The authors report the results they obtained by bone scintigraphy using technetium pyrophosphate. We present an autopsy case of primary leptomeningeal melanoma visualized with technetium-99m-hexamethylpropyleneamine oxime (Tc-99m-HMPAO) single photon emission CT (SPECT) of the brain. Increased uptake of Tc-99m-HMPAO coincided with leptomeningeal enhancement on MR images and with the tumor location of the autopsy findings. It was thought that Tc-99m-HMPAO could correlate closely with melanoma. Tc-99m-HMPAO SPECT clearly showed primary leptomeningeal melanoma and severe hypoperfusion induced by intracranial hypertension and tumor proliferation.

We describe herein a case of parathyroid carcinoma accompanied with metastatic calcification identified by technetium-99m methylene diphosphonate (Tc-99m MDP) scintigraphy in the lungs, kidneys and stomach. A 99mTc-MDP bone scan performed on a 52-yr-old female for possible bone metastasis revealed prominent hepatic uptake. Subsequently, a 99mTc-SC scan revealed tracer uptake in the caudate lobe with diminished uptake in the remainder of the liver. Further imaging with Doppler ultrasound and hepatic venography confirmed a diagnosis of Budd-Chiari syndrome. Hepatic necrosis, demonstrated on CT imaging, was secondary to Budd-Chiari syndrome and was felt to be the cause of 99mTc-MDP hepatic uptake in this patient.

In 11 non-smoking AIDS patients suspected of pneumocystis carinii pneumonia (PCP), the results of Tc-99m DTPA aerosol clearances, gallium scans, and arterial blood gases were compared with those of bronchoalveolar lavage (BAL). The authors, from their experience in 60 patients, demonstrate that technetium-99m-labelled red blood cell scintigraphy is a sensitive (91%) and specific (76%) diagnostic procedure with a high predictive value, when both positive (81%) and negative (92%). Two HEDP complexes of technetium (either Tc-99 or a mixture of Tc-99 and Tc-99m) have been prepared without the use of stannous ion. Technetium-99m stannous pyrophosphate (99mTc-PYP) myocardial scintigrams were obtained in 68 patients during acute myocardial infarction (AMI) and at follow-up 15.9 +/- 8.8 weeks later.
