The analysis of symptom checklists completed by 3196 patients before treatment--1970 females and 1226 males starting the therapy due to neurotic disorders--revealed that 12 symptoms occur im more than 80% of patients; 9 symptoms in 70-80%, 18 dysfunctions in 70-60% and 17 other dysfunctions in 60-50% of examined population. This means that a large number of symptoms including tension, mood depression an anxiety as well as difficulties in concentration, lack of self-confidence, sense of tiredness, loss of energy, persistent thoughts and images, constant fear, absent-mindedness, motor tensions,

Bipolar disorder (BD) and trauma symptoms commonly co-occur and present a high rate of suicidality. Prolonged exposure (PE) is a well-established and efficacious treatment for symptoms of PTSD, untested in patients with co-occurring BD but promising in its effectiveness for reducing suicidal thoughts and behaviors. The current study evaluates the preliminary efficacy of PE in reducing suicide risk in patients with co-occurring BD and trauma symptoms. Participants (N = 32) were enrolled in a 10-session, weekly PE program and completed assessments of trauma symptoms, suicidality, state and trait

Tinnitus, the phantom perception of sounds, generates distress and anxiety in those affected. Cognitive behavioral treatment approaches reproducibly help patients in managing chronic tinnitus. This study systematically evaluated the usefulness of a tinnitus app (with areas of attention and relaxation, mindfulness, acceptance, self-efficacy), which is prescribed for a total of nine months. One hundred eighty-seven participants with chronic tinnitus were equally randomized to an intervention arm that used a smartphone-based intervention -marketed as Kalmeda Tinnitus app-. and a control arm with

The aim of this paper is to reflect on the way that we as clinicians may play an inadvertent role in perpetuating eating disordered behaviour. This is considered within the theoretical framework of Schmidt and Treasures' maintenance model of anorexia nervosa (AN). The model includes four main domains; interpersonal factors, pro-AN beliefs, emotional style and thinking style. Interpersonal reactions are of particular relevance as clinicians (as with family members) may react with high expressed emotion and unknowingly encourage eating disorder behaviours to continue. Hostility in the form of co

BACKGROUND: Satisfactory results have been reported from long-term studies on bipolar hemiarthroplasty for femoral neck fracture. However, long-term results of this procedure for osteoarthritis of the hip (OA) and idiopathic necrosis of the femoral head (ION) have given rise to pessimism. These poor results have often reported migration of the bipolar head, but few had described the direction of this migration. The purpose of the present study was to conduct a long-term follow-up analysis of bipolar hemiarthroplasty for OA and ION, including the direction of migration.
METHODS: We retrospecti

BACKGROUND: Until now therapy of carcinoma of the esophagus has presented an aura of pessimism, resulting in an attitude among physicians that cure was impossible. Presently the overall 5-year survival is between 38.3% and 55% thanks to new radical surgical techniques. The aim of this work is to evaluate lymph node dissection in treatment of esophageal carcinoma by analyzing morbidity, mortality, survival and quality of life.
METHODS: From 1975 to 1995, 170 patients with carcinoma of the esophagus and cardia underwent operation, of whom 165 underwent 2-field lymphadenectomy and four 3-field l

OBJECTIVE: This study examines the effects of an applet-based Acceptance and Commitment Therapy focused on Repetitive Negative Thinking (RNT-ACT) for adults with comorbid anxiety and depression, and explores the mediating role of Repetitive Negative Thinking (RNT) in its efficacy.
METHODS: In a randomized controlled trial, participants with the co-occurrence of elevated anxiety and depressive symptoms (GAD-7 and PHQ-9 scores ≥10; N = 94) were randomly assigned to either the RNT-ACT intervention group (N = 49) or a wait-list control group (N = 45). The intervention group completed a 14-day onl

OBJECTIVE: Insomnia and repetitive negative thinking (RNT) are both prevalent among cancer survivors, yet little work has investigated their interrelationship. To explore the hypothesis that RNT and insomnia are related, we conducted secondary analyses on data from a pilot clinical trial of cognitive behavioral therapy for insomnia (CBT-I) for cancer survivors.
METHODS: This study analyzed survey data from 40 cancer survivors with insomnia who participated in a pilot randomized trial of CBT-I. Correlations and linear regression models were used to determine associations between aspects of RNT

Repetitive negative thinking (RNT) is a maladaptive response to sadness and a transdiagnostic risk-factor. A critical challenge hampering attempts to promote more adaptive responses to sadness is that the between-person characteristics associated with the tendency for RNT remain uncharacterized. From the perspective of the impaired disengagement hypothesis, we examine between-person differences in blood-oxygen-level-dependent (BOLD) functional networks underlying cognitive conflict signaling, self-referential thought, and cognitive flexibility, and the association between sadness and RNT in da

Co-rumination has been shown advantageous for friendship quality, but disadvantageous for mental health. Recently, two components have been distinguished, with co-brooding predicting increases in depressive symptoms and co-reflection decreases. The current study aimed to replicate these findings and investigated whether both components also show differential relations with friendship quality. Gender was investigated as a moderator. Path analyses were used on data of 313 adolescents aged 9-17 (50.5% girls). Co-brooding was related to more concurrent and prospective depressive symptoms in girls.

