Functional outcomes of operative fixation of clavicle fractures in patients with floating shoulder girdle injuries.

BACKGROUND: Double disruptions of the superior suspensory shoulder complex, commonly referred to as 'floating shoulder' injuries, are ipsilateral midshaft clavicular and scapular neck/body fractures with a loss of bony attachment of the glenoid. The treatment of 'floating shoulder' injuries has been debated controversially for many years. The purpose of this study was to demonstrate the clinical and functional outcomes of patients with 'floating shoulder' injuries who underwent operative fixation of the clavicle fracture only. MATERIALS AND METHODS: Between 2002 and 2010, 32 consecutive float

Overuse upper extremity injuries in the skeletally immature patient: beyond Little League shoulder and elbow.

Overuse injuries of the shoulder and elbow are common in the skeletally immature throwing athlete. This review goes beyond describing the classic imaging findings of Little League shoulder and elbow and probes deeper into new insights suggesting a unifying etiology of metaphyseal or metaphyseal equivalent injury as the cause of many of the bony lesions described. Injury patterns change depending on the athlete's skeletal maturation, and the spectrum of abnormalities is reviewed.

MR imaging of the thrower's shoulder. Internal impingement, latissimus dorsi/subscapularis strains, and related injuries.

In conclusion, internal impingement apparently occurs in nearly all patients and is demonstrable on MR imaging. Pathologic changes associated with internal impingement seem to develop with repetitive placement of the arm into a position of extreme external rotation and abduction. Findings may include lesions of the posterior superior labrum, undersurface irritation, or tearing of the supraspinatus-infraspinatus junction near the attachment site and cystic changes of the posterior superior glenoid and posterior lateral greater tuberosity. There is no evidence for a particular sequence of pathol

[Ultrasonography of the shoulder in subacromial syndromes with disorders and injuries of the rotator cuff].

Shoulder sonography was performed prospectively in 4172 patients with 4588 shoulders affected with pathology of the subacromial space, among them 1227 operated cases with 425 rotator cuff tears (292 total tears and 133 partial tears). A two-criterion model with formal as well as echogenic and dynamic criteria was used for the diagnosis of cuff tears. The overall sensitivity in diagnosing rotator cuff tears was 95.3% (97.3% in total tears and 91.0% in partial tears). False-negative results were found overall in 1.6% of cases. Overall accuracy for all defects was 94.9%. Involvement of the supras

Prevalence of shoulder subluxation diagnosed via ultrasound screening in infants with birth brachial plexus injuries.

INTRODUCTION: Posterior shoulder subluxation and glenohumeral dysplasia are known secondary complications of birth brachial plexus palsies (BBPP) that contribute to functional limitations. The prevalence of shoulder subluxation in infants with BBPP and the timeline of when subluxation occurs in the first year of life remain unclear, with prior literature ranging from 7% to 56% prevalence, in part owing to lack of standardized protocols for screening. OBJECTIVE: To determine the prevalence of posterior shoulder subluxation in infants with BBPP and persistent neurological deficits with universa

Nerve Grafting for Axillary Nerve Injuries Following Shoulder Trauma: A Systematic Review of Surgical Outcomes.

The axillary nerve is particularly susceptible to injury following shoulder trauma such as dislocation and fracture-dislocation, leading to loss of abduction, external rotation, and sensory deficits. Severe cases with loss of continuity require surgical intervention. Nerve grafting is the mainstay when direct repair is not feasible, though outcomes are influenced by timing, graft length, and patient selection. This review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. A systematic search of PubMed, Embase, Scopus, a

Joint hypermobility in athletes is associated with shoulder injuries: a systematic review and meta-analysis.

BACKGROUND: Joint hypermobility in athletes is associated with increased risk of knee injuries, but its role in relation to shoulder injuries has not been scrutinized. Therefore, our aim was to synthesize the evidence on the association between joint hypermobility and shoulder injuries in athletes. METHODS: Data sources were MEDLINE, CINAHL, EMBASE, and SPORTDiscus from inception to 27th February 2021. Eligibility criteria were observational studies of athletes (including military personnel), mean age ≥ 16 years, and with a transparent grouping of those with and without joint hypermobility. A

Acute shoulder injuries: A clinical review of diagnosis and management in the deployed Naval environment.

Musculoskeletal injuries of the shoulder are a common presentation in primary care. Injuries to this highly mobile region can have a profound impact on the range of movement, resulting in severe functional limitation. The shoulder can also be one of the more complicated regions to examine due to its high mobility, poor localisation of pain and numerous supporting structures. This article aims to review the anatomy of the shoulder, examination technique and the pathology underlying common acute injuries in order to provide guidance to medical personnel deployed with the Royal Navy and Royal Mar

MR imaging in the evaluation of SLAP injuries of the shoulder: findings in 10 patients.

OBJECTIVE: Injuries to the superior portion of the glenoid labrum, called SLAP (superior labrum, anterior and posterior) injuries, are recently recognized injuries consisting of tears of the long head of the biceps tendon anchor/superior labral complex. The purpose of this study was to determine if the MR imaging findings in patients with surgically proved SLAP injuries correspond to the abnormalities found at arthroscopy. MATERIALS AND METHODS: Four variants of SLAP lesions have been described; they are based on the degree of compromise of the superior portion of the glenoid labrum, biceps t

Reduction of acute anterior shoulder dislocations using the Milch technique: a study of ski injuries.

