Neurophysiology of epidurally evoked spinal cord reflexes in clinically motor-complete posttraumatic spinal cord injury.

Increased use of epidural Spinal Cord Stimulation (eSCS) for the rehabilitation of spinal cord injury (SCI) has highlighted the need for a greater understanding of the properties of reflex circuits in the isolated spinal cord, particularly in response to repetitive stimulation. Here, we investigate the frequency-dependence of modulation of short- and long-latency EMG responses of lower limb muscles in patients with SCI at rest. Single stimuli could evoke short-latency responses as well as long-latency (likely polysynaptic) responses. The short-latency component was enhanced at low frequencies

Intraoperative spinal cord monitoring during surgery for aortic aneurysm: application of spinal cord evoked potential.

Spinal cord evoked potentials elicited by direct stimulation of the spinal cord were monitored in 21 patients during thoracic or thoraco-abdominal aortic aneurysm surgery. Flexible catheter-type electrodes were used for both stimulating and recording. The basic pattern of the spinal cord evoked potential consisted of an initial spike and a subsequent polyphasic component. The earliest and most frequent alterations after cross-clamping of the aorta were changes in the configuration or amplitude of the polyphasic component. In 13 patients who exhibited no change except minor alterations of the p

Intramedullary spinal cord metastasis associated with pencil-shaped softening of the spinal cord: case report.

A 57-year-old man presented with a pulmonary mass and subacute onset of paraplegia. Laboratory examination revealed an increased protein content in the cerebrospinal fluid and a normal myelogram. Autopsy disclosed epidermoid carcinoma in the left lower lobe of the lung, intramedullary metastasis at the midthoracic level, and a central pencil-shaped softening above and below the metastatic lesion. The pencil-shaped softening was an ischemic infarct rather than of hemorrhagic or congestive origin. The pathogenesis of this rare association may be explained by the hypothesis of a tumor embolus in

Comparison of spinal cord evoked potentials and peripheral nerve evoked potentials by electric stimulation of the spinal cord under acute spinal cord compression in cats.

STUDY DESIGN: Spinal cord evoked potentials and peripheral nerve evoked potentials after spinal cord stimulation were recorded under acute spinal cord compression in 19 cats. OBJECTIVES: To investigate the effects of acute compression upon grey matter and white matter by comparing both potentials. METHODS: We compared peripheral nerve evoked potentials, recorded at the biceps brachii branch of the musculocutaneous nerve, with descending spinal cord evoked potentials, recorded from the lumbar spinal cord, by stimulation to the C2 level, under compression of the C6 segment. RESULTS: The ampli

Spinal Angiolipoma: A Rare Cause of Spinal Cord Compression.

Spinal angiolipomas (SALs) are uncommon benign tumors composed of mature adipocytes and abnormal blood vessels, with symptoms usually appearing gradually due to the progressive compression of the spinal cord. In this context, we report the case of a 34-year-old female patient with no significant medical history who presented with heaviness in both lower limbs and difficulty walking for the past six months. MRI showed an epidural posterior lesion extending from the first to the sixth thoracic vertebrae, strongly compressing the spinal cord, and surgical intervention for spinal cord decompressio

Spinal fixation surgery for acute traumatic spinal cord injury.

BACKGROUND: If the spine is unstable following traumatic spinal cord injury (SCI), surgical fusion and bracing may be necessary to obtain vertical stability and prevent re-injury of the spinal cord from repeated movement of the unstable bony elements. It has been suggested that this spinal fixation surgery may promote early rehabilitation and mobilisation. OBJECTIVES: To answer the question: is there a difference in functional outcome and other commonly measured outcomes between people who have a spinal cord injury and have had spinal fixation surgery and those who have not? SEARCH STRATEGY:

Spinal cord compression from spinal tuberculosis in a child.

Tuberculosis in children most frequently involves the pulmonary system. Spinal tuberculosis is very rare and insidious in children. We present a 1-year-old girl who was brought to our clinic with complaints of a bulge on her back and weakness in the feet. She was investigated with magnetic resonance imaging and at the thoracic level which revealed vertebral destruction and para- and perispinal abscess. Computed tomography of the lungs showed tuberculous lesions. The child's mother gave a post history of recovered lung tuberculosis. Due to appearance paraparesis secondary to vertebral destructi

Ganglioglioma in the spinal cord of a steer.

