Learning health systems: Driving real-world impact in mental health and substance use disorder research.

The Veterans Health Administration (VHA), under the U.S. Department of Veterans Affairs (VA), is one of the largest single providers of health care in the U.S. VA supports an embedded research program that addresses VA clinical priorities in close partnership with operations leaders, which is a hallmark of a Learning Health System (LHS). Using the LHS framework, we describe current VA research initiatives in mental health and substance use disorders that rigorously evaluate national programs and policies designed to reduce the risk of suicide and opioid use disorder (data to knowledge); test i

Leveraging Learning Health Systems to Address Racial and Ethnic Disparities in Veteran Patient Experience.

Learning health systems (LHS) use continuous data-driven learning cycles to improve care delivery, but seldom apply this approach to advance health equity. This perspective describes how the Veterans Health Administration (VA) applied LHS principles through a research-operations partnership between the Office of Health Equity (OHE) and the VA Health Equity-Quality Enhancement Research Initiative (QUERI) National Partnered Evaluation Center (PEC) to address racial and ethnic disparities in patient experience among VA users. The PEC analyzed the Survey of Healthcare Experience of Patients data t

Drug interactions: the effects of alcohol and meprobamate applied singly and jointly in human subjects. I. Theoretical considerations and literature review.

The considerations necessary to describe the effects of combinations of drugs in a biological system are reviewed. The terms which express these effects-additive, potentiative, antagonistic, synergistic-have not had specific operations applied to them and mathematical models have been sought to define these operations. The models should (1) describe the nature of the action of single drugs, (2) classify the results of drug combinations, (3) provide a set of operations for deciding the outcome of combinations, and (4) predict all possible results of a combination from a knowledge of each drug a

Understanding safety threats and resilience supports in the operating room: a mixed-methods protocol study using surgical video analysis and clinician interviews.

INTRODUCTION: Preventable intraoperative adverse events (iAEs) are common despite widespread implementation of surgical quality improvement initiatives. These events often result from the interaction of multiple system-based factors (safety threats, STs) that coalesce to compromise safety. Existing research does not fully capture how STs vary across institutions, and how surgical teams either recover from or anticipate challenges (resilience supports, RSs). Consequently, efforts to design and align interventions are hindered by an incomplete understanding of the system-level contributors to pa

Barriers and Solutions in Implementing Evidence-Based Recommendations to Prevent Intraoperative Inadvertent Hypothermia: A Qualitative Study.

In this study, it was aimed to understand the barriers and solutions for operating room (OR) nurses and anesthesiologists to implement evidence-based recommendations to prevent intraoperative inadvertent hypothermia (IIH). A qualitative, inductive, and descriptive study was conducted. This qualitative interview study was conducted face-to-face with 19 participants working in OR units between February and March 2023. The interviews were analyzed using qualitative content analysis. The COREQ checklist was followed. Two main themes and five sub-themes were identified as a result of content analys

Quantifying the Burden of Interhospital Cost Variation in Pediatric Surgery: Implications for the Prioritization of Comparative Effectiveness Research.

IMPORTANCE: Practice variation is believed to be a driver of excess health care spending, although few objective data exist to guide the prioritization of comparative effectiveness research (CER) in pediatric surgery. OBJECTIVE: To identify high-priority general pediatric surgical procedures for CER on the basis of the following 2 complementary measures: the magnitude of interhospital cost variation as a surrogate for the need for and potential effect of CER at the patient level and the cumulative fiscal burden of this cost variation when considering the case volume from all hospitals as a su

[Detection of meningococcal meningitis epidemics in Africa: a new recommendation].

In sub-Saharan Africa, the control of meningococcal meningitis epidemics relies on early epidemic detection and mass vaccination. However, experience shows that interventions are often initiated too late to have a significant impact on the epidemic. A new recommendation drafted by participants of a consensus meeting proposes an alert threshold and an epidemic threshold based on the weekly number or incidence of meningitis cases, according to the population size and the epidemic risk, resulting in indicators with high sensitivity and specificity for the detection of an emerging epidemic. [Modern view on the effectiveness of surgical methods for treating obstructive sleep apnea syndrome].

Obstructive sleep apnea (OSA) syndrome is not only a widespread pathology, but also has far-reaching social consequences due to patients' poor quality of nighttime sleep and high daytime sleepiness. To date, a large number of methods, both conservative and surgical, have been developed for the treatment of OSA. Surgeries performed for OSA are aimed at correcting the structures of the nose, pharynx, larynx, as well as the hyoid and jaw bones and the muscles attached to them. Despite the seventy-five-year history of the use of surgical treatment methods, there is still no complete clarity regard

An observational study of mortality on bison farms in Saskatchewan with special emphasis on malignant catarrhal fever.

