Personality styles of patients asserting environmental illness.

Case reports and chart reviews of patients asserting environmental illness suggest that they suffer from psychiatric difficulties, typically somatization disorder. We assert that viewing these patients solely as somatizers or hysterical characters searching for a nurturant relationship will undermine the doctor-patient relationship. Rather, many of these patients are obsessive/paranoid characters searching for a medical explanation to their physical symptoms. This distinction is highlighted by contrasting the clinical presentations of hysteric/somatizing patients with those environmental illne

Cannabis psychosis and paranoid schizophrenia.

The initial clinical symptoms of 25 consecutive cases of cannabis psychosis of the paranoid type and 25 consecutive cases of paranoid schizophrenia were studied and compared, in order to delineate features that would enable a differentiation of the two conditions. It was observed that the patients with cannabis psychosis substantially differed in terms of behavioral manifestations. Most of these patients were violent and panicky and demonstrated bizzare behavior, but they possessed some insight into the nature of their illness. Schizophrenic patients manifested these disturbances and character

Course of paranoid psychoses in relation to diagnostic grouping.

The author stresses the importance of diagnosis in psychiatry and gives a short presentation of the Scandinavian concepts of reactive psychoses and schizophreniform psychoses. On the basis of his own personal follow-up investigations on 301 consecutively admitted patients to the University Psychiatric Clinic in Oslo, followed up through 5-18 years, he concludes that the schizophreniform (schizo-affective) psychosis also in prognostic respects is a group in between. Of the patients with a discharge diagnosis of reactive psychosis, 81% had a favourable course compared to 61% of the patients with

New dimensions in the quantitative classification of mental illness.

CONTEXT: Patterns of comorbidity among mental disorders are thought to reflect the natural organization of mental illness. Factor analysis can be used to investigate this structure and construct a quantitative classification system. Prior studies identified 3 dimensions of psychopathology: internalizing, externalizing, and thought disorder. However, research has largely relied on common disorders and community samples. Consequently, it is unclear how well the identified organization applies to patients and how other major disorders fit into it. OBJECTIVE: To analyze comorbidity among a wide r

Folie a deux versus genetically driven delusional disorder: case reports and nosological considerations.

Folie a deux is thought to occur in persons who live close together, so that delusions of the primarily ill patient are adopted by the partner. Alternatively, if both patients are related, they may share the same genetically driven psychiatric illness. We present the cases of two psychotic couples (monozygotic twins and a mother and daughter pair, respectively), in which both subjects featured an almost identical psychopathological syndrome with the same delusional content and thus fulfilled the diagnostic criteria of folie a deux. When Leonhard's diagnostic system was applied, all four patien

Word associative production in affective versus schizophrenic psychoses.

The production of association word to stimulus words, which was found to be correlated with conceptual disorganization, as clinically measured by the Brief Psychiatric Rating Scale, was developed as a quantifiable measure of formal thought disorder. Associative word production in patients with affective psychoses (acute episodes of mania or schizoaffective disorder) was found to be higher in a statistically significant manner than in patients with acute episode of paranoid schizophrenia. The production of associative words in the two groups of acutely psychotic patients was significantly highe

Conspiracy theories, personality dimensions and personality disorders.

PURPOSE OF REVIEW: Conspiracy theories are not a recent phenomenon, but their dissemination has been facilitated by the internet and modern means of communication such as social media. This article reviews personality-based factors that increase the likelihood of endorsing conspiracy theories. RECENT FINDINGS: Most studies used a correlational approach and examined personality traits and dimensions rather than personality disorders. The strongest and most consistent relationships were found between endorsement of conspiracy theories and suspiciousness and paranoid ideation, pseudoscientific t

[The Othello syndrome].

A case is described and 7 others are discussed of the Othello Syndrome, characterized by cognitive, affective and conative manifestations plus non-specific psychosomatic accesory symptoms. The nuclear symptom is the delusional or delusion-like idea of jealousy. The syndrome is seen in both sexes, as part of a number of clinical entities (paranoia, psychoses, organic brain syndromes, neuroses and personality disorders). Premorbid personality and family history are always abnormal. Cases of cocaine abuse, involutional melancholia and borderline syndrome are remarkers. The management of this synd

Persecutory delusions: a cybernetic model.

The authors propose a cybernetic model for the formation and maintenance of persecutory delusions. During the formation of persecutory ideation, the threat of loss of control over the self or others interacts with predictions of control from others. This interaction may result in feelings of alien control followed by persecutory explanations. Since the persecutory explanations provide the individual with a new goal of resisting others' influence in order to prevent being controlled, they reduce the threat of loss of self-control. This is how persecutory delusions are maintained.

Are deluded people unusually prone to illusory correlation?

