To What Degree Do Patients' and Clinicians' Ratings of Appropriateness of TKA Align, and Were Expected Outcomes Associated With Those Ratings?

BACKGROUND: TKA is performed only after the patient and the surgeon agree that the potential benefits of surgery outweigh potential risks. However, as many as 20% of patients are dissatisfied after TKA, suggesting that the shared decision-making process does not consistently serve our patients well. Expected outcomes are increasingly used in clinical practice, as an addendum to preoperative clinical and demographic factors, to inform this decision; however, the added value of this information in aligning patients and surgeons regarding the benefit and risk of TKA remains poorly understood. Cultural and urban city living expectations of knee motion in a Southeast Asian city: implications on total knee arthroplasty outcomes.

BACKGROUND: The prevalence of knee osteoarthritis in Southeast Asia has increased steadily over the years. When conservative management options fail, total knee arthroplasty (TKA) is a reliable surgical option. Despite over 90% post-operative satisfaction, the high volume of TKAs performed means that even a small percentage of dissatisfied patients holds significance. Dissatisfaction often arises from unmet expectations, which are influenced by cultural and urban factors specific to the region. Surgeons must counsel patients on likely post-operative activities, but limited data on knee range o

Extranodal diffuse large B-cell lymphoma: Clinical and molecular insights with survival outcomes from the multicenter EXPECT study.

BACKGROUND: Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of aggressive non-Hodgkin's lymphoma with distinct clinical and molecular heterogeneity. DLBCL that arises in extranodal organs is particularly linked to poor prognosis. This study aimed to determine the clinical and molecular characteristics of extranodal involvement (ENI) in DLBCL and assess the actual survival status of the patients. METHODS: In this population-based cohort study, we investigated the clinical features of 5,023 patients newly diagnosed with DLBCL. Their clinical conditions, eligibility criteria, an

Perinatal outcomes in dichorionic diamniotic twins with multifetal pregnancy reduction versus expectant management: A systematic review and meta-analysis.

BACKGROUND: Published findings on perinatal outcomes of multifetal pregnancy reduction (MPR) of dichorionic diamniotic (DCDA) twin pregnancy to singleton are controversial. We performed a meta-analysis to appraise the effects of MPR of DCDA twin pregnancy versus expectant management on perinatal outcomes. METHODS: Four electronic databases were searched from their inception to June 15, 2019, to identify publications that appraised MPR before 15 weeks of gestation. Studies reporting perinatal outcomes of both MPR of DCDA twin pregnancy to singleton and expectant management were considered. The

Intrapartum intervention rates and perinatal outcomes following induction of labour after 41 + 0 weeks compared to expectant management.

OBJECTIVE: There is limited evidence regarding the incidence of intrapartum fetal compromise in women who are induced compared to those managed expectantly. The aim of this study was to investigate intrapartum and perinatal outcomes in women who were induced at >41 + 0 weeks compared to an expectantly managed cohort. METHODS: This was a retrospective cohort study of singleton, non-anomalous pregnancies delivering between 41 + 0 to 43 + 0 weeks at the Mater Mothers' Hospital, Brisbane. We compared outcomes between women who were induced and those that laboured spontaneously. RESULTS: Six thou

Midtrimester preterm prelabour rupture of membranes (PPROM): expectant management or amnioinfusion for improving perinatal outcomes (PPROMEXIL - III trial).

BACKGROUND: Babies born after midtrimester preterm prelabour rupture of membranes (PPROM) are at risk to develop neonatal pulmonary hypoplasia. Perinatal mortality and morbidity after this complication is high. Oligohydramnios in the midtrimester following PPROM is considered to cause a delay in lung development. Repeated transabdominal amnioinfusion with the objective to alleviate oligohydramnios might prevent this complication and might improve neonatal outcome. METHODS/DESIGN: Women with PPROM and persisting oligohydramnios between 16 and 24 weeks gestational age will be asked to participa

Long-term outcome in couples with unexplained subfertility and an intermediate prognosis initially randomized between expectant management and immediate treatment.

BACKGROUND: We recently reported that treatment with intrauterine insemination and controlled ovarian stimulation (IUI-COS) did not increase ongoing pregnancy rates compared with expectant management (EM) in couples with unexplained subfertility and intermediate prognosis of natural conception. Long-term cost-effectiveness of a policy of initial EM is unknown. We investigated whether the recommendation not to treat during the first 6 months is valid, regarding the long-term effectiveness and cumulative costs. METHODS: Couples with unexplained subfertility and intermediate prognosis of natural

Differential UCS expectancy bias in spider fearful individuals: evidence toward an association between spiders and disgust-relevant outcomes.

Recently, differential UCS expectancies were found for high- and low-predatory fear-relevant animals [Davey, G. C. L., Cavanagh, K., & Lamb, A. (2003). Differential aversive outcome expectancies for high- and low-predation fear-relevant animals. Journal of Behavior Therapy and Experimental Psychiatry, 34, 117-128]. The present study extends these findings to spider phobia. In a hypothetical experiment, high (n = 27) and low (n = 28) spider fearful individuals estimated the probability that slides of spiders, maggots, pit bull terriers, or rabbits would be followed by a sip of nauseating juice,

Distinct Neural Dynamics Underlying Reward Expectation Formation and Outcome Processing: Insights From ERP and Time-Frequency Analyses.

Reward expectations are fundamental to theories of decision making and reinforcement learning. While prior research has focused on how expectations can influence outcome processing, far fewer studies have investigated how these expectations are actually formed. To address this gap, we measured EEG activity from participants as they completed a two-stage binary-choice task designed to separate the formation of expectations about outcome probabilities from the processing of actual reward outcomes. To more fully examine the neural mechanisms underlying each stage, we measured the Reward Positivit

Sex Differences in Clinical Outcomes and Loss of Life Expectancy After Aortic Valve Surgery for Infective Endocarditis: A SWEDEHEART Study.

BACKGROUND: Female patients with infective endocarditis (IE) are referred to surgery less often than male patients, and research on sex differences after IE surgery is inconclusive. Understanding the impact of sex can help clinicians optimize patient care. Therefore, we investigated sex-based differences in loss of life expectancy and net survival after aortic valve surgery for IE. METHODS: We conducted a population-based, observational cohort study, including all patients undergoing aortic valve surgery for IE in Sweden from 1997 to 2022. Data came from national registers, including SWEDEHEA
