Crosstalk between alcohol use disorder and obesity: two sides of the same coin?

Investigating similarities and differences between alcohol use disorder (AUD) and obesity is important because both AUD and obesity are public health concerns and share neurobiological and periphery-brain mechanisms. Furthermore, AUD and obesity often present with similar medical consequences related to organ damage, including liver and cardiovascular diseases. There is also growing evidence of changes in alcohol drinking in people who undergo bariatric surgery for obesity. In this non-systematic critical review, we identified relevant articles through PubMed searches, previous knowledge, and

Development of functional gastrointestinal disorder symptoms following laparoscopic cholecystectomy: a prospective cohort study.

BACKGROUND: The casual relationship between the role of cholecystectomy and functional gastrointestinal disorders (FGIDs) are a controversial clinical challenge. This study aimed to investigate: (1) the overlap of FGIDs before cholecystectomy and its long-term outcome after surgery in patients with symptomatic cholelithiasis, and (2) the incidence of new-onset FGIDs after cholecystectomy. METHODS: Patients with symptomatic gallstone disease who underwent elective, laparoscopic cholecystectomy were prospectively enrolled. Healthy populations who underwent medical check-ups were selected as age

A new overlapped delta-shaped anastomosis technique using linear staplers with reinforced bioabsorbable material for intracorporeal anastomosis during laparoscopic colectomy.

AIM: Totally laparoscopic colectomy with intracorporeal anastomosis (IA) is associated with less surgical trauma and wound related complications compared to laparoscopy-assisted colectomy with extracorporeal anastomosis. Delta shaped anastomosis is a widely popular IA technique. In this study, we discuss a construction technique for an overlapped delta-shaped anastomosis using a linear stapler with a reinforced bioabsorbable material. METHODS: We excised the bowel at a point 10 cm from the tumour on either sides and laid them in an overlapped fashion. The entry hole was created at a point 3 c

Rare Association of Bilateral Central Salzmann Nodular Degeneration and Fuchs Endothelial Dystrophy Managed by DMEK: A Case Report.

Salzmann nodular degeneration is a slowly progressive, rare, degenerative disease of the cornea, usually associated with other ocular pathologies such as dry eye disease, previous ocular surgery, involving the peripheral cornea. We present the surgical management of a rare case of a patient diagnosed with Fuchs endothelial dystrophy where the Salzmann nodules present an unusual bilateral central corneal involvement and overlap the area with the most pronounced endothelial dysfunction. The patient underwent anterior superficial keratectomy, cataract surgery and Descemet membrane endothelial ker

Variations in surgical anatomy of the endolymphatic sac.

Twenty-nine specimens of the extraosseous portion of the human endolymphatic sac (ES) were serially sectioned longitudinally. The length and width of the extraosseous ES were measured and the surface area was calculated. As the specimens included the sigmoid sinus (SS), the relationship between the ES and the SS was analyzed. The extraosseous ES varied considerably in size. The lumen either consisted of a single tube or was subdivided into several tubules. The distal part of the ES overlapped the SS in one third of the specimens. The results indicate that a minute extraosseous ES could explain

Refining macrostomia correction: Case series applying square flap technique and Z/W-plasty skin closure for enhanced aesthetic and functional outcome.

INTRODUCTION AND IMPORTANCE: Macrostomia is a congenital deformity found in Tessier no. 7 facial clefts defined as an enlargement of the mouth at the oral commissure. Several techniques are described in literature to achieve optimal functional and aesthetic results, with varying results and surgeon preferences. In this case series we report surgical repair of macrostomia with a vermillion square flap method for the oral commissure combined with either Z-plasty or W-plasty closure for the skin. CASES PRESENTATION: A retrospective case analysis of 12 patients with macrostomia operated over the

Transfusion practice in elderly patients across surgical disciplines in Germany - a secondary data analysis of over 21 million patients.

BACKGROUND: Clinicians are increasingly confronted with elderly patients across nearly all surgical disciplines. Advanced age is an independent risk factor for complications, prolonged hospitalization, and particularly for red blood cell (RBC) transfusion. The elderly suffer more frequently from anemia, which is associated with higher perioperative transfusion requirements, increased complication rates, and mortality Surgical disciplines differ not only in primary diagnoses but also in the demographics of their typical patient population. MATERIAL AND METHODS: Hospitalized surgical patients i

Laparoscopic suture repair for perforated peptic ulcer disease: a meta-review and trial sequential analysis.

BACKGROUND: The number of systematic reviews (SRs) and meta-analyses in surgery is growing exponentially. Meta-epidemiology, as a form of evidence synthesis, allows for the pooling of data and assessment of the diversity present in multiple and overlapping SRs. AIM OF THE RESEARCH: This study aimed to summarize evidence from systematic reviews of randomized controlled trials and reanalyze outcome data on laparoscopic suture repair of perforated peptic ulcers using trial sequential analysis (TSA). MATERIALS AND METHODS: The Cochrane Library, PubMed, Embase, CINAHL, eLibrary, and ClinicalTrial

Cardiac surgery 2014 reviewed.

