The clinical and genetic spectrum of paediatric speech and language disorders.

Speech and language disorders are known to have a substantial genetic contribution. Although frequently examined as components of other conditions, research on the genetic basis of linguistic differences as separate phenotypic subgroups has been limited so far. Here, we performed an in-depth characterization of speech and language disorders in 52 143 individuals, reconstructing clinical histories using a large-scale data-mining approach of the electronic medical records from an entire large paediatric healthcare network. The reported frequency of these disorders was the highest between 2 and 5

Pre-pragmatic language use in toddlerhood: Developmental antecedents, aetiological factors, and associations to autism.

BACKGROUND: Pragmatic language is key for adaptive communication, but often compromised in neurodevelopmental conditions such as autism spectrum disorder (ASD). Decontextualized language-to talk about events and things beyond here and now-develops early in childhood and can be seen as a pre-pragmatic ability. Little is known about the factors that contribute to decontextualized language use in toddlers and whether these are different from factors contributing to general language development. METHODS: We studied longitudinal associations between parent-rated core language and non-verbal socio-

Investigation of Bias Using ChatGPT-4 in Pain Management and Patient Profiling.

BACKGROUND: Large language models, such as ChatGPT (cGPT), are being integrated increasingly into clinical workflows and medical education. However, concerns persist regarding their susceptibility to bias, especially in high-stakes areas like pain management, when disparities across race, socioeconomic status, and substance use history are well documented. OBJECTIVE: This study investigated whether cGPT-4 generates consistent and equitable pain management recommendations when patient demographic variables are altered. METHODS: Using cGPT-4, researchers created six clinical scenarios represen

Uncovering the Social-Cognitive Contributors to Social Dysfunction in Borderline Personality Disorder Through Language Analysis.

Borderline personality disorder (BPD) is characterized by severe interpersonal dysfunction, yet the underlying nature of such dysfunction remains poorly understood. The present study adopted a behavioral approach to more objectively describe the social-cognitive contributors to interpersonal dysfunction in BPD. Participants (N = 530) completed an online survey comprising validated measures of BPD features and other problematic interpersonal traits (e.g., narcissism), as well as a writing prompt where they were asked to share their personal thoughts about relationships. Computerized language an

Advancing Our Understanding of the Link between Statistical Learning and Language Acquisition: The Need for Longitudinal Data.

Mastery of language can be a struggle for some children. Amongst those that succeed in achieving this feat there is variability in proficiency. Cognitive scientists remain intrigued by this variation. A now substantial body of research suggests that language acquisition is underpinned by a child's capacity for statistical learning (SL). Moreover, a growing body of research has demonstrated that variability in SL is associated with variability in language proficiency. Yet, there is a striking lack of longitudinal data. To date, there has been no comprehensive investigation of whether a capacity

Sociocognitive performance in autism spectrum disorders and interference of the therapeutic environment.

PURPOSE: To analyze the sociocognitive performance of children and adolescents with autism spectrum disorders in two environments of language therapy, which differ as to the physical structure. METHODS: Ten children and adolescents with ages between 4 and 13 years, of both genders, diagnosed with autism spectrum disorders took part in the study. For data collection, eight filming sessions were performed during individual language therapy lasting 30 minutes, being four in a room with conventional environmental organization (common room) and four in a room with specific ambiance [children's int

Resting state auditory-language cortex connectivity is associated with hallucinations in clinical and biological subtypes of psychotic disorders.

BACKGROUND: Auditory hallucinations are prevalent across the major psychotic disorders, but their underlying mechanism is poorly understood. Limited prior work supports a hypothesis of altered auditory/language brain systems. To more definitively assess this, we examined whether alterations in resting state connectivity of auditory and language cortices are associated with hallucination severity in a large sample of individuals in the schizo-bipolar spectrum. METHODS: Whole brain resting state connectivity of auditory and language cortex (primary auditory cortex, unimodal auditory association

Progressive aphasia, apraxia of speech and agraphia in corticobasal degeneration: A 12-case series clinical and neuropsychological descriptive study.

BACKGROUND: Despite initial underreporting of language dysfunctions in corticobasal syndrome (CBS), aphasia is now recognized as a frequent feature of this disease. Aphasia in CBS seems clinically overlying to a non-fluent/agrammatic primary progressive aphasia (nfaPPA), which is also a clinical phenotype associated with corticobasal degeneration (CBD) pathology. However, the clinical features of aphasia in CBS still remain poorly delineated, resulting in misjudgements in the differential diagnosis from a PPA presentation of the disease. AIMS: To investigate the language disorders of this syn

The clinical practice of speech and language therapists with preschool-age children with syntactic impairment.

