BACKGROUND: Tears of the rotator cuff tendons, which surround the joints of the shoulder, are one of the most common causes of pain and disability in the upper extremity.
OBJECTIVES: To review the efficacy and safety of common interventions for tears of the rotator cuff in adults.
SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group specialised trail register (July 2002), the Cochrane Controlled Trials Register (The Cochrane Library issue 2, 2002), MEDLINE (1966 to December 2001), EMBASE (1974 to December 2001), Biological Abstracts (1980 to December 2001), LILACS (1982 t

BACKGROUND: Degenerative rotator cuff tears are common and are often treated with surgical repair. Randomised trials have not shown a clear advantage to surgery over non-surgical treatment, but there have been no published placebo-controlled trials investigating rotator cuff repair. This study aims to compare arthroscopic shoulder surgery with rotator cuff repair to surgery without rotator cuff repair (placebo) for improving shoulder pain and function in people with shoulder pain and full-thickness degenerative rotator cuff tears.
METHODS: The study is a multicentre two-parallel arm, blinded,

BACKGROUND: A common complication of rotator cuff (RC) repair is a postoperative stiffness. Postoperative stiffness may develop and lead to inferior functional outcomes. Rotator cuff repair with additional rotator interval (RI) capsular release can be done to prevent postoperative stiffness in rotator cuff tears. However, routine rotator interval capsular release in rotator cuff repair is controversial. Therefore, we conducted a systematic review and meta-analysis to compare the postoperative outcomes between RC repair with and without RI capsular release for RC tears with and without concomit

BACKGROUND: The muscle quality of the rotator cuff (RC), measured by atrophy and fatty infiltration (FI), is a key determinant of outcomes in RC injury and repair. The ability to regenerate muscle after repair has been shown to be limited.
PURPOSE: To determine if there is a source of resident endogenous stem cells, fibroadipogenic progenitor cells (FAPs), within RC injury patients, and if these cells are capable of adipogenic, fibrogenic, and pro-myogenic differentiation.
STUDY DESIGN: Controlled laboratory study.
METHODS: A total of 20 patients between the ages of 40 and 75 years with par

BACKGROUND: Treatments for rotator cuff tendinopathy include rotator cuff muscle strengthening to promote better muscle recruitment in order to minimize subacromial narrowing during active movement. Glenohumeral adductor recruitment has also been shown to prevent such narrowing in asymptomatic individuals; therefore, adding glenohumeral adductor coactivation during rotator cuff strengthening could enhance the efficacy of rotator cuff strengthening. However, no study has explored its benefits.
OBJECTIVES: To compare the short-term efficacy of adding glenohumeral adductor coactivation to a rota

BACKGROUND: Arm adductor co-activation during abduction has been reported as a potential compensation mechanism for a narrow subacromial space in patients with rotator cuff dysfunction. We assessed differences in acromiohumeral distance at rest and the amount of humerus translation during active abduction and adduction in patients with rotator cuff tears (n=20) and impingement (n=30) and controls (n=10), controlled for deltoid, pectoralis major, latissimus dorsi and teres major activation (electromyography).

BACKGROUND: It was the objective of this study to evaluate the contact area and pressure between a repaired rotator cuff tendon and bone for several different rotator cuff repair techniques, with and without a subacromial balloon spacer. It was hypothesized that the contact area and pressure at the rotator cuff footprint would increase with a subacromial balloon spacer following all repair techniques evaluated.
METHODS: Ten cadaveric shoulders were mounted onto a biomechanical shoulder simulator. A full-thickness 2-cm rotator cuff tendon tear was simulated, and repaired using single-row, doub

Study Design Clinical measurement. Background Recently, the Western Ontario Rotator Cuff Index (WORC) was shortened, but few studies have reported its measurement properties. Objective To compare the validity and responsiveness of the short version of the Western Ontario Rotator Cuff Index (Short-WORC) and the WORC (disease-specific measures) with those of the Shoulder Pain and Disability Index (SPADI) and the simple shoulder test (SST) (joint-specific measures); the Disabilities of the Arm, Shoulder and Hand (DASH) (a region-specific measure); and the Medical Outcomes Study 12-Item Short-Form

