Experimental craniosynostosis in growing rabbits. The role of the periosteum.

Reports on the role of the periosteum in premature sutural synostosis have been contradictory. The present study summarizes a series of six experiments designed to clarify these previously conflicting findings. Twenty-five male New Zealand White rabbits were divided into six experimental groups. In four of the groups, methyl-2-cyanoacrylate was used to glue the frontal and parietal bones together and temporarily immobilize the coronal suture. In the other two groups, the sutures were not immobilized. Polyethylene was used to separate the cyanoacrylate from the periosteum in two of the groups.

Acute surgical intervention for a depressed skull fracture causing a laceration to the brain parenchyma from a bite wound in a dog.

A 5-month-old spayed female mixed breed dog was attacked by another dog causing multiple fractures of the left calvarium with a fragment penetrating through the gray matter of the parietal lobe. Surgery was performed to remove the bone fragment. A 6-month follow-up showed dramatic improvement in neurologic status.

Fetus with asymmetric parietal encephalocele, and hydrops secondary to laryngeal atresia.

We describe a fetus with an asymmetric, nonmidline, parietal encephalocele that appeared to result from "expulsion" through the center of the membranous bone; the fetus also had laryngeal atresia, which caused pulmonary overdistension (fetal Valsalva maneuver) with consequent hydrops fetalis. A common cause and clear pathogenetic relationship between these anomalies is not apparent.

The role of bone centers in the pathogenesis of craniosynostosis: an embryologic approach using CT measurements in isolated craniosynostosis and Apert and Crouzon syndromes.

This paper describes the role of the displacement of bone centers, i.e., the tubers, in the pathogenesis of craniosynostosis. This displacement was studied in 54 patients with isolated or syndromic craniosynostosis in the form of CT scans as well as in two dry neonate skulls with Apert syndrome. For comparison, 49 fetal and 8 normal infant dry skulls were studied. Our investigation was restricted to the coronal and metopic sutures. The results showed a significantly more occipital localization of the frontal bone center and a more frontal localization of the parietal bone center at the side of

Unilateral Pterional Polycraniosynostosis Treated with Craniectomy and Helmet Therapy.

Craniosynostosis is a condition in which one or more of the cranial sutures have fused prematurely, affecting the growth pattern and contours of the infant skull. The pterion is the junction of temporal, frontal, parietal, and sphenoid bones of the skull. We present a case of unilateral pterional craniosynostosis, which was treated with strip craniectomy and helmet therapy.

Composite skull and dura defect reconstruction using combined latissimus dorsi musculocutaneous and serratus anterior muscle-rib free flap coupled with vascularized galea transfer: a case report.

Recalcitrant epidural abscess following cranioplasty is a complicated problem, which becomes even more trying when large span of dura and skull bone are being replaced by alloplastic materials. A 22-year-old male underwent right fronto-temporo-parietal craniectomy and duroplasty with artificial dura graft after traumatic brain injury. Epidural abscesses recurred after cranioplasty with autologous bone graft as well as with a methyl methacrylate bone plate. The massive defects of both the dura and skull bone (15 × 9 cm) caused by radical debridement were reconstructed successfully with a combin

The increase of metopic synostosis: a pan-European observation.

Metopic synostosis is thought to have an incidence of about 1 in 15,000 births. Traditionally, this makes it the third most frequent single-suture craniosynostosis, after scaphocephaly (1 in 4200-8500) and plagiocephaly (1 in 11,000). Our units have, independently from each other, noted a marked increase in the number of metopic synostosis over the recent years. This is a pan-European, retrospective epidemiological study on the number of cases with metopic synostosis born between January 1, 1997, and January 1, 2006. This number was compared to the prevalence of scaphocephaly, the most frequen

Calvarial bone grafting of facial defects.

Bony defects of the face continue to challenge the reconstructive surgeon. Traditional sources of autologous bone from the rib or pelvis have been associated with the limitations of pain at the donor site, the need for a second operative field, variable "take" of the graft with poor eventual survival and, ultimately, less than optimal reconstruction. Calvarial bone grafting provides a readily accessible source of bone from an inconspicuous donor site, usually within the same operative field. The morbidity is minimal. The calvarial bone graft is especially useful in repair of the frontal sinus,

[Effect of artificial skull deformation in vitro on sensory performance in the human. Morphologic and tomographic study based on the calvarium E2 of Tell es Sultan/Jericho (7th millenium b.c.)].

The question of an influence of artificial skull deformations on sensory performances in man has been discussed since Torquemada (1615). It was inquired into by means of the individual E2 of the Tell es Sultan/Jericho (7th millenium B.C.). At first a morphological expert evidence and then an ear tomography (Mündnich and Frey's method) were carried out. Hereby abnormal displacements of all head organs or their parts could be seen. Evaluable pathological findings, however, did not result. Thus an influence of artificial skull deformation on sensory performance in man has to be refused in the sam

Frontal bone advancement stability with or without microplate fixation: an experimental study in rabbits.

