Laparoscopic sleeve gastrectomy for linear scleroderma.

Linear scleroderma is a rare type of autoimmune connective-tissue disorder and is one of five known types of localized, nonsystemic scleroderma. This type of localized scleroderma affects the skin and subcutaneous tissues and induces extensive fibrosis and atrophy of the affected limbs. The disease is, characteristically, a unilateral phenomenon and usually affects the lower extremities. Due to its underlying pathophysiology, linear scleroderma is rarely associated with morbid obesity. This article documents the case of a 45-year-old female with a 35-year history of unilateral linear scleroder

[Diagnostic and therapeutic problems of scleroderma].

Scleroderma is the autoimmunologic disease with induration and fibrosis of the skin, subcutaneous tissue; sometimes refers to muscles, bones and other internal organs. Pathogenesis of morphea is still unknown. There are two main types of scleroderma: the first type is related only to the skin (localized scleroderma--morphea), while the second is connected with fibrosis and induration of the skin, lungs, heart and other organs (systemic sclerosis). There are various clinical forms of scleroderma circumscripta: linear scleroderma (the most frequent in children), morphea en plaque, generalized mo

Linear scleroderma with prominent multiple lymphadenopathy followed by the development of polymyositis: A case report and review of published work.

Localized scleroderma is an inflammatory disorder affecting the skin and underlying tissues, a certain subset of which develops other autoimmune diseases on the basis of a prominent autoimmune background. We here report a unique case of linear scleroderma presenting with a sclerotic plaque on the left thigh, multiple lymphadenopathy in bilateral inguinal and para-aortic lymph nodes, and hepatosplenomegaly, who later developed polymyositis. We describe the detailed disease course of our case and discuss the clinical significance of multiple lymphadenopathy in localized scleroderma based on a re

Juvenile Scleroderma-What has Changed in the Meantime?

BACKGROUND: Juvenile scleroderma is a rarely seen chronic connective tissue disorder characterized by stiffening of the skin. The frequency of the disease was reported as one per million. According to organ involvement, the disease is divided into two main forms: systemic and localized scleroderma. OBJECTIVE: Since it is uncommon in children, many aspects of the disease remain discussable. With this review, we aimed to revise recent findings and new developments in this rare condition. METHOD: A systematic literature research was performed, using the following medical databases: Pubmed/ Medl

Association of acrodermatitis chronica atrophicans and morphea.

A patient developed successively acrodermatitis chronica atrophicans (ACA) and localized scleroderma. ACA was demonstrated to be a tick-borne disease, whereas morphea is only suspected to be one. As morphea appeared after successful treatment of ACA, it suggests that localized scleroderma is not directly an infectious disease but might be rather the consequence of spirochetal infection.

Advanced Neuroimaging and Clinical Findings in a Case of Linear Scleroderma En Coup De Sabre Presenting With Seizures.

Scleroderma is a rare connective tissue disease categorized as systemic or localized. Linear subtype of localized scleroderma usually manifests as a cutaneous linear scar-like lesion most commonly on the scalp. It may present with neurologic, ophthalmologic, and rheumatologic symptoms. We present a case of a 24-year-old female patient with a band-like scalp brown sclerotic lesion in the left upper frontal region presenting with headaches and a first-time seizure who underwent 3.0-T magnetic resonance and described the structural and advanced neuroimaging findings, including perfusion and spect

Topical calcipotriene for morphea/linear scleroderma.

BACKGROUND: Morphea and linear scleroderma are characterized by erythema, induration, telangiectasia, and dyspigmentation. There is no universally effective treatment. Oral calcitriol has been beneficial in the treatment of localized and extensive morphea/scleroderma, but the use of topical calcipotriene has not been reported. OBJECTIVE: The purpose of this study was to evaluate the efficacy and safety of topical calcipotriene 0.005% ointment in the treatment of localized scleroderma. METHODS: In a 3-month open-label study, 12 patients aged 12 to 38 years with biopsy-documented active morphe

Clinical Profile of Methotrexate-resistant Juvenile Localised Scleroderma.

Methotrexate has demonstrated its efficiency for the treatment of juvenile localized scleroderma but some patients may be resistant. The aim of our study was to define the profile of such patients. We performed an observational retrospective multicenter study between 2007 and 2016 and included all children seen in the French Paediatric Dermatology and Rheumatology departments with active localized scleroderma treated by methotrexate for a minimum of 4 months. Metho-trexate efficacy was assessed clinically and/or by imaging between the fourth to twelfth months of treatment. A total of 57 patien

Scleroderma in hospital settings in Lomé: 50 cases.

The aim of this study was to document the epidemiological and clinical profile, treatment used, and outcome of patients with scleroderma in hospital settings in Lomé. This descriptive study examined the records of all patients seen as outpatients or admitted for scleroderma in hospital dermatology and rheumatology departments in Lomé during the 20-year period of 1993-2012. During the study period, 50 (0.04%) of the 121,021 patients seen in these departments had scleroderma. There were 29 cases of localized scleroderma and 21 systemic cases, predominantly women (sex-ratio=0.2). The patients' me

Scleroderma-like Disorders.

