Klinefelter syndrome comorbidities linked to increased X chromosome gene dosage and altered protein interactome activity.

Klinefelter syndrome (KS) (47,XXY) is the most common male sex chromosome aneuploidy. Diagnosis and clinical supervision remain a challenge due to varying phenotypic presentation and insufficient characterization of the syndrome. Here we combine health data-driven epidemiology and molecular level systems biology to improve the understanding of KS and the molecular interplay influencing its comorbidities. In total, 78 overrepresented KS comorbidities were identified using in- and out-patient registry data from the entire Danish population covering 6.8 million individuals. The comorbidities extr

Hepatitis C Virus entry: the early steps in the viral replication cycle.

Approximately 170 million are infected with the hepatitis C virus (HCV) world wide and an estimated 2.7 million are HCV RNA positive in the United States alone. The acute phase of the HCV infection, in majority of individuals, is asymptomatic. A large percentage of those infected with HCV are unable to clear the virus and become chronically infected. The study of the HCV replication cycle was hampered due to difficulties in growing and propagating the virus in an in vitro setting. The advent of the HCV pseudo particle (HCVpp) and HCV cell culture (HCVcc) systems have made possible the study of

Isolation of Candida auris in Clinical Specimens.

Candida auris is a multidrug-resistant yeast causing healthcare-associated outbreaks of blood stream infections worldwide. Currently, C. auris isolation and identification is complicated by issues such as misidentification and long turnaround time associated with application of commonly used diagnostic tools. Based on phenotypic characteristics, differentiation of C. auris from related Candida haemulonii complex spp. is problematic. Candida auris can be misidentified using biochemical-based systems such as VITEK 2 YST, API 20C, BD Phoenix yeast identification system, and MicroScan. C. auris gr

People get ready! A new generation of Alzheimer's therapies may require new ways to deliver and pay for healthcare.

The development of disease-modifying therapies (DMTs) for Alzheimer's disease (AD) has progressed over the last decade, and the first-ever therapies with potential to slow the progression of disease are approved in the United States. AD DMTs could provide life-changing opportunities for people living with this disease, as well as for their caregivers. They could also ease some of the immense societal and economic burden of dementia. However, AD DMTs also come with major challenges due to the large unmet medical need, high prevalence of AD, new costs related to diagnosis, treatment and monitori

Developing a Clinical Prototype to Guide Surgeons for Intraoperative Label-Free Identification of Parathyroid Glands in Real Time.

BACKGROUND: Patients undergoing thyroidectomy may have inadvertent damage or removal of the parathyroid gland(s) due to difficulty in real-time parathyroid identification. Near-infrared autofluorescence (NIRAF) has been demonstrated as a label-free modality for intraoperative parathyroid identification with high accuracy. This study presents the translation of that approach into a user-friendly clinical prototype for rapid intraoperative guidance in parathyroid identification. METHODS: A laboratory (lab)-built spectroscopy system that measures NIRAF in tissue was evaluated for identifying par

Comparison of tactile, auditory, and visual modality for brain-computer interface use: a case study with a patient in the locked-in state.

This paper describes a case study with a patient in the classic locked-in state, who currently has no means of independent communication. Following a user-centered approach, we investigated event-related potentials (ERP) elicited in different modalities for use in brain-computer interface (BCI) systems. Such systems could provide her with an alternative communication channel. To investigate the most viable modality for achieving BCI based communication, classic oddball paradigms (1 rare and 1 frequent stimulus, ratio 1:5) in the visual, auditory and tactile modality were conducted (2 runs per

[NA Rare Bacterial Pathogen in a Patient with Perihilar Cholangiocarcinoma: Erwinia persicina; First Case From Turkey].

Members of the Erwiniaceae family, which can be found saprophytic in humans, have been identified several times as an infectious agent after their first identification in 1920. Erwinia persicina was first identified as a plant pathogen by being isolated from cucumber, tomato and banana in 1990, and it was shown to cause disease in many plant species in the following years. E.persicina was diagnosed as a urinary tract infection agent in an 88-year-old female patient in 1998. Our case, a 30-year-old male patient, was hospitalized for perihilar cholangiocarcinoma while being examined with the com

The IMPACT-FH Renewal Protocol: Evaluating FH Cascade Testing Implementation in Primary Care through a Pragmatic Trial, Economic Evaluation, and Mixed-Methods Sustainability Assessment.

INTRODUCTION: Familial hypercholesterolemia (FH) is a common autosomal dominant genetic disorder conferring a high risk of premature atherosclerotic cardiovascular disease (ASCVD), without early diagnosis and treatment. Underdiagnosis remains a major public health problem; cascade testing (systematically identifying blood relatives) can mitigate this burden. The Identification Methods, Patient Activation, and Cascade Testing for FH (IMPACT-FH, 5R01HL148246-04) study evaluated family communication strategies for FH cascade testing and significantly increased uptake in a population genomic scree

Learning by Example: An International Perspective on Reflex-Testing for Lynch Syndrome.

