonstrated a Jefferson burst fracture with a posterior dislocation of the atlantoaxial joints and odontoid process Anderson and D'alonzo type II fracture. A computed tomography angiography (CTA) sho

Retro-odontoid pseudotumors are mainly caused by aging or rheumatoid arthritis. We treated a very elderly patient with retro-odontoid pseudotumor. A 92-year-old man was admitted with the chief complaints of difficulty walking and progressive numbness in the right upper and lower extremities. Neurological examination revealed muscle weakness and exaggerated tendon reflexes of the right upper and lower extremities, and disturbance in skilled motor activities of the fingers, bilaterally. He had no bladder or rectal disturbances. The Japanese Orthopaedic Association(JOA)score for cervical myelopat

Fractures of the second cervical vertebra with involvement of the odontoid process can mostly be treated conservatively by immobilization. In the case of high-grade unstable fractures and pseudarthrosis dorsal C1/2 spondylodesis should be considered.Based on established atlantoaxial osteosynthesis techniques for dorsal C1/2 spondylodesis, a polyaxial screw-rod system with C1/2 transarticular screws connected with C1 lateral mass screws was carried out in a 56-year-old male patient with dislocated odontoid fracture. No intraoperative or postoperative complications occurred. Posterior dislocation without any associated fracture of odontoid is exceedingly rare and only 11 cases have been reported so far. A 32 year old male presented with pain, stiffness in neck, difficulty in breathing, associated lacerations on face and deformity of mandible and inability to open mouth. His plain radiographs, CT scan, MRI demonstrated a posterior dislocation of the atlas with respect of axis and a flake of bone from odontoid process on CT scan. He was successfully managed by closed reduction, C1C2 lateral mars pedicular screw stabilization and inter facetal fusion with synthetic b

OBJECTIVE: Crowned dens syndrome (CDS), caused by calcium pyrophosphate or hydroxyapatite deposition around the odontoid process, is an under-recognized cause of acute severe neck pain and headache. It can closely mimic septic arthritis, osteomyelitis, giant cell arteritis (GCA), polymyalgia rheumatica (PMR), or cervical spondylitis. We report a case of CDS in a patient with diffuse systemic sclerosis initially managed for presumed infection.
METHODS: A 55-year-old man with diffuse cutaneous systemic sclerosis on mycophenolate mofetil and methotrexate presented with severe throbbing headache,

The cervical spine was examined with MR imaging and conventional radiography in 23 patients with severe rheumatoid arthritis. All patients had neck pain and 17 also had neurologic symptoms. MR provided detailed information about soft-tissue lesions, vertebral dislocation, and narrowing of the spinal canal. Pannus surrounding the odontoid process was revealed in 14 patients, all with horizontal atlantoaxial subluxation. Compression of the medulla and/or spinal cord, caused by dislocated vertebrae and/or the soft-tissue mass around the odontoid process, was seen in 15 patients. When there was mo

Crowned dens syndrome (CDS) is a relatively uncommon presentation of calcium pyrophosphate dihydrate (CPPD) deposition disease that manifests as acute attacks of neck pain with fever, neck rigidity and elevated inflammatory markers related to radiodense deposits of CPPD in ligaments around the odontoid process. We present a case of CDS.

BACKGROUND: The transoral transpharyngeal odontoidectomy, followed by occipitocervical fixation, have traditionally been a recognized method for ameliorating ventral compression at the craniovertebral junction (CVJ), despite its associated comorbidities. As an alternative, the endoscopic endonasal odontoid resection is a viable approach for various CVJ abnromalities that preserve the oropharynx and leads to fewer procedure-related complications(1-4). We present our case to detail the technical nuances of the procedure and its advantages over other techniques.

This paper describes ten children with a developmental defect in the midportion of the odontoid process that may be related to congenital os odontoideum. Six of the patients had a skeletal dysplasia, the seventh had an associated congenital C5-C6 fusion, and the last three patients had no other congenital anomalies.

A case of a Jefferson fracture is presented. A new diagnostic sign, which is viewed on a lateral radiograph and consists of interposition of the basion between the anterior arch of C-1 and the odontoid process, is described. This sign may aid in the rapid identification of Jefferson fractures.

