[Rehabilitation patients after fractures of upper limb].

Rehabilitation after fractures of upper limb is very complicated and should be coordinated with orthopedic procedures. A choice of method and schema is depended on general health status of patient and used orthopedic treatment. This process should start immediately after surgeon's intervention. An aim of rehabilitation is improvement of range movement, straight of muscles and acceleration of callus formation. It is very important to restore proprioception in a region of trauma. An intensity and velocity of rehabilitation is the greatest after extra bone consolidation and the slowest after cons

Clinical experience with low molecular weight heparins.

Low molecular weight heparins (LMWHs) have been prepared by means of different fragmentation or fractionation procedures with the aim of obtaining a more effective and/or safer antithrombotic agent than standard heparin (SH). LMWHs resulting from different production procedures are structurally heterogeneous; this could account for the different efficacy and safety observed clinically. Many clinical trials have been carried out on the prevention of deep-vein thrombosis (DVT) and pulmonary embolism in patients undergoing major abdominal or orthopedic surgery, hemodialysis or affected by non-hem

Massive transfusion during major surgery in a child with anti-Knopsa and one kidney.

Massive transfusion has been defined as the replacement of one-third to one-half of the blood volume of the patient. It usually implies the presence of shock and is associated with multiple traumatic injuries. Blood replacement is almost always banked blood given in large quantities. However, massive transfusion with autologous blood can also be applied to elective cases and is becoming a useful and clinically beneficial service both in the quality of the products transfused service both in the quality of the products transfused and avoidance of diseases transmitted by blood transfusion from h

Orthopedic surgery and its complication in systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a multi-systemic immune-complex mediated autoimmune condition which chiefly affects women during their prime year. While the management of the condition falls into the specialty of internal medicine, patients with SLE often present with signs and symptoms pertaining to the territory of orthopedic surgery such as tendon rupture, carpal tunnel syndrome, osteonecrosis, osteoporotic fracture and infection including septic arthritis, osteomyelitis and spondylodiscitis. While these orthopedic-related conditions are often debilitating in patients with SLE which n

Outcomes Following Open Reduction Internal Fixation of Ankle Fractures (ORIF) By Podiatric Surgeons.

Open reduction internal fixation of closed ankle fractures is a common procedure performed by podiatric surgeons. This is the first study to specifically evaluate complication rates of this procedure among podiatric surgeons using a large patient cohort. The rates of podiatric surgical complications were also compared to the complication rates reported in the orthopedic literature. There was no significant difference between podiatric surgeons and orthopedic surgeons with regards to surgical site infection. However, complication rates, including 1-year postsurgery deep vein thrombosis (DVT), m

Surgical management and limb salvage outcomes in orthopedic trauma patients with concomitant popliteal artery injury: a single-center retrospective analysis.

BACKGROUND: Traumatic popliteal artery injury often occurs with complex orthopedic and soft-tissue damage and requires coordinated vascular, skeletal, decompressive, and wound management. We evaluated real-world treatment patterns and limb-salvage outcomes in a single-center cohort of patients with combined orthopedic trauma and popliteal artery injury. METHODS: We retrospectively reviewed 48 hospitalization records from 46 patients treated between 2002 and 2025. Repeated hospitalizations related to the same injury were reconciled into 46 independent patient/injury episodes. The primary outco

Effects of lip adhesion and presurgical orthopedics on facial growth: an evaluation of four treatment protocols.

Differences in craniofacial morphology in uni and bilateral cleft lip and palate were studied by roentgenocephalometry: 35 American children, treated with a lip adhesion procedure, and 26 Dutch children treated with a preoperative orthopedic procedure, both followed by a lip and/or palatal repair, were compared. A multivariate statistical approach was used to study the effects of the surgical procedures and of the age of the child when operated upon. The variables affected by the type of operation were: the relative length of the maxilla (PNS-ANS); the SNA angle; and the relative length of the

The relationship between preoperative anemia and length of hospital stay among patients undergoing orthopedic surgery at a teaching hospital in Ethiopia: a retrospective cohort study.

INTRODUCTION: Preoperative anemia in orthopedic surgery is linked to adverse outcomes such as longer hospital stays, higher rates of blood transfusion, and increased risk of death. Effectively addressing and managing this condition is essential for improving patient outcomes and shortening the length of hospital stays. In Ethiopia and other low-income countries, studies on preoperative anemia and its impact on the length of hospital stay following orthopedic surgery are limited. Therefore, this study aimed to assess the relationship between preoperative anemia and length of hospital stay among

Litigation Patterns of Acute Compartment Syndrome: Distinctions Between Orthopaedic and Non-Orthopaedic Cases and Factors Predicting Successful Defense.

