PURPOSE OF REVIEW: Neonatal seizures continue to present a diagnostic and therapeutic challenge to paediatricians worldwide, and are a worrying sign for both parents and clinicians alike. The present review summarizes recent evidence regarding the diagnosis, aetiology and treatment of neonatal seizures. It is timely because there is new evidence that seizures are damaging to the neonatal brain, and because prolonged electroencephalographic recordings during treatment have provided information that challenges established treatment regimens.
RECENT FINDINGS: Neonatal seizures can permanently di

Pyridoxine-dependent seizure is a rare autosomal recessive disorder that usually presents with neonatal intractable seizures. This syndrome results from an inborn abnormality of the enzyme glutamic acid decarboxylase, which results in reduced pyridazine-dependent synthesis of the inhibitory neurotransmitter gamma amino butyric acid. The full range of symptomatology is unknown; but can be associated with autism, breath holding and severe mental retardation, bilious vomiting, transient visual agnosia, severe articulatory apraxia motor dyspraxia, microcephaly and intrauterine seizures. Parenteral

Febrile seizures are the most common form of convulsion, occurring in 2-5% of infants in Europe and North America and in 6-9% in Japan. In large families, the febrile seizure (FS) susceptibility trait is inherited by the autosomal dominant pattern with reduced penetrance. Two putative FS loci, FEB1 (chromosome 8q13-q21) and FEB2 (chromosome 19p13.3) have been mapped. A clinical subset of FS, termed generalized epilepsy with febrile seizures plus (GEFS+), was reported. In GEFS+ families, a mutation in the voltage-gated sodium channel beta1 subunit gene (SCN1B) at chromosome 19q13.1 and two muta

PURPOSE: To outline the clinical characteristics of seizures in our large series of Behçet disease (BD) patients with neurologic involvement.
METHODS: All files of 223 patients with neuro-BD were evaluated retrospectively, and the group with clearly documented seizures was included in the current study. Clinical characteristics, EEG, neuroimaging and cerebrospinal fluid findings were reevaluated, and the seizures were classified according to the new proposed criteria of the International League Against Epilepsy. On excluding the patients in whom the seizures were due to possible seizure-provo

OBJECTIVE: To describe an adverse effect with intravenous codeine in a chid diagnosed with sickle cell anemia.
CASE SUMMARY: A seven-year-old boy with sickle cell anemia was admitted to the emergency department with severe pain unresponsive to high doses of oral acetaminophen; subsequently, intravenous codeine phosphate was administered. The patient immediately developed a tonic-clonic seizure, which was treated with intravenous diazepam and naloxone.
DISCUSSION: Seizures associated with the intravenous administration of codeine phosphate have not been extensively reported in the literature,

Although neonatal seizures are quite common, there is controversy regarding their consequences. Despite considerable evidence that seizures may cause less cell loss in young animals compared with mature animals, there are nonetheless clear indications that seizures may have other potentially deleterious effects. Because it is known that seizures in the mature brain can increase neurogenesis in the hippocampus, we studied the extent of neurogenesis in the granule cell layer of the dentate gyrus over multiple time points after a series of 25 flurothyl-induced seizures administered between postna

It is imperative for a radiologist to determine the type of seizure a patient has prior to magnetic resonance (MR) imaging to optimally provide the clinician with the information he or she requires. Specifically, complex partial seizures require evaluation of the frontal lobes and the hippocampus (for mesial temporal sclerosis). These are best evaluated with fluid-attenuated inversion recovery (FLAIR) imaging; the use of intravenously administered contrast material is not required. Other types of chronic seizures are best evaluated with nonenhanced FLAIR or T2-weighted imaging for low-grade tu

Many disorders can mimic complex partial seizure (CPS), and clinicians must be careful to make an accurate diagnosis. Complex partial seizures must be distinguished from generalized-onset seizures (such as absence seizures), which require a different treatment regimen. Treatment of CPS should begin with monotherapy with a standard antiepileptic drug (AED), such as phenytoin or carbamazepine. Increasing evidence indicates that the newer AEDs, such as lamotrigine, topiramate, and possibly gabapentin, are effective as monotherapy. These AEDs may prove to be as effective as the standard AEDs but w