An 18-mo-old Aberdeen Angus steer was unable to rise without assistance; and, when standing, the steer walked with flexed forelimbs. Due to the poor prognosis, it was euthanized and autopsied. Grossly, the cervical spinal cord was markedly enlarged at C5-C6 by a yellow, soft, well-demarcated nodule that affected ~70% of the parenchyma. Microscopically, the expansive, multinodular, unencapsulated neoplasm affected mostly the white matter and consisted of 2 distinct cell populations. Small, pleomorphic cells with scant cytoplasm predominated; nuclei were positive for oligodendrocyte transcriptio

Spinal Cord Medulloepithelioma in a Cat.

A 13-month-old, neutered, male, domestic shorthair cat was referred with a history of progressive paraparesis, proprioceptive ataxia, and lumbar spinal pain. Neurological examination revealed non-ambulatory paraparesis consistent with L4-S1 myelopathy. Magnetic resonance of the thoracolumbar spinal cord identified a dorsal intradural extramedullary space-occupying lesion extending from L5 to L6. It was homogeneously hyperintense in T2-weighted imaging and isointense in T1-weighted imaging and exhibited marked and homogeneous contrast enhancement in the T1-weighted post-contrast imaging. Syringomyelia in the Tethered Spinal Cords.

Cases of syringomyelia associated with spinal dysraphism are distinct from those associated with hindbrain herniation or arachnoiditis in terms of the suspected pathogenetic mechanism. The symptoms of terminal syringomyelia are difficult to differentiate from the symptoms caused by spinal dysraphism. Nonetheless, syringomyelia has important clinical implications, as it is an important sign of cord tethering. The postoperative assessment of syringomyelia should be performed with caution.

Endoscopic approaches to the spinal cord.

Minimally invasive procedures have been used to treat various diseases in medicine. Great improvements in these techniques have provided intraventricular, transnasal and more recently cisternal intracranial accesses used to treat different conditions. Endoscopic approaches have been proposed for the treatment of disk herniation or degenerative disease of the spine with great progress in the recent years. However the spinal cord has not yet been reached by video-assisted procedures. This article describes our recent experience in procedures to approach the spinal cord itself in order to provide

[Anesthetic mechanisms in the spinal cord].

The essential elements of anesthesia are : hypnosis, amnesia, analgesia, immobility, and inhibition of untoward reflexes. The spinal cord is responsible for the latter three. Suppression of excitatory transmission and stimulation of inhibitory transmission are the anesthetic mechanisms in the spinal cord. Each anesthetic, however, has a unique effect on the transmission systems in the spinal cord. Some exclusively suppress excitatory transmission or stimulate inhibitory transmission, and others have a dual effect. The minimum alveolar/anesthetic concentration (MAC) is spinally mediated. Tuberculosis of the spine and spinal cord.

Tuberculosis remains an important problem in underdeveloped countries, especially in individuals under the age of 20 years. In developed countries, there was an increase of the disease due to several factors, especially the appearance of A.I.D.S. The arterial haematogenous seeding is the most important way to the propagation of the disease to the rachis. The bacillus mycobacterium tuberculosis is characterized by a slow growth, propention for an oxygen rich environment and the absence of proteolytic enzymes. These characteristics are responsible for the imaging particularities of tuberculous o

[Inflammatory pathology of the spinal cord].

Diagnosis of myelopathy whatever its cause, is based on clinical findings: paraparesis, loss of sensibility, sphincterian disturbance. Diagnosis of myelopathy requires magnetic resonance imaging, which rules out compressive and vascular disorders. Main causes of inflammatory myelopathies are multiple sclerosis multisystemic disorders, infectious and post-infectious diseases. Absence of identifiable causes in one quarter of the cases leads to the diagnosis of idiopathic myelitis.

[Mechanical pathology of the spinal cord].

Syringomyelia is a cyst within the spinal cord, associated with numerous causes, most frequently combined with the Chiari abnormality. Many other causes of craniovertebral junction anomalies are described. In those cases the syringomyelia is so-called hind-brain related syringomyelia. Other dysraphic states and spinal causes of syringomyelia are spina bifida cystica, lumbar lipomas, diastematomyelia or split notochord syndrome. Mechanical causes of spinal cord compression are mostly of tumoral origin. The vast majority of spinal metastases occur extradurally and originate from carcinomas of th
