INTRODUCTION: Extrapontine myelinolisis is a rare condition caused by severe hydroelectrolytic disorders. Following an initial stage of diffuse encephalopathy, the neurological sequelae are usually of diffuse cognitive deficits and extrapyramidal or cortico-bulbo-spinal disorders. We report a case of extrapontine myelinolisis following hypertonic peritoneal lavage due to rupture of a hydatid cyst of the liver (HCL), which was followed by a syndrome of isolated amnesia due to bilateral hippocampal lesions.

A study of 406 consecutive children operated upon for appendicitis from July 1982 to July 1987 was compared with a previously published study of 657 children with the same diagnosis operated upon between 1972 and 1982. The prognostic value of three predictive indices was compared in 436 attacks of acute pancreatitis in 418 patients. INTRODUCTION: Today the management of the different forms of peritonitis is generally standardised. The classification of primary and secondary peritonitis is well accepted. From a pathophysiological point of view, postoperative and post-traumatic peritonitis should be considered as independent entities. Hydatid disease or echinococcosis is caused by a tapeworm of the genus Echinococcus. Several cases of hepatic hydatid cysts are reported worldwide, but disseminated peritoneal hydatidosis with spontaneous rupture of cysts leading to ascites is a rare entity, and only a few instances in literature can be found. We would like to report case of disseminated peritoneal hydatidosis, which was secondary to the rupture of the primary hepatic hydatid cyst. Midline laparotomy with partial pericystectomy with removal of daughter cyst and peritoneal lavage was done.

Bile peritonitis after injury to the biliary tree is a serious complication that requires exploratory laparotomy. Our patient had an obstructing ampullary carcinoma, and generalized bile peritonitis developed from attempted percutaneous transhepatic cholangiography. The patient's condition was managed by peritoneal lavage and endoscopic transampullary stenting, with immediate relief of pain and toxicity. Exploratory laparotomy was avoided, and an eventual pylorus-sparing Whipple resection was the definitive treatment. We believe this to be the first report of successful nonoperative treatment
