Compensation of otolith function.

Horizontal eye displacement in total darkness during lateral tilting about the body axis was examined in 14 patients with unilateral loss of labyrinth function, including 8 cases of positional vertigo or dizziness and 6 symptom-free cases. The change in eye displacement following labyrinthectomy was also studied in 2 cases of labyrinthitis and one case of delayed hydrops. Seven out of 8 subjects suffering from positional vertigo or dizziness exhibited abnormal eye displacement on tilting to the affected side, while 5 out of 6 symptom-free subjects showed normal eye displacement. The correlatio

[Pathophysiology of peripheral, paroxysmal benign position vertigo].

Peripheral paroxysmal, benign positional vertigo is one of the most common peripheral vestibular disorders. According to the "cupulolithiasis hypothesis", it is assumed that inorganic material is attached to the cupula of the posterior vertical canal which renders this organ sensitive to gravitational forces rather than angular accelerations. Quantitative and well-controlled positioning manoeuvres, however, show that the characteristics of this kind of vertigo and the corresponding nystagmus cannot be explained by gravitation-dependent cupular reactions. It seems more probable that the typical

Procedures for restoring vestibular disorders.

This paper will discuss therapeutic possibilities for disorders of the vestibular organs and the neurons involved, which confront ENT clinicians in everyday practice. Treatment of such disorders can be tackled either symptomatically or causally. The possible strategies for restoring the body's vestibular sense, visual function and co-ordination include medication, as well as physical and surgical procedures. Prophylactic or preventive measures are possible in some disorders which involve vertigo (bilateral vestibulopathy, kinetosis, height vertigo, vestibular disorders when diving (Tables 1 (T

Resting discharge patterns of macular primary afferents in otoconia-deficient mice.

Vestibular primary afferents in the normal mammal are spontaneously active. The consensus hypothesis states that such discharge patterns are independent of stimulation and depend instead on excitation by vestibular hair cells due to background release of synaptic neurotransmitter. In the case of otoconial sensory receptors, it is difficult to test the independence of resting discharge from natural tonic stimulation by gravity. We examined this question by studying discharge patterns of single vestibular primary afferent neurons in the absence of gravity stimulation using two mutant strains of

Residual dizziness after the first BPPV episode: role of otolithic function and of a delayed diagnosis.

Residual dizziness (RD) following the resolution of a benign paroxysmal positional vertigo (BBPV) episode is frequently reported by patients. Possible causes are still under debate in the literature. This study discusses the possible role of otolithic function and of elapsed time from onset of symptoms to diagnosis in the genesis of RD. In total, 116 patients younger than 65 years with their first episode of BPPV and without any other comorbidities were enrolled in the study. Before a bedside examination, subjective visual vertical (SVV) was determined in the case of a history suggestive of BP

The linear vestibulo-ocular reflex in patients with skew deviation.

PURPOSE: The linear vestibulo-ocular reflex (LVOR) is mediated primarily by the otolith organs in the inner ear. Skew deviation is a vertical strabismus believed to be caused by imbalance of otolithic projections to ocular motor neurons (disynaptically through the medial longitudinal fasciculus in the brain stem or polysynaptically through the cerebellum). The authors postulated that if skew deviation is indeed caused by damage to these projections, patients with skew deviation would show abnormal LVOR responses. METHODS: Six patients with skew deviation caused by brain stem or cerebellar les

Size and cell number of the utricle in kinetotically swimming fish: a parabolic aircraft flight study.

Humans taking part in parabolic aircraft flights (PAFs) may suffer from space motion sickness (SMS, a kinetosis). Since it has been repeatedly shown earlier that some fish of a given batch also reveal a kinetotic behavior during PAFs (especially so-called spinning movements and looping responses) and due to the homology of the vestibular apparatus among all vertebrates, fish can be used as model systems to investigate the origin of susceptibility to motion sickness. Therefore, we examined the utricular maculae (they are responsible for the internalization of gravity in teleosteans) of fish swi

Otolith dysfunction during vertiginous attacks in Ménière's disease.

OBJECTIVE: Vertiginous attacks of Ménière's disease (MD) are characterized by various types of vertigo, namely rotatory vertigo, dizziness and drop attack. When a patient complains of dizziness without spontaneous nystagmus, otolith dysfunction cannot be ruled out. The purpose of this study was to evaluate otolith dysfunction during vertiginous attacks of MD or delayed endolymphatic hydrops. MATERIAL AND METHODS: Vestibular function tests were carried out daily for several days in 11 patients (MD, n = 9; delayed endolymphatic hydrops, n = 2) during vertiginous attacks. Otolith function was ev

Mortality drives production dynamics of Atlantic cod through 1100 years of commercial fishing.