Previous research indicates that sleep difficulties and stress are prevalent in college students. The main aim of the current study was to investigate the role of repetitive negative thinking (RNT), cognitive emotion regulation, and negative affect as mediators of the relationship between stress and sleep difficulties. We also intended to analyse the associations between all these variables and the gender differences. A sample of 549 college students completed self-report measures assessing the mentioned variables. Descriptive and correlational analyses showed significant differences between g

Although considerable evidence has linked sleep disturbance to symptoms of psychopathology, including repetitive negative thinking, few studies have examined how sleep disturbance may predict repetitive negative thinking over time. Further, no study to date has examined specific mechanisms that may account for this relationship. The present study sought to address these gaps in the literature by testing focusing and shifting attentional control as two potential mediators of the relationship between sleep disturbance and repetitive negative thinking over a 6-month period. A final sample of 445

Waiting for uncertain news is often distressing, at times even more distressing than facing bad news. The goal of this article was to investigate strategies for "waiting well" during these periods of uncertainty. Specifically, we propose 2 definitions of waiting well. First, people can wait in such a way as to ease their distress during the waiting period. Second, people could wait in such a way as to ease the pain of bad news or enhance the thrill of good news. We conducted a longitudinal study of law graduates (N = 230) awaiting their result on the California bar exam. Participants completed

OBJECTIVES: The Social Identity Model of Identity Change (SIMIC) provides a framework for understanding how social identities support adjustment to major life transitions. We examined whether the number of social identities predicts psychological wellbeing among older adults living with dementia.
METHODS: Using a mixed-methods design, we analyzed 245 blog posts written by 49 people with dementia (25 men, 24 women).
RESULTS: Bloggers who referenced a greater number of social identities in their online narratives reported higher positive wellbeing and greater empowerment. This association betw

Background and Objectives: Static prognostication in comatose out-of-hospital cardiac arrest (OHCA) survivors may overlook delayed recovery, risking premature withdrawal of life-sustaining therapy (WLST). This study aimed to identify distinct longitudinal phenotypes of consciousness recovery and determine the prevalence and characteristics of the Late Awakener phenotype. Materials and Methods: We applied K-means clustering to serial Glasgow Coma Scale motor scores (0, 24, 48, 72 h, Day 5) in 417 adult OHCA survivors treated with targeted temperature management at Seoul St. Mary's Hospital (200

OBJECTIVES: Lamotrigine is used to treat bipolar depression despite inconsistent evidence. Here we present the results of an exploratory item-level analysis of pooled data from five randomized placebo-controlled trials of lamotrigine for acute bipolar depression. The goal was to determine if certain depression scale items were more responsive to lamotrigine treatment.
METHODS: The pooled sample contained 1072 adult outpatients treated for up to 7-10 weeks. Depressive symptoms were measured with the Hamilton Depression Rating Scale and the Montgomery-Åsberg Depression Rating Scale. Change scor

Engel (1959) suggested that negative physical or emotional experiences in childhood predispose to the development of chronic pain. Studies have shown that physical and sexual abuse in early life is connected with chronic pain. Emotional adversities are much less studied causes contributing to the development of chronic pain and disability. Early emotional abuse, neglect, maltreatment and other adversities are deleterious childhood experiences which, according to Young's schema theory (1990), produce early maladaptive schemas (EMSs). The primary goal of this study was to examine whether early a

INTRODUCTION: Old age depression is difficult to treat. It is presumed that the underlying biochemical mechanism differs from that of depression of younger age. Clinical manifestations also differ, as elderly people manifest more irritability, anxiety, conduct and cognitive disorders than pessimism or guilt. Response to treatment is believed to be poorer than in younger patients.
METHOD: Fifty patients, with a mean age of 66.65, suffering from major depression or dysthymia according to DSM-IV criteria, took part in the study. Their mean HDRS score was 16.77. They were prescribed 100 mg of flu

Autistic people are more likely to have mental health problems than non-autistic people. We know that having repetitive and negative thoughts can contribute to multiple mental health problems such as depression and obsessive-compulsive disorder. Autistic people often do the same behaviours repetitively, and they may also have more repetitive thinking styles. This could contribute to higher rates of mental health problems in autistic people. In this research, we wanted to find out if higher rates of repetitive behaviours contributed to depression and anxiety, and whether this relationship was b
