The author discusses the achievements of contemporary traumatology in the treatment of intraarticular fractures by the method of perosseous osteosynthesis with external fixation apparatuses. The best functional effect was achieved by the application of hinged distraction apparatuses of Volkov-Oganesyan and other equipment with hinged devices. The results of treatment of 160 patients with severe open and closed intraarticular fractures testified to the advantage of the new hinged apparatuses according to biomechanics of the joints. The constructions of the author's apparatuses allow for strong

The Fixateur Interne (F.I.) is a new device developed by W. Dick and F. Magerl for the internal fixation of the thoracolumbar spine. This is a completely new system since it does not act as a four point bending system like long rod instrumentation but it is a two point fixation system and it is stable in flexion by itself. Segmental stabilization is attained in the best way by it. The device consists of two pairs of Schanz screws, short connecting threaded rods and hinges. Instrumentation is quite simple: from a posterior approach long Schanz screws are driven into the bodies in transpedicular

Orthopedic device-related infections remain a serious challenge to treat. Central to these infections are bacterial biofilms that form on the orthopedic implant itself. These biofilms shield the bacteria from the host immune system and most common antibiotic drugs, which renders them essentially antibiotic-tolerant. There is an urgent clinical need for novel strategies to prevent these serious infections that do not involve conventional antibiotics. Recently, a novel antibiofilm coating for titanium surfaces was developed based on 5-(4-bromophenyl)-N-cyclopentyl-1-octyl-1H-imidazol-2-amine as

BACKGROUND: To our knowledge, there are no comprehensive clinical studies of implant-related fractures in children. Our goal was to identify the incidence, skeletal location, and associated diagnoses of implant-related fractures.
METHODS: We reviewed our institutional database to identify cases of implant insertion (7584 cases) in patients less than 18 years old from January 1, 1995 through December 31, 2009. We calculated the overall incidence of these fractures and stratified the incidence by skeletal location and preoperative diagnoses. Fisher exact test was used to ascertain differences i

BACKGROUND: Open fractures constitute an important mortality and morbidity cause among all musculoskeletal system injuries and bring along many social and economic problems. The cost occurring due to both long treatment duration and the delay in returning to work made these conditions more complicated. The present study aims to evaluate of the complications which may occur in cases with an application of internal fixation following external fixator in upper and lower extremity open fractures retrospectively.
METHODS: Forty-nine patients, who applied to the emergency service between 2007 and 2

Fracture of the distal radius is the type of fracture most commonly seen in emergency departments. The understanding of nonsurgical and surgical care of distal radius fractures is evolving with recently developed methods of fixation. It is worthwhile to review some new methods of treatment, the role of bone grafting and synthetic substitutes, the principles of complex fracture management, and the treatment of common complications of distal radius fractures.

An intramedullary Kirschner wire was combined with an intra-osseous wire loop for fixing finger fractures. The mechanical stability of this technique to resist a bending moment in the sagittal plane was compared with four other conventional methods: single loop, loop and an oblique Kirschner wire, crossed Kirschner wires and dorsal plating. It was found that when the applied force was small, the technique was much more stable than the crossed Kirschner wires and was similar to the others. The ability to withstand mechanical failure was much better than most of the methods except the dorsal pla

The successful use of external fixation to stabilize severe open fractures elsewhere in the body has led to its adaptation in the hand. It is particularly useful when extensive wounds prohibit internal fixation and when comminution and bone loss render the fracture difficult to control by other methods. External fixation permits simultaneous wound access and fracture management. It can be used alone or adjunctively with other methods of fixation such as Kirschner wires, screws, or plates for definitive fracture management. Alternatively, external fixation can be used for provisional fracture m

A new implant system has been designed to rigidly fix the spine anteriorly at a single level above and below the injury site for treatment of severe thoracolumbar fractures. The primary system consists of two rods rigidly linked by vertebral body bridges. These bridges are secured to each vertebra at two points: one posterolaterally on the body with a vertebral body screw and the other more anterolaterally with a vertebral body staple. This implant system has been used in 48 consecutive thoracolumbar injuries. There have been three late failures to attain spine fusion, but in no patient has th

In a prospective survey of severe open tibial fractures, the incidence of serious complications with relatively rigid one- or two-plane unilateral fixator frames was considerably lower than with internal fixation by plates and screws. Unilateral configurations that contain rigid components provide better wound access than bilateral frames and interfere less with knee and ankle motion; they allow unencumbered partial and even full weight-bearing. This increases comfort and mobility and may have a positive effect on fracture healing. No standard formula exists, however, for the application of an

Experience with a successfully treated transepiphyseal fracture-dislocation of the femoral head in a 4-year-old boy, a review of 6 similar injuries from the literature, and consideration of the anatomy of the immature femoral head and neck, suggest that the treatment of choice for this injury is immediate open reduction and internal fixation with a smooth pin followed by cast protection. Contrary to recommendations in the literature, closed manipulation does not seem justified at any time, since it could eliminate residual blood supply to the femoral head by disruption of the soft tissue hinge

Long-term review of fifty-two displaced supracondylar humeral fractures in children treated by open reduction and crossed Kirschner-wire fixation demonstrated functional results similar to those obtained by closed methods, but fewer and milder cases of cubitus varus deformity (only 25 per cent of patients). The deformity appeared to result from faulty reduction with medial angulation of the distal fragment. Surgical treatment of these fractures may offer, as its principal advantages: reduced hospitalization time, fewer sequelae, more stable fixation, and slightly better anatomical results. No

BACKGROUND: Pediatric supracondylar humeral fractures present considerable surgical challenges due to the difficulty of achieving proper fracture alignment and stable fixation while avoiding injury to the ulnar nerve. This study assesses the biomechanical performance of a novel Kirschner wire (K-wire) fixation device (KFD), designed to enhance stability and reduce complications linked to traditional K-wire configurations.
METHODS: Using finite element analysis (FEA), we evaluated four fixation strategies for treatment of pediatric supracondylar humeral simple transverse fractures: crossed pin

Marlex rectopexy is a popular and effective procedure for the repair of rectal prolapse. Heretofore, this operation has required a generous laparotomy. As videolaparoscopy provides superior pelvic exposure, performing a modified Ripstein procedure was a logical progression of minimally invasive surgery. Experience with the first sutured Marlex rectopexy suggested the need for a simple fixation device to secure the mesh to the sacrum. A commercially available orthopedic staple allows quick and secure fixation. Five cases of laparoscopic Marlex rectopexy provide our initial clinical experience.
