Preventable feto-infant mortality: application of a conceptual framework for perinatal health surveillance to Manitoba perinatal outcomes.

BACKGROUND: Perinatal health surveillance systems have lacked conceptual frameworks to translate data into information for policy and program planning. This paper demonstrates the application of a conceptual framework in the analysis of feto-infant mortality data in the province of Manitoba. METHODS: Fetal and infant deaths were categorized according to a two-dimensional framework of birthweight and age-at-death, and grouped into four broad categories of contributors to perinatal health: Maternal Health, Maternal Care, Newborn Care, and Infant Care. Birth Weight Proportionate Mortality Rates

[Differences in perinatal mortality between provinces: dependence on many factors].

The Dutch perinatal mortality rate is relatively high when viewed in a European perspective. There appear to be also substantial differences in perinatal mortality between provinces, large cities and even between neighbourhoods. Accumulation of obstetric risk factors as well as socioeconomic and urban risks seems to be involved as well. This should be taken into account in the process of risk selection to define a high risk population needing hospital care. In 85% of all cases of perinatal mortality, one or more of the following perinatal morbidities are present, designated as the 'Big 4': con

Evaluating perinatal mortality rates: effects of referral and case mix.

OBJECTIVE: To evaluate perinatal mortality rates as a method of auditing obstetric and neonatal care after account had been taken of transfer between hospitals during pregnancy and case mix. DESIGN: Case-control study of perinatal deaths. SETTING: Leicestershire health district. SUBJECTS: 1179 singleton perinatal deaths and their selected live born controls among 114,362 singleton births to women whose place of residence was Leicestershire during 1978-87. MAIN OUTCOME MEASURE: Crude perinatal mortality rates and rates adjusted for case mix. RESULTS: An estimated 11,701 of the 28,750 women

Perinatal mortality in Victoria, Australia: role of group B Streptococcus.

Group B beta-hemolytic streptococcus is the most common infective cause of neonatal morbidity and mortality. It is therefore surprising that no agreement exists for an approach to its prevention. There is also increasing evidence that occult infection may play an etiologic role in premature rupture of the membranes and preterm labor. In this report we review the role of group B beta-hemolytic streptococcal sepsis as a cause of perinatal wastage in the state of Victoria, Australia during the period 1982 to 1987. Group B beta-hemolytic streptococcus accounted for 1.7% of overall perinatal deaths

Parity specific perinatal mortality. A longitudinal study based on sibships.

Perinatal mortality in sibships has been examined using the Medical Birth Registry of Norway. Using linear logistic regression, parity specific perinatal mortality in the period 1967-1981 has been analysed, controlling simultaneously for maternal age, year of birth and survival of earlier births. The risk of a perinatal loss was increased by a factor of up to 6.0 following one earlier perinatal loss, and with three earlier losses the increase was 17.0. The general reduction in mortality between the different five year periods varied according to parity and maternal age. No secular reduction in

Trends of stillbirth, early neonatal and perinatal mortality rates and sociodemographic differentials of perinatal mortality rates in 17 low- and middle-income countries: A modeling study.

BACKGROUND: Stillbirths and early neonatal deaths account for a substantial proportion of global child mortality. Reliable measurement of these outcomes remains challenging in settings where civil registration systems are incomplete and household surveys are the primary data source. However, survey-based estimates are often affected by underreporting and misclassification between stillbirths and early neonatal deaths. We aimed to apply a statistical adjustment approach to improve estimates of stillbirth, early neonatal, and perinatal mortality trends and to assess socioeconomic inequalities in

Racial and ethnic disparities in perinatal mortality: applying the perinatal periods of risk model to identify areas for intervention.

OBJECTIVES: To determine the feto-infant mortality rate for New York City, assess racial/ethnic variations and identify areas for intervention using the Perinatal Periods of Risk (PPOR) approach. METHODS: The PPOR model examines fetal and infant deaths by age at death (fetal, neonatal, postneonatal) and birthweight (500-1499, > or =1500 g). It groups age at death and birthweight into four categories to identify problems hypothesized to lead to the death: factors related to Maternal Health and Prematurity, Maternal Care, Newborn Care and Infant Health. The model was applied to fetal and infant

[A statistical study of the countermeasures to reduce the maternal mortality based on the relationship between the maternal mortality rate and perinatal mortality rate].

Although both the Japanese maternal mortality rate (MMR) and the perinatal mortality rate (PMR) have decreased remarkably in recent decades, the former is still high compared with other advanced countries. A statistical analysis on the relationship between the MMR and the PMR was performed to investigate this discrepancy. During the period of study, both rates fell by over 50 percent. There were definite statistical differences among the PMR for each prefecture but almost none among the MMR. The prefectures with high PMR remained generally high in the PMR and the prefectures with low PMR also

Perinatal mortality in eastern Africa: A systematic review and meta-analysis.

BACKGROUND: Every day, over 5000 stillbirths and more than 6000 newborn deaths are estimated to occur worldwide, the majority in sub-Saharan Africa and South Asia. Estimates, however, rely on data that might underestimate these deaths. Further, outside major categories such as preterm birth, infection, and complications at delivery, little is known about the causes of death for newborns and even less for stillbirths. OBJECTIVES: This systematic review and meta-analysis aimed to synthesize evidence on the incidence, causes, and risk factors for perinatal mortality in East Africa to inform publ

An investigation into the history of maternal risk factors in the incidence of perinatal mortality.

ObjectivesThe rate of perinatal mortality is an important indicator of maternal and neonatal health. Identification of risk factors for perinatal mortality may be effective in its prevention. Accordingly, this study aimed to identify maternal risk factors of perinatal mortality.MethodsA cross-sectional study was conducted on 547 stillbirths and 709 neonatal deaths at Ghaem Hospital of Mashhad, Iran, during 2008-2019. The data were collected using a researcher-made questionnaire in two parts of stillbirth (maternal diseases, main complication, and final diagnosis) and specifications of neonatal

Death audits and reviews for reducing maternal, perinatal and child mortality.

BACKGROUND: The United Nations' Sustainable Development Goals (SDGs) include reducing the global maternal mortality rate to less than 70 per 100,000 live births and ending preventable deaths of newborns and children under five years of age, in every country, by 2030. Maternal and perinatal death audit and review is widely recommended as an intervention to reduce maternal and perinatal mortality, and to improve quality of care, and could be key to attaining the SDGs. However, there is uncertainty over the most cost-effective way of auditing and reviewing deaths: community-based audit (verbal an

Perinatal mortality audits and reviews: Past, present and the way forward.

Perinatal deaths are devastating for families and staff involved. Failure to examine perinatal deaths for substandard care prevents learning and may lead to recurrence of events, as well as prolonged morbidity in bereaved families and hospital staff. Perinatal mortality reviews can identify factors contributing to suboptimal care. An integrative literature review was carried out to study the different types of perinatal mortality reviews currently being done internationally, establishing a comparison and examining promising new developments. We start by outlining issues with the classification
