Management of recurrent stress incontinence following a sling.

Management of recurrent or persistent stress urinary incontinence (RSI) following primary insertion of a synthetic midurethral sling (MUS) remains a challenge for the urologist since no consensus is available to favor one treatment over another. Complete workup should be carried out, including cystoscopy, urodynamics and potentially a pelvic floor ultrasound as a diagnostic adjunct. Various surgical options have been described for RSI, apart from another MUS, including tightening of or shortening a previously placed MUS, a mini-sling, a salvage spiral sling, a colposuspension, the ACT® system,

[Surgical treatment of female urinary stress incontinence].

Since the description of the sub urethral technique for genuine stress incontinence all the others have been abandoned. The surgical indications have not changed and are related to the poor quality of life of the patient induced by stress incontinence. A careful clinical examination and a pre operative urodynamique investigation should prevent from the failures due to a wrong indication. All the techniques are the same when it comes to the result except the mini slings which still have to be evaluated. There are two types of complications: the ones which are specific to the using of a hexogen

Recurrent urinary stress incontinence: an overview.

AIM: We review the literature regarding recurrent urinary stress incontinence (USI) following various anti-incontinence procedures and analyze possible risk factors for failure of different anti-incontinence procedures. Management options for recurrent USI and future trends are also covered. METHODS: A PubMed search was undertaken for English language literature using the MeSH terms 'stress urinary incontinence', 'recurrence', 'treatment', and 'failure'. Literature from the year 2001 to 2010 was selected. We only selected those studies including randomized control trials, meta-analysis, and p

Transobturator tension free vaginal tapes and bladder injury.

INTRODUCTION: Tension free vaginal tapes have transformed the surgical management of female urodynamic stress incontinence (USI). Transobturator approach of tension free vaginal tapes are emerging as one of the surgical options for USI. MATERIALS AND METHODS: A literature search to answer question of routine cystoscopy during transobturator tape procedures. CONCLUSION: Though, both TOT and TVT-O has been shown not to enter pelvis on anatomical dissection on female cadavers, many cases of bladder injury have been reported. Delorome and De Leval strongly suggest avoiding routine cystoscopy dur

[Vaginal mesh fixed by transobturator tape can lead to ureteral injury in cystocele surgery].

The lower third ureteral injuries are not rare during pelvic surgeries. We report for the first time a case of ureteral injury during a transvaginal cure of an anterior vaginal wall prolapse with a synthetic mesh, whose the four arms are fixed by transobturator way. This lesion is itself included in the lesions of the low urinary tract during vaginal surgery with transobturator tape. The difficult diagnosis of these iatrogenic injuries must be carried early in order to decrease their morbidity and in the case of ureteral injury to preserve the nephrogenic capital.

Tension-free vaginal tape obturator system in the treatment of stress urinary incontinence.

Surgical approach to the treatment of female stress urinary incontinence has changed significantly since 1995, when Ulmsten first described the procedure providing tension-free support in the middle part of urethra. Although retropubic placement of TVT tapes seemed to meet all the standards of minimally invasive surgical procedures, the occurrence of complications (bleeding, hematomas, and bladder, urethral of intestinal injuries) necessitated a new obturator approach. This study presents a transobturator approach to TVT tapes placement (Gynecare, Johnson-Johnson Company) with "inside-out" tec

Urinary stress incontinence treatment with Remeex in a patient with epispadias: a case report.

Epispadias is a rare condition in women. A postmenopausal 53-year-old woman with a known problem of epispadias came to our outpatient clinic complaining of persistent severe urinary stress incontinence without overactive bladder symptoms. She had previously undergone an inefficacious tension-free vaginal tape (TVT) procedure in another institution. We implanted a Remeex (Mechanic External Regulation) device without applying any tension to the sling. Two tape regulations were performed the day after surgery and 2 months later to gain continence. At 13 months of follow up, this woman was asympto

Multifilament polypropylene mesh for urinary incontinence: 10 cases of infections requiring removal of the sling.

One hundred and forty-nine women received a suburethral woven polypropylene mesh insertion for urodynamic stress incontinence. Eleven patients (7.5%) presented between 4 and 17 months after surgery (mean 10 months) with symptoms of retropubic infection. Ten patients required surgical exploration, drainage of the collection and removal of the tape either unilaterally (six) or bilaterally (four). Bacterial cultures were negative in nine women. Following removal of the tape, eight patients reported residual incontinence, either stress leakage (five) or de novo urge incontinence (three). Multifila

Percutaneous bone anchor sling using synthetic mesh associated with urethral overcorrection and erosion.

Percutaneous bone anchor bladder neck suspension has been recommended as a less morbid alternative to traditional anti-incontinence procedures. Specifically, it has reported to be associated with shorter duration of hospitalization, catheterization and urinary retention, and equivalent short-term cure rates. Recently, there have been reports of pubic osteomyelitis associated with bone anchor placement, and high incidences of recurrent incontinence. To improve the effectiveness of the procedure the placement of a suburethral synthetic collagen-impregnated mesh without tension was recommended. A

Transobturator surgery for female stress incontinence: a comparative anatomical study of outside-in vs inside-out techniques.

OBJECTIVE To assess the specific risks of injury to neural and vascular structures inherent in two approaches to transobturator surgery for inserting a suburethral sling, i.e. the outside-in (standard technique) and inside-out approaches. MATERIALS AND METHODS The study comprised seven cadavers, providing 14 obturator regions. Five specimens had a tape inserted outside-in on one side, and inside-out on the other; of the remaining two cadavers, one had an inside-out tape and one an outside-in tape, bilaterally. After tape insertion, the cadavers were dissected. Particular attention was paid to

Long-term outcomes of retropubic tension-free vaginal tape for stress urinary incontinence after a transobturator tape failure: a retrospective study.

INTRODUCTION AND HYPOTHESIS: Management of stress urinary incontinence (SUI) after a transobturator tape (TOT) failure is a controversial issue. There are few long-term data on the different treatment options. The aim of this study is to evaluate the long-term effectiveness and safety of retropubic suburethral slings (RP-TVT) in this setting. METHODS: A descriptive retrospective study was performed among women with persistent/recurrent SUI treated at the Vall d'Hebron University Hospital between January 2006 and December 2014. All women were preoperatively evaluated to rule out complications

Detection of biofilms on explanted urinary incontinence slings and their relationship with pain, exposure, or urinary tract complications.

UNLABELLED: Midurethral slings (MUS) are synthetic mesh implants used to treat stress urinary incontinence in women. Positioned via retropubic or transobturator approaches, they support the mid-urethra to maintain closure during increases in intra-abdominal pressure. Although generally effective, long-term MUS complications have been associated with chronic inflammation, pain, and infection, with explanted slings often showing bacterial colonization. The underlying mechanisms remain unclear, particularly the role of microbial biofilms. We examined the presence and characteristics of biofilms o

Stress urinary incontinence.

Incontinence is a major clinical issue in women's health and well-being. Various interventions have been developed to address this issue; however, these carry a risk of significant potential complications. In recent years, the safety profile of mesh surgery in particular has been a focus of renewed debate. In this report, a case of complex urogynaecological management of stress urinary incontinence is discussed in detail, with a particular emphasis on an understanding of different treatment options and practical learning points.
