Muscles may contribute to shoulder dislocation and stability.

Joint instability is one of the most common human afflictions, affecting not only the hip, knee, and shoulder but all joints of the body. Surgical repair for joint instability is done commonly, yet has not been able to restore function consistently. Pain, recurrence of instability, joint stiffness, muscle injury, and degenerative joint disease are all too common sequelae. Because the glenohumeral joint is most commonly dislocated, efforts have been directed to understanding shoulder instability. Study has included not only the static restraints (capsulolabrum, articular surfaces, intracapsular

Modified Bankart procedure for recurrent anterior shoulder dislocation.

BACKGROUND AND PURPOSE: Recurrent anterior shoulder dislocation is a serious condition, for which the Bankart procedure is a standard treatment. Having made three modifications to the original procedure, we examined the efficacy of this modified Bankart procedure in the treatment of patients with recurrent anterior shoulder dislocation. PATIENTS AND METHODS: The medical records of 21 patients who received a modified Bankart procedure for recurrent anterior shoulder dislocation during the period from 1989 through 1998 were retrospectively analyzed. The average age at initial dislocation was 22

Primary anterior shoulder dislocation and rotator cuff tears.

In a prospective controlled study 167 patients with 167 primary traumatic anterior shoulder dislocations underwent early ultrasonograpic evaluation for rotator cuff tears. We found 53 (31.7%) full-thickness cuff tears in this group. Compared with a group of 93 healthy volunteers, we found with statistical significance more cuff tears in the patients aged < 60 years. Women ruptured the cuff more often than men. If the patient is not able to elevate the affected arm more than 90 degrees in the scapular plane 2 weeks after the dislocation, there should be a high suspicion of rotator cuff tear (76

[Etiopathogenetic factors of congenital habitual shoulder joint dislocation].

Six of 56 patients (10.7 per cent) surgically treated for anterior habitual and recurrent dislocation of the shoulder joint exhibited congenital habitual dislocation. Surgery revealed agenesis of the anterior and upper parts of the capsular wall. The lower part of the capsular wall was used for reconstruction.

[Sonographic assessment of typical lesions in shoulder dislocation].

The traumatic dislocation of the shoulder joint is the most often joint luxation. The involved patients are often sporting and between the age of 20 to 40 years. The therapeutic and prognostic point of interest is to recognize and reconstruct injured structures. Among the soft tissue injuries the Bankart lesions, among the osseous injuries the Hill-Sachs defect are numeric well to the fore. Hitherto existing diagnostic methods for these injuries are either invasive or with X-rays. Its is demonstrated, that dynamic ultrasonography examination of the shoulder joint can visualize besides the rota

Voluntary anterior dislocation of the shoulder: case study.

Anterior shoulder dislocation can be willfully produced by a subject through abnormal unbalanced muscle contraction at the shoulder joint. It may be associated with pathologic lesions at the shoulder. Treatment consists of vigorous shoulder strengthening exercises and/or surgery to increase anterior stability at the joint. For any treatment approach to be successful the subject must avoid dislocation of the shoulder. A psychologic component frequently associated with this condition is its use as a mechanism for attention, sympathy or defense--a factor that has a significant negative effect on

Extensive shoulder capsule tearing as a main cause of recurrent anterior shoulder dislocation.

Anterior capsular and ligamentous injuries occur frequently at initial traumatic shoulder dislocation. However, these injuries have not been considered to be one of the main causes of recurrent anterior dislocation, which is believed to heal spontaneously. We evaluated five joints of five patients in which tears in the middle portion of the anterior capsule including the inferior glenohumeral ligament were considered to be the main cause of recurrent dislocation. History of present illness and physical and plain x-ray results were not specific. The only specific finding was a diverticulum-like

Traumatic Anterior Shoulder Dislocation: Epidemiology, Diagnosis, and Treatment.

BACKGROUND: Traumatic anterior shoulder dislocation is the most common type of joint dislocation, with an incidence of 11 to 29 per 100 000 persons per year. Controversy still surrounds the recommendations for treatment and the available procedures for surgical stabilization. METHODS: This review is based on pertinent publications (2014-2024) that were retrieved by a selective search in the PubMed and Google Scholar databases. Meta-analyses and ran - domized controlled trials (RCTs) with evidence levels I and II were included. RESULTS: The typical injury mechanism is forcible external rotati

Delayed Appearance of Neuropathy After Inferior Shoulder Dislocation.

