Postoperative evaluation of moyamoya disease with perfusion-weighted MR imaging: initial experience.

BACKGROUND AND PURPOSE: Encephaloduroarteriosynangiosis (EDAS) has become the main treatment for moyamoya disease, a chronically progressive cerebrovascular occlusive disease in children. We aimed to assess the utility of perfusion-weighted MR imaging for evaluating hemodynamic changes before and after EDAS. METHODS: Thirteen patients with angiographically confirmed moyamoya disease who underwent EDAS were investigated, and results were compared with those of a control group (n = 5). Perfusion MR imaging was performed before and after EDAS by using a T2*-weighted contrast material-enhanced te

Patterns of perfusion-weighted imaging in patients with carotid artery occlusive disease.

BACKGROUND: The importance of hemodynamic factors in the pathophysiology of stroke and transient ischemic attacks in patients with internal carotid artery (ICA) stenosis or occlusion remains controversial. OBJECTIVE: To investigate whether perfusion-weighted imaging (PWI) could identify pathophysiologically meaningful categories in patients with symptomatic and asymptomatic ICA occlusive disease. METHODS: Thirty-eight consecutive patients with occlusion (22 patients) or severe stenosis (16 patients) of the ICA, determined by ultrasonography or magnetic resonance angiography, were identified

Scapular hibernoma fortuitously discovered on myocardial perfusion imaging through Tc-99m tetrofosmin.

A 49-year-old man was hospitalized for precordial pain. The result of a resting electrocardiograph was normal. Tc-99m tetrofosmin stress myocardial imaging was performed and uptake in the area of the left scapula was seen. The abnormality was caused by a subcutaneous tumor. Histologic examination revealed a hibernoma.

[Diffusion- and perfusion-weighted MR imaging during the hyperacute phase of stroke].

The sensitivity of diffusion-weighted MR imaging to detect a lesion within 6 hours of stroke onset was approximately 90%. The false negative results were usually small lesions (1 ml), were seen early, and were usually located in the brain stem. The specificity of this technique was nearly 100% when it was used correctly. The volume and the value of the apparent diffusion coefficient of the detected lesions provided prognostic information. After injection of a contrast agent (perfusion imaging), a time series of volumes were obtained using a T2* sensitive gradient echo EPI sequence. Hemodynamic

Evaluation of carotid endarterectomy with sequential MR perfusion imaging: a preliminary report.

BACKGROUND AND PURPOSE: Current indications for carotid endarterectomy are determined by balancing the relative risks of surgery with the benefits of reduced risk of subsequent stroke. Our purpose was to use MR perfusion imaging to assess patients being considered for carotid endarterectomy and to monitor sequential changes in MR perfusion characteristics after surgery. In particular, we wished to determine whether this technique could be used to detect changes that might be related to post-carotid endarterectomy hyperemia. METHODS: We used a single-section gradient-recalled echo sequence to

Atraumatic quantitation of cerebral perfusion in cats by 19F magnetic resonance imaging.

We have noninvasively produced low-resolution, quantitative nuclear magnetic resonance images of cerebral blood flow in 2-ml voxels in eight cats. Typical signal-to-noise of 4 to 1 was obtained in cerebral voxels in 16.5-s epochs. Mean flow during normocapnia (paCO2 = 39 +/- 4 mm Hg) and hypercapnia (paCO2 = 62 +/- 4 mm Hg) was 53 +/- 20 ml/100 g-min and 140 +/- 36 ml/100 g-min, respectively. Fast flows in normocapnia were 94 +/- 13 and 182 +/- 39 ml/100 g-min in hypercapnia. These results suggest that an atraumatic quantitative imaging assessment of cerebral perfusion may be possible in human

Pancreatic grafts. Nuclear perfusion imaging to detect vascular complications and rejection crises.

Perfusion studies with 99m-Tc-DTPA were used routinely to investigate renal grafts. Efforts were made to employ this technique in monitoring the perfusion of pancreatic grafts. A total perfusion failure is as reliably detectable as in renal grafts. Smaller perfusion alterations could be demonstrated by follow up studies. It appears to be feasible to differentiate the salivary edema, well known from animal experiments, from a rejection reaction with the help of other parameters (e.g. creatinine). Further clinical studies, however, are necessary to confirm these results.

Perfusion imaging abnormalities in acute cerebral venous sinus thrombosis: A case series.

The clinical value of perfusion imaging in arterial ischemic stroke is well established, but its interpretation in cerebral venous sinus thrombosis (CVST) remains less clearly defined. We present a case series of five patients with acute CVST who demonstrated perfusion abnormalities on initial CT perfusion (CTP) or MR perfusion (MRP). The observed findings demonstrated a spectrum of venous hemodynamic patterns. Several patients showed delayed transit metrics, including increased MTT or Tmax, with preserved or increased CBV, suggesting venous congestion or vasogenic edema rather than arterial-t

Fluorescence imaging for assessing tissue perfusion after revascularization in peripheral arterial disease.

Methods for assessing peripheral arterial disease (PAD) include ankle-brachial index (ABI) and transcutaneous oxygen pressure (TcPO2) measurements. Fluorescence imaging with indocyanine green (FI) has been proposed as a promising tool for tissue perfusion assessment, despite lack of robust evidence due to small study cohorts or missing comparison of methods. In this prospective study, patients underwent FI, TcPO2, and ABI measurements before and after revascularization. FI quantification included slope of fluorescence intensity (SFI), background-subtracted peak fluorescence intensity (BSFI), a

Perfusion stability in acute stroke: An observational study exploiting repeated CTP imaging.