Most edible fish species have been fished for centuries or millennia, leaving little record or understanding of their population responses prior to human impact and thus no baseline for population conservation. Here, we reconstruct the population dynamics of Atlantic cod, one of the world's most harvested fish species, from the pristine state during the Viking era through more than 1100 years of fishing. Analysis of cod otoliths recovered during archaeological excavations of Icelandic fish processing sites revealed that cod in the 10th to 12th centuries were 25% larger and up to 300% older tha

Vestibular rehabilitation by auditory feedback in otolith disorders.

Rehabilitation strategies have been applied successfully over the last few decades to initiate central compensation of the tonus imbalance and to facilitate substitution in different types of peripheral vestibular dysfunction. However, these vestibular rehabilitation strategies are often not successful in patients with isolated otolith disorders. The aim of the present study was therefore to evaluate a specific rehabilitation strategy for patients with an isolated otolith disorder by using an auditory feedback system. Thirteen patients, which suffered from different types of otolith disorders,

Function tests of the otolith or statolith system.

PURPOSE OF REVIEW: This review aims to provide an overview of recent advances in tests to evaluate otolith function over the last 2 years. RECENT FINDINGS: Over the last 2 years, many papers have focused on the application of the vestibular evoked myogenic potentials (VEMP). Several aspects are under survey: a search for optimal stimuli, search for normative data, search for which labyrinthine function losses and what kind of pathologies induce abnormal VEMPs. The review shows that some fundamental problems still have to be solved to improve reproducibility and to increase sensitivity. Other

The canalith repositioning maneuver in patients with benign positional vertigo.

The canalith repositioning maneuver (CRM), as defined by Epley, can be an effective treatment for benign paroxysmal positional vertigo (BPPV). The staff at Başkent University's Ear Nose and Throat Clinic performed CRM on 68 cases of canalithiasis in 64 BPPV patients from June 1996 to August 1997. Symptoms resolved after the first session in 49 patients (72%) and after the second session in 11 cases (16.2%). It was necessary to repeat the maneuver three times in two cases (2.9%) and four times in one patient (1.5%). Discounting three patients who were lost to follow-up, only two patients in our

Postural stability using different neck positions in normal subjects and patients with neck trauma.

Subjects with neck problems, such as whiplash injuries, often complain of disturbed equilibrium and, in some instances, provocation of the neck position can elicit such problems. The importance of neck proprioceptors for maintaining balance is gaining increased interest, moreover the function or malfunction of the otoliths may disturb equilibrium in certain head positions. The aim of the study was to create a reference material for postural control and its dependence on head position in healthy subjects and to compare this with a set of patients with known neck problems and associated vertigin

[Mechanism of benign, peripheral, paroxysmal positional vertigo (BPPV)].

BACKGROUND: Positioning nystagmus of the peripheral benign type (BPPV) has long been considered to be due to cupulolithiasis-i.e. attachment of inorganic material to the cupula-of the posterior vertical semicircular canal. Meanwhile it has generally been recognized that not all characteristics of this type of nystagmus/vertigo can be explained by assuming a gravity-dependent reaction of the posterior canal. Canalolithiasis-i.e. floating material within the canal, heavier than endolymphe-of the posterior semicircular canal is now widely regarded as the cause of BPPV. Again, however, this is a c

Vertical vestibular input to and projections from the caudal parts of the vestibular nuclei of the decerebrate cat.

1. To investigate the type of vestibular signals that neurons in the caudal parts of the vestibular nuclei transmit to the cerebellum and spinal cord, we studied their responses to natural vestibular stimulation in vertical planes in decerebrate cats with the caudal cerebellum removed. Most neurons were in the caudal half of the descending vestibular nucleus, the remainder at corresponding levels of the medial nucleus or the medial-descending border. 2. Dynamics of the responses of spontaneously firing neurons were studied with sinusoidal tilts delivered at 0.05-1 Hz near the plane of body rot

Influence of vestibular and visual climbing fiber signals on Purkinje cell discharge in the cerebellar nodulus of the rabbit.

Extracellular recordings of the climbing fiber responses (CFRs) of single Purkinje cells were made in the cerebellar nodulus and ventral uvula of anesthetized pigmented rabbits. Natural vestibular and optokinetic stimuli were used to evoke responses from these Purkinje cells. Using a null response technique we sorted vestibularly-evoked CFRs into two canal-related categories. CFRs were related to either the ipsilateral anterior and contralateral posterior semicircular canal (iAC+cPC) or to the ipsilateral posterior and contralateral anterior semicircular canal (iPC+cAC). Both canal-related CFR
