Volumetric modulated arc therapy with robust optimization for larynx cancer.

PURPOSE: The aim of this study was to perform a comparison between robust optimization and planning target volume (PTV)-based optimization plans using volumetric modulated arc-therapy (VMAT) by evaluating perturbed doses induced by localization offsets for setup uncertainties in larynx cancer radiation therapy. METHODS: Ten patients with early-stage (T1-2N0) glottis carcinoma were selected. The clinical target volume (CTV), carotid arteries, and spinal cord were contoured by a radiation oncologist. PTV-based and robust optimization plans were normalized at D50% to the PTV and D98% to the CTV,

Geometric dose prediction model for hemithoracic intensity-modulated radiation therapy in mesothelioma patients with two intact lungs.

The presence of two intact lungs makes it challenging to reach a tumoricidal dose with hemithoracic pleural intensity-modulated radiation therapy (IMRT) in patients with malignant pleural mesothelioma (MPM) who underwent pleurectomy/decortications or have unresectable disease. We developed an anatomy-based model to predict attainable prescription dose before starting optimization. Fifty-six clinically delivered IMRT plans were analyzed regarding correlation of prescription dose and individual and total lung volumes, planning target volume (PTV), ipsilateral normal lung volume and ratios: contr

CBCT-guided adaptive radiotherapy using self-supervised sequential domain adaptation with uncertainty estimation.

Adaptive radiotherapy (ART) is an advanced technology in modern cancer treatment that incorporates progressive changes in patient anatomy into active plan/dose adaption during the fractionated treatment. However, the clinical application relies on the accurate segmentation of cancer tumors on low-quality on-board images, which has posed challenges for both manual delineation and deep learning-based models. In this paper, we propose a novel sequence transduction deep neural network with an attention mechanism to learn the shrinkage of the cancer tumor based on patients' weekly cone-beam compute

Conformal and stereotactic radiotherapy in hepatocellular carcinoma.

Historically, Radiation Therapy (RT) has played a minor role in the treatment of hepatocellular carcinoma, especially because of toxicity related to conventional techniques. However, high radiation doses can now be given safely by means of 3D-conformal techniques and other more specialized techniques. 3D-conformal RT, in which both target definition and treatment planning are based on three-dimensional image data sets, provided good local control in several series. As an evolution of cranial radiosurgery, stereotactic body radiation therapy (SBRT) is currently being employed as a new tool for

Measurement of the volume CT dose index on spiral CT scanning with a real-time ionization chamber.

BACKGROUND: The measurement of computed tomography dose index 100 ( C T D I 100 $CTD{{I}_{100}}$ ), which is feasible only through axial scanning, requires that the clinical spiral protocols be replaced with those for axial scanning. The real-time ionization chamber detects the integral of radiation dose rate profile, enabling the direct verification of the volume CTDI on spiral CT scanning ( C T D I v o l S p i r a l $CTDI_{vol}^{Spiral}$ ). PURPOSE: This study aimed to develop a direct measurement technique for C T D I v o l S p i r a l $CTDI_{vol}^{Spiral}$ and compare its accur

The effects of mega-voltage CT scan parameters on offline adaptive radiation therapy.

TomoTherapy involves image-guided radiation therapy (IGRT) using Mega-voltage CT (MVCT) for each treatment session. The acquired MVCT images can be utilized for the retrospective assessment of dose distribution. The TomoTherapy provides 18 distinct imaging conditions that can be selected based on a combination of algorithms, acquisition pitch, and slice interval. We investigated the accuracy of dose calculation and deformable image registration (DIR) depending on MVCT scan parameters and their effects on adaptive radiation therapy (ART). We acquired image values for density calibration tables

A multi-modal vision-language pipeline strategy for contour quality assurance and adaptive optimization.

