Patellar-tendon transfer by the slot-block method for recurrent subluxation and dislocation of the patella.

In sixty-five knees followed for an average of 4.5 years after treatment of subluxation or dislocation of the patella by the slot-block method of medial patellar-tendon transfer, the results were satisfactory in fifty-four (83%) and unsatisfactory in eleven (17%). Recurrent subluxation or dislocation occurred in three (5%) of the knees and was attributed to insufficient displacement. Chondromalacia of the patella became worse after transfer in three of the sixty-two knees in which the distal realignment was technically correct. Two knees (3%) required patellectomy because of patellofemoral sym

Patient characteristics influencing knee injury and osteoarthritis outcome scores vary with time from patellar dislocation and number of dislocations.

OBJECTIVES: Demographic characteristics of the patient population influence patient reported outcome measures (PROMs) following patellar dislocations. The time from injury and number of dislocations can also vary within the patient population. The hypothesis of the study is that characteristics of the patient population influencing Knee injury and Osteoarthritis Outcome Score (KOOS) measures of pain, function, and quality of life vary with time from patellar dislocation and number of dislocations. METHODS: Outcome scores were evaluated for subjects in four groups: within five months of a firs

Osteochondral Fractures in Acute Patellar Dislocations in Adolescents: Midterm Results of Surgical Treatment.

BACKGROUND: Osteochondral fractures (OCFs) are common injuries during acute patellar dislocation (APD), carrying a high risk of early joint deterioration if left untreated. The recommended approach is reduction and stable fixation; however, data on the results of such treatment are limited. PURPOSE: To evaluate midterm results of fixation of APD-related OCFs in adolescents and to identify predictive factors for poor outcomes. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: This was a retrospective analysis of adolescent patients who underwent internal fixation of APD-related OCFs

Upper pole sleeve fracture of the patella secondary to patellar dislocation: A case report.

RATIONALE: Upper pole sleeve fractures of the patella are rare in adolescents; however, they are serious injuries that require early diagnosis and treatment. PATIENT CONCERNS: We present a rare case of a 15-year-old girl who suffered a sleeve fracture at the superior pole of the right patella. The patient had a history of dislocation of the patella 2 weeks ago. Physical examination showed tenderness on palpation over the upper pole of the patella and absence of active movement of the knee accompanied by swelling and joint effusion. A plain radiograph showed an avulsed fragment of the superior

Open posterior dislocation of the knee with rupture of the patellar tendon and a tibial plateau fracture.

Knee dislocations are rare injuries. They represent a severe soft tissue injury following high-energy blunt trauma. We report a case of open posterior knee dislocation with rupture of the patellar tendon and a fracture of the tibial plateau. The treatment was surgical and consisted of reduction of the knee dislocation, fixation of the tibial plateau fracture by lag screws, and transosseous sutures for the patellar tendon protected by a patellotibial cerclage. The result was successful with full range of motion.

A case of intra-articular synovial lipoma of the knee joint causing patellar dislocation.

Intra-articular synovial lipoma is rare and clinically manifested mostly by mass effect and occasionally by torsion of the stalk. Patellar dislocation rarely occurs owing to a mass effect of intra-articular tumor of the knee joint. The authors present a case of large intra-articular synovial lipoma at the lateral gutter of the knee joint, which presents as patellar dislocation. A 19-year-old man visited our clinic due to pain and limitation of motion owing to mass at his left knee. MRI revealed an intra-articular soft tissue mass at the lateral gutter of the knee joint with considerable mass e

Pes anserinus transposition for patellar dislocations. Long-term follow-up results.

Transposition of part of the pes anserinus was used to treat 96 knees in 78 patients with recurrent, habitual or permanent dislocations of the patella. The knees with habitual (48) and permanent (27) dislocations also had extensive release of superolateral contracture. The patients were reviewed at an average follow-up of 9 years 8 months. The medial stability of the patella had been improved in all the cases. There were no recurrences in 21 knees with recurrent dislocations. There were four recurrences (5.3%) in knees with habitual and permanent dislocations due to inadequate release of the s

An evaluation of the Elmslie-Trillat procedure for management of patellar dislocations and subluxations: a preliminary report.

The Elmslie-Trillat procedure involves lateral retinacular release, medial capsular reefing, and medial transposition of the anterior tibial tubercle hinged on a distal periosteal attachment. It is an easily performed procedure and is an excellent method for realignment of the extensor mechanism in cases of dislocation and subluxation of the patella. In our series of 52 cases with 6-24 months postsurgical evaluation, 46 cases, or 88 percent were rated as good or excellent. It must be emphasized that this technique is used only if the epiphyseal plates are closed.

Persistent Patellar Dislocation Following Correction of a Lower Limb Length Discrepancy: A Case Report.

Lower limb length discrepancy is common, but only a small proportion of cases are clinically significant. Distraction osteogenesis allows correction at any age without reducing final height, though long-term biomechanical consequences may arise. We report the case of a young patient who underwent femoral lengthening for a 4.5 cm discrepancy and, five years later, developed chronic patellar instability with persistent dislocation. Imaging revealed trochlear dysplasia, lateralization of the tibial tuberosity, medial patellofemoral ligament (MPFL) insufficiency, and increased quadriceps tension.

Combined medial patellofemoral and patellotibial reconstruction with soft tissue fixation in recurrent patellar dislocation.

INTRODUCTION: The medial patellofemoral and patellotibial ligaments (MPFL and MPTL) are the main passive restraints to lateral patellar translation. When nonoperative management of patellofemoral dislocations fails, surgical options can be considered to restore patellofemoral stability. Several reconstruction procedures of the MPFL with semitendinosus, gracilis, quadriceps tendon, and synthetic grafts have been described. No clear superiority of one surgical technique over another is evident. MATERIALS AND METHODS: Patients who suffered at least two documented episodes of unilateral patellar

Tibial tubercle proximalization as quadriceps lengthening in treating severe habitual patellar dislocation in adults.

BACKGROUND: To describe a novel 'four-in-one' procedure - including tibial tubercle proximalization, extensive lateral release, tibial tubercle medialization, and medial patellofemoral ligament reconstruction - for treating severe habitual patellar dislocation (HPD) in adults, and to report its early clinical outcomes. METHODS: Thirteen patients (13 knees) with severe primary HPD received this procedure. Results of the physical examinations, including apprehension tests and patellar tracking throughout full range of motion, were recorded pre-operatively and at final follow-up. Radiological as

Dislocation of a constrained total knee arthroplasty with patellar tendon rupture after trivial trauma.

Constrained total knee prostheses are used in knees with severe deformities and insufficiency of collaterals to provide stable and mobile knees. Dislocation after constrained knee prosthesis is an extremely rare and dreaded complication. When dislocation is associated with patellar tendon rupture, the management includes restoration of the extensor apparatus along with a stable knee. Repair of the patellar tendon is challenging due to poor soft tissue coverage in the area and a bulky repair can put tension on the wound closure. Ideal method of restoration of the extensor apparatus is a matter
