Dose planning study of proton versus photon radiotherapy for head and neck squamous cell carcinoma of unknown primary in the primary and recurrent setting.

BACKGROUND: Patients with head and neck squamous cell carcinoma of unknown primary (HNCUP) are often treated with extensive radiotherapy (RT). Frequently, the bilateral nodal clinical target volume (nCTV) and the volumes of suspected mucosal primary sites (mCTV) of the pharynx and larynx is irradiated. This treatment is effective but toxic. New data suggest that omission of the contralateral nCTV and mCTV, results in few recurrences. The present study explores photon versus proton therapy, in the primary and recurrent setting. MATERIAL AND METHODS: An analysis of twelve patients previously tr

Repeated SBRT for in- and out-of-field recurrences in the liver.

PURPOSE: To evaluate the feasibility and toxicity profile of repeated stereotactic body radiotherapy (SBRT) for recurrent primary or secondary liver tumors. METHODS: Consecutive patients with primary (hepatocellular carcinoma [HCC] or cholangiocarcinoma [CCC]) or secondary liver cancer (LM), with intrahepatic recurrence or progression after SBRT, underwent re-SBRT in 3 to 12 fractions with a median time of 15 (range 2-66) months between treatments. RESULTS: In all, 24 patients which were previously treated with SBRT (30 lesions) were retreated with SBRT for "in- and out-of-field" recurrences

A Phase 1 Dose-Escalation Trial of Reirradiation for Patients With Recurrent Glioblastoma.

PURPOSE: A dose-escalation trial of reirradiation (reRT) was conducted in patients with a diagnosis of recurrent glioblastoma (rGBM) to increase the total overall dose given to the tumor without increasing toxicity. The primary objective of this study was to establish the maximum tolerated dose (MTD) for reRT in patients with rGBM. METHODS AND MATERIALS: A phase 1 dose-escalation study using a "3 plus 3" design aimed at identifying reRT MTD with secondary study endpoints of overall survival, progression-free survival, Health-Related Quality of Life, and neurocognitive outcomes. Patients with

Acute Toxicity and Cosmesis Following a Tumor Bed Boost After Whole Breast Moderate Dose Hypofractionation.

PURPOSE: Randomized evidence suggests that tumor bed boost improves local control following whole breast radiation therapy (WBRT) but may lead to more acute and late toxicity. However, little data are available about toxicity in real world practice from a tumor bed boost delivered after WBRT, particularly following moderate dose hypofractionation (MH). We evaluated acute toxicity and cosmesis effects associated with use of a boost after MH-WBRT in a large prospective cohort. METHODS AND MATERIALS: This analysis includes women undergoing adjuvant MH-WBRT without regional nodal irradiation, pro

Oncological outcomes and safety after spinal reirradiation with stereotactic body radiotherapy.

OBJECTIVE: Spinal metastases pose a significant challenge in oncology, with incidence rates increasing alongside improved survival rates. Radiation therapy (RT) has played a crucial role in managing spinal disease progression and reducing associated neurological morbidity. However, management of spinal metastases for which prior RT failed is challenging, and there are limited data regarding the safety and efficacy of stereotactic body radiotherapy (SBRT) for reirradiation. The authors present the largest series to date of patients undergoing SBRT for reirradiation of spinal metastases. METHOD

Reirradiation of head and neck squamous cell carcinomas: a pragmatic approach, part II: radiation technique and fractionations.

INTRODUCTION: Reirradiation (reRT) of local recurrent/second primary tumors of the head and neck represents a potential curative treatment for patients not candidate to a salvage surgery. Aim of the present study is to summarize literature data on modern radiation techniques and fractionations used in this setting of patients. MATERIALS AND METHODS: A narrative review of the literature was conducted on three topics: (1) target volume delineation (2) reRT dose and techniques and (3) ongoing studies. Patients treated with postoperative reRT and palliative intent were not considered for the curr

Organs at risk radiation dose constraints: 2025 update.

We present the updated recommendations from the Société française de radiothérapie oncologique (SFRO, the French society for radiation oncology) regarding radiation dose constraints for organs at risk in adult patients, with the aim of supporting clinical decision-making in contemporary radiotherapy practice. These guidelines address a wide range of clinical scenarios, including normofractionation, hypofractionation, stereotactic radiotherapy, and reirradiation. Dose constraints are essential to ensure both the safety and efficacy of radiotherapy, especially with the growing use of advanced te

REMIT: Reirradiation of Diffuse Midline Glioma Patients -A Nordic Society of Paediatric Haematology and Oncology Feasibility Study.

Diffuse midline glioma (DMG) continues to be an aggressive brain stem cancer among children and young adults. It has a dismal prognosis, with less than 10% of patients alive two years after diagnosis. Radiotherapy has been demonstrated to be effective, albeit transient. Hence, radiotherapy is considered a cornerstone in the treatment. Reirradiation has, in retrospective studies, shown promising overall survival and palliative effect, but no pan-European consensus for reirradiation exists. The REMIT (Reirradiation of diffuse Midline glioma paTients) protocol evaluates safety and the palliative

Radioprotection of thyroid cells mediated by methimazole.

