Effectiveness of Corticosteroid Injections in Diabetic Patients With De Quervain Tenosynovitis.

PURPOSE: We sought to determine the effectiveness of corticosteroid injections (CSIs) for de Quervain tenosynovitis in patients with diabetes mellitus. METHODS: We retrospectively identified all patients with diabetes receiving a CSI for de Quervain tenosynovitis by 16 surgeons over a 2-year period. Data collected included demographic information, medical comorbidities, number and timing of CSIs, and first dorsal compartment release. Success was defined as not undergoing an additional CSI or surgical intervention. The mixture of a corticosteroid and local anesthetic provided in each injection

The Radial Synergy Test: An Aid to Diagnose de Quervain's Tenosynovitis.

BACKGROUND: Diagnosis of de Quervain's tenosynovitis is made clinically. Finkelstein's and Eichoff's tests are commonly utilized examination maneuvers. Their specificity has been questioned due to a propensity to provoke pain in asymptomatic patients. Using the principle of synergism, the novel radial synergy test takes advantage of isometric contraction of the first dorsal compartment with resisted abduction of the small finger. METHODS: Electromyography was performed on 3 authors and the first dorsal compartment sampled during the maneuver. Sensitivity evaluation was performed via retrospec

Acute rheumatic fever associated with tenosynovitis and a unique cytokine profile.

Acute rheumatic fever (ARF), caused by group A β-hemolytic streptococcus infection, is characterized by inflammation affecting several organs. There are few reports on magnetic resonance imaging (MRI) findings in patients with ARF. An 8-year-old Japanese boy presented with a prolonged fever of unknown cause and swelling of his right hand. MRI of his hand revealed tenosynovitis. Migratory arthritis and erythema marginatum appeared following the hand swelling. We diagnosed him as having ARF based on the clinical course and serological testing for group A β-hemolytic streptococcus. His serum inte

Oyster-related tenosynovitis: a rare case of Mycobacterium szulgai in the immunocompromised.

A 66-year-old man with a history of renal transplant on chronic immunosuppression presented to his primary care physician with a swollen right index finger. On examination, mild swelling was present. Conservative management failed and initial plain films were negative. Corticosteroid injection was performed by orthopaedics, but symptoms recurred several months later and an MRI showed flexor digitorum tenosynovitis and abscesses of the superficialis and profundus tendons. A flexor tenolysis was performed with cultures positive for Mycobacterium szulgai, a rare, non-tuberculous mycobacterial inf

Flexor Tenosynovitis Due to Tuberculosis in Hand and Wrist: Is Tenosynovectomy Imperative?

The treatment of flexor tenosynovitis in the hand and wrist due to tuberculosis is controversial. Although some authors recommend the antituberculous chemotherapy, the others recommend the surgical treatment. In this article, 12 patients with synovial tuberculosis of the flexor aspect of the hand and the wrist were evaluated with respect to diagnosis and treatment modalities. None of the patients had a history of tuberculosis, concomitant disease, immunosuppressive drug use, drug abuse, and human immunodefficiency virus positivity. A chest x-ray and family screening were performed in all of th

The diagnostic accuracy of inflammatory blood markers for purulent flexor tenosynovitis.

PURPOSE: For patients with purulent flexor tenosynovitis, our purpose was to (1) calculate the diagnostic accuracy of white blood count (WBC), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) for those who underwent surgical drainage, (2) to correlate these markers for those treated with antibiotics alone, and (3) to evaluate the accuracy of diagnosis for surgical patients. METHODS: A total of 82 consecutive patients (71 surgical and 11 nonsurgical) with flexor tenosynovitis were identified from orthopedic databases at 2 academic centers. We evaluated inflammatory markers (W

Focal radial styloid abnormality as a manifestation of de Quervain tenosynovitis.

OBJECTIVE: de Quervain disease is a stenosing tenosynovitis of the first dorsal wrist compartment. The purpose of this study was to determine whether focal radial styloid abnormality (cortical erosion, sclerosis, or periosteal bone apposition) as shown by radiography can be an indicator of de Quervain tenosynovitis. MATERIALS AND METHODS: A retrospective review of 49 radiographs from 45 patients in whom the clinical diagnosis of de Quervain tenosynovitis was confirmed (positive findings on Finkelstein's test) and 64 radiographs from 62 asymptomatic patients was carried out independently by tw

A retrospective study of 192 horses affected with septic arthritis/tenosynovitis.

