Posterior tibial tendinopathy associated with matrix metalloproteinase 13 promoter genotype and haplotype.

BACKGROUND: Posterior tibial tendon (PTT) is particularly vulnerable and its insufficiency is recognized as the main cause of adult acquired flat foot. Some patients have a predisposition without a clinically recognized cause, suggesting that individual characteristics play an important role in tendinopathy. The present study investigated whether genetic variants in matrix metalloproteinases (MMPs) are associated with PTT dysfunction. METHODS: One hundred women who presented PTT dysfunction, with histopathological examination of the tendon and magnetic resonance imaging (MRI) confirming tendi

Effect of eccentric exercise program for early tibialis posterior tendinopathy.

BACKGROUND: Morphology and vascularization of painful tibialis posterior (TP) tendons before and after an intervention targeting the degenerated tendon were examined. Functional status and pain level were also assessed. MATERIALS AND METHODS: A10-week twice daily, progressive eccentric tendon loading, calf stretching program with orthoses was implemented with ten, early stage TP tendinopathy subjects. TP tendons were imaged by grayscale and Doppler ultrasound at INITIAL and POST evaluations to assess the tendon's morphology and signs of neovascularization. The Foot Functional Index (FFI), Phy

The relationship of hip muscle performance to leg, ankle and foot injuries: a systematic review.

OBJECTIVES: Hip control affects movement and muscle firing patterns in the leg, ankle and foot, and may contribute to overuse injuries. Muscle performance can be measured as strength, endurance or muscle activation patterns. Our objective was to systematically review whether hip muscle performance is associated with leg, ankle and foot injuries. DATA SOURCES: A structured and comprehensive search of six medical literature databases was combined with forward and backward citation tracking (AMED, CINAHL, EMBASE, Medline, Scopus and SportDiscus). STUDY SELECTION: Eligible studies measured hip m

So What Exactly Is Progressive Collapsing Foot Deformity?

Progressive collapsing foot deformity is a new nomenclature coined by a group of expert foot and ankle orthopaedic surgeons who published numerous articles on adult flatfoot deformities. The consensus statements were published in 2020 due to concern that traditional terminologies such as "posterior tibial tendon dysfunction," "adult acquired flatfoot deformity," and numerous others could be confusing to physicians/health care professionals and do not adequately encompass the deformity. The consensus group also designed a new classification system to simplify the decision-making process that co

Isolated subtalar arthrodesis.

Forty-eight isolated subtalar arthrodeses in 44 patients with an average follow-up of 59.5 months were retrospectively reviewed. Original diagnoses included talocalcaneal coalition, healed calcaneal fracture with subtalar arthrosis, acquired flatfoot because of posterior tibial tendon dysfunction, degenerative subtalar arthrosis, subtalar instability, and psoriatic arthritis. Ninety-three percent of patients were very satisfied or satisfied with their treatment. Pain and function improved significantly, and the American Orthopaedic Foot and Ankle Society ankle-hindfoot score at follow-up was 8

Influence of tibialis posterior muscle activation on foot anatomy under axial loading: A biomechanical CT human cadaveric study.

BACKGROUND: Collapse of the medial longitudinal arch and subluxation of the subtalar joint are common occurrences in adult flatfoot deformity. Controversy exists about the role of the tibialis posterior (TP) tendon as first and/or essential lesion. Subtle changes in the foot configuration can occur under weight bearing. PURPOSE: This human cadaveric study is designed to investigate the effect that isolated actuation of the TP tendon has on the medial longitudinal arch and the hindfoot configuration under simulated weight bearing. METHODS: A radiolucent frame was developed to apply axial load

Nonoperative care for the treatment of adult-acquired flatfoot deformity.

Nonoperative therapy for adult-acquired flatfoot is a reasonable treatment option that is likely to be beneficial for most patients. In this article, we describe the results of a retrospective cohort study that focused on nonoperative measures, including bracing, physical therapy, and anti-inflammatory medications, used to treat adult-acquired flatfoot in 64 consecutive patients. The results revealed the incidence of successful nonsurgical treatment to be 87.5% (56 of 64 patients), over the 27-month observation period. Overall, 78.12% of the patients with adult-acquired flatfoot were obese (bo

Novel technique for deltoid spring complex reconstruction in progressive collapsing foot disorder.

INTRODUCTION: Adult Acquired Flatfoot Deformity (AAFD) is a progressive condition characterized by the collapse of the medial foot arch, often caused by posterior tibial tendon dysfunction or deltoid spring ligament incompetency. Flexible type 2 AAFD results in peritalar subluxation and instability. The deltoid and spring ligament complex plays a critical role in foot stability. Current surgical treatments include ligament reconstructions, tendon transfers, and osteotomies, but comprehensive approaches for advanced cases are limited. This study compares two novel techniques-Quadrangular and Tr

Foot and Ankle Conditions: Pes Planus.