BACKGROUND: Children with syntactic impairment associated with autism spectrum disorder and developmental language disorder benefit from speech and language therapy intervention. Despite that, few studies focus on the clinical practice of the speech and language therapist (SLT). AIM: The aim of the study is to explore the clinical practice of SLTs with preschool-age children with syntactic impairment. METHODS & PROCEDURES: A cross-sectional survey was carried out in Portugal, using a questionnaire designed specifically for this study. The survey is composed of 109 questions divided into eigh

Diagnostic accuracy of parent reports for speech-language disorders in young children: a meta-analysis.

BACKGROUND: Speech and language disorders are among the most common developmental concerns in early childhood, yet there is limited consensus in pediatrics regarding how accurately parent-report instruments identify affected children. Prior reviews have combined heterogeneous screening formats, making it difficult to evaluate the performance of parent-completed measures widely used in developmental surveillance and primary care. METHODS: We conducted a systematic review and meta-analysis of studies evaluating the diagnostic accuracy of parent-report instruments for identifying speech and/or l

The atypical functional connectivity of Broca's area at multiple frequency bands in autism spectrum disorder.

As a developmental disorder, autism spectrum disorder (ASD) has drawn much attention due to its severe impacts on one's language capacity. Broca's area, an important brain region of the language network, is largely involved in language-related functions. Using the Autism Brain Image Data Exchange (ABIDE) dataset, a mega-analysis was performed involving a total of 1454 participants (including 618 individuals with ASD and 836 healthy controls (HCs). To detect the neural pathophysiological mechanism of ASD from the perspective of language, we conducted a functional connectivity (FC) analysis with

Increased sense of identity in delusional disorders.

Language, which is unique in each subject, can reflect how a patient copes with disease. The method ALCESTE used here made it possible at the same time to analyse the subject's verbal behavior and speech patterns at several levels. The present study was designed to analyse during a 3-mo. period the language production of subjects with paranoia exhibiting delusional disorder (nonbizarre delusions without any hallucination) of imaginative subtype. The subjects produced very specific speech without any semantic or syntactic impairment and disruption in language or thinking processes, but with a p

[Developmental dysphasia].

Developmental dysphasia is characterized by severe, specific and unexplained developmental language impairment. Phonologic and syntactic disorders of expressive language and perceptual deficit are almost always present. Moreover, certain specific linguistic symptoms are particular to each dysphasic child, explaining the efforts to classify different forms of dysphasia. The most important point is to separate receptive dysphasia from the expressive on the one hand, and severe forms from the mild which are closed to "simple language delay", on the other. The evolution of oral language is variabl

Adaptation and normative data for the Comprehensive Aphasia Test in Catalan (CAT-CAT).

Assessment tools for diagnosing aphasia in languages other than English are scarce, particularly for minority languages such as Catalan. The present study introduces the Catalan adaptation of the Comprehensive Aphasia Test (CAT-CAT), the first assessment tool of its kind in Catalan, which was developed with careful consideration of cultural and psycholinguistic factors. Additionally, the study provides normative data based on a sample of 110 Catalan-dominant speakers without language or speech disorders in order to establish the range of non-pathological performance and cut-off scores. We also

Differentiating phonological delay from phonological disorder: Executive function performance in preschoolers.

BACKGROUND: The conversational speech of most children can be understood by people outside the family by the time they reach 4 years. However, for some children, speech sound disorders (SSDs) persist into their early school years, and beyond, despite adequate hearing, oromotor function, and language learning opportunities. One explanation for children's SSDs are domain-general cognitive-linguistic deficits that impair the child's ability to correctly derive rules governing how speech sounds legally combine to form words in a specific language. AIMS: To explore whether there are differences in

Propositional speech in unselected stroke: The effect of genre and external support.

Distinguished from nominal language, propositional language generation refers to the spontaneous and voluntary aspect of language that introduces novel concepts to a specific context. Propositional language can be impaired in a range of neurological disorders, including stroke, despite well-preserved nominal language. Although external support can increase speech rate in patients with reduced propositional speech, no specific investigation of propositional speech has been carried out in unselected stroke patients. The current study investigated propositional language in an unselected post-acut

Optimal intervention intensity for emergent literacy: what we know and need to learn.

In response to Baker (2012) unpacking critical components of optimal intervention intensity, this article explores what is currently known regarding intervention intensity for emergent literacy for children at risk for delays in this area of development. Studies specifically addressing intervention intensity related to phonological awareness and print knowledge suggest that more intensity does not always yield better results, and other factors suggested by Baker may influence the impact of intensity on children's outcomes. However, none of the research to date includes speech-language patholog

Genetic influences on language impairment and phonological short-term memory.

It has been known for some years that specific language impairment (SLI), an unexpected failure to acquire age-appropriate language skills, is highly heritable. However, molecular genetic studies have been hampered by the heterogeneity of the disorder and the predominant lack of clear genotype-phenotype relationships. We review recent studies suggesting that a better understanding of the genetics of SLI might emerge if we move away from clinical criteria for diagnosis to look instead at a theoretically based quantitative and cognitive measure of the phenotype: a test of phonological short-term
