Immunomodulatory effects of anti-estrogenic drugs.

There are substantial experimental, epidemiological and clinical evidences that show that breast cancer pathology is influenced by endogenous estrogens. This knowledge is the foundation upon which endocrine deprivation therapy has been developed as a major modality for the management of breast cancer. Tamoxifen, which functions as a competitive partial agonist-inhibitor of estrogen at its receptor, has been widely used for more than three decades for adjuvant endocrine treatment in breast cancer. Currently, other effective drugs for endocrine therapy include raloxifene, different aromatase inh

Endocrine manipulation in advanced breast cancer: recent advances with SERM therapies.

Tamoxifen is one of the most effective treatments for breast cancer through its ability to antagonize estrogen-dependent growth by binding estrogen receptors (ERs) and inhibiting breast epithelial cell proliferation. However, tamoxifen has estrogenic agonist effects in other tissues such as bone and endometrium because of liganded ER-activating target genes in these different cell types. Several novel antiestrogen compounds have been developed that are also selective ER modulators (SERMs) but that have a reduced agonist profile on breast and gynecological tissues. These SERMs offer the potenti

Phytoestrogens: implications in neurovascular research.

The early discontinuation of the Women's Health Initiative trial evaluating the effects of estrogen plus progestin due to concerns about the risk-benefit ratio of this steroid combination therapy emphasizes the need to examine alternative methods of estrogen replacement. One such alternative is isoflavone consumption of soy-derived dietary phytoestrogens that have received prevalent usage due to their ability to decrease age related disease (cardiovascular and osteoporosis), hormone-dependent cancers (breast and prostate), and peri- and postmenopausal symptoms. Differences in dietary phytoestr

Pharmacology and clinical use of sex steroid hormone receptor modulators.

Sex steroid receptors are ligand-triggered transcription factors. Oestrogen, progesterone and androgen receptors form, together with the glucocorticoid and mineralocorticoid receptors, a subgroup of the superfamily of nuclear receptors. They share a common mode of action, namely translating a hormone-i.e. a small-molecule signal-from outside to changes in gene expression and cell fate, and thereby represent "natural" pharmacological targets. For pharmacological therapy, these receptors have originally been addressed by hormones and synthetic hormone analogues in order to overcome pathologies re

The effect of long-term oral tamoxifen and metabolism in the mouse ovary.

Tamoxifen (TAM) is a selective estrogen receptor modulator (SERM) widely used as long-term chemotherapy for premenopausal women with estrogen-sensitive breast cancer. We developed a murine model of chronic TAM exposure by administering TAM admixed in mouse chow (TAM-chow), a less stressful and physiologically relevant alternative to daily gavage or injections. Mice received either 36 mg/kg/day or 72 mg/kg/day for 2 or 5 weeks. Body weights were monitored every 2-days; blood and tissue samples collected at endpoints. TAM and key metabolites (N-desmethyl-TAM (NDM-TAM), 4-hydroxytamoxifen (4-OH-T

[Treatment of bone mass loss in postmenopausal women].

In this revision, the results of different controlled clinical trials and metaanalyses on the efficacy in the increase of bone mineral density (BMD) and reduction of risk of fracture on postmenopausal osteoporosis treatments are summarized. Most of the drugs studied produce significant BMD increases but with significant differences regarding fracture risk reduction, especially regarding extravertebral fractures. Bisphosphonates and selective estrogen receptor modulators would constitute the first line of treatment of postmenopausal osteoporosis with previous fractures. Head to head studies wou

Endometrial hyperplasia: a review.

UNLABELLED: Endometrial hyperplasia is a precursor to the most common gynecologic cancer diagnosed in women: endometrial cancer of endometrioid histology. It is most often diagnosed in postmenopausal women, but women at any age with unopposed estrogen from any source are at an increased risk for developing endometrial hyperplasia. Hyperplasia with cytologic atypia represents the greatest risk for progression to endometrial carcinoma and the presence of concomitant carcinoma in women with endometrial hyperplasia. Abnormal uterine bleeding is the most common presenting symptom of endometrial hyp

New management options for osteoporosis with emphasis on SERMs.

Albright was the first of many to show that loss of bone mass due to estrogen deficiency is an important part of the pathogenesis of postmenopausal osteoporosis. This led to the use of estrogen therapy which was shown to prevent bone loss at menopause and to reduce the risk of important fragility fractures. Selective estrogen receptor modulators (SERMs), with salutary estrogen-like skeletal effects and with protection from breast cancer, have important roles in the management of young postmenopausal women. New members of the SERM family may approach the effectiveness of estrogen in preventing

Selective steroid receptor modulators in reproductive medicine.