For the year 2014, more than 17,000 published references can be found in Pubmed when entering the search term "cardiac surgery". The last year has been characterized by a vivid discussion in the fields where classic cardiac surgery and modern interventional techniques overlap. Specifically, there have been important contributions in the field of coronary revascularization with either percutaneous coronary intervention or bypass surgery as well as in the fields of interventional valve therapy. Here, the US core valve trial with the first demonstration of a survival advantage at 1 year with tran

Perianal Mucinous Adenocarcinoma: A Case of Recurrent Anal Fistula.

An anal fistula is a fairly common clinical condition, with a very low incidence of malignant transformation. Mucinous adenocarcinoma is a subtype of adenocarcinoma, and its occurrence within perianal fistula tracts is quite rare. This case report describes a 54-year-old male patient with recurrent anal fistula, initially suspected of Crohn's disease (CD), and ultimately diagnosed with perianal mucinous adenocarcinoma. After our joint internal medicine, surgery, and imaging reassessment, the diagnosis was confirmed. Anal fistula is usually considered a benign lesion, but it may also be associa

Personal Guide Assisted Orthognathic Surgery: Application of a T-shaped Tooth Bone Combined Supporting Osteotomy Guide Plate and Positioning Guide Plate.

The precise movement of the maxilla is particularly important for orthognathic surgery, especially for patients with maxillary segmentation. In this preliminary study, the authors present a new tooth bone combined with a supporting osteotomy guide and positioning guide to guide the osteotomy and reduction of the maxilla. Through our preoperative simulation and postoperative image fusion, the authors found that the overlapping area is more than 90%. According to compare of the virtual plans and the postoperative results based on distances from the maxillary land- marks to the horizontal plane,

Fascia lata onlay patch for repairing infected incisional hernias.

Abdominal incisional hernias represent the most common wound complication after abdominal surgery and infected abdominal incisional hernias are difficult to manage. We describe a simple, safe and effective method of using a free fascia lata patch to repair a large infected incisional hernia. This procedure involves the following steps: incising the skin and subcutaneous tissue and identifying the edges of the hernia defect; incising each anterior rectus sheath and completely suturing the medial edges of the fascia with 0 polypropylene; creating a fascia lata patch; and overlapping the defect i

Indeterminate colitis: a distinctive clinical pattern of inflammatory bowel disease in children.

Inflammatory bowel diseases such as Crohn disease and ulcerative colitis are frequently clinical conditions in children. Another clinical entity, indeterminate colitis, is considered a subgroup of pediatric inflammatory bowel disease. It is generally characterized by early onset in the first years of life, and clinical behavior is rapidly progressive to pancolitis. The definition of indeterminate colitis has changed over the years, but it is usually used to identify severe colitis with overlapping features of ulcerative colitis and Crohn disease. Ileal pouch-anal anastomosis is the surgical tr

Double whammy: Acute suppurative appendicitis on top of Meckel's diverticulum.

Meckel's diverticulum (MD), a congenital abnormality occurring in 0.3% to 3% of individuals, shares overlapping clinical features with acute appendicitis (AA), a common surgical emergency, making simultaneous presentation diagnostically challenging. An 11-year-old boy presented with right lower abdominal pain, fever, and loss of appetite, exhibiting positive Rovsing and Psoas signs. Elevated inflammatory markers and ultrasound findings suggested acute appendicitis with concurrent Meckel's diverticulitis. Surgery confirmed acute suppurative appendicitis and an inflamed MD, both of which were su

Xanthogranulomatous osteomyelitis: multimodality imaging of a rare case involving tibial diaphysis with review of literature.

The case report discusses xanthogranulomatous osteomyelitis, a rare inflammatory bone condition that can resemble malignancy on imaging studies. The report details a case of a 42-year-old woman with left leg pain, swelling, and redness lasting 4 months. Imaging revealed a lytic lesion in the left tibia with endosteal erosion, and MRI showed extraosseous soft tissue extension. An open biopsy confirmed xanthogranulomatous osteomyelitis. Following surgical curettage and bone grafting, the patient recovered well at subsequent follow-ups until 1 year after surgery. The article emphasizes the import

Atypical vascular lesions extending outside the radiation field: a diagnostic challenge.

Atypical vascular lesion (AVL) is an uncommon, benign vascular proliferation seen in previously irradiated skin, most commonly after radiotherapy for breast cancer. Atypical vascular lesion and angiosarcoma may share overlapping clinical and histopathologic features. We report the first case of AVL occurring outside the field of radiation. This patient's clinical course and histopathology was overall consistent with AVL, including two biopsies with focal MYC positivity. However, due to variations in the interpretation of her histopathology, the management plans devised by two centers involved

Primary renal angiosarcoma: a diagnostic and therapeutic challenge.

Primary renal angiosarcoma is an exceedingly rare and aggressive neoplasm. Although it may occur in youth, this tumour is frequently reported in the sixth and seventh decades of life. The clinical presentation is frequently varied. Pathogenesis remains largely unknown and it has overlapping features with other tumours of the kidney. Current treatment options include variable combinations of surgery, chemotherapy and radiotherapy. Reports regarding the disease prognosis and natural history are limited. In this article, we chronicle the case of a patient with primary renal angiosarcoma presentin