BACKGROUND: Type 2 odontoid fractures may be associated with varying degrees of atlantoaxial dislocation (AAD). Atlantoaxial spondyloptosis (AAS) is a rare cervical injury with reported etiologies including congenital, trauma, and rheumatoid disease, defined as having the facets of the atlas fixed anteriorly to the facets of the axis. When associated with a type 2 odontoid fracture, there is complete translocation of the C1 arch and the fractured odontoid process complex, anteriorly to the C2 vertebral body, representing a severe form of fixed or irreducible anterior AAD. The challenging anato

Crowned dens syndrome (CDS) typically manifests as acute neck pain accompanied by calcifications around the odontoid process. However, such findings may obscure more serious underlying pathologies. We report a case of an 85-year-old man on edoxaban who presented with acute neck pain and atlantoaxial calcifications on computed tomography (CT), initially suggestive of CDS. Subsequent mild neurological deficits prompted cervical magnetic resonance imaging (MRI), which revealed a spontaneous cervical epidural hematoma (SCEH) extending from C2 to C4. The patient was managed conservatively and achie

BACKGROUND: Odontoid process fractures are one of the most common spine fractures, especially in patients over age 70. There is still much controversy over the ideal candidate for anterior odontoid screw fixation (AOSF), with outcomes affected by characteristics such as fracture morphology, nonideal body habitus, and osteoporosis. Therefore, this systematic review seeks to discuss the optimal criteria, indications, and adverse postoperative considerations when deciding to pursue AOSF.
METHODS: This investigation was conducted from experiential recall and article selection performed using the

PURPOSE: Odontoid process fractures can extend rostral into the C2 arch. We investigated the clinical impact of a concurrent fracture of the pars interarticularis on odontoid failure. To overcome the surgical challenges related to the morphology of these fractures, we describe a novel surgical strategy using atlantoaxial joint distraction arthrodesis.
METHODS: We conducted a single centre cohort study of 13 consecutive patients with odontoid fractures extending into the pars treated between June 2016 and June 2018. Criteria for a stable fibrous non-union were: Atlanto-Dens Interval (ADI) < 3

BACKGROUND: Microscopic transoral approach (TOA) has been considered as the gold standard for anterior decompression of the cranio-vertebral junction (CVJ). The progress in endonasal endoscopic procedures has made it possible to perform odontoidectomy via a minimally invasive transnasal route.
OBJECTIVE: The aim is to critically analyze our cases of odontoid subluxation and those described in the literature that were treated by the endonasal endoscopic odontoidectomy (EEA).
METHODS: Seven consecutive patients that failed conservative treatment and other surgical procedures of atlanto-axial d

The endoscopic endonasal corridor for odontoidectomy (EEO) is a valuable route in expert's hand to manage craniovertebral junction (CVJ) pathologies of the midline. The possibility to preserve part of the C1 anterior arch along to an adequate preoperative planning, makes this option a viable choice especially in elderly patients with multiple comorbidities who cannot sustain a second procedure closed in time. Indeed, the current case shows the results of EEO in the presence of ventral compression due to degenerative non-rheumatoid retro-odontoid pannus in an 80-year-old male. Due to the absenc

BACKGROUND: Odontoidectomy is a challenging yet effective operation for decompression of non-neoplastic craniovertebral junction disease. Though both the endoscopic endonasal approach (EEA) and the transoral approach (TOA) have been discussed in the literature, there remain few direct comparisons between the techniques.
OBJECTIVE: To evaluate the perioperative outcomes of EEA vs TOA odontoidectomy.
METHODS: A retrospective review of all cases undergoing odontoidectomy by either the EEA or TOA was performed. Attention was paid to the need for prolonged nutritional support, prolonged respirato

STUDY DESIGN: Nationwide Readmissions Database Study.
OBJECTIVE: The aim of this study was to investigate readmission rates and factors related to readmission after surgical and nonsurgical management of odontoid fractures.
SUMMARY OF BACKGROUND DATA: Management of odontoid fractures, which are the most common isolated spine fracture in the elderly, continues to be debated. The choice between surgical or nonsurgical treatment has been reported to impact mortality and might influence readmission rates. Hospital readmissions represent a large financial burden upon our healthcare system. Factor

Odontoid fractures are the most common cervical spine fracture in elderly patients, with falls being the most common mechanism of fracture in these patients. The clinical symptoms range from mild neck pain to quadriplegia, or even death. Treatment options include surgical and non surgical therapies, such as cervical collar or halo vest. We present the case of an 87 year-old patient with a history of recurrent falls and progressive loss of strength in all 4 extremities that worsened after a dental revision. The CT scan showed a cervical odontoid fracture, with displacement confirmed on MRI. Con