Complications regarding inferior shoulder dislocations (ISD) are predominately rotator cuff injuries, neuropathies, and vascular insults. To our knowledge, there are no studies regarding the delayed appearance of neuropathies with inferior shoulder dislocations. A 32-year-old previously healthy male presented with an inferior shoulder dislocation that required open reduction and internal fixation after failed attempts at closed reductions in the emergency room and operating room. At the time of discharge, the patient had no neurologic complaints, which continued to the two-week follow-up appoi

Bilateral Fracture-Dislocation of the Shoulder After Defibrillation.

BACKGROUND: There are multiple reported injuries associated with cardiopulmonary resuscitation, most of them caused by the force of compressions, like sternal and rib fractures, abdominal organ injuries like splenic rupture, liver lacerations, and injuries to the upper airway and skin. Injuries related to defibrillation and cardioversion are rare, mostly related to skin and muscle injuries on where the defibrillation paddles were placed. CASE REPORT: A 52-year-old man presented to the Emergency Department with crushing chest pain. The patient was suffering from a myocardial infarction, and du

First-time traumatic anterior shoulder dislocation: current concepts.

The management of first-time traumatic anterior shoulder dislocations has been a topic of extensive study yet remains controversial. Development of a treatment plan requires an understanding of patient-specific considerations, including demographics, functional demands, and extent of pathology. Each of these can influence rates of recurrence and return to activity. The purpose of this review is to provide a framework for decision-making following a first-time anterior shoulder dislocation, with particular focus on the high-risk young and athletic population. A summary of surgical treatment opt

Axillary artery laceration after anterior shoulder dislocation reduction.

INTRODUCTION: Glenohumeral dislocation is the most commonly encountered dislocation in the emergency department. The most frequent complications of glenohumeral dislocation are rotator cuff tears and an increase in the risk of recurrent dislocation. Less common acute complications include fractures, neurological complications and vascular injuries. The incidence of axillary artery injury associated with shoulder dislocation is reported to be about 1-2%. CASE: An 81-year-old male presented to the emergency department with pain in the right shoulder after a fall. On physical examination, the sh

Bipolar bone defect in the shoulder anterior dislocation.

In the anterior shoulder instability with glenoid bone loss among 25 % or more of the inferior glenoid diameter (inverted-pear glenoid), the consensus of recent authors is that glenoid bone grafting (Latarjet procedure) should be performed. The engaging Hill-Sachs lesion has been recognized as a risk factor for recurrent anterior shoulder instability. We have developed a method using radiographic and arthroscopic studies and the concept of the glenoid track to determine whether a Hill-Sachs lesion will engage the anterior glenoid rim, whether or not there is concomitant anterior glenoid bone l

Anticoagulant-induced hemarthrosis presenting as anterior shoulder dislocation.

This is a case of nontraumatic shoulder pain initially diagnosed on x-ray as an anterior dislocation. The patient was on anticoagulants and, in actuality, had severe hemarthrosis that caused the subluxation. Attempts to reduce the dislocation in this situation might have resulted in worsening of the intra-articular bleed. There has been only 1 similar reported case in the European Journal of Emergency Medicine in 2013 of a 53-year-old woman who was thought to have a nontraumatic anterior shoulder dislocation, and attempts were unsuccessful at reduction. Definitive therapy involved hemarthrosis

A Case of Simultaneous Bilateral Anterior Shoulder Dislocation.

INTRODUCTION: Anterior dislocation of shoulder is commonest dislocation one encounters in day to day Orthopaedic practice. But bilateral shoulder dislocations are relatively uncommon frequently posterior and secondary to violent muscle contraction. Simultaneous bilateral anterior dislocations of shoulder following trauma is rare occurrence. CASE REPORT: 35 year old male presented to emergency department with history fall by tripping on a stone (fall on outstretched hand). He complained of pain and difficulty in moving both the shoulders. On clinical examination, patient's both upper limbs wer
