Predisposing factors for developing gastric volvulus and the role of imaging.

Two cases of gastric volvulus are presented to highlight the predisposing factors, mechanism and different types of volvulus, and the role of imaging in making the diagnosis. Eventration of the diaphragm and hiatus hernia are precipitating factors for developing organo-axial and mesentero-axial volvulus. Imaging is key to making the diagnosis and distinguishing the types of volvulus in order to decide on management.

Using the NEISS database to understand pressure cooker related injuries in the USA.

OBJECTIVE: To determine the incidence of pressure cooker related injuries in US hospital emergency departments. To quantify injury patterns associated with pressure cookers and inform prevention recommendation messaging. METHODS: The National Electronic Injury Surveillance System (NEISS) was queried to identify injuries associated with pressure cookers between 1 January 2003 and 31 December 2019. Case narratives were reviewed to identify precipitating or contributing factors of pressure cooker related injuries. Negative binomial regression was employed to test for trends over time. RESULTS:

Factors associated with delirium severity among older persons with dementia.

Delirium is a common cause of functional and cognitive decline, morbidity, and mortality among hospitalized elderly individuals. Several studies reveal that the prognosis of delirium is worse among elderly individuals with severe delirium. In light of these findings, it is important to identify which factors are associated with delirium severity: individual (predisposing) or environmental (precipitating) factors. This study wanted to investigate individual and environmental factors associated with delirium severity among older persons with delirium superimposed on dementia. This study is a sec

Elderly patients with a hip fracture: the risk for delirium.

This prospective study investigated risk factors for delirium in elderly hip fracture patients that could be recognized by nurses. Data were collected on predisposing and precipitating factors for delirium from 92 elderly patients with a hip fracture. Predisposing factors included age, gender, sensory impairments, functional impairment before the hip fracture, residency before admission, pre-existing cognitive impairment, comorbidities, and medication use. Precipitating factors included factors related to surgery and to the postoperative period. Factors related to surgery included time between

Functional Versus Anatomical Nasolacrimal Obstruction: Are There Differences in Epiphora Severity, Symptoms, or Effects on Quality of Life?

PURPOSE: To elucidate whether the specific cause of nasolacrimal duct drainage impairment impacts the severity, symptoms, and implications on visual tasks and quality of life (QOL) in epiphora. METHODS: A retrospective review of consecutive adult patients with epiphora attending a tertiary lacrimal clinic from June 2011 to March 2021 was conducted. Cases with evidence of canalicular stenosis, a mucocele, or other identifiable causes of epiphora were excluded. A structured interview assessed the epiphora severity (Munk), symptomatology, precipitating factors, and effects on visual tasks and QO

Symmetrical peripheral gangrene and Roth spots in a patient with infective endocarditis.

We present a case of a young man with symmetrical peripheral gangrene (SPG) resulting from Streptococcus viridans-related infective endocarditis, an association which has not previously been reported. SPG is associated with up to 40% mortality and may necessitate amputation; early identification and treatment of the precipitating factors is very important.

Delirium.

Delirium, a syndrome characterized by an acute change in attention, awareness and cognition, is caused by a medical condition that cannot be better explained by a pre-existing neurocognitive disorder. Multiple predisposing factors (for example, pre-existing cognitive impairment) and precipitating factors (for example, urinary tract infection) for delirium have been described, with most patients having both types. Because multiple factors are implicated in the aetiology of delirium, there are likely several neurobiological processes that contribute to delirium pathogenesis, including neuroinfla

Pubertal process and green-sickness in Renaissance drama: a form fruste of anorexia nervosa?

OBJECTIVE: The aim of this paper is to investigate the relationship between puberty and the onset of green-sickness in early modern popular culture with findings on puberty and the onset of anorexia nervosa. METHOD: Four plays from the late 16th and early 17th centuries dealing with 14-15-year-old girls were analysed for information on puberty and for precipitating factors for the onset of green-sickness. These were then contrasted with the literature on anorexia nervosa where puberty has been proposed as a potential risk factor for the development of the disorder. RESULTS: These four plays

Environmental factors and cervical artery dissection.

A history of a minor precipitating event is frequently elicited in patients with a spontaneous dissection of the carotid or vertebral artery. Other precipitating events associated with hyperextension or rotation of the neck include practicing yoga, painting a ceiling, coughing, vomiting, sneezing, the receipt of anesthesia, and the act of resuscitation. Chiropractic manipulation of the neck has been associated with carotid artery dissection and, particularly, vertebral artery dissection. Another risk factor for spontaneous dissections seems to be a recent history of a respiratory tract infecti

Risk calculation for hyperkalaemia in heart failure patients.

BACKGROUND: We aimed to develop a model to estimate the risk of hyperkalaemia in patients treated for heart failure in a tertiary reference hospital and to identify precipitating factors. METHODS: 125 congestive heart failure (CHF) patients were studied retrospectively. Thirty of these patients developed episodes of hyperkalaemia (K>or=5.5 mmol/l). Both groups were compared for possible risk factors for hyperkalaemia (age, glomerular filtration rate (GFR), New York Heart Association (NYHA) class, diabetes mellitus (DM), ejection fraction and medication use (ACE inhibitors, angiotensin recepto

Management of hepatic encephalopathy: role of rifaximin.

Hepatic encephalopathy (HE) is a neuropsychiatric syndrome, which develops in patients with acute or chronic liver failure. It is widely accepted to be due to impairment of hepatic clearance of toxic products from the gut such as ammonia. Accumulation of ammonia induces a glutamate neurotoxicity leading to an increased tone of the gamma-aminobutyric acid A (GABA-A) receptor system in the brain which results in HE. Factors either increasing the ammonia levels (protein load, constipation, sepsis, or gastrointestinal bleeding) or potentiating the functional activity of the GABAergic system [natur

Mucogingival considerations for the orthodontic patient.

Recession of the gingival margin during or subsequent to orthodontic therapy may occur as either pseudorecession or true recession. Assessment of both predisposing and possible precipitating factors will assist in evaluating the need for surgical intervention. Unfortunately, several parameters cannot be evaluated by objective measures. The clinician must address both objective and subjective criteria. Factors predisposing a site to recession are elucidated by evaluating the anatomy of the area, including the width of keratinized and attached gingiva, the facial-to-lingual dimension of both sof

Delirium tremens and related clinical states. Aetiology, pathophysiology and treatment.

Definitions of Delirium Tremens (DT) and related clinical states are discussed together with the concepts of aetiology and pathogenesis in relation to psychiatric disease. The withdrawal theory which considers reduction or cessation of alcohol intake as an important precipitating factor in DT is discussed; this theory is supported by experimental studies of ethanol withdrawal in man and by studies indicating cross dependence between ethanol and several other CNS depressors; the arguments in the literature for and against the withdrawal theory are discussed. Other possible aetiological factors