BACKGROUND AND PURPOSE: CT perfusion (CTP) is widely used to assess infarct core in acute stroke, yet real-world data on its reproducibility and temporal dynamics are limited. MATERIALS AND METHODS: We retrospectively identified patients with repeated CTP scans. Core and hypoperfusion volumes were quantified using standard thresholds (CBF <30 %, Tmax >6 s). Clinical and imaging data were reviewed to identify cases with disruptive events. We analyzed scan-to-scan differences in core volume, hypoperfusion volume, ASPECTS, and intensity metrics, including median Tmax (in hypoperfusion), CBF, and

18F-Flurpiridaz PET in myocardial perfusion imaging: comparing effectiveness against traditional tracers.

Myocardial perfusion imaging (MPI) has served as a cornerstone of coronary artery disease (CAD) evaluation for over four decades, including many other cardiac diseases. MPI is extremely valuable for both diagnostic and prognostic purposes in clinical environments, enabling clinicians to evaluate a patient's coronary health. Perfusion scanning utilizes two primary imaging techniques: single-photon emission computed tomography (SPECT) and positron emission tomography (PET). Each relies on radioactive tracers, which are radioactive substances injected into a patient that spread across myocardial

Rapid noncontact laser speckle imaging for perfusion-based differentiation of wound necrosis.

Traditional wound assessment relies on subjective visual inspection, which may miss early microvascular changes. In this study, laser speckle contrast imaging (LSCI) was used to differentiate necrotic from nonnecrotic wounds using quantitative perfusion under a shortened protocol. Forty wound patients (20 acute, 15 chronic, and 5 necrotic) and 20 participants without wound lesions were included. Within-subject normalized perfusion (percent change relative to adjacent healthy skin) served as the predictor. Significant differences were observed among wound types (ε 2 = 0.23-0.40, moderate-to-lar

Enhancing Hand Vascular Assessment: The Role of Hyperspectral Imaging in Perfusion Monitoring.

PURPOSE: Accurate assessment and monitoring of tissue perfusion are critical in diagnosing and managing vascular diseases of the hand. Traditional methods, including clinical examination, pulse oximetry, and Doppler ultrasound, have limitations in detecting early or subtle perfusion deficits. Hyperspectral imaging (HSI) is a noninvasive, real-time technology that generates quantitative perfusion maps by measuring oxygen saturation, oxyhemoglobin, and deoxyhemoglobin levels. To the best of our knowledge, this is the first study of its kind to evaluate the clinical use of HSI in assessing perfus

Real-Time Bladder Perfusion Assessment Using ICG Fluorescence Imaging Enables Successful Reconstruction.

INTRODUCTION: We report a case where intraoperative indocyanine green (ICG) imaging was utilized to evaluate bladder perfusion, enabling successful bladder preservation and bilateral ureteral reimplantation. CASE PRESENTATION: A 49-year-old woman underwent resection of a recurrent pelvic tumor. The tumor involved both ureters and bilateral vesical arteries, which were sacrificed during surgery. ICG fluorescence imaging demonstrated uniform bladder wall perfusion, guiding the decision to proceed with bladder preservation. Postoperatively, contrast-enhanced CT and cystoscopy confirmed preserved

Pre-transplant kidney quality evaluation using photoacoustic imaging during normothermic machine perfusion.

Due to the shortage of kidneys donated for transplantation, surgeons are forced to use the organs with an elevated risk of poor function or even failure. Although the existing methods for pre-transplant quality evaluation have been validated over decades in population cohort studies across the world, new methods are needed as long as delayed graft function or failure in a kidney transplant occurs. In this study, we explored the potential of utilizing photoacoustic (PA) imaging during normothermic machine perfusion (NMP) as a means of evaluating kidney quality. We closely monitored twenty-two p

Myocardial Perfusion Imaging by Cardiovascular Magnetic Resonance: Research Progress and Current Implementation.

Cardiovascular diseases pose a significant health and economic burden worldwide, with coronary artery disease still recognized as a major problem. It is closely associated with hypertension, diabetes, obesity, smoking, lack of exercise, poor diet, and excessive alcohol consumption, which may lead to macro- and microvascular abnormalities in the heart. Coronary artery stenosis reduces the local supply of oxygen and nutrients to the myocardium and results in reduced levels of myocardial perfusion, which can lead to more severe conditions and irreversible damage to myocardial tissues. Therefore,

Clinical application of brain perfusion imaging in detecting stroke mimics: A review.

Stroke mimics constitute a significant proportion of patients with suspected acute ischemic stroke. These conditions may resemble acute ischemic stroke and demonstrate abnormalities on perfusion imaging sequences. The most common stroke mimics include seizure/epilepsy, migraine with aura, brain tumors, functional disorders, infectious encephalopathies, Wernicke's encephalopathy, and metabolic abnormalities. Brain perfusion imaging techniques, particularly computed tomography perfusion and magnetic resonance perfusion, are being widely used in routine clinical practice for treatment selection i

Mapping of Perfusion During Full-Thickness Blepharotomy Using Laser Speckle Contrast Imaging.

PURPOSE: The aim of this study was to monitor how the blood perfusion in human upper eyelids is affected during full-thickness blepharotomy. METHODS: Seven eyelids in 5 patients with upper eyelid retraction due to Graves' disease underwent full-thickness blepharotomy. Perfusion was measured using laser speckle contrast imaging in the eyelid margin and in the conjunctival pedicle. RESULTS: Immediately following the procedure, a nonsignificant reduction in perfusion was observed in the skin of the pretarsal eyelid margin, being 66% of the initial value ( p = n.s.). However, a statistically sig
