Psychophysical and functional imaging evidence supporting the presence of central sensitization in a cohort of osteoarthritis patients.

OBJECTIVE: The groin pain experienced by patients with hip osteoarthritis (OA) is often accompanied by areas of referred pain and changes in skin sensitivity. We aimed to identify the supraspinal influences that underlie these clinical manifestations that we consider indicative of possible central sensitization. METHODS: Twenty patients with hip OA awaiting joint replacement and displaying signs of referred pain were recruited into the study, together with age-matched controls. All subjects completed pain psychology questionnaires and underwent quantitative sensory testing (QST) in their area

The tooth, the whole tooth, and nothing but the tooth: can dental pain ever be the sole presenting symptom of a myocardial infarction? A systematic review.

BACKGROUND: Pain symptoms related to cardiac ischemia can vary greatly from patient to patient. However, should emergency physicians consider the possibility of myocardial infarction in patients who present solely with dental pain? OBJECTIVE: This is a systematic review of the literature investigating the incidence of jaw, tooth, or facial pain as the sole symptom of cardiac ischemia. METHODS: Studies investigating jaw, tooth, or facial pain of cardiac origin were identified using the PubMed database. All English studies in which cardiac pain originated in the face, teeth, or jaw were screen

Noninvasive discrimination of brachial plexus involvement in upper limb pain.

The ability of Elvey's "brachial plexus tension test" to identify pain referred from the cervical region was investigated in 50 patients with unilateral shoulder and upper arm pain: 25 reported symptoms commencing after open heart surgery, indicating a high probability of referred pain due to that procedure; 25 were athletes with injuries from throwing movements, a cause appearing much less likely to cause referred pain. Twenty-five asymptomatic subjects with no history of spinal or limb pain also were tested. Cervical and upper limb maneuvers of the test were assessed goniometrically. The car

Cervical spine causes for referred otalgia.

OBJECTIVE: Present experience in diagnosis and treatment for referred otalgia secondary to cervical spine degenerative disease (CSDD). STUDY DESIGN: A retrospective study of 123 patients with ear pain. SUBJECTS AND METHODS: All patients had a normal otologic examination and diagnosed with unspecified otalgia. The causes for referred otalgia were categorized into Group I: otalgia from non-cervical spine disease (n = 72), and Group II: cervical spine disease-referred otalgia (n = 51). Pain relief following cervical spine physical therapy (CSPT) was assessed. RESULTS: The most common cause for

Myofascial pain with referral: Role of nociceptive drive and central pain mechanisms.

Referred pain frequently occurs in musculoskeletal pain disorders. The differential role of peripheral (nociceptive drive) and central (pain processing) domains associated with referred pain was investigated. Utilizing TMJ IMPACT Project data, 293 participants were classified into 2 groups: referred pain (n=118) and local pain (n=175). Each domain of nociceptive drive and central processing was comprised of 7 observed variables as predictors based on face validity. Latent variables for each domain were constructed using confirmatory factor analysis (CFA). Univariate associations of predictors

[Published books on pain and its treatment in Spain. Analysis with the ISBN database].

INTRODUCTION: Although analyses have been done on the publishing of scientific articles on pain in Spanish, book publications in the field have not been studied. This article fills that gap. MATERIAL AND METHODS: A bibliography of books with pain approached from a medical standpoint was compiled from ISBN CD-ROM database (updated for 1993). Books going into more than one edition were considered single titles. Multi-volume collections were considered single books. We analyzed type of book, subject, ISBN classification, year, language (of publication and original), publisher and place of public

[Significance of subjective sensitivity in evaluation of keratoconjunctivitis sicca].

In patients with keratoconjunctivitis sicca (KCS) frequently a mismatch between the symptoms and clinical signs of impaired tear film is found. Thus, we conducted a study to determine whether tests can be found that provide information on increased sensitivity in KCS patients. Thirty patients with and without KCS were investigated by the following methods: a specific anamnestic questionnaire, esthesiometry and provocation of ocular pain by instillation of a local anesthetic eye drop. Esthesiometry only showed a shift towards higher sensitivity, but the other tests indicated significantly incre

Efficacy of IDET for relief of leg pain associated with discogenic low back pain.