BACKGROUND: Lynch syndrome (LS), an autosomal dominant cancer syndrome, is the most common cause of hereditary colon cancer. Currently, however, less than 5% of patients with LS have been identified. Reflex-testing programs (in which tumors of patients with colorectal cancer are routinely evaluated for LS) have been proposed for better identification of affected individuals, yet the uptake of these programs within health care systems is limited. This study explored the structure, implementation challenges, and future directions of existing international population-based reflex LS testing progr

Artificial Intelligence as a Prognostic Tool in Cardiac Amyloidosis: A Review.

Cardiac amyloidosis (CA) poses a significant prognostic challenge due to its varied presentations and frequent delays in identification. While traditional prognosticators, such as cardiac biomarkers and imaging parameters, offer valuable information, there are significant challenges with individualizing prognosis and accounting for its complex and heterogeneous nature. Artificial intelligence (AI) has enhanced the precision across multiple modalities and has emerged as a prognostic tool in cardiac amyloidosis, demonstrated through models that predict disease progression and stratify patient ri

Bacteremia due to Clostridium hathewayi in a patient with acute appendicitis.

Clostridium hathewayi is a newly described Clostridium species isolated from the feces of healthy human individuals, but its clinical significance is not known. We describe a case of human infection associated with C. hathewayi. The bacterium (strain HKU18) was isolated from the blood culture of a 39-year-old patient with acute gangrenous appendicitis complicated by septic shock. The cells were strictly anaerobic, nonmotile rods that stained gram negative. Conventional phenotypic tests and commercial identification systems failed to identify HKU18 to the species level. 16S rRNA gene analysis s

Inertial Measurement Unit-Based Frozen Shoulder Identification from Daily Shoulder Tasks Using Machine Learning Approaches.

Frozen shoulder (FS) is a common shoulder condition accompanied by shoulder pain and a loss of shoulder range of motion (ROM). The typical clinical assessment tools such as questionnaires and ROM measurement are susceptible to subjectivity and individual bias. To provide an objective evaluation for clinical assessment, this study proposes an inertial measurement unit (IMU)-based identification system to automatically identify shoulder tasks whether performed by healthy subjects or FS patients. Two groups of features (time-domain statistical features and kinematic features), seven machine learn

A Confidence-Based Shared Control Strategy for Robotic Electrosurgery.

Robotic-assisted surgery (RAS) systems take advantage of dexterous tools, enhanced vision, and motion filtering to improve patient outcomes. Whereas most RAS systems are directly controlled by surgeons, the development and application of autonomous RAS are growing owing to their repeatability and precision. Although full autonomy is a long-term goal, human intervention in RAS is still essential. In this work, we develop and test a shared control strategy for robotic electrosurgery in which autonomous robot controllers and human operators collaborate. We designed and implemented identification

Disturbed lipid and amino acid metabolisms in COVID-19 patients.

The Coronavirus disease 2019 (COVID-19) pandemic is overwhelming the healthcare systems. Identification of systemic reactions underlying COVID-19 will lead to new biomarkers and therapeutic targets for monitoring and early intervention in this viral infection. We performed targeted metabolomics covering up to 630 metabolites within several key metabolic pathways in plasma samples of 20 hospitalized COVID-19 patients and 37 matched controls. Plasma metabolic signatures specifically differentiated severe COVID-19 from control patients. The identified metabolic signatures indicated distinct alter

Artificial intelligence in pituitary surgery: the path to clinical solutions.

Artificial intelligence (AI) is rapidly moving from conceptual innovation to high-performing algorithms across the pituitary patient pathway, promising benefits to patients, endocrinologists and surgeons alike. Pre-operatively, machine learning applied to facial imaging and natural language processing of electronic health records show potential for earlier identification of pituitary adenomas and timelier referral from primary providers to a specialist endocrinologist. Intraoperatively, computer vision systems have already augmented surgical training but have the potential to become integrated

Complicated multiple organ infection of Purpureocillium lilacinum and varicella-zoster virus infection in a patient with Evans' syndrome.

Purpureocillium lilacinum (P lilacinum) is a rare pathogenic fungus, which mainly involves immunocompromised individuals. Here, we report a case of complicated multiple-organ infections involving skin, lungs, and spleen in a 63-year-old female with Evans' syndrome after 9 months of glucocorticoid treatment. Microbial examinations of skin biopsy and blood samples revealed P lilacinum infections. Posaconazole was effective in this patient. During anti-fungi treatment, she developed varicella-zoster virus infection and was diagnosed through next-generation sequencing examination. In conclusion, P