Objective.Accurate delineation of organs-at-risk (OARs) is a critical step in radiotherapy. The deep learning generated segmentations usually need to be reviewed and corrected by oncologists manually, which is time-consuming and operator-dependent. Therefore, an automated quality assurance (QA) and adaptive optimization correction strategy was proposed to identify and optimize 'incorrect' auto-segmentations.Approach.A total of 586 CT images and labels from nine institutions were used. The OARs included the brainstem, parotid, and mandible. The deep learning generated contours were compared wit

Influence of Fractionation and Beam Sequencing on Absorbed Dose to Circulating Lymphocytes During Ultra-High Dose Rate and Conventional Radiotherapy: An In Silico Study.

PURPOSE: Several in silico models concluded that ultra-high dose rate (UHDR) radiotherapy could spare large quantities of circulating lymphocytes. However, preclinical studies failed to show a reduction in radiation-induced lymphopenia after UHDR irradiation compared with conventional dose rates (CONV). This study aims to investigate the influence of fractionation and beam sequencing on absorbed dose to circulating lymphocytes during CONV and UHDR irradiations. MATERIALS AND METHODS: The LymphoDose framework was applied to a cohort of 162 patients treated for brain tumors with 3D conformal ra

Commissioning a standalone adaptive radiotherapy linac in a multi-vendor environment.

Current radiotherapy machines intended to perform streamlined online adaptive therapy are designed to be standalone, which makes it challenging to integrate them with the rest of the clinic. This work describes the installation of a standalone CT-guided online adaptive system, the Varian Ethos, in a busy clinic utilizing products from multiple vendors, including RayStation as the treatment planning system (TPS) and MOSAIQ as the oncology information system (OIS). The aim was to develop solutions that minimized workload increases for staff using redundant systems and to implement this new techn

Novel multileaf collimator designs with tongues and grooves in leaf end and Monte Carlo simulations.

In this study, we proposed 2 new multileaf collimator leaf designs to eliminate leaf gaps for closed leaf pairs so that radiation leakage can be avoided. In the new designs, multi tongues and grooves were added to the conventional multileaf collimators leaf ends. Thus, when a pair of leaves closed, tongues of a leaf can enter grooves of its opposing leaf. Consequently, there would be no radiation leakage through closed leaves. One design was named finger-shaped MLC, and another design with doubled leaf end thickness was named hand-shaped MLC. Monte Carlo simulations were performed to simulate

Light Pareto robust optimization for IMRT treatment planning.

BACKGROUND: Robust optimization (RO) has been proposed to mitigate breathing motion uncertainty during treatment in intensity-modulated radiation therapy (IMRT) planning for breast or lung cancer. RO is a pessimistic approach that implicitly trades off average-case for worst-case treatment plan quality. Pareto robust optimization (PRO) provides a mechanism for improving nonworst-case plan outcomes, but often remains overly conservative in the average case. PURPOSE: The goal of this study is to characterize the trade-off between the optimality of robust IMRT plans in the worst case and the tre

Robust VMAT treatment planning for extremity soft tissue sarcomas.

Volumetric modulated arc therapy (VMAT) is a frequently employed and guideline-recommended radiotherapy (RT) modality for extremity soft tissue sarcomas (eSTS). Prior studies have demonstrated that significant tumor volume changes during treatment result in loss of target volume coverage with highly conformal techniques such as VMAT, but few solutions exist to these issues aside from adaptive replanning. Here, we describe a related but novel phenomenon in which relatively minor changes in surface volume contour (whether due to daily setup uncertainty, edema of peritumoral tissue, or progressio

Probabilistic evaluation of plan quality for time-dependent anatomical deformations in head and neck cancer patients.

PURPOSE: In addition to patient set-up uncertainties, anatomical deformations, e.g., weight loss, lead to time-dependent differences between the planned and delivered dose in a radiotherapy course that currently cannot easily be predicted. The aim of this study was to create time-varying prediction models to describe both the average and residual anatomical deformations. METHODS: Weekly population-based principal component analysis models were generated from on-treatment cone-beam CT scans (CBCTs) of 30 head and neck cancer patients, with additional data of 35 patients used as a validation co
