In this brief history, the authors have attempted to highlight salient aspects of psychotherapy with physically disabled individuals across a span ranging from the prepsychoanalytic to the contemporary. Particular attention is given to the prepsychoanalytic work of Charcot and Janet on neurological diseases and trauma, respectively. Psychoanalytic concepts are reviewed as they relate to physical disability and they are compared with contemporary themes involving trauma and loss. The paper has a distinctly psychoanalytic bias about the psychology of congenital and acquired physical differences.

Little is known about prognostic factors in the treatment of bulimic patients. In the context of an ongoing study we looked at 1-year symptomatic outcome of 37 outpatients fulfilling DSM-III-R criteria for bulimia nervosa in relation to a variety of measures at first assessment. In a randomised controlled trial of different types of psychotherapy for bulimia 92 women were assigned to receive cognitive-behaviour therapy (n = 32), behaviour therapy (30), or group therapy (30) for 15 weeks and a further 20 (controls) assigned to remain on a waiting list for 15 weeks. Behavioral modification psychotherapies, largely diffused in the anglo-saxon countries, are used here for treating severely autistic children, hospitalized in a child psychiatry service for at least one year. In this work I tried to classify Anorexia nervosa into two groups, the central group and the reactive group. The central group contains four different subgroups, the immature, the schizoid, the hysterical and the compulsive type. In the first two subgroups the lack of appetite appears primarily at the onset of the disease, and the fear of growing up plays an important role. In the management of patients it is often important to combine physical and psychotherapeutic treatment methods according to the individual patient's needs. In analytically oriented psychotherapy more attention can usefully be paid to the patient's bodily attitudes and behavior than has been customary. A psychosomatic approach which can be applied to the psychotherapeutic process is described, and theoretical considerations are discussed.
