Myotonic Dystrophy and Huntington's Disease Care: "We Like to Think We're Making a Difference".

BACKGROUND: Patient-centered care for individuals with myotonic dystrophy (DM1) and Huntington's disease (HD)-chronic, progressive, and life-limiting neurological conditions-may be challenged by patients' cognitive and behavioral impairments. However, no research has explored health care providers' (HCPs') perspectives about patient-centered care provision for these patients along their disease trajectory. METHODS: Constructivist grounded theory informed the iterative data collection and analysis process. Eleven DM1 or HD HCPs participated in semistructured interviews, and three stages of cod

A novel TUFM homozygous variant in a child with mitochondrial cardiomyopathy expands the phenotype of combined oxidative phosphorylation deficiency 4.

Translation of mitochondrial-specific DNA is required for proper mitochondrial function and energy production. For this purpose, an elaborate network of dedicated molecular machinery including initiation, elongation and termination factors exists. We describe a patient with an unusual phenotype and a novel homozygous missense variant in TUFM (c.344A>C; p.His115Pro), encoding mtDNA translation elongating factor Tu (EFTu). To date, only four patients have been reported with bi-allelic mutations in TUFM, leading to combined oxidative phosphorylation deficiency 4 (COXPD4) characterized by severe e

Botulism and cavernous sinus thrombosis induced by acute rhinosinusitis: A case report.

BACKGROUND: Botulism is an acute and rapidly progressive descending paralytic disease caused by a neurotoxin of clostridium botulinum. CASE PRESENTATION: A 28-year-old woman presented with severe generalized ascending symmetrical muscle paralysis. The patient was intubated and transferred to the medical intensive care unit with several symptoms including: severe headache, dysphagia, dyspnea, ptosis, diplopia, and dry mouth. Despite being alert, pupils were bilaterally midriatic and had absent corneal reflux. Pansinusitis was seen in the paranasal sinus scan. At first, the movement of eyelids,

A Case of Smith-Lemli-Opitz Syndrome Diagnosed with Hypertrophic Pyloric Stenosis.

Smith-Lemli-Opitz syndrome (SLOS) is caused by a deficiency in the enzyme 7-dehydrocholesterol reductase (DHCR7) that results in an abnormality in cholesterol metabolism. SLOS is inherited as an autosomal recessive genetic disorder. In this case, we describe a 34-day-old patient with postnatal progressive projectile vomiting, diagnosed with hypertrophic pyloric stenosis, who was suspected to have SLOS during treatment clinical and biochemical profile. A 34-day-old patient with progressively worsening vomiting and abdominal distention, diagnosed as hypertrophic pyloric stenosis, was operated by

Point-of-care ultrasound-guided fluid management in acute respiratory failure complicating preeclampsia and thyrotoxicosis: a case report.

The coexistence of preeclampsia and uncontrolled thyrotoxicosis leading to acute respiratory failure is rare and poses significant diagnostic and therapeutic challenges, particularly with respect to safe fluid management in the obstetric intensive care setting. We report the case of a 35-year-old woman at 37-week gestation who presented with progressive dyspnea, acute pulmonary edema, and fetal distress, necessitating emergency cesarean section and subsequent intensive care admission. Point-of-care ultrasound (POCUS) demonstrated impaired left ventricular systolic function (ejection fraction 4

Making strides in preventing ICU-acquired weakness: involving family in early progressive mobility.

As advances in research and technology expand our ability to optimize the short-term outcomes in critical illness, survivors are often left with long-term functional impairments. The complications of bed rest can be nearly as devastating as the illness itself. Prolonged periods of immobility during the acute phase of illness have been linked to severe weakness, self-care deficits, poor quality of life, and mortality in patients up to 5 years after discharge from the intensive care unit. Interventions targeted at early, progressive mobility have been shown to reduce the burden of these outcomes

Respiratory failure because of neuromuscular disease.

PURPOSE OF REVIEW: Understanding the mechanisms and abnormalities of respiratory function in neuromuscular disease is critical to supporting the patient and maintaining ventilation in the face of acute or chronic progressive impairment. RECENT FINDINGS: Retrospective clinical studies reviewing the care of patients with Guillain-Barré syndrome and myasthenia have shown a disturbingly high mortality following step-down from intensive care. This implies high dependency and rehabilitation management is failing despite evidence that delayed improvement can occur with long-term care. A variety of m

Juvenile Amyotrophic Lateral Sclerosis: A Case Report of a Rare and Aggressive Presentation in a 22-Year-Old Filipino Male.

Amyotrophic Lateral Sclerosis (ALS) is a rare neurodegenerative disorder primarily affecting adults, but juvenile-onset ALS is exceptionally rare. We report a rare case of a 22-year-old Filipino male patient who exhibited early-onset weakness, muscle atrophy, and tongue fasciculations, followed by rapidly progressive dysphagia and respiratory distress. Electromyography - Nerve Conduction Velocity (EMG-NCV) findings showed evidence for a chronic, active predominantly motor neuronal-axonal loss type of neuropathy involving the tongue and limb muscles bilaterally consistent with a motor neuron di

Assisting patients with motor neurone disease to make decisions about their care.