PURPOSE: The radioprotective effect of antithyroid drugs on radioiodine treatment is a controversial issue. However, it is of clinical relevance whether antithyroid medication has to be interrupted for therapy and when antithyroid medication can be continued after radioiodine treatment. We investigated DNA damage caused by internal or external radiation using thyroid cells (sodium iodine symporter [NIS] positive). MATERIALS AND METHODS: Adherent thyroid cells were irradiated following incubation with the mediators methimazole and perchlorate using either X-ray tube or Re-188-perrhenate. DNA d

Patterns of neuroglial proliferation in spinal cord white matter following exposure to ionizing radiation.

X-irradiation temporarily decreases the proliferative activity of neuroglia in immature rat spinal cord. Later, the proliferative activity in these irradiated regions surpasses that noted in control rats. Areas adjacent to the irradiated region have a greater than normal percentage of labelled neuroglia and may also be a source of neuroglia which re-populate the irradiated zone.

Hypofractionated reirradiation for feline nasal epithelial tumors: A retrospective study.

BACKGROUND: Feline nasal epithelial tumors are locally invasive and radiosensitive, and radiotherapy is the standard treatment. However, recurrence is common, and the role of reirradiation in cats remains unclear. AIM: This retrospective study aimed to evaluate the efficacy and safety of hypofractionated reirradiation in cats with recurrent nasal epithelial tumors. METHODS: The medical records of cats treated with hypofractionated radiotherapy for intranasal epithelial tumors at Azabu University Veterinary Teaching Hospital between 2014 and 2022 were reviewed. Follow-up continued until Augus

Efficacy of postoperative radiation treatment for bone metastases in the extremities.

BACKGROUND AND PURPOSE: Impending or pathological fractures due to bone metastases may require surgical fixation. Postoperative radiation is often recommended to reduce local progression and prevent prosthesis displacement, hence reducing the need for second surgery. The objectives of this study were to investigate the need for second surgery, and to report on rates of re-irradiation, tumor progression and prosthesis displacement following postoperative radiation. MATERIALS AND METHODS: Data were collected from 65 patients who received postoperative radiation to 74 sites in the extremities in

Histopathological Findings After Reirradiation Compared to First Irradiation of Spinal Bone Metastases With Stereotactic Body Radiotherapy: A Cohort Study.

BACKGROUND: Stereotactic body radiotherapy (SBRT) of the spine provides superior tumor control, but vertebral compression fractures are increased and the pathophysiological process underneath is not well understood. Data on histopathological changes, particularly after salvage SBRT (sSBRT) following conventional irradiation, are scarce. OBJECTIVE: To investigate surgical specimens after sSBRT and primary SBRT (pSBRT) regarding histopathological changes. METHODS: We assessed 704 patients treated with spine SBRT 2006 to 2012. Thirty patients underwent salvage surgery; 23 histopathological repo

A Clinical Case of 5 Times Irradiated Recurrent Orbital Hemangiopericytoma.

Orbital hemangiopericytomas (HPCs) are rare mesenchymal tumors with a high tendency to recur. Treatment options are quite limited in case of a relapse, but re-irradiation can be useful. Most of the available data limit the possibility of re-irradiation, while novel techniques (e.g., pencil beam proton therapy [PT]) open new approaches for the safe repeating of treatment. To the best of our knowledge, this is the first well-documented case of multi-times (>3) irradiation of tumors located intracranially. The case reported here describes orbital HPCs with proton irradiation performed two times s

Early circulating tumor DNA changes predict outcomes in head and neck cancer patients under re-radiotherapy.

Local recurrence after radiotherapy is common in locally advanced head and neck cancer (HNC) patients. Re-irradiation can improve local disease control, but disease progression remains frequent. Hence, predictive biomarkers are needed to adapt treatment intensity to the patient's individual risk. We quantified circulating tumor DNA (ctDNA) in sequential plasma samples and correlated ctDNA levels with disease outcome. Ninety four longitudinal plasma samples from 16 locally advanced HNC patients and 57 healthy donors were collected at re-radiotherapy baseline, after 5 and 10 radiation fractions,

Mesenchymal stem cells with high telomerase expression do not actively restore their chromosome arm specific telomere length pattern after exposure to ionizing radiation.

BACKGROUND: Previous studies have demonstrated that telomeres in somatic cells are not randomly distributed at the end of the chromosomes. We hypothesize that these chromosome arm specific differences in telomere length (the telomere length pattern) may be actively maintained. In this study we investigate the existence and maintenance of the telomere length pattern in stem cells. For this aim we studied telomere length in primary human mesenchymal stem cells (hMSC) and their telomerase-immortalised counterpart (hMSC-telo1) during extended proliferation as well as after irradiation. Telomere le

Attempts to induce resistance in mice to Schistosoma japonicum and Schistosoma mansoni by exposure to crude schistosome antigens plus cloned glutathione-S-transferases.

Several attempts have been made to induce resistance in mice to Schistosoma japonicum (Philippines) or Schistosoma mansoni by exposure to living male and/or female adult worms, their antigens or irradiated cercariae. No resistance was demonstrated in the following cases: re-exposure of mice to cercariae following praziquantel (PZQ) treatment of existing infection; re-exposure of mice following cyclosporin A (CsA) treatment at the time of first cercarial exposure; subcutaneous or intraperitoneal deposition of living male or female worms; repeated intranasal administration of crude worm homogena