A key step forward in the treatment of hormone-dependent diseases has been made since the discovery of tissue-specific steroid receptor modulators (SRMs), or receptor ligands that elicit agonistic or antagonistic effects in a cell- and tissue-dependent manner. In reproductive medicine, selective estrogen receptors modulators (SERMs) and selective progesterone receptors modulator (SPRMs) were discovered and developed since the 1970s. SERMs were first released for the treatment of infertility and later developed in areas such as osteoporosis prevention and breast cancer treatment. The later disc

The intersection of the HER2-low subtype with endocrine resistance: the role of interconnected signaling pathways.

Since its introduction in the 1970s, endocrine therapy that targets the estrogen receptor alpha (ERα) signaling pathway has had tremendous success in the clinic in estrogen receptor positive (ER+) breast cancer. However, resistance to endocrine therapy eventually develops in virtually all patients with metastatic disease. Endocrine resistance is a primary unaddressed medical need for ER+ metastatic breast cancer patients. It has been shown that tumors become resistant through various mechanisms, converging on the acquisition of genetic alterations of ER, components of the MAP kinase pathway, o

Molecular Pharmacological Interventions for Endometrial Cancer: A Comprehensive Review.

Endometrial cancer, a common malignancy of the female reproductive system, has a rising incidence and complex clinical management due to its diverse molecular subtypes. This review examines the molecular mechanisms underlying EC, particularly the roles of the Bcl-2 family in apoptosis regulation and estrogen receptor signaling in tumor progression. We explore pharmacological interventions targeting these pathways, including BH3 mimetics and selective estrogen receptor modulators, which show promise but face challenges such as resistance and adverse effects. Additionally, we highlight the poten

Therapies to improve bone mineral density and reduce the risk of fracture: clinical trial results.

Osteoporosis is a skeletal disorder that is associated with lowered bone mineral density and increased risk of fracture. Numerous clinical trials have been conducted to study drugs and lifestyle changes that can either prevent bone loss in perimenopausal women or prevent fractures in women with established osteoporosis. The role of estrogen alone or in combination with androgen as hormone replacement therapy is discussed for its potential value in osteoporosis prevention and treatment. Additional therapies that are examined for prevention and treatment of osteoporosis include tibolone, bisphos

[Antiresorptive therapies in osteoporosis].

Osteoporosis is a systemic skeletal disorder that is characterised by low bone mineral density (BMD) and deterioration of bone tissue. Eventually, this leads to disruption of bone microarchitecture, a compromised bone strength and an increase risk of fracture. Antiresorptive therapies are used to increase bone strength and reduce the risk of fracture in individuals with osteoporosis. The most common antiresorptives are bisphosphonates, estrogens, selective estrogen receptor modulators (SERM) and the human monoclonal antibody against RANKL (Denosumab).

ERα Micromap (µMap) proximity labeling reveals fulvestrant mechanisms.

Estrogen Receptor alpha (ERα) is the primary driver in ER+ breast cancer and can be targeted using small molecules such as fulvestrant, a selective estrogen receptor degrader (SERD). Despite being approved over two decades ago and used regularly in post-menopausal women, its precise anti-tumor mechanism remains unclear, with several reports showing efficacy without the requirement for ERα degradation. To investigate the molecular basis of fulvestrant's action, we employed split intein-based µMap photo-proximity labeling to profile ERα-associated interactions. Our findings reveal that fulvestra

Inherited coagulopathies in OB/GYN.

There is an increased risk of venous thrombosis in women with inherited plasma protein coagulopathies (thrombophilias). Although a general screening for thrombophilia is impractical, risk assessment based on identification of other hypercoagulable states-such as a personal or family history of venous thrombosis-can identify patients who may benefit from testing. Those patients testing positive for thrombophilia need to be counseled regarding the implications of oral contraceptive use, hormone replacement therapy, use of selective estrogen receptor modulators, and pregnancy.

Estrogens, episodic memory, and Alzheimer's disease: a critical update.

Estrogen-containing hormone therapy initiated during late postmenopause does not improve episodic memory (an important early symptom of Alzheimer's disease), and it increases dementia risk. Cognitive consequences of exogenous estrogen exposures during midlife are less certain. Observational evidence implies that use of hormone therapy at a younger age close to the time of menopause may reduce risk of Alzheimer's disease later in life. However, there are concerns that observational findings may be systematically biased. Partial insight on this critical issue may be gleaned from results of ongoi

Unraveling the hormonal approaches for the treatment of rheumatoid arthritis and its complementary interventions.

Rheumatoid Arthritis (RA) is an autoimmune, chronic, systemic inflammatory disease that causes redness, swelling, stiffness, and joint pain. It is a long-lasting disease that can have a widespread impact on the body, often affecting the hands, feet, and wrists. The immune cells, such as dendritic cells, T cells, B cells, macrophages, and neutrophils, play a significant role in bone degradation and inflammation. Several cytokines, including TNF-α and IL-17A, play a significant role in causing bone erosion, cartilage deterioration, and joint inflammation. Progesterone and estrogen have a crucial
