Neurosyphilis is a Central Nervous System infection that can manifest as a psychiatric condition. Although neurosyphilis is not widely considered in the differential when patients present with psychiatric symptoms, routine Rapid Plasma Reagin (RPR) screening is indicated when assessing new-onset psychiatric illness. This case report will illustrate the usefulness of RPR testing for patients admitted to inpatient or outpatient psychiatric treatment.

Objective: To compare the accuracy of different methods by reviewing syphilis serologic screening external quality assessment (EQA) data of Chinese primary health care institutions. A group of 182 workers exposed to diisocyanates (varnishers, carpenters, producers of polyurethane resins and expanded plastic) were examined to study the immunogenic capacity of these substances. Gastric syphilis in the setting of HIV is rare in the literature with only one other case reported. Herein, we present a case of a 40-year-old HIV-infected man with gastric syphilis. Evaluation revealed a rapid plasma reagin (RPR) of 1:2056, and cerebrospinal fluid with evidence of neurosyphilis. An esophagogastroduodenoscopy (EGD) was performed revealing nodular masses, which resolved after appropriate therapy with penicillin.

In a prospective study of the development of reaginic allergy, infants of allergic parents were either subjected to an allergen-avoidance regimen from birth for six months or managed conventionally. The group on the allergen-avoidance regimen had less eczema at six months and one year than did the control group at six months. They also had a lower mean serum-total-IgE level at six weeks.

Since 1995 infectious syphilis has re-emerged in Albania. As syphilis has become more common, more unusual presentations are being recognized. We present a case of an HIV-negative man with primary syphilis presenting with multiple penile ulcers and negative rapid plasma reagin and Venereal Disease Research Laboratory tests. The case illustrates the challenges of diagnosing early syphilis and the importance of not relying on non-treponemal tests.

We describe a 17-year-old man who developed penile annular and scrotal eczematoid syphilids with penile chancre redux. Dermoscopy showed linear-irregular and hairpin vessels with white scales in annular lesions. Histopathology displayed psoriasiform hyperplasia with perivascular lymphoplasmacytic dermal infiltrate. Rapid plasma reagin and Treponema pallidumparticle agglutination assays were positive. The lesions disappeared after intramuscular benzathine penicillin.

Ocular syphilis has a wide range of presentations; however, the most common findings are ophthalmalgia, blurry vision, and erythematous conjunctiva. This report presents a case of ocular syphilis in a human immunodeficiency virus-positive patient with an allergy to penicillin. On presentation, the patient was diagnosed using a fluorescent treponemal antibody absorption test, in addition to rapid plasma reagin. In this case, the patient received alternative treatment with doxycycline prior to penicillin desensitization, with marked improvement of his symptoms.

Safe and efficacious alternative treatment options for syphilis are necessary. This randomized, 2-arm, noncomparative pilot study evaluated the efficacy of oral cefixime 400 mg in achieving a ≥4-fold rapid plasma reagin titer decrease by 3 or 6 months after treatment. This case report describes a middle-aged male who presented with symptoms of dementia. During a routine screening in a lumbar puncture, a total of 209 × 109 leukocytes was found in his cerebrospinal fluid, 208 of which were mononuclear. The patient was admitted to an infectious disease ward, and serum analyses showed a positive Wassermann reaction and a rapid plasma reagin of 128. The man was diagnosed with neurosyphilis and started on penicillin treatment. This lasted 14 days, and the symptoms of dementia decreased substantially.

Among patients with primary syphilis, sensitivity of the toluidine red unheated serum test was 55% (6/11) compared with darkfield microscopy and 60% (9/15) compared with Treponema pallidum polymerase chain reaction. Sensitivity of rapid plasma reagin was 78% (38/49) and 93% (43/46), respectively. Treponema pallidum particle agglutination had a sensitivity of 95% (41/43) compared with darkfield microscopy and 96% (43/45) compared with polymerase chain reaction.

Serological tests for syphilis (with rapid plasma reagin) were performed in 2 767 women attending the antenatal clinic of King Edward VIII Hospital, Durban, for their first, booking visit. The test was repeated at the 36th week of pregnancy. A total of 152 women (5,5%) were assumed to have syphilis. In addition, 4 (11,8%) of 34 patients who were seronegative at less than 32 weeks of pregnancy had a positive serological test result at 36 weeks.

A case of acute, severe allergic conjunctivitis is reported in a biochemist who had been grinding Bandeiraea simplicifolia, the source of an anti-B lectin. A crude extract of these beans produced immediate-type skin test reactivity, and reaginic antibodies were demonstrated in her serum. Several other examples of occupationally induced allergy in laboratory and research workers in the field of allergy-immunology are briefly cited. Exposure to potential allergens in a dry, powdered form seems particularly hazardous.

Congenital syphilis is an outcome of maternal syphilis that occurs due to the transmission of Treponema pallidum via the placenta of untreated or inadequately treated pregnant women to their newborns. It is now a very rare cause of neurological, developmental, and musculoskeletal disability and death in infants after the advent of penicillin. Here, we report a case of late congenital syphilis presented with classic stigmata of syphilis at the age of 10 years. Reactive serological titer of rapid plasma reagin test and venereal disease research laboratory test confirmed the diagnosis.