Intradiscal electrothermal annuloplasty (IDET) is an effective treatment for chronic discogenic low back pain (LBP). However, efficacy of IDET for the treatment of referred leg pain has not been examined. This study was performed to assess the long-term efficacy of IDET for the treatment of referred leg pain in chronic discogenic LBP patients. Data were retrospectively analyzed as an IDET case series from January 1999 to December 2000. The IDET procedure was performed at 1-3 symptomatic levels confirmed by pressure-controlled discography. General pain outcome was evaluated by Visual Analog Sca

Quantification of changes in myofascial trigger point sensitivity with the pressure algometer following passive stretch.

In order to determine the relationship between trigger point sensitivity and the referred symptoms of myofascial pain, VAS ratings of referred pain intensity and pressure algometer measures of myofascial trigger point sensitivity were taken pre and post treatment of the muscle containing the trigger point with passive stretch. The results in 20 subjects, experiencing unilateral or bilateral myofascial head and neck pain, showed that myofascial trigger point sensitivity decreases in response to passive stretch as assessed by the pressure algometer, and that trigger point sensitivity and intensi

Non-pharmacological treatment gap preceding surgical consultation in thumb carpometacarpal osteoarthritis - a cross-sectional study.

BACKGROUND: Osteoarthritis (OA) in the thumb carpometacarpal joint (CMCJ) is a prevalent disease which may lead to structural damage, severe pain and functional limitations. Evidence-based treatment recommendations state that all patients with hand OA should be offered non-pharmacological treatment. Surgery should be considered only when other treatment has proven insufficient in relieving pain. The purpose of this study was to investigate prior treatment and characteristics of patients referred to specialist health care surgical consultation due to CMCJ OA. The study includes exploring differ

Rurality and use of specialty clinic referrals in primary care management of chronic pain: a retrospective study.

BACKGROUND: We compared referrals to pain management specialists between rural and urban primary care clinics within a single academic healthcare network (integrating hospital and outpatient services) in the Southeastern United States. MATERIALS & METHODS: We retrospectively queried primary care visits in 2022 with chronic pain-associated diagnosis codes, stratified by rural vs. urban clinic location. Referrals (pain clinic, orthopedics, neurology, rheumatology, or any of the above) were tracked through June 2024 and analyzed using Cox proportional hazard models. While some patients were refe

Referred otalgia in head and neck cancer: a unifying schema.

Pain referred to the ear is a commonly encountered clinical event, and the differential diagnoses that must be considered for pain in a normal ear are numerous. For physicians involved in the treatment of patients with referred ear pain, especially those involved in the care of patients with head and neck malignancies, a basic understanding of the mechanisms involved to produce this phenomenon is required. Several sources offer figures outlining the neuroanatomic basis of nonotogenic ear pain. On occasion, there has been omission of various components in this referred otalgia pathway, however.

Toothache referred from heart disease and lung cancer via the vagus nerve.

New epidemiologic evidence suggests that referred craniofacial pain in coronary heart disease is more common than previously believed. Current medical reports suggest that in addition to coronary disease, thoracic disorders such as aortic dissection, pericarditis, and lung cancer can cause referred craniofacial pain. Recent physiologic evidence from animals and humans suggests that the vagus nerve mediates this referral of cardiac pain to the maxillofacial region. This article discusses the critical role of the dentist in patient education and recognition of referred head and neck pain in thor

Effects of Bepridil and Pimozide, Existing Medicines Capable of Blocking T-Type Ca2+ Channels, on Visceral Pain in Mice.

T-Type Ca2+ channels (T-channels), particularly Cav3.2, are now considered as therapeutic targets for treatment of intractable pain including visceral pain. Among existing medicines, bepridil, a multi-channel blocker, used for treatment of arrhythmia and angina, and pimozide, a dopamine D2 receptor antagonist, known as a typical antipsychotic, have potent T-channel blocking activity. We thus tested whether bepridil and pimozide could suppress visceral pain in mice. Colonic and bladder pain were induced by intracolonic administration of 2,4,6-trinitrobenzene sulfonic acid (TNBS) and systemic ad

Generalised muscular hyperalgesia in chronic whiplash syndrome.

The whiplash syndrome has immense socio-economic impact. Despite extensive studies over the past years, the mechanisms involved in maintaining the pain in chronic whiplash patients are poorly understood. The aim of the present experimental study was to examine the muscular sensibility in areas within and outside the region involved in the whiplash trauma. Eleven chronic whiplash patients and 11 sex and age matched control subjects were included in the study. Before the experiment, the whiplash patients had pain in the neck and shoulder region with radiating pain to the arm. Five patients repor
