[Biopsy of the pericardium. Diagnostic value in subacute and chronic pericarditis, apropos of 70 cases].

The aetiology of pericarditis is often difficult to assess. To try to clear up this problem, a pericardial biopsy through a left lateral thoracotomy was performed in 70 cases in which the cause could not be established by the usual means (including pericardial paracenthesis in 32 cases). They included either subacute pericarditis, dry or with effusion (biopsy being undertaken as an average 45 days after the clinical onset), or chronic pericarditis with effusion. Tuberculosis was thus demonstrated in 14 cases (8 of which were aged more than 60 years), inaugural pericardial tumours in 8 and supp

Likely tuberculous myocarditis mimicking an acute coronary syndrome.

Acute systemic infections may involve the heart, mostly represented by myocarditis and pericarditis. We report the case of a likely myopericarditis in an adult, leading to the diagnosis of tuberculosis infection. The clinical presentation was an acute coronary syndrome with elevated troponin Ic. An alternative diagnosis of myopericarditis was considered. Chest X-ray depicted a miliary pattern and a CT-scan demonstrated bilateral micronodules with a "tree-in-bud" pattern associated with parenchymal consolidations in the apical segment of the left upper lobe, suggesting infectious bronchiolitis.

Adenosine Deaminase Diagnostic Testing in Pericardial Fluid.

A 65-year-old Vietnamese man with hypertension, type 2 diabetes mellitus, and chronic hepatitis B with cirrhosis presented with a 2-week history of shortness of breath at rest, orthopnea, and lower extremity edema. He reported a 4-month history of nonproductive cough, 5-kg weight loss, and fatigue. He immigrated to the United States as an adult more than 20 years before presentation. His temperature was 37°C, heart rate was 78/min, respiratory rate was 17/min, and blood pressure was 158/95 mm Hg. A chest radiographic image suggested cardiomegaly and a computed tomographic scan demonstrated a m

Tuberculosis in the intensive care unit.

The overwhelming majority of patients infected with tuberculosis may be very adequately treated without hospitalization on an out-patient basis. Until tuberculosis is completely eradicated, however, some patients with extensive disease will require hospitalization for diagnosis and the institution of appropriate chemotherapy. A minority of these who are desperately ill and require admission to an intensive care unit may still recover by the prompt diagnosis of the illness, appropriate supportive care, and the rapid institution of appropriate antituberculous chemotherapy.

Angiosarcoma - a rare fatal cause of recurrent pericardial effusions.

A 40-year-old man presented to the Hospital Sultanah Bahiyah, Alor Setar, Kedah, with constitutional and respiratory symptoms. Physical examination and echocardiogram demonstrated massive pericardial effusion. Patient required multiple attempts of pericardiocentesis due to recurrent pericardial effusion. Initial workup including pericardial fluids examination and computed tomography imaging did not reveal any apparent cause. Magnetic resonance imaging showed a suspicious mass infiltrating into the right atrium. Autoimmune screening was negative. Patient was subsequently treated as having tuber

Diagnosis and management of pericardial effusion.

Pericardial effusion is a common finding in everyday clinical practice. The first challenge to the clinician is to try to establish an etiologic diagnosis. Sometimes, the pericardial effusion can be easily related to a known underlying disease, such as acute myocardial infarction, cardiac surgery, end-stage renal disease or widespread metastatic neoplasm. When no obvious cause is apparent, some clinical findings can be useful to establish a diagnosis of probability. The presence of acute inflammatory signs (chest pain, fever, pericardial friction rub) is predictive for acute idiopathic pericar

Tuberculous pericardial disease: a focused update on diagnosis, therapy and prevention of complications.

Tuberculous pericarditis (TBP) is the most important manifestation of tuberculous heart disease and is still associated with a significant morbidity and mortality in TB endemic areas. The high prevalence of the disorder over the last 3 decades has been fueled by the human immunodeficiency virus/AIDS (HIV/AIDS) pandemic in these areas. The objective of this review is to provide a focused update on developments in the diagnosis and therapy of this condition, prevention of its complications, as well as future novel therapies. The definitive diagnosis of a tuberculous etiology in patients with sus

Cardiac manifestations of HIV infection: an African perspective.

The pericardium, myocardium, coronary arteries and pulmonary arteries are the main targets for cardiac disease in people who are infected with HIV. Geography and access to highly active anti-retroviral therapy (HAART) have a major influence on which of these targets is affected. In sub-Saharan Africa, where tuberculosis is endemic and access to HAART is limited, the dominant forms of HIV-associated heart disease are pericardial tuberculosis and cardiomyopathy. However, in industrialized countries, where tuberculosis is rare and HAART is widely available, coronary artery disease is the main cau

[Thanatogenesis of respiratory tuberculosis under the present conditions].