BACKGROUND: Pseudoparalysis is defined as active forward flexion less than 90° with full passive motion. There is controversy about the ideal surgical management of a massive rotator cuff tear with pseudoparalysis.
PURPOSE/HYPOTHESIS: The purpose of this study was to prospectively analyze the ability to reverse pseudoparalysis with an arthroscopic rotator cuff repair (ARCR). The hypothesis was that in the absence of substantial glenohumeral arthritis, preoperative fatty infiltration of grade 3 or higher and an acromiohumeral interval (AHI) of less than 7 mm would not prevent reversal of pseud

Thirty-three patients (35 full-thickness rotator cuff tears) who underwent arthroscopically assisted mini open repair between June 1987 and January 1990 were evaluated for shoulder function and cuff integrity. The study population was composed of 19 women and 14 men with an average age of 63 years (range 35-76) and an average follow-up of 3.7 years (range 2.5-5.1). Functional results were obtained using the UCLA Shoulder Rating Scale. Integrity of the rotator cuff was established by shoulder arthrography at a minimum 2 years postoperatively. UCLA Shoulder Rating Scale showed 86% good/excellent

PURPOSE: Because recurrent or persistent defects in the rotator cuff after repair are common, we sought to clarify the correlation between structural integrity of the rotator cuff and clinical outcomes through a systematic review of relevant studies.
METHODS: Medline, CINAHL (Cumulative Index to Nursing and Allied Health Literature), and the Cochrane Central Register of Controlled Trials were searched for all literature published from January 1966 to December 2008 that used the key words shoulder, rotator cuff, rotator cuff tear, rotator cuff repair, arthroscopic, integrity, healed, magnetic

PURPOSE: The quantitative assessment of muscle atrophy has a degree of importance in prognosticating rotator cuff treatment. However, it has been conjectured that muscle fat increases with aging. Therefore, we thought that the quantitative assessment of the supraspinatous would be better if made in comparison with a standard of reference such as the deltoid. Consequently, we performed a two-part study, first evaluating supraspinatous changes compared with the deltoid in "normals" with aging, and second, determining if in patients with cuff tears the supraspinatous fat exceeds that of the delto

BACKGROUND: Massive rotator cuff tears (RCTs) with significant retraction are challenging due to the high retear rates and limitations associated with existing repair techniques. Biceps tendon rerouting has recently emerged as an effective reinforcement technique for massive RCTs, particularly in achieving low retear rates.
INDICATIONS: Arthroscopic RCT repair with proximal biceps tendon rerouting is a viable option for nonarthritic, active patients with massive, retracted RCTs for whom nonoperative treatment has failed and the long head of the biceps tendon is intact.
TECHNIQUE DESCRIPTION:

PURPOSE: Traumatic full-thickness rotator cuff tears (FT-RCTs) are often treated surgically due to the acute nature of the tear. Presently, there is not a clear consensus on the appropriate timing for repair of FT-RCTs following injury. This study aims to evaluate functional outcomes based on timing of surgical repair.
METHODS: In total, 141 patients who underwent RCT repair for a tear caused by a traumatic injury from 2014 to 2021 were retrospectively reviewed. Patients were stratified based on time to surgical repair from the date of injury. Patient reported outcomes (PROMs), complication r

Irreparable rotator cuff tears present significant challenges owing to tear size, tendon retraction, and poor tissue quality. This article describes a surgical approach integrating biologic tuberoplasty with rotator cuff repair, using an acellular human dermal allograft to re-establish the rotator cuff footprint and prevent bone-on-bone contact between the humeral head and acromion. Footprint reconstruction is defined as allograft coverage of the tuberosity combined with partial cuff repair that includes some contact of the native cuff over the allograft. Changing the nomenclature to "biologic

Rotator cuff injuries are a common cause of shoulder pain, primarily affecting individuals older than 40 years, with the incidence increasing progressively with age. Advancements in surgical techniques have led to the development of arthroscopic techniques. This evolution has transitioned through various arthroscopic suture anchor rotator cuff repair configurations, including single-row, double-row, and transosseous-equivalent constructs. Although these innovations have replicated the biomechanics of the original transosseous technique conducted using an open approach, they often require multi

The use of imaging after shoulder surgery should be considered carefully and should provide a real benefit for the patient. It is not a routine request. Currently, there is no consensus on imaging indications for patients showing good postoperative progress, without complications and with satisfactory functional outcomes. In cases of unfavorable progress, the indications are clearer. Conventional radiography remains essential, and when combined with ultrasound, forms the first-line approach. Second-line imaging techniques, such as CT scans or MRI, are selected based on the type of surgery perfor