BACKGROUND: Acute compartment syndrome (ACS) is a medical emergency and a cause of medical litigation across multiple specialties. We sought to compare the characteristics and outcomes of ACS-related litigation levied against surgeons in orthopaedics compared with other specialties. METHODS: The Westlaw database was queried for ACS-related cases filed within the United States between 1980 and 2023 using the search term "compartment syndrome." Inclusion criteria were defined as all jury verdicts or settlements tied to alleged medical malpractice concerning ACS of the spine and extremities. ACS

Improved Agreement With American Academy of Orthopaedic Surgeons Clinical Practice Guidelines on Carpal Tunnel Syndrome.

The American Academy of Orthopaedic Surgeons (AAOS) publishes clinical practice guidelines on the diagnosis and management of carpal tunnel syndrome (CTS). Previous versions made controversial recommendations, which failed to yield universal endorsement. Updated guidelines were published in 2016, and we aimed to evaluate agreement with their highest-strength recommendations among members of the American Society for Surgery of the Hand. An online questionnaire was sent to American Society for Surgery of the Hand members. There were 22 questions that inquired about respondents' specialty, experi

Musculoskeletal surgery in eighteenth century America.

Surgeons in America in the eighteenth century treated wounds, fractures, dislocations, and gunshot injuries of the skeletal structures and also contended with osteomyelitis and tuberculosis of bones and joints. In meeting this challenge, they proved innovative, adaptive and courageous, and developed principles of surgery, some of which still are in use today.

Trends in Use of Medications for Opioid Use Disorder among Commercially Insured U.S. Surgical Patients, 2016 to 2022.

BACKGROUND: The optimal management of perioperative pain in patients using medications for opioid use disorder (MOUD) is unclear. To motivate and inform efforts to develop evidence-based guidelines for perioperative pain management in these patients, it is important to evaluate whether the prevalence of MOUD use in surgical patients is increasing and to identify which procedures have the highest rate of MOUD use. METHODS: This cohort study analyzed adults 18 to 64 yr undergoing 1 of 1,083 major surgical procedures from 2016 to 2022 from the Merative MarketScan Commercial Database, which inclu

Survey of terminology used for the intraoperative direction of C-arm fluoroscopy.

BACKGROUND: Orthopedic surgeons depend on the intraoperative use of fluoroscopy to facilitate procedures across all subspecialties. The versatility of the C-arm fluoroscope allows acquisition of nearly any radiographic view. This versatility, however, creates the opportunity for difficulty in communication between surgeon and radiation technologist. Poor communication leads to delays, frustration and increased exposure to ionizing radiation. There is currently no standard terminology employed by surgeons and technologists with regards to direction of the fluoroscope. METHODS: The investigatio

Reimagining Orthopedic Practical Examinations: The Expanding Role of Objective Structured Clinical Examination.

INTRODUCTION: Assessment of orthopedic clinical competence has traditionally relied on long-case examinations, short-case discussions, and viva voce assessments. Although these methods evaluate theoretical knowledge and bedside skills, they are often limited by examiner bias, variability in case complexity, and lack of standardization. The Objective Structured Clinical Examination (OSCE) has emerged as a structured and objective method for assessing multiple domains of orthopedic competence within a uniform examination framework. OBJECTIVE: The objective of the study is to discuss the expandi

Native American Representation and Diversity Initiatives Within Orthopaedic Surgery: An Update and Road Map to Success.

Orthopaedic surgery lags behind other specialties in terms of diversity. This lack of representation is especially evident within the Native American population. Native American/Alaskan Natives represent approximately 2.9% of the overall US population but comprise just 0.4% of orthopaedic surgeons within the United States. Currently, no Native American-specific orthopaedic inclusion or recruitment programs exist. There are a multitude of programs that exist to recruit Native American applicants into health care. These include pipeline programs and scholarships from the Indian Health Service (I

Hand Surgeons Are Tackling Tougher Scaphoids: A Study of ABOS Candidate Data.

BACKGROUND: This study will evaluate whether those who have completed a hand fellowship treat a disproportionate number of scaphoid fractures based on recent American Board of Orthopaedic Surgery (ABOS) data. We hypothesize that surgeons who have completed a fellowship in hand surgery will address most surgically treated scaphoid fractures, particularly those with fracture nonunions or requiring graft. METHODS: We queried the ABOS database for case log information submitted for part II of the ABOS examination. This search included all cases with Current Procedural Terminology codes for proced

Preoperative gait analysis has a substantial effect on orthopedic decision making in children with cerebral palsy: comparison between clinical evaluation and gait analysis in 60 patients.

BACKGROUND: There is still some debate regarding the role of 3-dimensional gait analysis in routine preoperative evaluation of children with cerebral palsy. The aim of this prospective study was to evaluate to what extent introduction of 3-D gait analysis changes preoperative surgical planning. METHOD: Before gait analysis, 60 ambulatory children aged 10 (4-18) years with spastic cerebral palsy had a specific surgical plan outlined, based on clinical examination by orthopedic surgeons. After gait analysis, the proposed surgical procedures were reviewed to determine the frequency with which th