In December 2011, the Malignant Catarrhal Fever (MCF) Task Force in Saskatchewan recommended that research be conducted on the relationship between the proximity of bison and sheep under typical commercial production settings and bison deaths due to MCF. The objective of this study was to evaluate all causes of death in bison herds and compare the incidence of MCF in herds at varying distances of exposure from sheep operations. Necropsies were completed on 76 of 133 bison reported to have died during the 18-month study period. A total of 7 MCF deaths was reported from 2 large herds within 1.0

[Postoperative homeostatic imbalance after trauma surgical interventions of various degrees in polytrauma].

The objective of this study was to evaluate and compare the derangement of body homeostatis and the inflammatory response after different types of traumatological operations in patients with multiple injuries. These were determined in a total of 60 operations. The procedures comprised osteosynthesis of the femur (n = 28), the pelvic girdle (n = 11) the spine (n = 8), and facial and basal skull reconstructions (n = 13). Specific and unspecific parameters of the inflammatory response were determined on the morning of the operation, immediately after the procedure, every 6 h on the 1st day and 48

Operations that are Questionable for Control of Obesity.

At the last review of the literature, there were 43 different operations or modifications thereof used for the control of obesity. An effective operation for obesity control should produce a significant weight loss (>/= 25% excess weight loss) to ameliorate the associated medico-sociopsycho-economic ramifications of the obesity. The operative and postoperative significant morbidity should be less than 20% and mortality less than 1%. The weight lost should be maintained for a long period (>/= 5 years). The effects of the operation should be reproducible and documentable by more than two authors

Adoption of awake spine surgery - trends from a national registry over 14 years.

BACKGROUND CONTEXT: Awake spine surgery is growing in popularity, and may facilitate earlier postoperative recovery, reduced cost, and fewer complications than spine surgery conducted under general anesthesia (GA). However, trends in the adoption of awake (ie, non-GA) spine surgery have not been previously studied. PURPOSE: To investigate temporal trends in non-GA spine surgery utilization and outcomes in the United States. STUDY DESIGN/SETTING: A retrospective observational study. PATIENT SAMPLE: Patients undergoing cervical or lumbar decompression or/and fusion from the American College o

The relationship between resilience and neurophysiological stress in Special Operations Forces combat service members.

Military resilience research is increasing due to the growing literature associating resilience with stress adaptation. This study aimed to investigate which physiological stress adaptation components were associated with resilience in Special Operations Forces combat service members. Special Operations Forces combat service members (n = 117) self-reported resilience (ER89) and lifetime clinician-confirmed mild traumatic brain injury history. Participants also underwent transcranial Doppler ultrasonography to measure middle cerebral artery velocity during rest and a breath-holding task. Neithe

Causes of mortality by autopsy findings of combat casualties and civilian patients admitted to a burn unit.

BACKGROUND: Approximately 5% of combat-related injuries include burns. Previous studies have shown similar mortality rates between military and civilian burn casualties; but causes of death were not detailed. STUDY DESIGN: We retrospectively reviewed autopsy reports of patients with burns treated at the US Army Institute of Surgical Research Burn Center from 2004 to 2007. RESULTS: Of 1,255 admissions, 100 (8%) died, with autopsies performed on 74 (36 burned during military operations). Causes of death included infection (61%); disorders of the pulmonary (55%), cardiac (36%), renal (27%), gas

Digital Clinical Trials for Substance Use Disorders in the Age of Covid-19.

: As a result of the coronavirus 2019 (Covid-19) pandemic, clinical research for substance use disorders (SUDs) has been impeded due to widespread stay-at-home mandates limiting the operations of "non-essential" work. Although appropriate to proceed with an abundance of caution to prevent viral spread, there will be detrimental consequences for patients with SUDs if clinical trials research cannot adapt and continue uninterrupted. The field of digital health has strong evidence for its feasibility and effectiveness and offers tools that can facilitate the continuation of SUD clinical trials re

Recovery housing and on-the-ground research priorities: a scoping study through the lens of community based participatory research.

INTRODUCTION: Though communities have featured recovery housing (RH) for several decades, the base of evidence for best practices continues to grow - especially evidence needed by, and known to, those who operate and receive these services. The Initiative for Justice and Emerging Adult Populations (JEAP) engaged with three community boards (CBs) - consisting of young adults with experience in recovery from substance use issues, people who have a history of criminal legal system involvement and recovery, and payers and provider of substance use services and harm reduction - to understand on-the
