[Updated recommendations for vaccination in adults with respiratory diseases. Argentine Association of Respiratory Medicine, 2023].

Adult smokers, those with comorbidities, and the elderly, are at greater risk of contracting infections and their complications. Community acquired respiratory infections due to viruses, pneumococcus and other bacteria, affect both healthy and sick adults. There are vaccines that the pulmonologist must know and prescribe. The target strains of the influenza vaccine are defined by the WHO for the Southern hemisphere considering those involved in the previous influenza season in the Northern hemisphere. Its effectiveness depends on virulence, concordance between circulating and vaccine strains,

Per-Esophageal Needle Aspiration of Parenchymal Lung Lesions and Mediastinal Lymph Nodes Using an Endobronchial Ultrasound Bronchoscope.

BACKGROUND: Transesophageal endoscopic ultrasound-guided fine-needle aspiration using a bronchoscope (EUS-B-FNA) allows clinicians to determine mediastinal staging and lung mass evaluation of lesions not accessible by endobronchial ultrasound (EBUS) or where endobronchial ultrasound-guided transbronchial needle aspiration might not be safe. OBJECTIVES: To evaluate the safety, diagnostic accuracy, and feasibility of EUS-B-FNA. METHODS: The study comprised patients who underwent a pulmonologist-performed EUS-B-FNA of mediastinal lymph nodes and parenchymal lung lesions between June 2015 and Se

Initial staging of head and neck squamous cell carcinoma. What is the place of bronchoscopy and upper GI endoscopy?

To determine the place of bronchoscopy and upper GI endoscopy in the initial staging of head and neck squamous cell carcinoma (HNSCC). A 10-year retrospective study was conducted on a series of 838 patients. As part of initial staging of the tumor, all patients were examined by neck and chest CT scan, 487 patients were examined by bronchoscopy and 588 patients were examined by upper GI endoscopy. Esophageal cancer was detected in 4.25 % of cases and lung cancer in 6.35 % of cases. Chest CT scan was statistically superior to bronchoscopy to detect second lung cancers (p < 0.05). On multivariate

Clinician-Perceived Barriers and Facilitators for Initiation and Management of Noninvasive Ventilation Among Patients With Chronic Hypercapnia From COPD: A Qualitative Study.

BACKGROUND: Long-term non-invasive ventilation (NIV) improves survival and re-admission risk for patients with chronic hypercapnia from chronic obstructive pulmonary disease (COPD). However, outpatient NIV prescription remains low. METHODS: We conducted a qualitative study using semi-structured interviews with pulmonary physicians. We identified a convenience sample of community-affiliated outpatient pulmonologists who were recruited via email. Deductive and inductive thematic content analysis were used to identify how physicians screen and manage patients with COPD at risk for chronic hyperc

The rise of artificial intelligence in respiratory primary care and pulmonology: a scoping review.

Artificial intelligence (AI) is rapidly advancing respiratory disease management, from diagnosis to population lung health. This scoping review synthesizes the most promising uses of AI in respiratory medicine, with a particular focus on pulmonologists and family physicians interested in lung health. In diagnostics, deep-learning systems streamline chest-imaging workflows by triaging radiographs, detecting COVID-19 pneumonia, and classifying lung nodules on CT. In pulmonary function testing, algorithms detect technical errors and classify spirometric patterns, some claiming to outperforming pu

Diagnosis in Children With Prolonged or Recurrent Cough: Findings From the Swiss Paediatric Airway Cohort.

INTRODUCTION: Prolonged or recurrent cough is a common reason for referral to pediatric pulmonologists, yet few studies have assessed its causes. We examined records of children visiting respiratory outpatient clinics in Switzerland and assessed how diagnoses vary by age. METHODS: We analyzed data from the multicenter Swiss Paediatric Airway Cohort study. We included 363 children (median age 6 years, range 0-16) referred for prolonged or recurrent cough. From outpatient records, we extracted information on diagnostic investigations, final diagnoses proposed by pediatric pulmonologists, and tr

The impact of interstitial lung disease in patients with acid sphingomyelinase deficiency (ASMD) - A case series.

