[Atraumatic and aseptic osteonecrosis of large joints].

Subchondral osteonecrosis of large weight-bearing joints (hip and knee joints) represents a severe and joint-threatening disease. Apart from idiopathic forms, glucocorticoid medication and alcohol abuse have been identified as major causative factors for atraumatic osteonecrosis. In addition to direct cytotoxic factors, a critically reduced perfusion of the subchondral osseous space will eventually lead to bone marrow ischemia and osseous necrosis. Magnetic resonance imaging (MRI) is capable of visualizing a reactive border-like zone in this early, already irreversible and often asymptomatic s

[Differential diagnoses of osteonecrosis of large joints].

Subchondral insufficiency fractures and idiopathic bone marrow edema syndrome are the most important differential diagnoses to be distinguished from osteonecrosis because they have a deviating and as a rule more favorable prognosis and also different therapeutic strategies. Osteochondritis dissecans represents a different entity in many respects, which should not be confused or unified with osteonecrosis. Based on recent knowledge intractable subchondral insufficiency fractures may be the underlying cause of rapidly destructive osteoarthritis at least in some cases. Septic involvement of bone

Osteonecrosis of the femoral head: genetic basis.

PURPOSE: Genetic factors and hereditary forms of osteonecrosis of the femoral head (ONFH) have been elucidated through genetic association studies. The significance of these cases is that they suggest an alternative hypothesis to the development of the disease. This review presents a summary of single nucleotide polymorphisms (SNPs) and other genetic mutation variations found in association with ONFH, including our recent identification of a novel mutation in the transient receptor potential vanilloid 4 (TRPV4) gene in association with inherited ONFH. The purpose of this review is to consolida

[Progress on medication-related osteonecrosis of the jaw].

Medication-related osteonecrosis of the jaw (MRONJ) is a severe complication of bisphosphonates (BPs) or other targeted agent therapies. MRONJ appears as exposed bone, pus, and swelling in the oral and maxillofacial regions. However, neither surgery nor conservative therapy can eliminate symptoms thoroughly. In addition to BPs, several antiresorptive and antiangiogenic agents, such as denosumab and bevacizumab, as well as targeted agents, such as sunitinib and temsirolimus, can cause osteonecrosis of  the  jaw according to the literature. This review aims to summarize the research progress on

Hip osteonecrosis and pregnancy in healthy women.

PURPOSE: Osteonecrosis has been reported to be associated with pregnancy in the absence of other known risk factors for the disease. Few population-based data exist to support an association since the association is rare. We investigated the association of the femoral head and pregnancy to define if pregnancy was a risk factor and to define the risk period. METHODS: Using a case-crossover method design among 652 patients who were healthy (without any known cause of osteonecrosis) during or after pregnancy, we defined the periods of risk based on the timing of reported osteonecrosis and pregna

[Focus: Drug-related osteonecrosis of the jaw].

Antiresorptives and antiangiogenics are treatments that have proven effective in oncology and the treatment of osteoporosis and they are increasingly prescribed. The care of these patients requires collaboration between the prescriber and the oral health professional to establish an optimized treatment plan. Therapeutic education of the patient is essential for him to understand the issues of good oral health and the adverse effects that can be caused by these treatments. The management is essentially based on the individual benefit/risk balance resulting from the general, local and inherent o

Spontaneous osteonecrosis of navicular and talus (SONNT).

We describe a rare case of simultaneous and spontaneous osteonecrosis affecting bilateral navicular and talar bones in a young male. On both sides, there was associated symptomatic talonavicular and naviculocuneiform arthritis. The more symptomatic left foot was managed successfully by talonavicular-cuneiform (TNC) arthrodesis and core decompression of talar body. A probable cause of this rare disorder was most likely due to a developmental anomaly in the preaxial ray of the foot. As no literature is currently available for this rare condition, our case report is intended to provide a referral

Osteonecrosis of the jaw associated with ziv-aflibercept.

Medication-related osteonecrosis of the jaw (MRONJ) has been associated with medications that include bisphosphonates (BPs), denosumab, bevacizumab and sunitinib. Ziv-aflibercept is a recombinant human vascular endothelial growth factor (VEGF) receptor which has been used to treat patients with various advanced solid tumors. We report three patients without a history of the use of medications known to cause MRONJ presenting with jaw osteonecrosis typical for MRONJ following therapy with ziv-aflibercept. All patients had metastatic gastrointestinal cancer treated with ziv-aflibercept and were e

Post-traumatic osteonecrosis of the proximal humerus.

Post-traumatic osteonecrosis of the proximal humerus represents a challenging problem to the surgeon. It is commonly seen following multi-fragmentary fractures of the proximal humerus which may affect the long-term functional recovery after such injuries. This review summarises the current evidence on risk factors, reasons why estimating its epidemiology is difficult, the vascular supply of the humeral head, classification, and management options.

Steroid-Induced Osteonecrosis in Dermatology: A Review.

BACKGROUND: Steroid-induced osteonecrosis is unpredictable and highly morbid. METHODS: MEDLINE (Ovid) and PubMed search for relevant literature. RESULTS: Oral steroid doses typically considered to be associated with the condition are >20 mg of prednisone (or equivalent) per day or >2 g within a period of 2 to 3 months. Symptoms may be vague. CONCLUSIONS: Dermatologists must exercise a high degree of clinical suspicion and warn patients about steroid-related risks and conditions for follow-up.

Osteonecrosis of the mandible associated with bevacizumab therapy.

Bevacizumab is a humanized antibody that blocks vascular endothelial growth factor and is of great value for the treatment of advanced cancer. Several adverse effects following its administration have been reported. To date, only 8 cases of osteonecrosis of the jaws associated with bevacizumab (without any association with bisphosphonates) have been reported. The aim of this article was to describe an original case of bevacizumab-related osteonecrosis of the jaw. A 61-year-old man diagnosed with advanced renal cell carcinoma was undergoing treatment with intravenous bevacizumab and temsirolimu

Vertebral osteonecrosis in an HIV-infected patient.

Vertebral osteonecrosis, also known as Kümmell's disease, is a disorder resulting in non-consolidation of a vertebral fracture caused by ischaemia. The main differential diagnosis of vertebral osteonecrosis is osteoporotic vertebral fracture; however, the former is associated more frequently with severe pain and neurological complications. Although HIV-infected patients have an increased risk of developing osteonecrosis in peripheral locations and osteoporotic vertebral fractures, the occurrence of vertebral osteonecrosis has not been previously reported in any of the large series of HIV-assoc

Potential pathophysiological mechanisms in osteonecrosis of the jaw.

Bisphosphonates are used in the treatment of hypercalcemia of malignancy, skeletal complications associated with metastastic bone disease, Paget's disease, and osteoporosis. Osteonecrosis of the jaw (ONJ) is a recently described clinical condition that has been associated with the use of nitrogen-containing bisphosphonates. Reports describing this entity first appeared in the literature in 2003. While there have been significant numbers of case reports and a limited number of retrospective and prospective studies examining risk factors associated with ONJ, the pathophysiology of this condition

Bisphosphonate-related osteonecrosis of the jaw: an overview.

Bisphosphonates are widely used in the management of metastatic disease to bone and in diseases of altered bone turnover. Recently, multiple-case series and retrospective studies have established a relationship between necrotic bone lesions localized to the jaw and the use of chronic bisphosphonate therapy. This condition has been named bisphosphonate-related osteonecrosis of the jaw (BRONJ). To evaluate the potential risks associated with this new and emerging complication, stage-specific management strategies and guidelines have been developed. In view of the widespread use of chronic bispho