Pes planus or pes planovalgus (ie, flatfoot) is a common condition among young children and also is encountered in adults. In children, congenital pes planus typically resolves with age as the foot musculature strengthens. Flexible pes planus is defined as a normal arch during non-weight-bearing activity or tiptoeing, with a flattening arch on standing. In rigid pes planus, the arch remains stiff and collapsed with or without weight bearing. Patients with rigid pes planus should be referred for subspecialist treatment. Patients with flexible pes planus, in the absence of signs of rheumatologic

Isolated Proximal Tibiofibular Joint Reconstruction With Semitendinosus Tendon Autograft in a Collegiate Athlete.

BACKGROUND: Proximal tibiofibular joint (PTFJ) instability is a rare and often underrecognized cause of lateral knee pain and dysfunction, most commonly presenting as anterolateral subluxation of the fibular head after trauma. While conservative management is first-line, persistent instability may require operative intervention. INDICATIONS: We present the case of a 20-year-old male athlete who sustained a PTFJ injury during sport and noted instability. After a successful PTFJ diagnostic taping trial, he elected for surgical reconstruction after completing his competitive season. Imaging conf

Calcaneocuboid distraction arthrodesis and first metatarsocuneiform arthrodesis for correction of acquired flatfoot deformity in a cadaver model.

BACKGROUND: Lateral column lengthening has been associated with residual forefoot supination and symptomatic lateral overload in treatment of acquired flatfoot. A medial column procedure may be useful to redistribute load to the medial column. We evaluated radiographic and pressure changes in a severe flatfoot model with lateral column lengthening and investigated the effect of an added first metatarsocuneiform arthrodesis. METHODS: Ten cadaver specimens were loaded in simulated double-legged stance, and radiographic and pressure data were collected for all tested states. Calcaneocuboid arthr

Orthopaedic issues in the musculoskeletal care of adults with cerebral palsy.

AIMS: Orthopaedic care of adults with cerebral palsy (CP) has not been well documented in orthopaedic literature. This paper focuses on some of the common problems which present themselves when adults with CP seek orthopaedic intervention. In particular, we review the most common orthopaedic issues which present to the Penn Neuro-Orthopaedics Program. METHOD: A formal review of consecutive surgeries performed by the senior author on adults with CP was previously conducted. This paper focuses on the health delivery care for the adult with orthopaedic problems related to cerebral palsy. Ninety-

Tibialis posterior muscle pain effects on hip, knee and ankle gait mechanics.

BACKGROUND: Tibialis posterior (TP) dysfunction is a common painful complication in patients with rheumatoid arthritis (RA), which can lead to the collapse of the medial longitudinal arch. Different theories have been developed to explain the causality of tibialis posterior dysfunction. In all these theories, pain is a central factor, and yet, it is uncertain to what extent pain causes the observed biomechanical alterations in the patients. The aim of this study was to investigate the effect of experimental tibialis posterior muscle pain on gait mechanics in healthy subjects. METHODS: Twelve

Ligament of anterior horn of medial meniscus of knee joint: case reports and literature review.

BACKGROUND: The meniscus is essential for knee joint biomechanics, contributing to load distribution and stability. Variations such as the anterior medial meniscofemoral ligament (AMMFL) originating from the anterior horn of the medial meniscus (AHMM) are rare but significant. These variations may predispose individuals to medial meniscus injuries and knee dysfunction, though their biomechanical implications remain unclear. The AMMFL disrupts normal anchorage of the AHMM to the tibial plateau, increasing meniscal mobility and potentially altering load transmission and predisposing to degenerat

Partial Rupture of the Tibialis Posterior Tendon Associated with a Closed Medial Malleolar FractureA Case Report.

Rupture of the tibialis posterior tendon associated with ankle fracture is rare and difficult to diagnose. This rupture can be easily overlooked because the clinical examination is limited owing to acute pain related to a closed ankle fracture. Complete rupture of the tibialis posterior tendon can be identified by a loss of tension during ankle fracture fixation, but partial rupture is more difficult to detect because the tibialis posterior tendon can maintain its tension. A few cases of complete rupture of the tibialis posterior tendon combined with ankle fracture have been reported. It is we

Load response of the medial longitudinal arch in patients with flatfoot deformity: in vivo 3D study.

BACKGROUND: The acquisition of flatfoot by an adult is thought to primarily be caused by posterior tibial tendon dysfunction, although some other causes, such as congenital flexible flatfoot or an accessory navicular, may also be responsible. The objective of this study was to evaluate the bone rotation of each joint in the medial longitudinal arch (MLA) and compare the response in healthy feet with that in flat feet by analyzing the reconstructive three-dimensional (3D) CT image data during weightbearing. METHODS: CT scans of 20 healthy feet and 24 feet with flatfoot deformity were taken in