BACKGROUND: Lysosomal storage diseases (LSD) are inherited diseases caused by mutations affecting genes encoding the function of lysosomal enzymes. Acid sphingomyelinase deficiency (ASMD) is an ultra-rare, progressive, and often fatal LSD with an estimated prevalence of 1-in-50,000-250,000 individuals. The PrevASMD study established the ASMD prevalence in Spain at 35 patients. Almost all present asymptomatic interstitial lung disease (ILD), since >90 % patients with chronic visceral ASMD have radiographic evidence of ILD. OBJECTIVE: This case series aims to emphasize the importance of ASMD in

Lung Diseases and Rare Disorders: Is It a Lysosomal Storage Disease? Differential Diagnosis, Pathogenetic Mechanisms and Management.

Pulmonologists may be involved in managing pulmonary diseases in children with complex clinical pictures without a diagnosis. Moreover, they are routinely involved in the multidisciplinary care of children with rare diseases, at baseline and during follow-up, for lung function monitoring. Lysosomal storage diseases (LSDs) are a group of genetic diseases characterised by a specific lysosomal enzyme deficiency. Despite varying pathogen and organ involvement, they are linked by the pathological accumulation of exceeding substrates, leading to cellular toxicity and subsequent organ damage. Less se

Evaluating the consistency with guideline recommendations for diagnosis and management of idiopathic pulmonary fibrosis in non-academic settings.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a progressive lung disease with elevated mortality. Delay in diagnosis lead to worse outcomes. Guidelines developed at academic medical centers are difficult to replicate in the community. OBJECTIVES: Our primary objective was to ascertain consistency with the 2011 IPF guidelines. Our secondary objective was to conduct an interdisciplinary review to ascertain whether the evidence supported the original diagnosis of IPF or not. METHODS: We asked permission from pulmonologists to review records of patients diagnosed with IPF after 2011. We col

Pleuroscopy or video-assisted thoracoscopic surgery for exudative pleural effusion: a comparative overview.

Exudative pleural effusions, such as malignant and tuberculous pleural effusions, are associated with notable morbidity and mortality. Unfortunately, a significant number of these effusions will remain undiagnosed despite thoracentesis. Traditionally, closed pleural biopsies have been the next best diagnostic step, but the diagnostic yield of blind closed pleural biopsies for malignant pleural effusions is insufficient. When image-guided targeted biopsies are not possible, both pleuroscopy and video-assisted thoracoscopic surgery are reasonable options for obtaining pleural biopsies, but the d

High Complication Rate after Introduction of Transbronchial Cryobiopsy into Clinical Practice at an Academic Medical Center.

RATIONALE: Transbronchial cryobiopsy is an emerging technique for obtaining biopsies of lung parenchyma. Despite limited evidence of its safety and efficacy in direct comparison with other available biopsy procedures, pulmonologists are integrating this technique into clinical practice with the hope of avoiding the risks of surgical lung biopsy. OBJECTIVES: To report the rate of severe complications and diagnostic outcomes immediately after introduction of transbronchial cryobiopsy into the clinical practice of a single-center, high-volume, interventional pulmonary group at a large academic m

Feasibility and safety of outpatient medical thoracoscopy at a large tertiary medical center: a collaborative medical-surgical initiative.

BACKGROUND: Medical thoracoscopy (MT) is performed by relatively few pulmonologists in the United States. Recognizing that an outpatient minimally invasive procedure such as MT could provide a suitable alternative to hospitalization and surgery in patients with undiagnosed exudative pleural effusions, we initiated the Mayo Clinic outpatient MT program and herein report preliminary data on safety, feasibility, and outcomes. METHODS: All consecutive patients referred for outpatient MT from October 2011 to August 2013 were included in this study. Demographic, radiographic, procedural, and histol

Utilization Patterns of Nebulized Glycopyrronium in Patients Hospitalized for Acute Exacerbations of Obstructive Airway Disease (AEOAD)-Indian Expert Perspectives.

Background: Acute exacerbation of obstructive airway disease (AEOAD) is a major cause of hospitalization, morbidity, and premature mortality in India. Hospitalized patients for the same are predominantly treated with short-acting bronchodilators, which require frequent administration and are associated with systemic adverse effects. Despite the availability of nebulized long-acting muscarinic antagonists (LAMAs) with quick onset of action, such as glycopyrronium, their role in acute care remains unclear in India. Methods: A pan-India expert opinion-building initiative was conducted among 220 p