Autoptic findings of 120 patients with respiratory tuberculosis indicate that under the present-day conditions, the prevalent cause of death was a progressive tuberculous process as caseous pneumonia (89% of cases), which is much higher than our previous findings (42.9% in 1989 versus 66.% in 1994). Caseous pneumonia chiefly resulted from fibrous-cavernous tuberculosis, less frequently from an independent entity or manifestations of disseminated tuberculosis. Fibrous-cavernous tuberculosis, the major clinical form in the deceased, was frequently associated with specific involvement of intratho

Cardiac Tamponade Secondary to Massive Pericardial Effusion in Severe Primary Hypothyroidism: A Case Report.

BACKGROUND: Pericardial effusion is a recognized manifestation of overt hypothyroidism, but progression to cardiac tamponade is rare. In tuberculosis-endemic settings, diagnostic focus often favors tuberculous pericarditis, risking delayed recognition of reversible endocrine causes. CASE PRESENTATION: A 65-year-old Black African woman from Northern Uganda presented with five months of progressive dyspnea, orthopnea, and generalized edema, followed by acute pleuritic chest pain. She had a history of hypertension and type 2 diabetes mellitus. No prior thyroid disease, tuberculosis, malignancy,

Pulmonary tuberculosis and tuberculous pleuropericarditis in a patient with pyoderma gangrenosum treated with adalimumab: a case report.

BACKGROUND: Adalimumab is a tumor necrosis factor-α inhibitor, which is associated with an increased risk of tuberculosis. However, evidence regarding tuberculosis development in patients with pyoderma gangrenosum is limited, and simultaneous involvement in lungs, pleura, and pericardium is rare. This report describes a case of pulmonary tuberculosis and tuberculous pleuropericarditis associated with adalimumab administration. CASE PRESENTATION: The patient was a 59-year-old Filipino female. She began receiving adalimumab to treat pyoderma gangrenosum at the dermatology department of our hosp

Tuberculosis on regular hemodialysis--a case of pericardial tamponade.

The patient presented in this paper had been stable for 3 months after the induction of hemodialysis, when nausea, vomiting and hepatomegaly suddenly developed. A chest film revealed rush cardiomegaly, and massive pericardial effusion was demonstrated by echocardiography. One liter of hemorrhagic fluid was removed by pericardiocentesis and subsequent pericardial drainage under echocardiography. The patient received chemotherapy against pulmonary tuberculosis 30 years ago and calcification on chest film was apparent. Although sputum smear and pericardial effusion was negative for acid-fast orga

Aflatoxins in the lungs of children with kwashiorkor and children with miscellaneous diseases in Nigeria.

Autopsy lung specimens from 20 children with kwashiorkor and 20 with other miscellaneous diseases, at the Obafemi Awolowo Teaching Hospital complex, Ile-Ife, Nigeria, were analyzed for the presence of aflatoxin using high-performance liquid chromatography. Aflatoxins were detected in 18 children who died from kwashiorkor but only in 13 of those who died from miscellaneous diseases. Of the 10 children, 5 in each group, who died with pneumonia, all had detectable levels of aflatoxins in their lungs. The two children with congestive cardiac failure, one secondary to pneumonia and the other second

Impact of Socioeconomic Status, Ethnicity, and Urbanization on Risk Factor Profiles of Cardiovascular Disease in Africa.

Africa is a continent characterized by marked ethnic, sociodemographic, and economic diversity, with profound changes in many regions over the past 2 decades. This diversity has an impact on cardiovascular disease presentation and outcomes. Within Africa and within the individual countries, one can find regions having predominantly communicable diseases such as rheumatic heart disease, tuberculous pericarditis, or cardiomyopathy and others having a marked increase in noncommunicable disease such as hypertension and hypertensive heart disease. Ischemic heart disease remains rare in most countri

Ethiopian cardiovascular studies. Case-finding by mass miniature radiography.

No large series of patients with cardiovascular disease has yet been reported from Ethiopia, where only limited means for investigation are at present available. The authors therefore studied the types of heart disease detected by mass miniature radiography in a largely self-selected population at the Addis Ababa Tuberculosis Centre, and examined the value of this method of cardiac case-finding. Rheumatic heart disease occurred in 34.8% of patients, but syphilitic aortitis, hypertension, "cardiomyopathy" and tuberculous pericarditis were also common. Endomyocardial fibrosis was not seen; this

A Comprehensive Review of Effusive-Constrictive Pericarditis, Diagnosis, and Management.

Effusive-constrictive pericarditis (ECP) is characterized by fluid accumulation in the pericardial space and a rigid, fibrotic pericardium that restricts heart filling. Its diverse causes include infectious agents, systemic inflammatory conditions, malignancies, and iatrogenic factors. ECP is more prevalent in areas burdened by contagious diseases, such as tuberculous pericardial effusion, and it is found in about 6.7% of patients with tuberculous pericardial effusion. The diagnosis of ECP has improved with advanced imaging techniques, yet challenges persist. Although Doppler echocardiography