BACKGROUND: Scleroderma is a term used to describe diseases that involve hardening and tightening of the skin and the underlying subcutaneous connective tissue. It could be localized to skin and subcutaneous tissue, or may involve the internal organs too in systemic sclerosis. OBJECTIVE: There are disorders that can cause hardening and tightening of skin and mimic scleroderma but are rarely associated with Raynaud phenomenon, sclerodactyly, and autoantibodies in the serum, features specific to scleroderma/systemic sclerosis. CONCLUSION: These are termed as "scleroderma variants" or "sclerode

Scleroderma with an update about clinico-pathological correlation.

Scleroderma is divided into a systemic form called systemic sclerosis and a localized form also called morphea. According to 2013 ACR/EULAR Classification Criteria for Systemic Sclerosis, developed by the American College of Rheumatology (ACR) and the European League Against Rheumatism (EULAR), skin thickening of the fingers extending proximal to the metacarpophalangeal joints is sufficient for a patient to be classified as having scleroderma. Histological examination is not included in the diagnostic criteria and is not routinely performed. Skin biopsy is recommended only in the case of diagn

Chromosomal alterations in scleroderma.

To bring some new arguments in support of the genetic theory regarding the etiology and pathogenesis of scleroderma 12 patients (2 males and 10 females, ranging in age between 13 and 64 years) with both localized and systemic scleroderma were studied. For each patient chromosomal preparations were made in cell cultures from peripheral blood. The structural cytogenetic aberrations, observed in proportion of 81.81%, were, in the order of frequency: chromatid breakage, gaps, acentric fragments, deletions. The alterations appeared both in the systemic scleroderma and in the localized one but canno

Linear scleroderma en coup de sabre and brain calcification: is there a pathogenic relationship?

Extracutaneous calcifications are rare in scleroderma and related conditions. We describe a female patient with linear scleroderma en coup de sabre and a longstanding clinical history of tonic and clonic convulsions. Radiographic study showed extensive cerebral calcifications in the right occipital hemisphere, homolateral to the involved side of her face. This report further suggests a relationship between localized scleroderma and neurological manifestations. Brain imaging studies should be routinely performed in scleroderma patients exhibiting neurological manifestations, especially seizure

[Coincidence of scleroderma and primary biliary cirrhosis. Results of a systematic study of a dermatologic patient sample].

We present for the first time in the German dermatologic literature results of systematic investigations on the relationship between scleroderma, antimitochondrial autoantibodies and primary biliary cirrhosis (PBC). 40 patients with different clinical pictures of systemic and localized scleroderma were examined. By means of indirect immunofluorescence technique, in the sera of 5 cases (12.5%) antimitochondrial autoantibodies (AMA) could be detected. The target autoantigens for the AMA were identified as pyruvate dehydrogenase, branched-chain alpha-ketoacid dehydrogenase, alpha-ketoglutarate de

Interferons dominate damage and activity in juvenile scleroderma.

OBJECTIVES: Juvenile scleroderma is a heterogeneous group of diseases associated with sclerotic skin lesions, grouped as juvenile systemic sclerosis and juvenile localized scleroderma. This study aims to measure the cytokine and chemokine levels involved in interferon (IFN) signalling in patients with juvenile scleroderma and determine their correlation with disease severity. METHODS: Twenty-nine juvenile localized scleroderma, five juvenile systemic sclerosis, and nine healthy controls were included in the study. Cytokines and chemokines involved in IFN gene signalling (IL-1, IL-6, IL-8, IP-

Use of Extracorporeal Photopheresis in Scleroderma: A Review.

BACKGROUND: Scleroderma is a heterogeneous group of diseases that can be localized or systemic. Localized scleroderma is a fibrosis of the skin characterized by inflammation and thickening due to excessive collagen deposition, and systemic sclerosis (SSc) is characterized by vasculopathy, immune dysregulation and skin fibrosis. In general, the prognosis of scleroderma highly depends on the degree of visceral involvement and relates to the degree of skin fibrosis. Despite the numerous therapies used for patients with scleroderma, the disease-related morbidity and mortality are high. Studies hav

A case of linear scleroderma and myasthenia gravis.

We report a case of a 28-year-old woman with myasthenia gravis who developed linear scleroderma seven years later. Myasthenia gravis and scleroderma are rarely found in direct association with each other; there are only five such reported cases, all of which were systemic scleroderma patients. Although localized and systemic scleroderma are distinct entities, autoimmunity is believed to be involved in the pathogenesis of both. Myasthenia gravis and scleroderma may occur coincidentally, but an autoimmune predisposition seems to be the more likely underlying cause, as evidenced by an increased i