Recent advancements in surgical correction of coronal suture craniosynostosis involve the overcorrection of a frontal bone segment to allow unrestricted growth of the developing brain. However, problems with segment stability and collapse have been reported. Such problems may be alleviated with microplate fixation of the segments. The present experimental study tests this hypothesis in a growing rabbit frontal bone advancement model. Sixteen 6-week-old rabbits were divided into three groups, consisting of animals with short bone segments advanced with two bone struts and fixed with Vicryl, lon

Validation of a simulator for cranial graft lift training: Face, content, and construct validity.

PURPOSE: Surgical skills can be improved through practical exercise. The use of specimens, human as well as animal, or live animals for surgical training is limited due to ethical concerns. Drawbacks of simulators are costs, fidelity and creditibility. Thus, simulators must be evaluated objectively to determine their validity before they can be used as teaching modalities. The aim of this study was to verify the face content and construct validity of a novel model-based simulator for lifting tabula externa transplants from the parietal skull. Ruptured cerebral arteriovenous malformation associated with enlarged parietal foramina: a rare surgical case with pathogenetic implications.

Enlarged parietal foramina (EPF) are rare, bilateral, symmetric calvarial defects, most commonly inherited in an autosomal-dominant manner. We report an exceptionally rare case of a ruptured parietal arteriovenous malformation (AVM) situated immediately beneath an EPF and resected via the calvarial defect. A 46-year-old man with no prior medical history presented with acute left upper-limb incoordination. Head computed tomography revealed a 3-cm right parietal subcortical hemorrhage and bilateral symmetric parietal defects; similar defects in his mother, uncle, and grandfather supported a diag

Parietal foramina clavicular hypoplasia. An autosomal dominant syndrome.

A male infant was evaluated with macrocephaly, scaphocephaly , a high forehead, a parietal foramen, a midline posterior occipital dermoid, and sloped shoulders. A skeletal survey also showed distal hypoplasia of the clavicles with bilateral loss of the acromion. Similar features were evident in his father, grandfather, and two paternal great-aunts. To our knowledge, this is the second report of this syndrome, which may be designated as parietal foramina-cleidocranial dysplasia. Our three-generation family and male-to-male transmission clearly established an autosomal dominant mode of inheritan

Cell mixing at a neural crest-mesoderm boundary and deficient ephrin-Eph signaling in the pathogenesis of craniosynostosis.

Boundaries between cellular compartments often serve as signaling interfaces during embryogenesis. The coronal suture is a major growth center of the skull vault and develops at a boundary between cells derived from neural crest and mesodermal origin, forming the frontal and parietal bones, respectively. Premature fusion of these bones, termed coronal synostosis, is a common human developmental anomaly. Known causes of coronal synostosis include haploinsufficiency of TWIST1 and a gain of function mutation in MSX2. In Twist1(+/-) mice with coronal synostosis, we found that the frontal-parietal

Computed Tomography Findings of Children Under 3 Years of Age with Mild Traumatic Brain Injury (TBI) and No Neurological Focal Signs.

Background/Objectives: Mild traumatic brain injury (mTBI) is a leading cause of pediatric emergency department visits, particularly among children under three years old. Although computed tomography (CT) is the gold standard for diagnosing intracranial injuries, its use in young children poses radiation risks. Identifying reliable clinical indicators that justify CT imaging is essential for optimizing both patient safety and resource utilization. Objective: This study aimed to evaluate CT findings in children under three years of age with mTBI and no focal neurological deficits, as well as to

Further observations on the fate of pedicle osteocutaneous grafts.

Recent work has shown that pedicle osteocutaneous grafts retain their viability and participate actively in the restructuring of transferred bone. The present study investigates two aspects of this process: (1) whether or not healthy recipient bone is needed for the survival of these grafts and (2) the degree of periosteal involvement in osteogenesis. Six adult dogs were used. Pedicle osteocutaneous grafts were constructed using the anterior table of the frontal sinus. Rotation of these flaps allowed implantation of the bone transplant into the ipsilateral parietal muscle. Free bone grafts wer

Anatomy and development of skull-neck boundary structures in the skeleton of the extant crocodylian Alligator mississippiensis.

A system-by-system approach dominates morphological and evolutionary study; however, some structures that are better understood within the context of an interface between two systems or traditional units remain less well understood. As part of a larger goal to clarify aspects of skull-neck boundary evolution, we herein describe the morphology and development of the occiput and atlas-axis complex in the crocodylian Alligator mississippiensis. We apply micro-computed tomography scanning, clearing and double staining, and histological analyses to skull-neck boundary structures at three stages of

A new syndrome of congenital generalized osteosclerosis and bilateral polymicrogyria.

We report a 29-week male fetus with healthy consanguineous parents. He showed a severe sclerosing bone disorder affecting all skeletal elements, resulting in insufficient modeling, generalized densification, and fragility of the skeleton. This skeletal dysplasia was associated with an abnormal craniofacial development (hypertelorism, severe microretrognathia, cleft palate, absent epiglottis, reduced number, and mineralization of teeth buds) and abnormal terminal phalanges. Neuropathologic examination showed bilateral fronto-parietal cerebral polymicrogyria. This syndrome appears to represent a