Motor neurone disease (MND), is a progressive terminal illness affecting the central nervous system, causing paralysis of the muscles affecting limb movement, breathing and bulbar function, with an average life expectancy of 2-4 years. Patients are presented with repeated loss and the constant need to make adjustments to their lifestyle and expectations. Within palliative care there has been a move to formalise planning by undertaking advance care planning, giving the patient the opportunity to plan whether they would consider medical interventions and how they would like their care and death

Is there a future for the use of left ventricular assist devices in Duchenne muscular dystrophy?

Duchenne muscular dystrophy (DMD) is the most common form of childhood muscular dystrophy resulting in progressive muscle wasting and weakness. With advancements in respiratory care and the use of glucocorticoids, cardiomyopathy has surpassed respiratory compromise as the leading cause of morbidity and mortality in this patient population. As muscular dystrophy remains a relative contraindication to heart transplantation, end-stage heart failure management represents a major therapeutic challenge. Long-term left ventricular assist device (LVAD) therapy has emerged as a promising management str

Advance directives as a tool to respect patients' values and preferences: discussion on the case of Alzheimer's disease.

BACKGROUND: The proposal of the new criteria for the diagnosis of Alzheimer's disease (AD) based on biomarker data is making possible a diagnosis of AD at the mild cognitive impairment (MCI) or predementia/prodromal- stage. Given the present lack of effective treatments for AD, the opportunity for the individuals to personally take relevant decisions and plan for their future before and if cognitive deterioration occurs is one the main advantages of an early diagnosis. MAIN BODY: Advance directives are largely seen as an effective tool for planning medical care in the event the subject become

The patient's voice: real-world experiences of a transthyretin amyloidosis focus group.

BACKGROUND: Transthyretin amyloidosis (ATTR) is a progressive, multisystemic disorder that severely impacts patient quality of life. Despite therapeutic advancements, significant delays in diagnosis and care persist, often due to insufficient awareness among healthcare professionals. We aimed to highlight the experiences of patients living with ATTR in Spain in order to understand their daily challenges within the healthcare system. METHODS: As part of the CARABELA-ATTR initiative, we conducted a focus group discussion with six patients with confirmed ATTR from different regions of Spain. The

Rapidly Progressive Necrotizing Fasciitis of the Lower Limb Following Self-Drainage of an Abscess: Prompt Recognition and Initial Management at a District Healthcare Centre.

Necrotizing fasciitis (NF) is a rare but rapidly progressive infection of the fascia and subcutaneous tissue that can arise following trauma, surgical wounds, or minor skin breaches. Early stages often mimic cellulitis, making timely diagnosis challenging. This case report describes a 52-year-old male patient who presented with erythema and swelling of the left foot and ankle, initially diagnosed as cellulitis in the setting of septic shock. Despite nonspecific imaging and laboratory findings, the rapid progression of cutaneous changes raised clinical suspicion for NF. Emergent surgical debrid

Perceptions of palliative care in Huntington's disease: A qualitative study.

INTRODUCTION: Palliative care focuses on improving patient and family quality of life by managing symptoms, psychosocial issues and spiritual concerns. Huntington's disease is a progressive neurodegenerative disorder with no current disease modifying therapy. Although the palliative care model has been postulated to be an integral part of HD care, there are gaps in knowledge about how this care should be implemented. This study aims to identify perceptions of palliative care in Huntington's Disease (HD), palliative care needs of people living with HD, and at what point they feel they would ben

Toxic epidermal necrolysis in pregnancy: case report and review of the literature.

The authors report the case of a 29-year-old pregnant woman (2g1p) in the 16th week of gestation presenting with extensive toxic epidermal necrolysis (TEN). The cutaneous symptoms began at hands, feet, and in the mouth and developed during the course of 10 days to cover 75% of her TBSA, whereas total epidermolysis was present on more than 40% of her TBSA. Because of progressive swelling and bleeding of the oral mucosa, tracheal intubation was necessary to secure the airway of the patient. Critical care management required sedation, tracheotomy and artificial ventilation (14 days), prolonged fl

Fulminant Heterotopic Ossification following COVID-19 associated Systemic Inflammatory Response Syndrome: Manifestations in Radiology, Nuclear Medicine, and Clinical Application.

Heterotopic ossification (HO) is an unclear etiological trigger that results in diverse extra-skeletal bone formation in muscles and soft tissues. This often results in morbidity and reduced quality of life with pain, contractures, and mobility impairment. We present two patients with HO with a history of severe COVID-19 infection requiring 1-month-long mechanical ventilation. The first patient was found to have progressive stiffening of the right knee and left elbow, with clear demonstration of radiographic findings and active dystrophic calcification by nuclear medicine three-phase bone scan