During the 1977-78, 1978-79, and 1979-80 ski seasons, 76 acute anterior shoulder dislocations were treated by one of three Orthopedic Surgeons at the Rutland Vermont Hospital Emergency Room: 68 (89.4%) were reduced on first attempt using the Milch technique of abduction and external rotation. Four (5.2%) required general anesthesia and four were reduced using meperidine hydrochloride (Demerol, Winthrop) and diazepam (Valium, Roche) and a traction-countertraction technique. Of the 68 shoulders reduced with the Milch technique, 47 (69.1%) required no analgesics or muscle relaxants. There were no

Shoulder girdle injuries involving the medial clavicle differ from lateral clavicle injuries with a focus on concomitant injuries and management strategies: A retrospective study based on nationwide routine data.

INTRODUCTION: Although shoulder girdle injuries are frequent, those of the medial part are widely unexplored. Our aim is to improve the knowledge of this rare injury and its management in Germany by big data analysis. METHODS: The data are based on ICD-10 codes of all German hospitals as provided by the German Federal Statistical Office. Based on the ICD-10 codes S42.01 (medial clavicle fracture, MCF) and S43.2 (sternoclavicular joint dislocation, SCJD), anonymized patient data from 2012 to 2014 were evaluated retrospectively for epidemiologic issues. We analyzed especially the concomitant in

Shoulder stretching versus shoulder muscle strength training for the prevention of baseball-related arm injuries: a randomized, active-controlled, open-label, non-inferiority study.

Glenohumeral internal rotation deficit (GIRD) and weakness in prone external rotation are risk factors for shoulder and elbow injuries in high school baseball pitchers. While a shoulder-stretching prevention program to improve GIRD decreases the injury rate, the effects of external rotation strength remain unclear. This non-inferiority (NI) study investigates the hypothesis that external rotation strength training is not inferior to sleeper stretching for shoulder and elbow injury prevention in high school baseball pitchers. Participants were randomly allocated to the stretching (n = 62; activ

Irreducible Posterior Shoulder Dislocation With Concomitant Fracture of Both the Greater and Lesser Tuberosity: An Extremely Rare Shoulder Injury.

Posterior shoulder dislocation is a relatively rare injury representing only 5% of all shoulder dislocations. It is usually the result of a high-energy trauma or an epileptic seizure. Diagnosis is challenging with half of these injuries missed in the emergency room (ER). Often the dislocation is accompanied by a lesser tuberosity fracture as a result of the impact between the posterior glenoid and the proximal humerus. Additionally, fractures of the greater tuberosity or even the metaphysis are extremely rare, and their treatment remains challenging. We present a rare case of posterior locked

Modified Saha Procedure for Chronic Paralytic Shoulder after Reverse Shoulder Arthroplasty Complicated with Brachial Plexus Injury.

Nerve injuries after reverse total shoulder arthroplasty (rTSA) are rare, and fortunately, most patients recover without long-term sequelae. However, irreversible deltoid paralysis due to brachial plexus or axillary nerve injury is devastating, as the deltoid is the main force of movement of the humerus after rTSA. Denervation of the deltoid results in weakness in abduction, forward flexion, and elevation, as well as varying degrees of chronic pain and shoulder instability. Treatment options for chronic deltoid denervation after axillary nerve or brachial plexus injuries are limited, since ner

Iatrogenic nerve injury in upper limb surgery excepting shoulder.

Upper limb nerve injuries, although uncommon, represent a major source of morbidity and medicolegal issues. They may occur in various contexts: during patient positioning, regional anesthesia, or the surgical procedure itself. This article provides a synthesis of the mechanisms of injury, the nerves at risk according to anatomical regions (from the arm to the hand), and simple preventive measures to be adopted in each situation. We emphasize the importance of knowledge of anatomical landmarks, safety zones, and the clinical settings most likely to cause injury. A reasoned diagnostic and therap

Shoulder extension impairment with residual neonatal brachial plexus injury.

BACKGROUND: Impairment of both shoulder extension and behind-the-back function are common in patients with residual neonatal brachial plexus injury (NBPI), but have scarcely been studied or reported in the literature. Behind-the-back function is classically evaluated using the hand-to-spine task used for the Mallet score. Angular measurements of shoulder extension with residual NBPI have generally been studied utilizing kinematic motion laboratories. To date, no validated clinical examination method for this has been described. METHODS: Intraobserver and interobserver reliability analyses of

Prevalence and risk factors for nerve injury following shoulder dislocation.

BACKGROUND: The glenohumeral joint dislocation can be associated with major nerve injury. The reported prevalence and risk factors for major nerve injury are variable and this injury can have a severe and life-long impact on the patient. The objectives of this study were to analyse the prevalence of major nerve injury following shoulder dislocation and examine risk factors. Management and outcomes of nerve injury were explored. METHODS: A 1 year retrospective cohort study of 243 consecutive adults who presented with a shoulder dislocation was performed. Data were collected on patient demograp
