Editorial Commentary: Beyond the Number: Why Return to Sport After Hip Arthroscopy Can't Be One Size Fits All.

This editorial commentary reflects on the evolving field of hip arthroscopy in the elite athlete and the growing recognition that return-to-sport outcomes cannot be captured by a "one-size-fits-all" percentage. The heterogeneity we see in return-to-sport rates is not a flaw, but an authentic reflection of the athletes who we treat. Instead of relying on a universal return-to-sport benchmark, we should be embracing a more data-driven, individualized approach that allows us to counsel elite athletes with greater accuracy and nuance as they navigate recovery after hip arthroscopy.

Editorial Commentary: A Call for Clarity-Standardizing Return-to-Sport Definitions Is Essential for Interpreting Hip Arthroscopy Outcomes.

Outcome reporting in sports medicine has become increasingly sophisticated, yet sophistication alone does not ensure clinical usefulness. The value, or utility, of an outcome measure lies in its ability to inform patient expectations, shared decision-making, and treatment selection. This limitation is particularly evident in the reporting of return-to-sport (RTS) outcomes. Differences in RTS definitions can yield substantially different reported success rates without reflecting true differences in surgical effectiveness. For example, defining RTS as return to preinjury competitive level produc

Factors associated with return-to-sport outcomes following pathogen-confirmed acute respiratory infections in athletes: AWARE X study.

OBJECTIVE: To identify factors associated with return-to-sport (RTS) outcomes following confirmed acute respiratory infections (ARinf) in athletes. METHODS: This prospective cohort study included 114 confirmed ARinf cases among athletic individuals. Causative pathogens were identified using multiplex PCR testing, and illness severity was classified per International Olympic Committee consensus (mild, moderate and severe). RTS outcomes included time (days) to return-to-training (RTT), return-to-full-training (RTFT) and return-to-full-performance (RTFP). Cox regression models (HRs) assessed ass

Return-to-sport following lateral ankle sprain: Connecting scientific frameworks and real-world protocols of elite sports teams.

Lateral ankle sprains (LAS) are the most common lower-limb sports injuries and demonstrate high recurrence rates. Although several return-to-sport (RTS) frameworks exist, empirical, sport-specific criteria remain limited. Clinicians rely on heterogeneous or incomplete assessment strategies. This paper aims to answer how current RTS guidelines for LAS compare to RTS protocols in elite sports teams; and how RTS decision-making after LAS can be optimised by connecting conceptual frameworks and real-life protocols. Elite teams' protocols (n = 6) demonstrated large variability in RTS-phases, progre

Association Between Functional Muscle Assessment and Psychological Factors on Return to Sport and Graft Failure After ACL Reconstruction.

BACKGROUND: Few researchers have examined both psychological and physical factors simultaneously in return to sport (RTS) and reinjury. PURPOSE: To assess the role of psychological profiles and physical aspects in RTS and reinjury risk and to inform and develop innovative treatment strategies that may be beneficial after anterior cruciate ligament (ACL) reconstruction. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: During the rehabilitation process, patients (aged 18-45 years) with ACL reconstruction (using patellar tendon or hamstring autografts) were screened. A statistical an

High rates of return-to-sport and work in military personnel after arthroscopic posterior capsuloplasty of the wrist.

OBJECTIVE: Scapholunate ligament injuries lead to wrist pain exacerbated by hyperextension and a reduction in strength, posing challenges for military personnel who require high physical fitness for sports and heavy lifting. This study primarily aimed to assess return-to-work rates using the REFA physical workload scale (Reichsausschuss Für Arbeitszeitermittlung) and return-to-sport following isolated arthroscopic posterior capsuloplasty. Secondary objectives included evaluating pain during forced hyperextension, functional assessments, range of motion, and radiological outcomes. MATERIAL AND

Athletes with Implantable Cardioverter-Defibrillators: A Practical Framework for Device Selection, Programming, and Return-to-Sport Risk Mitigation.

Implantable cardioverter-defibrillators (ICDs) were historically viewed as a barrier to vigorous or competitive sport participation, based on theoretical concerns about arrhythmia induction, inappropriate shocks, device damage, and shock-related injury during activity. Contemporary registry data have reframed this paradigm, demonstrating that selected athletes with ICDs can participate in organized sport without the catastrophic outcomes once feared, although these data remain reassuring rather than definitive given selection bias and underrepresentation of high-consequence sports. This review

Objective Return-to-Sport (RTS) Decision Frameworks for Adolescent Shoulder and Elbow Injuries: Tests, Thresholds and Exercise Targets.

BACKGROUND: Adolescent shoulder and elbow injuries are increasing due to sports specialization and year-round training, yet return-to-sport decisions typically rely on adult-derived heuristics that fail to address the unique physiology, growth plate vulnerability, and recovery needs of developing athletes aged 10-19 years. OBJECTIVE: This review proposes a novel, objective three-pillar RTS framework integrating: (1) foundational tissue capacity, (2) dynamic movement quality and neuromuscular control, and (3) sport-specific load tolerance. We synthesize current evidence to establish provisiona

Differential Quadriceps Muscle Activation Preoperatively and at the Time of Return to Sport After Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: Patients with patellofemoral instability (PFI) often demonstrate altered movement patterns and strength deficits. The vastus medialis plays an important role in patellar stabilization as a dynamic medial force vector on the patella. HYPOTHESIS: It was hypothesized that limb-to-limb asymmetries in muscle activity would be present preoperatively and at the time of return to sport (RTS). STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A total of 25 adolescents with unilaterally treated PFI were evaluated before arthroscopic assisted medial patellofemoral ligamen

[Autologous hamstring versus quadriceps graft in anterior cruciate ligament plasty. Comparative study with focus on Return-To-Sport].

INTRODUCTION: anterior cruciate ligament rupture has evolved to be one of the most common sports injuries with a remarkable increase in its incidence in the last two decades, with Return-To-Sport being a crucial approach in the choice of autologous graft for its surgical repair, the most commonly used being hamstring and quadriceps tendon graft. MATERIAL AND METHODS: a prospective randomized study was performed with 32 patients divided into two groups, one for each type of graft, with 13 patients in each. They were evaluated before surgery, at 30, 180 and 360 days after the procedure using sc

The Role of Early and Delayed Surgery in Return to Sport After Anterior Shoulder Dislocation-A Scoping Review.

Background: Anterior shoulder dislocations are common in athletes, particularly in contact sports. Surgical stabilization reduces recurrence, but the optimal timing-early versus delayed-remains uncertain, especially for in-season athletes. Methods: A systematic search of PubMed, Embase, and Cochrane (2013-2023) yielded 945 articles; 15 met the inclusion criteria. Data were charted on procedure type, outcomes, follow-up, patient group, and timing of surgery. Search terms, e.g., 'shoulder', 'athlete', 'anterior' and 'shoulder dislocation', were used in a broad search protocol casting a wide net

Uninjured Youth Athlete Performance on Single-Leg Hop Testing: How Many Can Achieve Recommended Return-to-Sport Criterion?

BACKGROUND: Current anterior cruciate ligament reconstruction (ACLR) guidelines utilize single-leg hop tests (SLHTs) to assist in return-to-sport decision making. A limb symmetry index (LSI) of ≥90% is often required; however, after ACLR, most youth athletes cannot achieve this standard. Reporting the performance of age-matched normative controls will allow clinicians to compare post-ACLR performance with noninjured peers, improving the utility of SLHTs. The purpose of this study was to report hop test LSI within healthy youth athletes and determine whether athlete performance surpasses post-A