This study reexamines the possibility that paranoid individuals are unusually prone to perceive illusory correlations. The authors use emotionally neutral word pairs to examine the illusory correlation phenomenon in three diagnostic groups: nonparanoid schizophrenia (n = 10), paranoid schizophrenia or delusional disorder (n = 9), and depression (n = 10). A one-way analysis of variance shows that the three groups do not differ in their tendency to make illusory correlations. Three separate t tests that compare the data from each of the clinical groups in this study with normative data again rev

The validity of the DSM-IV diagnostic classification system of non-affective psychoses.

OBJECTIVE: The schizophrenia and other non-affective disorders categories listed in the DSM-IV, are currently under revision for the development of the fifth edition. The aim of the present study is to demonstrate the validity of these categories by investigating possible differences between diagnostic patient subgroups on various measures. METHODS: 1064 patients with a diagnosis of non-affective psychosis (schizophrenia N = 731 (paranoid type 82%), schizoaffective N = 63, schizophreniform N = 120, psychosis not otherwise specified/brief psychotic disorder N = 150) participated in this study.

Sex distribution of DSM-III personality disorders in psychiatric outpatients.

The author determined the sex distribution of 170 outpatients with a DSM-III diagnosis of personality disorder measured by standardized instruments and compared his findings with DSM-III predictions. They confirmed the prediction of more women diagnosed as having histrionic personality disorder and more men diagnosed as having paranoid, compulsive, and antisocial personality disorders. The predicted excess of women diagnosed as having borderline and dependent personality disorders was not confirmed.

Cerebrospinal anandamide levels are elevated in acute schizophrenia and are inversely correlated with psychotic symptoms.

The endocannabinoids are a family of bioactive lipids that activate CB1 cannabinoid receptors in the brain and exert intense emotional and cognitive effects. Here, we have examined the role of endocannabinoid signaling in psychotic states by measuring levels of the endocannabinoid anandamide in cerebrospinal fluid (CSF) of acute paranoid-type schizophrenic patients. We found that CSF anandamide levels are eight-fold higher in antipsychotic-naive first-episode paranoid schizophrenics (n = 47) than healthy controls (n = 84), dementia patients (n = 13) or affective disorder patients (n = 22). Narcolepsy, paranoid psychosis, and tardive dyskinesia: a pharmacological dilemma.

The case history of a 54-year-old man with concomitant narcolepsy, paranoid psychosis, and tardive dyskinesia is presented. These disorders may all result from alteration in catecholamines, serotonin, and/or acetylcholine in the central nervous system. The interactions of the various psychopharmacological agents usually used to treat the disorders when they occur separately are considered in terms of current neurotransmitter hypotheses. The management of this case creates a pharmacological dilemma; the agents used for treatment of each of the disorders separately exacerbate one or both of the

Are premorbid abnormal personality traits associated with behavioural and psychological symptoms in dementia?

OBJECTIVE: The study aims to investigate associations between behavioural and psychological symptoms of dementia (BPSD) and abnormal premorbid personality traits. METHODS: Data were obtained from 217 patients with a diagnosis of probable Alzheimer's disease. Behavioural and psychological symptoms of late-onset dementia were assessed with the Neuropsychiatric Inventory. Premorbid personality traits were assessed using the Standardised Assessment of Personality. Abnormal premorbid personality traits were categorised with Diagnostic and Statistical Manual of Mental Disorders fourth edition and I

TPH2 gene polymorphisms in the regulatory region are associated with paranoid schizophrenia in Northern Han Chinese.

In the last years, serotonin (5-HT) has been related with the pathophysiology of several psychiatric disorders, including schizophrenia. Thus, genes related to the serotonergic (5-HTergic) system are good candidate genes for schizophrenia. The rate-limiting enzyme of 5-HT synthesis is tryptophan hydroxylase 2 (TPH2). Single nucleotide polymorphisms (SNPs) in the regulatory regions of TPH2 gene may affect gene expression and biosynthesis of 5-HT triggering to various neuropsychiatric disorders related to 5-HT dysfunction. The present study explored the association of SNPs within the TPH2 gene w

[Paranoia erotica (de Clerambault syndrome) in affective disorder].

Erotomania is a rare delusional disorder usually associated with other psychic disorders, especially paranoid schizophrenia. The primary form without comorbidity is very rare. A case report is presented with characteristic features of the pure form of erotomania in a female patient that was nevertheless an affective disorder. A historical review is presented and the division of Clérambault's syndrome into primary and secondary categories critically discussed.

Monosymptomatic hypochondriacal psychosis.

In brief, therefore, we are dealing with an illness characterised by a single delusional system - in this instance with hypochondriacal content - which can occur at any age from late adolescence onwards, appears to affect the sexes equally, and has a very poor prognosis without treatment. Its presentation appears to be relatively independent of cultural factors. A previous history, or a family history, of psychotic illness seems very uncommon. However, substance abuse and/or head injury seem to be background factors in a high proportion of younger patients and I would speculate about the role
