Diagnostic Uncertainty in a Case of Heterotaxy Leading to Intestinal Malrotation and Volvulus.

We present the case of a baby with vomiting and constipation. The diagnostic tests revealed an inverted stomach, the absence of superior mesenteric vein visualization, and an elongated superior mesenteric artery, leading to the diagnosis of malrotation with situs ambiguous. Exploratory laparotomy with the Ladd's procedure was performed, and the child recovered well in the neonatal ICU. The case presented with diagnostic uncertainty in the face of vague symptoms, highlighting the need for a suspicion of malrotation and volvulus in neonates. With limited guidelines on diagnostic tests, this requ

Ileocolic intussusception mimicking the imaging appearance of midgut volvulus as a result of extrinsic duodenal obstruction.

Duodenal obstruction caused by ileocolic intussusception in the absence of intestinal malrotation is extremely rare. We present and discuss the imaging findings in an infant with an intussusception secondary to a duplication cyst in whom sonography also showed inversion of the orientation of the mesenteric vessels and a distended stomach. A contrast medium study revealed a proximal duodenal obstruction with a beak appearance suggestive of midgut volvulus. At surgery, an ileocolic intussusception causing duodenal obstruction without concomitant malrotation or volvulus was found. The combination

Secondary Mesenteroaxial Gastric Volvulus in an Elderly Female: A Rare Presentation of an Occult Diaphragmatic Hernia.

Gastric volvulus is a surgical emergency characterized by rotation of the stomach greater than 180°, potentially leading to ischemia and perforation. Mesenteroaxial volvulus associated with a late-presenting diaphragmatic hernia in adults is an uncommon clinical entity. A 70-year-old woman with diabetes presented with acute abdominal distension and lactic acidosis. Nasogastric decompression yielded 3 L of coffee-ground aspirate, and computed tomography (CT) identified a mesenteroaxial gastric volvulus. Emergency laparotomy revealed a left-sided diaphragmatic hernia containing the gastric antru

Hiatal hernia complicated by gastric volvulus: case report.

Gastric volvulus is a rare and potentially life-threatening complication of large hiatal hernias. It is defined as the abnormal rotation of the stomach by >180° around one of its axes, leading to closed-loop obstruction and risks of ischemia and perforation. We present a case of an 84-year-old female presented with severe epigastric pain associated with coffee ground vomiting. Esophagogastroduodenoscopy and computed tomography scan confirmed a large hiatal hernia complicated by gastric volvulus. Surgical intervention was done with a tension-free gastropexy following a partial reduction of the

Mesentero-axial gastric volvulus with gastric outlet obstruction.

This case report describes a woman in her late 50s with mesenteric volvulus, an uncommon and potentially fatal condition. She developed excessive abdominal distension, nausea and vomiting for a duration of 2 days. The preliminary ultrasound indicated a large, thick-walled, tight fluid region in her upper abdomen, indicating a probable gastric outlet obstruction. A subsequent contrast-enhanced CT scan of the abdomen confirmed the diagnosis, showing significant distension of the stomach with a thick septum displacing adjacent organs. The mesenteric volvulus was surgically treated as an emergency

Acute gastric volvulus: A rare case report and literature review.

INTRODUCTION AND IMPORTANCE: Gastric volvulus is the abnormal gastric rotation upon itself. It carries a considerable risk for gastric strangulation which may lead to gangrene and perforation, which can be fatal. It commonly presents with non-specific and vague abdominal symptoms making prompt diagnosis and management imperative. This is a case report describing a case of acute organo-axial gastric volvulus. CASE PRESENTATION: A 74-year-old female, with no comorbidities, presented to our department with non-specific abdominal symptoms. Gastric outlet obstruction was suspected. The patient was

Gastric outlet obstruction from a caecal volvulus, herniated through epiploic foramen: a case report.

A 55-year-old woman presented with sudden onset upper abdominal pain and vomiting. On examination she had tender epigastric mass with "succusion splash" on auscultation. Straight abdominal x ray showed a distended and displaced stomach with another gas filled viscus around it. Subsequent computed tomography suggested caecal volvulus herniated through the epiploic foramen obstructing the gastric outlet. The patient underwent reduction of the internal hernia and right hemicolectomy. Postoperative recovery was uneventful. Herniation of caecal volvulus through the epiploic foramen is a very rare c

Three cases of retrosternal diaphragmatic hernia (Morgagni hernia) are reported. Diagnosis have been made by conventional radiology (Chest X-ray, Upper G-I series) in 2 cases: the third one was found incidentally during a laparotomy. Peritoneography, CT scan, NMR are recommended in the difficult cases. ONe case presented as acute gastric outlet obstruction secondary to an intra-thoracic volvulus of the herniated stomach. Surgical treatment is indicated in all cases of retrosternal diaphragmatic hernia because of the high-risk of complications (gastric volvulus, colonic obstruction). Surgery th

Mesentero-axial gastric volvulus treated with laparoscopic gastropexy: a case report.