The medical records of 192 horses with septic arthritis/tenosynovitis 1979-1989 were reviewed. Forty-three horses developed infection after an intra-articular injection, 46 following a penetrating wound, 25 following surgery, 66 were foals less than 6 months old, and 12 were adult horses without a known aetiology. Haematogenous infection of a joint occurs in adult horses and should be considered as a differential diagnosis in horses with an acute onset of severe lameness. The aetiology of the infection had a significant effect on the type of bacteria identified by culture. Staphylococcus was c

Sporothrix tenosynovitis--differential diagnosis of granulomatous inflammatory disease of the joints.

Sporothrix tenosynovitis is difficult to distinguish from rheumatoid arthritis--the organisms are scarce in biopsy specimens, and both diseases cause a granulomatous synovitis which can affect multiple joints, have associated subcutaneous nodules, similar radiographic findings, and respond to heat and salicylates. This report describes a patient treated for over 2 years as having rheumatoid arthritis. Multiple tenosynovectomy specimens revealed granulomatous synovitis with sterile cultures. Eventually a fungal culture was positive for Sporothrix. This report discusses the histopathologic featu

Diffuse pigmented villonodular tenosynovitis of the ulnar bursa--a case report.

A case report of diffuse pigmented villonodular tenosynovitis involving the entire ulnar bursa and a review of the literature are presented. There have been few reports of the diffuse form of pigmented villonodular tenosynovitis involving the hand as compared to the localized nodular form, which is a common tumor of the hand. Complete tenosynovectomy of the involved areas is recommended.

"Daddy wrist": A high-resolution ultrasound diagnosis of de Quervain tenosynovitis.

De Quervain's tenosynovitis involves the first of the six dorsal compartments of the wrist, which contains the abductor pollicis longus (APL) and extensor pollicis brevis (EPB) tendons. It seems to be associated with female sex (F:M = 10:1), middle age (30-50 years) and activities involving repetitive hand and wrist motions such as typing, piano playing or repetitively lifting children head, such as in postpartum females (hence the term "baby wrist" or "mommy wrist"). Aim of this paper was to illustrate high-resolution ultrasound (US) features of the DQD by describing a well-documented case th

Infectious Flexor Tenosynovitis Following Trigger Finger Release: Incidence and Risk Factors.

Background: Trigger finger release (TFR) is one of the most commonly performed hand procedures and typically results in restoration of normal finger function. However, uncommon postoperative complications such as deep infection can have devastating consequences. The goal of this study was to evaluate the incidence, risk factors, and characteristics of infectious flexor tenosynovitis occurring after TFR. Methods: We searched billing records for the Current Procedural Terminology code for TFR to identify all adult patients who underwent TFR at our institution over a 10-year period. We defined in

Complex Regional Pain Syndrome-Type 1 Presenting as deQuervain's Stenosing Tenosynovitis.

AIM: To report the presentation of complex regional pain syndrome-1 (CRPS-1) as deQuervain's stenosing tenosynovitis (DQST). CASE REPORT: A 24-year-old woman presented with 3-year history of clinical diagnostic criteria (CDC) of CRPS-1. Conservative and surgical treatment for this as DQST had failed to relieve her. We diagnosed the problem as CRPS-1with CDC as inflammatory manifestations of a mechanical tendinoses of all her 5 digital tendons caused by movement of the fingers and hand tethered by agonist (flexor)/ antagonist (extensor) muscles in co-contraction. Surgical release of de Quervain's stenosing tenosynovitis postpartum: can it wait?

We compared the efficacy of conservative management of de Quervain's disease in 30 women postpartum (group 1) and 30 nonpregnant women (group 2). All patients underwent conservative management consisting of 2 weeks of splinting of the affected wrist, followed by physiotherapy and antiinflammatory drugs (NSAIDs). Clinical evaluation was performed using a functional score and the visual analogue scale (VAS) of Scott-Huskinson at entry to the study, 1 month later, and at a follow-up of 6 months. Conservative management gave good results in patients in group 1. At the 6-month follow-up a significa

De Quervain's tenosynovitis and ganglion over first dorsal extensor retinacular compartment.

Between 1988 to 1991, 80 cases of de Quervain's disease were managed at the Department of Orthopaedic Surgery, Singapore General Hospital. Six of them had an associated ganglion over the first dorsal retinacular compartment. Five out of 6 cases had intracompartment injections of hydrocortisone and lignocaine but the treatment was not effective in relieving the pain of de Quervain's tenosynovitis. All 6 cases had excision of the ganglion and decompression of the first extensor compartment. All the lumps were found arising from the dorsal aspect of the retinaculum and no intracompartment extensi
