Portal vein embolization (PVE) induces hypertrophy of the future liver remnant (FLR) in patients undergoing extensive hepatic resection. Portal vein access for PVE via the ipsilateral hepatic lobe (designated for resection) places veins targeted for embolization at acute angles to the access site requiring reverse curve catheters for access. This approach also involves access close to tumors in the ipsilateral lobe and requires care to avoid traversing tumor. Alternatively, a contralateral approach (through the FLR) risks damage to the FLR due to iatrogenic trauma or non-target embolization. T

BACKGROUND: Patients with adenocarcinoma of the pancreatic body/tail and associated vascular thrombosis or adjacent organ invasion are suboptimal candidates for resection. We hypothesized that extended distal pancreatectomy (EDP) for locally advanced adenocarcinoma is associated with a survival benefit.
METHODS: We retrospectively reviewed a prospectively collected database of patients who underwent distal pancreatectomy (DP) for adenocarcinoma at a single academic institution (1996 to 2011) with greater than or equal to 2 years of follow-up.
RESULTS: Among 680 DP patients, 93 were indicated

Eight patients have had selective variceal decompression after a splenectomy or splenic vein thrombosis with successful control of bleeding. The principle veins utilized in these patients, either alone or in combination, were: (a) the splenic remnant, (b) the coronary, (c) the gastroepiploic, and (d) an inferior mesenteric that joined the splenic. High quality preoperative angiography is essential but operative exploration is often required to assess fully the possible shunt options. Simple splenectomy for thrombocytopenia in portal hypertension is rarely justifiable and creates far more probl

We present an 8-year-old boy who was referred to our center with the complaint of upper gastrointestinal bleeding and was diagnosed with hypersplenism and progressive esophageal varices. Performing a computerized tomography (CT) scan, we discovered a suspicious finding in the venography phase in favor of thrombosis in the splenic vein. Once complementary examinations were done and due to recurrent bleeding and band ligation failure, the patient underwent splenectomy. And during the one-year follow-up obvious improvement of the esophageal varices was observed in endoscopy.

Bleeding esophageal varices (EV) have the highest mortality rate from all complications of liver cirrhosis (LC). Several Doppler ultrasound (DUS) studies have been done on the splenic or portal vein (PV) to evaluate the hemodynamic of the esophageal vein. Our study focused on finding a better index using the ratio from two parameters correlated with EV, splenic vein flow volume (SFV), and PV flow velocity. In this study, 28 patients with LC were evaluated using DUS to compare the SFV to PV flow velocity/speed (Sv/Ps) index and other measured DUS parameters with the EV degree. Afterward, the re

BACKGROUND: Splenic rupture because of metastasis from a distant organ is extremely rare.
CASE PRESENTATION: An 80-year-old man presented with left flank pain. A computed tomography (CT) demonstrated a poorly enhanced enlarged spleen with bulky thrombus in the splenic vein without extravasations. A CT on the following day showed increased intraperitoneal hemorrhage; therefore, an emergency laparotomy was performed. The spleen was enlarged and ruptured with lacerations on its surface. Macroscopic examination showed congestion with a thrombus in the splenic vein around the hilum. Pathology reve

Objectives: Demographic features of subjects, such as body mass index, height, weight, age, or sex, may affect diameters of vessels such as the splenic artery (SA) and splenic vein (SV). Some studies use alternative anatomical indicators, including the body of the first lumbar vertebra (L1), to improve standardization in the assessment of vascular pathologies. This study aimed to evaluate the relationship between splenic vessel diameters (SA and SV) and L1 using abdominopelvic computed tomography images obtained from individuals aged 1-80 years. Methods: Radiologic images of 800 subjects were

AIM: To determine risk factors for portal venous system thrombosis (PVST) after partial splenic artery embolisation (PSAE) in cirrhotic patients with hypersplenism.
MATERIALS AND METHODS: Between March 2014 and February 2022, 428 cirrhotic patients with hypersplenism underwent partial splenic artery embolisation and from these patients 208 were enrolled and 220 were excluded. Medical records of enrolled patients were collected. Computed tomography (CT) images were reviewed by two blinded, independent radiologists. Statistical analyses were performed by using SPSS.
RESULTS: Progressive PVST w

BACKGROUND: Splenic vein stenosis and occlusion, which are known causes of left-sided portal hypertension, often occur secondary to trauma, pancreatitis, or invasion or compression by pancreatic tumors. However, few reports have described idiopathic splenic vein stenosis.
CASE PRESENTATION: A 70-year-old man was referred to our hospital for examination of isolated gastric varices. He had no history of liver disease, pancreatitis, or abdominal trauma. Computed tomography revealed stenosis of almost the entire length of the splenic vein, and development of gastric fundal and short gastric varic

BACKGROUND: Surveillance endoscopy to detect varices needing treatment (VNT) is important to prevent bleeding and morbidity in portal hypertension. In adult and pediatric cirrhosis, platelet count and liver stiffness measurement (LSM) are useful in selecting patients for endoscopy. Such recommendations do not exist for extrahepatic portal vein obstruction (EHPVO). Splenic stiffness measurement (SSM) has been studied in adult and pediatric EHPVO with conflicting results and methodological errors. This study evaluates the role of platelet counts and SSM to predict VNT and bleeding in pediatric E

BACKGROUND: Spleen-preserving distal pancreatectomy with excision of splenic vessels (SPDP-ESV) has been widely adopted because of its simplicity. However, this simplified technique may lead to gastric varices and bleeding.
METHODS: For better understanding of the hemodynamic changes and risk of gastric variceal bleeding, we instituted a prospective study to perform serial follow-up contrast abdominal computed tomography (CT) and endoscopy for patients after SPDP-ESV.
RESULTS: There were 37 patients, with a mean follow-up period of 45.3 months (range 23-59 months), included in the analysis.

PURPOSE: Spleen-preserving distal pancreatectomy (SPDP) is performed for benign or low-grade malignant tumors of the pancreatic body and tail. The Warshaw technique (WT), involving the division of the splenic artery and vein, is associated with splenic infarction and gastric varices. Recently, SPDP with splenic artery resection and splenic vein preservation (SVP) has been introduced. However, its safety and feasibility remain unclear. This study compared the perioperative outcomes between the SVP and WT approaches.
METHODS: We retrospectively reviewed the data on patients who underwent laparo

The development of non-cirrhotic portal hypertension and esophagogastric varices is an uncommon, yet life-threatening complication of splenic vein thrombosis. Recognizing this association between splenic vein thrombosis and non-cirrhotic portal hypertension is critical in reducing morbidity and mortality in patients with hypercoagulable disease secondary to malignancy. Herein, we present a 45-year-old female with stage IV high-grade serous ovarian carcinoma and chronic splenic vein thrombosis found to have life-threatening esophagogastric varices secondary to non-cirrhotic portal hypertension.

BACKGROUND: Abdominopelvic arteriovenous malformation is an uncommon congenital vascular lesion, for which the diagnosis and treatment are usually difficult. Though embolization and sclerotherapy are the primary treatment strategies, traditional surgical resection remains a valuable option.
CASE PRESENTATION: Herein, we present a 32-year-old female diagnosed with a massive abdominopelvic arteriovenous malformation that originates from the splenic artery and drains into the portal vein. The vascular lesion was evaluated with multiple imaging modalities and then surgically resected successfully