BACKGROUND: Mesentero-axial gastric volvulus (MAGV) is an uncommon subtype of gastric volvulus (GV). However, reports of such cases in adult patients are very rare. We present an unusual case of idiopathic MAGV in an old woman. CASE PRESENTATION: An 84-year-old woman was referred to the emergency department for vomiting and abdominal pain. An abdominal computed tomography scan revealed a mesentero-axial gastric volvulus, which could be corrected endoscopically, and the symptoms were relieved. Contrast-enhanced examination was performed before the elective surgery to confirm the presence of sh

Redescending Stomach: A Rare and Potentially Lethal Complication of Gastric Herniation.

Large gastric hernias are common and usually cause minor symptoms. Rarely, complete intrathoracic herniation of the stomach is complicated by strangulation. The underlying mechanism can be gastric volvulus or the less recognized phenomenon of gastric fundus redescent. We describe a case where this rare but potentially lethal complication of gastric herniation is present. Additionally, we show that gastric pneumatosis, a sign associated with ischemia, can be initially visualized on a plain chest radiograph in this setting. Teaching point: Redescent of the fundus is a possible, but unrecognized

Small bowel volvulus resulting in infarction due to an anterior gastropexy band.

Volvulus of the small bowel, although rare, carries a high risk of strangulation and ischemic necrosis. It is usually caused by the rotation of a loop of small intestine around an adhesion band or stoma. We present a case of an anterior gastropexy band, giving rise to a small bowel volvulus, necessitating resection due to infarction. This band resulted from separation of the most distal anterior gastropexy suture from the anterior abdominal wall. The distensible nature of the stomach and its resultant postprandial gain in weight produced tremendous shearing forces on the gastropexy sutures, an

[A complicated paraesophageal diaphragmatic hernia. A clinical case report].

A case of diaphragmatic paraesophageal hernia complicated by omentum and gastric incarceration is reported. The pure paraesophageal hiatal hernia is a rare entity. The symptoms and complications are caused by the anatomic defect and not by gastroesophageal incompetence. Chronic, recurrent, asymptomatic blood-loss anemia is the most common complication. Gastric volvulus is frequent in which the stomach herniated in a huge parietal peritoneal sac in the chest behind the heart. Volvulus may be a chronic recurrent problem or an acute life threatening complication. The diagnosis depends on the pati

Gastric wall necrosis from organo-axial volvulus as a late complication of laparoscopic gastric banding.

We present a case of massive gastric wall necrosis associated with volvulus of the stomach occurring 17 months after laparoscopic gastric banding. The 19- year-old female was admitted to our hospital with acute abdominal pain with rapid deterioration from massive necrosis of a distended stomach which perforated distal to the gastric band, accompanied by splenic infarction. Total gastrectomy and spenectomy were required. We present this very rare complication which should be recognized and treated early, so that a less drastic operation is possible.

Delayed presentation of congenital diaphragmatic hernia with intrathoracic gastric volvulus.

BACKGROUND: Gastric volvulus (GV) occurs when the stomach abnormally rotates around one of its axes and is a rare upper gastrointestinal obstruction. We present an unusual case of intrathoracic GV associated with delayed manifestation of congenital diaphragmatic hernia. METHODS: A 16-month-old female infant presented with a history of projectile non-bilious vomiting for 2 days and mild hematemesis for the last day. Physical examination showed epigastric fullness and pain with abdominal palpation. Complaints of the patient disappeared on the 2nd day after hospital admission. On the 6th day non

Gastric dilatation-volvulus syndrome in dogs.

Gastric dilatation-volvulus is a medical and surgical emergency that principally affects large-breed dogs. Surgical treatment should be undertaken as soon as the patient has been stabilized with fluid therapy and decompression. A gastrectomy might be required if the stomach is becoming necrotic. A gastropexy is required to prevent recurrence.

Emergent gastric remnant volvulus in a post-OAGB patient with massive hiatal herniation: a life-threatening presentation in an asymptomatic course.

One anastomosis gastric bypass (OAGB) is a widely used bariatric procedure with favorable outcomes and a relatively low complication rate. However, both early and late postoperative complications can occur, including rare but serious events such as gastric remnant volvulus, particularly in the presence of massive hiatal herniation. The altered anatomy following OAGB can make the diagnosis of such complications challenging, especially when clinical presentations are atypical or silent. We report the case of a 67-year-old woman with a history of OAGB and prior hiatal hernia repair who presented

Gastric Volvulus: A Complication of Hiatal Hernia.

Gastric volvulus is a rare but potentially life-threatening condition, with difficult diagnosis. We present a case report of a demented woman aged 65 years that attended the emergency department with epigastric pain and vomiting for the past 10 days. The chest plain revealed a retrocardiac air-filled mass and the abdomino-pelvic computed tomography confirmed the diagnosis of gastric volvulus. The patient was admitted. Acute visceral obstruction.

Gastric volvulus is a rare cause for acute visceral obstruction, with a high mortality rate that rises with delay in definitive treatment. A 33-year-old woman with a rare chromosomal mutation (46,XX,del(6)(q25.1q25.3)) presented with evidence of acute visceral obstruction. Diagnostic difficulties delayed treatment and she clinically deteriorated. Urgent CT imaging revealed acute mesentero-axial gastric volvulus. At laparotomy, global failure of colonic and splenic fixation was found. The viscera were de-rotated, the stomach salvaged and gastropexy and colopexy were performed. This is the first