Phenytoin toxicity is an uncommon problem seen in clinical practice. The predisposing factors for toxicity are hypoalbuminemia, chronic renal failure, hepatic dysfunction and drugs which interfere with phenytoin metabolism. Common manifestations of toxicity, like confusion and ataxia, are well known. A less well known phenomenon is paradoxical seizures. In this condition, seizures develop as the serum phenytoin level rises and decrease in frequency as levels drop. It may or may not be accompanied by other features of toxicity. We present three patients with paradoxical seizures; their serum ph

While the evaluation and treatment of patients with seizures or epilepsy is often challenging, modern therapy provides many patients with complete seizure control. After a first seizure, evaluation should focus on excluding an underlying neurologic or medical condition, assessing the relative risk of seizure recurrence and determining whether treatment is indicated. Successful management of patients with recurrent seizures begins with the establishment of an accurate diagnosis of epilepsy syndrome followed by treatment using an appropriate medication in a manner that optimizes efficacy. The go

A retrospective analysis of seizure incidence in 511 patients who underwent posterior possa operations via a suboccipital craniectomy with prophylactic anticonvulsant agents, was performed. Thirty patients (5.9%) experienced seizures within 2 weeks postoperatively. Their mean age was 15 years. Twenty patients had seizures within 3 h of operation; 24 patients had generalized tonic clonic seizures. Focal motor seizures with secondary generalization and simple focal motor seizures were recorded in three patients each. The incidence of postoperative seizures was highest in patients with acoustic s

We report our preliminary observations on the role of Gabapentin as an add - on therapy in 16 patients with partial or secondary generalised seizures who were resistant to conventional antiepileptic drugs. There was a reduction in the seizure frequency by more than 50 in 11 (69), less than 50 in 3 (18.75), and increase in 1 (6.25). Adverse effects were noted in 9 (56.25 ) of them. The giddiness and sleepiness was seen in 5 (31.25 ); in one it was so severe that he withdrew from the study. Ataxic gait was observed in 1 (6.25). Reduction in the haemoglobin by 2 gms. occurred in 2 (12.5) and asym

Three cases are reported in which symptoms of occipital seizures resembled the visual aura of migraine. Careful recording of the characteristics and timing of such visual effects will often resolve the diagnostic dilemma.

Seizures are not an uncommon complication of medical conditions and should not be ascribed simply to "multifactorial" causes. Although many interacting factors may contribute to a lowered threshold for seizures, a careful systems-oriented search usually yields specific factors that are of primary importance. Not all medically complex patients with seizures have persistent or irreversible processes that lead to recurrent seizures or a need for maintenance antiepileptic drugs (AEDs). Because AEDs contribute to the potential for drug interactions, side effects, and toxicities in situations alread

Seizures are the most frequent neurological event in newborns (NBs), provoked often by noxae not apt to cause them in later life. This is because receptor families of excitatory amino acids (EAA) are overexpressed at this stage of brain ontogenesis, which is also why most neonatal seizures rapidly abate, even when neurological deficits persist. The brain's immaturities dictate distinct seizure phenotypes. A classification proposed in the late 1960s has been criticized, and a new one has been advocated, based on correlations between EEGs and behaviors, leading to a classification of seizures in

BACKGROUND: A review of the literature and an extensive Medline search revealed that this is the first case report of fetal seizures diagnosed by ultrasound.
CASE: A 23-year-old woman was found to have a fetus with bilateral choroid plexus cysts during a second-trimester ultrasound examination. Karyotype and alpha-fetoprotein level were normal. The cysts resolved, but idiopathic polyhydramnios was noted. Fetal testing beginning at 32 weeks revealed abnormal fetal behavior patterns on ultrasound and documented fetal seizures with rapid, repetitive limb movements noted on several occasions and
